Local control of mesenteric blood flow by the renin-angiotensin system.

The purpose of this study is to determine whether locally acting angiotensin II (ANG II) plays a direct role in the control of mesenteric blood flow after volume depletion in the anesthetized dog. Infusion of the ANG II receptor antagonist saralasin into the mesenteric artery at doses between 0.05 and 0.1 microgram.kg-1.min-1 attenuated the reduction in renal blood flow produced by intrarenal injection of ANG II. In contrast, infusion of saralasin at 0.01 microgram.kg-1.min-1 did not affect the change in renal blood flow produced by ANG II, indicating that at this dosage the antagonist did not

Etiology and Risk Factors for Splanchnic Vein Thrombosis in Non-Cirrhotic, Non-Neoplastic Patients: A Narrative Review.

Splanchnic vein thrombosis (SVT) is a heterogeneous group of disorders affecting the portal, mesenteric, splenic, and hepatic veins. While frequently associated with liver cirrhosis and malignancy, SVT also occurs in non-cirrhotic, non-neoplastic patients. This narrative review evaluates the epidemiology and risk factors for SVT in this population. The prevalence and incidence of SVT in non-cirrhotic, non-neoplastic patients remain incompletely characterized, with estimates varying widely across studies. The clinical significance of SVT relates to potential complications, including intestinal

Clinical manifestations and imaging tools in the diagnosis of splanchnic and cerebral vein thromboses.

Splanchnic vein thrombosis (SVT) and cerebral vein thrombosis (CVT) are uncommon manifestation of venous thromboembolism (VTE), occurring less frequently than deep vein thrombosis of the lower extremities and pulmonary embolism. SVT encompasses portal vein thrombosis, mesenteric vein thrombosis, splenic vein thrombosis and the Budd-Chiari syndrome. It is therefore a heterogeneous disease, with differences in clinical manifestations according to the site of thrombosis. CVT includes thrombosis of the cortical or deep cerebral veins and thrombosis of the major dural venous sinuses. Clinical prese

Noninvasive imaging of the abdominal vessels.

Color Doppler ultrasound, computed tomography (CT), multi detector CT, CT-angiography, magnetic resonance imaging (MRI), MR-angiography are playing an increasingly important role for noninvasive examinations of the abdominal vessels. The most frequently studied abdominal vessels are the aorta and the renal arteries. The most common abdominal vascular diseases include: renal artery stenosis, renovascular hypertension, ischemic nephropathy and end-stage renal disease. Scintigraphy, color Doppler US, CT-angiography and MR-angiography have been used for the evaluation of patients. The mesenteric a

Ammonia absorption from the intestine in adult cockerels.

Two ml of w mM ammonium acetate solution was introduced into 10 cm chicken intestinal sack having Meckel's diverticulum in the middle part. After the introduction, blood ammonia concentration in the mesenteric vein draining the sack reached a maximal increase within 5 min and returned to the initial level in 30 min. The difference of ammonia concentration between cardiac and mesenteric venous blood also showed a similar tendency. About 91% of the ammonia introduced into the intestinal lumen disappeared in 30 min. Amino acid metabolism in intestinal tissue appears to be affected by the introduc

[Ways to improve the diagnosis and treatment of mechanical intestinal obstruction of a nontumorous nature].

The authors analyze the results of surgical treatment of 629 patients with acute ileus. In order to increase the diagnostic information value of the radiopaque dynamic investigation the authors propose to administer the liquid barium suspension through the probe introduced through the nose into the initial portion of the jejunum. It has been established that the level of content of the alkaline phosphatase intestinal fraction is an indirect index of the mesenterial blood circulation state. The experience of employment of visual angiometry for the assessment of vital capacity of the intestine a

The carotid body detects circulating tumor necrosis factor-alpha to activate a sympathetic anti-inflammatory reflex.

Recent evidence has suggested that the carotid bodies might act as immunological sensors, detecting pro-inflammatory mediators and signalling to the central nervous system, which, in turn, orchestrates autonomic responses. Here, we confirmed that the TNF-α receptor type I is expressed in the carotid bodies of rats. The systemic administration of TNF-α increased carotid body afferent discharge and activated glutamatergic neurons in the nucleus tractus solitarius (NTS) that project to the rostral ventrolateral medulla (RVLM), where many pre-sympathetic neurons reside. The activation of these neu

Splanchnic venous thrombosis: aetiologies and a review of the literature.

BACKGROUND: Splanchnic vein thrombosis (SVT) is an uncommon yet potentially life-threatening manifestation of venous thromboembolism. The aim of this study was to present a retrospective analysis of a cohort of Western Australian patients diagnosed with SVT on imaging study, and a review of the literature surrounding the aetiology, location, anticoagulation treatment and outcomes of SVT. METHODS: All patients diagnosed with SVT over a five-year period from 2015 to 2020 in three tertiary hospitals in Western Australia were identified by using an electronic search engine of imaging reports. [Splanchnic vein thrombosis].

Splanchnic vein thromboses include thrombosis of the portal venous system (including the portal, mesenteric and splenic vein) and hepatic vein thrombosis (also called Budd-Chiari syndrome). They are rare manifestations of venous thromboembolism. These thromboses are frequently associated with local or systemic factors. The therapeutic approach is often complex due to heterogeneity of patients and limited available data in the literature. The cornerstone of treatment is anticoagulation. However, the bleeding risk, related to portal hypertension, should be accurately assessed to individualize th

Chronic mesenteric ischemia.

