Stenosis of the lumbar spinal canal in Forestier's disease.

Hyperostosis of the spine, Forestier's disease, is a rare, but well defined entity which results from silent, extensive paraspinal calcification. It has been regarded as an asymptomatic ankylosis requiring no medical or surgical treatment. Two patients with Forestier's disease are presented who show severe stenosis of the lumbar canal with early symptoms of neurological compression.

Percutaneous Endoscopic Lumbar Decompression for Lumbar Lateral Spinal Canal Stenosis: Classification of Lateral Region of Lumbar Spinal Canal and Surgical Approaches.

PURPOSE: The purpose of the present study was to provide a new classification of the lateral region of the lumbar spinal canal (LLSC) and evaluate the clinical outcomes of surgical treatment of LLSC stenosis guided by the classification. METHODS: We developed a new functional classification of the LLSC by dividing the region into 5 zones. Thirty random lumbar imaging studies from patients with single-level LLSC stenosis in our department from January 2015 to June 2015 were evaluated by 3 examiners to validate its reliability. Subsequently, a prospective clinical outcome study of 76 patients w

Nursing Management of Postoperative Lumbar Spinal Canal Stenosis.

Spinal stenosis most commonly occurs at L4-5, with 91% of patients experiencing nerve compression at this location. It results from a degenerative process that begins in the intervertebral disc, as viable cell counts, water, and proteoglycan content decrease in the nucleus pulposus. The disc height decreases as the nucleolus pulposus degenerates and the annulus fibrosus bulges. Stenosis is generally classified by descriptive elements, such as etiology (i.e., degenerative or congenital), location (i.e., central, lateral recess, foraminal, or extraforaminal), and severity of narrowing on advance

Three-Dimensional Reconstruction of the Genial Spinal Canal.

The aim of this anatomical study was to investigate the genial spinal canal histologically and to reconstruct it three-dimensionally to improve understanding of its anatomy and to reveal any differences between dentate and edentulous specimens. Two tissue blocks from the mandible between the left and right second incisors, one dentate and one edentulous, were fixed in 4.5% formaldehyde, decalcified and embedded in paraffin. Serial histological sections were prepared, stained with Azan and examined microscopically. Additionally, three-dimensional models of the blocks were reconstructed using mi

Posture of patients with lumbar spinal canal stenosis.

BACKGROUND: The available literature is lacking in reports on the quantitative analysis of posture in patients with lumbar stenosis. OBJECTIVE: The aim of this study was to analyze body posture in patients with lumbar spinal canal stenosis. METHODS: The study involved 100 people: 49 persons with severe lumbar spine stenosis and 51 control subjects without any history of back pain. All participatants were evaluated by a photogrammetric method. RESULTS: Photogrammetric measurements showed statistically significant differences in the shape of the anterior-posterior curvatures of the spine. In

Extradural nodular fasciitis arising in the spinal canal.

STUDY DESIGN: Case report. OBJECTIVE: To describe a patient with nodular fasciitis arising in the lumbar extradural space. SUMMARY OF BACKGROUND DATA: Nodular fasciitis is a benign proliferation of fibroblasts and myofibroblasts. It commonly occurs in the subcutaneous tissue of an upper extremity, trunk, head, and neck, but rarely arises in the spinal canal. METHODS: A 7-year-old boy experienced gradually increasing intense radiating pain from the bilateral buttocks to the lower extremities after a bruise on his lower back. Computed tomography and magnetic resonance imaging demonstrated a r

Vertebroplasty for osteoporotic fractures with spinal canal compromise.

BACKGROUND AND PURPOSE: Percutaneous vertebroplasty can aggravate spinal canal narrowing, especially in patients with preoperative retropulsion. The purpose of this study was to evaluate changes in spinal canal dimension during percutaneous vertebroplasty for osteoporotic fractures with retropulsion. MATERIALS AND METHODS: We reviewed all cases of osteoporotic vertebral fractures treated with vertebroplasty. Twenty-one patients (25 vertebrae) had a retropulsed fragment that compromised the dimension of the spinal canal on preoperative imaging. We measured the degree of retropulsion before and

Narrowing of thoraco-lumbar spinal canal in achondroplasia.

