Sarcomatosis of leptomeninges, brain and spinal cord coexisting with von Recklinghausen's neurofibromatosis. A case report and review of the literature.

A case of 56-year-old male with sarcomatosis of leptomeninges as well as of the brain and spinal cord coexisting with Recklinghausen's neurofibromatosis is presented. Neurological and neurophysiological symptoms of the disease resembled those of amyotrophic lateral sclerosis (ALS). Patient died 7 months after onset of the initial symptoms. The post-mortem examination revealed neoplastic infiltration of the leptomeninges of brain and spinal cord. Histologically sarcomatosis of the leptomeninges was diagnosed and immunohistochemical analysis of the neoplastic infiltrates can indicate fibrohistio

[Preoperative devascularization of a vertebral metastasis with a spinal artery at the same level].

An hypervascular and lytic thoracic (Th10) vertebral metastasis from kidney neoplasm causing cord compression was successfully resected after embolization. Because the anterior spinal artery arose at the same level, arterial embolization was only partial and completed by percutaneous vertebroplasty using Nbutyl cyanoacrylate.

Radiological insights into pachymeningitis imaging diagnosis: A state-of-the-art review.

Hypertrophic pachymeningitis, characterized by inflammation and thickening of the dura mater in the skull or spinal cord, poses diagnostic and management challenges. This review provides a comprehensive exploration of the condition, emphasizing the importance of a multidisciplinary approach involving clinical and radiological assessments. Clinical manifestations vary based on lesion location, with cranial involvement typically causing headaches and cranial nerve issues, while spinal cases present with back pain, radicular symptoms, and spinal cord compression. MRI emerges as the gold standard

Near-miss Thoracic Spine Solitary Plasmacytoma with Neurological Deficit during Pregnancy: A Case Report.

Solitary plasmacytoma (SPC) account for only 5% of plasma cell neoplasms, and the literature hardly reports spinal SPC with a neurological deficit. Furthermore, spinal surgical intervention during pregnancy is rarely encountered and often requires multidisciplinary collaboration and management. The objective of this case report is to highlight this near-miss diagnosis and spinal surgical intervention during pregnancy. A 31-year-old woman with 24 weeks gestation presented with sudden paralysis and incontinence, with an underlying history of chronic backpain over a two-month period. Initially, s

Diseases of the spinal canal in children: diagnosis with noncontrast CT scans.

The spinal cord and its coverings were assessed in 131 CT studies performed without intrathecal contrast material in 108 patients ages newborn to 20 years old. The spinal cord, including the conus medullaris, and the theca can be readily identified in most patients, except in the upper thoracic region. Correlation with other radiologic studies and surgery showed few false-positive and false-negative results. Of particular importance, in no patient was unnecessary surgery performed nor was necessary surgery not performed as a result of the method. The technique was a useful screening test, ofte

Giant cell tumor of the thoracic spine: An unusual cause for spinal cord compression.

Giant cell tumors (GCTs) of bone are uncommon neoplasms, typically located in the metaphysis of long bones, with rare occurrences in the spine, especially in the thoracic region. We report the case of a 34-year-old woman with a history of psoriasis and celiac disease, who presented with progressive inflammatory back pain and paraparesis. Imaging revealed an osteolytic mass at the T11 vertebra, causing dorsal spinal cord compression. Emergency surgery was performed, with histopathology confirming GCT. Despite initial recovery of motor function, surgical stabilization was later necessary to prev

Neoplasms of the Cervical Spine: Diagnostic Workup and Clinical Decision-making Algorithms.

STUDY DESIGN: Narrative review. OBJECTIVE: To synthesize current evidence on diagnostics and clinical decision-making for patients with cervical spine neoplasms. BACKGROUND: Although the cervical spine is disproportionately affected by primary spinal tumors and is the predominant site for intramedullary spinal cord tumors, there remains a relative paucity of reviews focused specifically on cervical spine neoplasms. METHODS: PubMed and Scopus were searched using a combination of terms related to cervical spine neoplasms, classification schemas, diagnostic evaluation, staging, and clinical de

Neurosurgical aspects of the treatment of neuroblastoma patients.

Neuroblastoma (NB) is the most common extracranial solid tumor in children. The neoplasm grows from progenitor cells of the sympathetic nervous system and can be detected anywhere along the sympathetic neurological circuit: retroperitoneally, mediastinally, cervically, and pelvically. Examination of children with suspected neuroblastoma is comprehensive and performed in strict compliance with a therapeutic protocol. A decision on the treatment regimen is made based on the tumor staging and the risk group of the patient. The diagnosis and treatment of NB patients are comprehensive and can be fu

MRI manifestations of leptomeningeal metastasis.

