In this paper, we describe the possibility of using a bone-mounted miniature robot based on the experience of different surgeries performed in 77 patients divided into four groups according to the general pathology (degenerative stenosis of the vertebral canal, fractures of vertebral bodies, spondylolisthesis, hemangiomas, and tumors). All the patients underwent surgical intervention using Spine Assist Mazor, such as stabilization using the Go-Lif system, transpedicular systems, vertebroplasty, and vertebral body biopsy. The new method and the technology of stabilizing surgeries using Spine As

Fractures of the thoracic (Th) and lumbar (L) vertebrae are among the most frequent fracture entities in Germany and particularly affect the thoracolumbar junction (TLJ; Th11-L2). Based on expert recommendations and consensus meetings, the thoracolumbar AOSpine injury score was established for patients with healthy bone and the osteoporotic fracture (OF) score for geriatric patients with the respective classifications for treatment decisions. In both cohorts, the treatment decision is based on the fracture morphology, neurological status and patient-specific contextual factors. In terms of fra

Charcot spinal arthropathy, a rare refractory progressive disease, is characterized by symptoms such as pain, deformity, and neurological impairment, which can significantly reduce functional ability, quality of life, and life expectancy. We report a case of Charcot spine at the L5/S1 level with long segment ankylosis to the L5 vertebra. We first performed thorough debridement via a posterior approach. We used antibiotic-containing cement as a spacer to fill the dead space, facilitating the second surgery approach. In the second surgery, transdiscal screws, which have a low profile and strong

Synovial chondromatosis (SC) is a rare, benign disease. It usually occurs in large joints such as the hip and knee. Few cases have been reported in the spine, especially in the lumbar spine. It is characterized by the presence of clusters of chondrocytes within the joints and free in the joint cavity. The main symptom is pain. Diagnosis requires a high level of suspicion, and malignancy must always be ruled out. Surgical management is a challenge. We present the case of a patient with an extensive spinal tumor that was managed under the principles of minimal spinal invasion. The exeresis was p

BACKGROUND: Degenerative processes of the lumbar spine may change the position of the sympathetic trunk which might cause failure of sympathetic blocks owing to inadequate distribution of local anaesthetic.
METHODS: The retroperitoneal spaces of 56 cadavers [24 males and 32 females; 79 (10) yr] embalmed with Thiel's method were investigated by dissection. The course of the lumbar sympathetic trunk (LST) was documented from the diaphragmatic level to the linea terminalis. Topography of the large vessels and the psoas muscle was documented. In the case of spondylophytes, the location or directi

A nerve root impingement within a stenotic neuroforamen is a common sequela of cervical degenerative arthritis and herniated nucleus pulposus. Understanding of the effects of cervical position on foraminal size is important in the assessment of pathology and injury, for selection of a provocative maneuver to elicit symptoms and in selecting a position of immobilization for the management of nerve root impingement syndrome. This biomechanical study of human cadaver cervical spines reports the measured variations in the sizes of neuroforamina as a function of cervical positioning. Five fresh fro

Cervical spines (with occiputs) of 44 patients with rheumatoid arthritis (RA) were compared with 44 control spines from patients without RA, matched for sex and age. There were six severe and seven slight atlanto-axial dislocations (both horizontal and vertical, two of which had been treated surgically) in the RA group and none in the control group. Ankylosis, synchondrosis, syndesmosis, bone erosions, and redness of synovial tissue in the atlanto-axial, atlanto-occipital and cervical intervertebral joints were more frequent in the RA group than in the control spines. STUDY DESIGN: Systematic review and proportional meta-analysis.
OBJECTIVE: To assess total and specific complication rates associated with lumbar biportal endoscopic spine surgery (BESS).
SUMMARY OF BACKGROUND DATA: In recent years, BESS has emerged as an effective minimally invasive technique for treating lumbar spine conditions, offering benefits such as reduced tissue damage and improved outcomes. However, the safety of BESS across lumbar pathologies is underexplored, with complication rates reported up to 50%.
METHODS: We registered on PROSPERO (CRD42024570377) and systematically search