Chronic mesenteric ischemia is an unusual but important cause of abdominal pain. Although this condition accounts for only 5% of all intestinal ischemic events, it can have significant clinical consequences. Among its many causes, atherosclerotic occlusion or severe stenosis is the most common. This disorder has an indolent course that results in extensive collateral vascular formation. Thus, symptoms occur when at least two of the three main splanchnic vessels are affected. Intestinal angina, weight loss, and sitophobia are common clinical features. Diagnosis can often be made by noninvasive

Intestinal ischemia.

Ischemic injury to the gastrointestinal tract can threaten bowel viability with potential catastrophic consequences, including intestinal necrosis and gangrene. The presenting symptoms and signs are relatively nonspecific and diagnosis requires a high index of clinical suspicion. AMI often results from an embolus or thrombus within the SMA, although a low-flow state through an area of profound atherosclerosis may also induce severe ischemia. Because most laboratory and radiologic studies are nonspecific in early ischemia an aggressive approach to diagnosis with imaging of the splanchnic vascul

Simultaneous mesenteric and renal bypass for visceral ischemia and renovascular hypertension.

Severe visceral ischemia and uncontrollable hypertension in a patient with previous aortofemoral and right aortorenal bypasses underwent surgical correction for stenosis of the celiac, superior mesenteric and left renal arteries. A Dacron jump graft from the junction of the aorto-celiac trunk to the superior mesenteric artery artery alleviated the mesenteric ischemia. A nonreversed vein graft connected the side of the Dacron prosthesis to the end of the left renal artery improved the renovascular hypertension. This modified, simple surgical approach, successfully corrected visceral ischemia an

Effect of indomethacin on mesenteric circulation in mongrel dogs.

Necrotizing enterocolitis has been attributed to the use of indomethacin (INDO) for medical closure of patent ductus arteriosus. To study the effect of INDO on cardiac output and mesenteric circulation, INDO was given by rectum (0.25 mg/kg, 0.5 mg/kg, 1.25 mg/kg--3 dogs in each group) and the control group received none. The cardiac output and organ blood flow were measured before and 1 hr after INDO with radioactive microspheres using 4 isotopes (Cr53, Ni95, Co57, Sn113). The blood flow to different parts of the GI tract was measured as percent of cardiac output using a gamma counter. Paired

Intestinal blood flow by Doppler ultrasound: the impact of clarithromycin treatment for feeding intolerance in preterm neonates.

OBJECTIVE: To compare the blood flow velocities of superior mesenteric artery (SMA) before versus after clarithromycin treatment for feeding intolerance in very low-birth weight infants. METHODS: A prospective study was conducted in a group of infant <1500 g with feeding intolerance who received clarithromycin 7.5 mg/kg/dose bid. Before and at the third day of the clarithromycin therapy, SMA blood flow velocity was measured with Doppler ultrasound. RESULTS: SMA peak systolic velocity (PSV) and mean systolic velocity (MV) on the third day of the treatment was found significantly higher than t

Colonoscopy-related colonic ischemia.

Colonoscopy is a risk factor for colon ischemia. The colon is susceptible to ischemia due to its minor blood flow compared to other abdominal organs; the etiology of colon ischemia after colonoscopy is multifactorial. The causative mechanisms include splanchnic circulation impairment, bowel preparation, drugs used for sedation, bowel wall ischemia due to insufflation/barotrauma, and introduction of the endoscope. Gastroenterologists must be aware of this condition and its risk factors for risk minimization, early diagnosis, and proper treatment.

Splanchnic vein thrombosis in myeloproliferative neoplasms.

Splanchnic vein thrombosis (SVT) is one of the most important complications of myeloproliferative neoplasms (MPN). Although MPN are common causes of SVT, the pathophysiological mechanisms underlying this predisposition, their epidemiology and natural history are not fully understood. Studies have concentrated on the generalized prothrombotic environment generated by MPN and their relationship with abnormal blood counts, thereby furthering our knowledge of arterial and venous thrombosis in this population. In contrast, there are few studies that have specifically addressed SVT in the context of

Splanchnic and systemic vasodilatation: the patient.

Arterial vasodilatation is one of the most important characteristics of cirrrhosis and portal hyptertension. Nowadays, it has been known that progressive vasodilatation is an essential factor contributing to hyperdynamic circulation and multiple organ dysfunction in liver cirrhosis. Over the past decades, numerous investigations have originated from the clinical observations. Clinicians and investigators have learned and applied new concepts of the pathophysiology of portal hypertension. For example, we now have effective pharmacologic treatment for hepatorenal syndrome. This review summarizes

Dose-dependent effects of a nitric oxide biosynthesis inhibitor on hyperdynamic circulation in two models of portal hypertension in conscious rats.

The role of nitric oxide (NO) in the hyperkinetic circulation in portal hypertension has not been clearly elucidated. Different doses of NO inhibitors, haemodynamic values and experimental conditions might explain the discrepant results. The aim of the present study was to investigate the acute effects of a specific biosynthesis inhibitor of NO, Nomega-nitro-L-arginine (L-NNA), on the systemic and splanchnic circulation in normal conscious rats and rats with portal hypertension due to either partial portal vein stenosis or secondary biliary cirrhosis. The administration of L-NNA (15 to 960 mic