Twenty-six cases with leptomeningeal metastasis were analyzed and Gd-DTPA enhanced MR imaging was found to be the most useful tool for making the diagnosis. The important manifestations in order of frequency were: 1) leptomeningeal enhancement; 2) ependymal enhancement; 3) plaque, nodule or mass in the subarachnoid space or ventricles; 4) nerve root thickening; and 5) ventricular dilatation not related to the obstruction of the primary neoplasm. A review of the literature has shown that intracranial leptomeningeal metastasis from spinal cord astrocytoma are very rare. In our series, 3 cases wi

Squamous cell carcinoma occurring within incision of recently implanted spinal cord stimulator.

Spinal Cord Stimulation (SCS) is a treatment option for chronic pain patients. Spinal cord stimulation has been employed in the treatment of chronic pain for more than 30 years. The most common indication for SCS is the failed back syndrome with leg pain. Its indications have expanded beyond back and lower extremities pain to include axial low back pain, CRPS, mesenteric ischemia, peripheral neuropathy, limb ischemia, and refractory angina pectoris. The SCS has become a more versatile form of analgesia. The number of wound complications will surely rise in conjunction with the increasing numbe

Spinal low-grade neoplasms with extensive leptomeningeal dissemination in children.

OBJECTIVE: The aim of this study is to re-appraise paediatric leptomeningeal disseminated spinal low-grade neoplasms putting forward the hypothesis that these tumours may represent a possible distinctive clinico-pathological entity. CASE REPORT: Three children affected by these rare neoplasms are reported, 2 boys and 1 girl aged 40 months, 7 and 12 years respectively. RESULTS: Despite treatment, 2 died at 9 months and 9 years, while 1 has survived with stable disease at 24 months after diagnosis. Histologically, these tumours shared the generic histological appearance of low-grade neuroectod

Multifocal Polymorphous Hemangioendothelioma of the Liver: Case Report and Review of Literature.

We report a case of polymorphous hemangioendothelioma, a rare vascular neoplasm, arising in the liver of a 51-year-old woman. Microscopically, this neoplasm is characterized by a polymorphous blend of solid, primitive vascular and angiomatous areas in variable proportions. Polymorphous hemangioendothelioma is considered to be an intermediate-grade vascular neoplasm. In most of the reported cases, its occurrence has been in lymph nodes, but a few cases have been found in extranodal sites such as the mediastinum, spinal cord, and liver. Its classification among other soft tissue neoplasms is sti

[A case of spinal dural arteriovenous malformation associated with the broad lesion within spinal cord--diagnostic value of MRI with Gd-DTPA enhancement].

A 70-year-old man, with slowly progressive gait disturbance, numbness in his lower limbs and dysuria, was admitted to our hospital in July, 1988. Neurological examination revealed flaccid paraparesis, hyperesthesia below the level of Th-10 combined with the disturbance of pain and temperature sensations below the level of Th-11, and neurogenic bladder. Midsagittal Tl-weighed image of MRI did not reveal any abnormal finding except for a mild enlargement of the conus medullaris. With Gadolinium (Gd)-DTPA injection, however, the spinal cord was diffusely enhanced below the level of the 9th thorac

Anterior thoracic surgical approaches in the treatment of spinal infections and neoplasms.

BACKGROUND: Thoracic surgeons are commonly consulted to provide anterior thoracic exposure for infection and malignant neoplasms involving the thoracolumbar spine. These cases can present significant technical and management challenges secondary to the underlying pathology, associated anatomic inflammation, and impaired functional status. In this study, we review the perioperative outcomes in patients undergoing anterior spinal exposure for infection and neoplasm. METHODS: 130 consecutive patients (61 women, 69 men) undergoing corpectomy, debridement, or debulking for osteomyelitis (n=50) or

Neoplasms of the central nervous system of lipoid origin.

In general tumours of lipoid origin are benign. A high proportion of cases are mesenchymal neoplasms localised in subcutaneous tissue. Neurological signs related to compression of neural structures by fat within the vertebral canal were first presented in 1975. The lipogenic neoplasms represent 1-4% of spinal tumours, and generally are associated with congenital disorders of the spine. These fatty swellings are usually benign, but penetrating deeply into the spine, and connecting with spinal cord, cauda equina or filium terminale, they produce deficits. The deficits may be present at birth but

Spinal intradural extramedullary mature cystic teratoma in an adult: A rare tumor with review of literature.

BACKGROUND: Spinal intradural extramedullary teratoma is a rare condition, more common in children than in adults often with a history of spinal dysraphism. METHOD: We reviewed the literature and Pubmed advanced search showed nine results of intradural extramedullary teratoma in adults which included five independent case reports and three cases in a case series. We reported a rare case of intradural extramedullary teratoma in an adult located opposite L4 vertebra with neither spinal dysraphism nor any history of the prior spinal procedure. RESULTS: The occurrence of teratomas in the spine i