The incidence of neurological deficits in achondroplastic subject is by no means negligible. We considered the clinico-radiological and therapeutic data of 35 achondroplasic patients (5 personal cases and 30 published in the available literature) harbouring myelo-radicular deficits related to narrowing of thoraco-lumbar canal. There was no significant sex prevalence. The clinical symptoms appear most frequently in the III or IV decades of life. Actually morphological anomalies of the spinal canal are already present at birth in achondroplasic dwarfs, and signs of cervical cord involvement are

Cervical sagittal spinal canal size in spine injury.

This study investigated the relationship between cervical spine sagittal canal diameter and neurologic injury in cases of spinal fracture-dislocation. A group of 98 patients with such injuries was reviewed; 45 had no neurologic deficits, 39 had incomplete quadriplegia, and 14 had complete quadriplegia. Spinal canal sagittal diameter was measured in all, and large diameter and small canals were defined. Small diameter canals were correlated significantly with neurologic injury, while large diameter canals allowed protection from neurologic injury in cervical fracture dislocation.

Vertebral body size in lumbar spinal canal stenosis.

The transverse and sagittal diameters of the lumbar vertebral bodies have been compared in patients with symptoms and myelographic findings consistent with stenosis of the lumbar canal, in patients with some narrowing of the lumbar canal, and in patients without clinical or myelographic indication of stenosis. No significant difference in the measured diameters was found.

[Stenosis of the cervical spinal canal: acquired? congenital].

The authors show that it is impossible to describe congenital stenosis of the cervical rachidian canal with a particular morphology that would make it possible to recognize the condition before the appearance of neurological signs. They do not deny its existence, but at the moment there is no proof, and hope of a prophylaxis for the neuropathy is not justified. On the other hand, there is clearly a possibility of an acquired stenosis. The term arthrosis is so generalized that it has lost nearly all its meaning. The rheumatologist knows the complex lesions comprise osteophytes and thickening of

Misplacement of a vena cava filter into the spinal canal.

We report the case of a 70-year-old male with a complication of misplacement of a vena cava filter into the spinal canal. This likely happened as a result of penetration of the wire and filter sheath through the iliac vein or vena cava into the retroperitoneum, vertebral foramina, and spinal canal at the level of L2 and L3. Due to the patient's condition, the filter was not removed and no neurologic symptoms have occurred. This represents the first reported case of a filter deployment into the spinal canal. Although placement of vena cava filters is a relatively safe procedure, complications a

The effect of translation of the C1-C2 on the spinal canal.

The stability of the atlantoaxial articulation depends fundamentally on the integrity of the odontoid process and the ligaments. Ligament stability mostly is maintained by two ligaments: the transverse ligament and the alar, apical ligaments. Failure of the transverse ligament can result in anterior translation of the atlas on the axis. The anteroposterior diameter of the ring of the atlas is approximately 3 cm. The spinal cord and the odontoid process are each approximately 1 cm in diameter, approximately 1/3 the diameter of the ring. According to Steel's rule of thirds, the remaining centime

Restoration of the spinal canal by the internal fixator and remodeling.

It has been observed that after reduction of an unstable vertebral fracture, treated either conservatively or operatively, the encroachment of the spinal canal remaining after reduction can decrease considerably with time. A series of 31 cases, each with a set of CT scans taken preoperatively, immediately after surgery and after implant removal respectively, was assessed using computer-aided planimetry. The mean initial narrowing of the spinal canal was 44.6%. Through surgery this was decreased by 20.4%, leaving a remaining deficit of 24.2%. After implant removal, 15 months after trauma and su