PURPOSE: Spinal disorders, including back and neck pain, are major causes of disability, economic hardship, and morbidity, especially in underserved communities and low- and middle-income countries. Currently, there is no model of care to address this issue. This paper provides an overview of the papers from the Global Spine Care Initiative (GSCI), which was convened to develop an evidence-based, practical, and sustainable, spinal healthcare model for communities around the world with various levels of resources.
METHODS: Leading spine clinicians and scientists around the world were invited t

Complications from spine surgery are not just devastating because they happen, or because we believe that complications can be avoided, but are magnified because many of the complications occur in elective cases. Catastrophic outcomes in surgeries that are intended to improve lifestyle and comfort do not fit our sensibility. In the first Neurosurgical Focus issue on complications published in October 2024, most of the complications shared involved cranial neurosurgery. Many of these complications occurred in cases in which there was no acceptable option except to operate, such as cases involvi

INTRODUCTION: Prolonged exposure to microgravity alters spinal biomechanics, increases disc herniation risk, and complicates perioperative care. With commercial and deep-space missions on the horizon, the need for in-orbit surgical capability has gained prominence as a safeguard for astronaut health.
METHODS: A narrative review of PubMed, EMBASE, and Google Scholar through November 2024 identified human, animal, and in vitro studies addressing spinal physiology, pathology, or surgical feasibility in actual or simulated microgravity. Studies unrelated to the spine or lacking English full text

Achieving fusion in spine surgery can be challenging because of patient factors such as smoking and diabetes. The consequences of pseudarthrosis can be severe, including pain, instability, and additional surgery. Autologous iliac crest bone graft is the historical standard for augmenting spine fusion, providing high rates of fusion throughout the cervical, thoracic, and lumbar spine. Harvest of autologous iliac crest bone can be associated with comorbidities and this has led to development of alternative biologic materials to enhance spine fusion. Substitutes include various forms of allograft

History Part one of this case appeared 4 months previously and may contain larger images. A 32-year-old woman presented to an ophthalmologist for bilateral blurry vision. She underwent MRI of the brain and orbits, which showed a focal abnormality within the pituitary gland. The patient was referred to an endocrinologist for further evaluation. Review of systems and physical examination by the endocrinologist revealed no symptoms or signs of endocrine dysfunction. Anterior pituitary hormone levels, including growth hormone, prolactin, thyroid stimulating hormone, follicular-stimulating hormone,

Soccer is the most popular sport in the world, with more than 270 million participants. It is characterized by repetition of short and intense actions that require high-coordination capacity. It is a sport where interactions with other players put the athletes at risk for traumatic injuries. Lower-limb injuries are the most prevalent injuries in soccer. Spine injuries are less frequent; however, they can impose serious and debilitating sequelae on the athlete. These injuries can be associated with long recovery periods preventing return to play. Moreover, specific repetitive activities (headin

PURPOSE: Mucopolysaccharidosis (MPS) are rare inherited metabolic diseases, causing lysosomal storage of mucopolysaccharides; clinical presentation involves skeletal system and particularly the spine. Anomalies include developing kyphosis at thoracolumbar junction, that can causes nervous symptoms, and dens hypoplasia with associated atlantoaxial subluxation that can cause myelopathy. We present our experience in the treatment of spine pathology in MPS.
METHODS: Medical treatments of MPS seem to have little impact on spine disease: treatment of cervical instability often includes surgical dec

BACKGROUND: The cervical spine is one of the main sites of manifestation in rheumatoid arthritis outside of the extremities. It can have a decisive influence on disease course via the occurrence of mechanical instabilities as well as neurologic symptoms. Both adequate diagnosis and the corresponding surgical treatment represent a challenge for the involved physicians.
MATERIALS AND METHODS: This review presents relevant diagnostic strategies and possibilities for surgical intervention which aim to avoid potentially fatal neurologic symptoms. Basic literature and expert opinions are also discu

Demographic trends make it incumbent on orthopaedic spine surgeons to recognize the special challenges involved in caring for older patients with spine pathology. Unique pathologies, such as osteoporosis and degenerative deformities, must be recognized and dealt with. Recent treatment options and recommendations for the medical optimization of bone health include vitamin D and calcium supplementation, diphosphonates, and teriparatide. Optimizing spinal fixation in elderly patients with osteoporosis is critical; cement augmentation of pedicle screws is promising. In the management of geriatric
