Surgical anatomy of the retroperitoneal spaces, Part IV: retroperitoneal nerves.

We present surgicoanatomical topographic relations of nerves and plexuses in the retroperitoneal space: 1) six named parietal nerves, branches of the lumbar plexus: iliohypogastric, ilioinguinal, genitofemoral, lateral femoral cutaneous, obturator, femoral. 2) The sacral plexus is formed by the lumbosacral trunk, ventral rami of S1-S3, and part of S4; the remainder of S4 joining the coccygeal plexus. From this plexus originate the superior gluteal nerve, which passes backward through the greater sciatic foramen above the piriformis muscle; the inferior gluteal nerve also courses through the gr

Retroperitoneal paraganglia and the peripheral autonomic nervous system in the human fetus.

By means of the AChE in toto staining method retroperitoneal paraganglia and the peripheral autonomic nervous system in human fetuses have been investigated. Many small retroperitoneal paraganglia are present near the sympathetic trunks close to the sympathetic trunk ganglia. In the thoracic region small paraganglia are present in the intercostal spaces. Small splanchnic nerves entering small paraganglia have been described. In the lower sacral region no paraganglia are present. The major splanchnic nerve arises at various levels from the sympathetic trunks as well as many smaller thoracic spl

The autonomic innervation of the large intestine of the toad (Bufo marinus).

1. A study was made of the pelvic and the splanchnic nerve supplies to the toad large intestine. 2. Stimulation of pelvic nerve fibres in the 9th and 10th spinal nerves caused a series of contractions of the circular muscle, only the first of which was abolished by hyoscine. The entire response was blocked by d-tubocurarine. The response was not affected by capsaicin treatment. 3. Stimulation of the splanchnic nerves caused a rapid contraction followed by a prolonged relaxation. The relaxation was abolished by bretylium. The contraction was selectively antagonised by prolonged exposure to caps

[A comparative study of the laparoscopic appearance and anatomy of the autonomic nervous in normal males].

OBJECTIVE: To further understand the anatomical basis of pelvic autonomic nerve preservation. METHODS: Autopsy of five adult male donated cadavers was performed. Meanwhile, ten videos of laparoscopic total mesorectal excision for male mid-low rectal cancer admitted from January to June 2012 were observed and studied. Anatomical features of pelvic autonomic nerve were compared between autopsy and laparoscopic appearance. RESULTS: Autopsy observations indicated that:the abdominal aortic plexus was situated upon the sides and front of the aorta, between the origins of the superior and inferior

Thoracic part of sympathetic chain and its branching pattern variations in South Indian cadavers.

INTRODUCTION: The sympathetic trunks are two ganglionated nerve trunks that extend the whole length of the vertebral column. The two trunks end by joining together to form a single ganglion, the ganglion impar. The thoracic part of the sympathetic chain runs downward and leaves the thorax behind the medial arcuate ligament. The preganglionic fibers which are grouped together to forms planchnic nerves and supply the abdominal viscera. Anatomical variations of the thoracic sympathetic trunk in relation to intercostal nerves may be one of the reasons that cause surgical failures. Therefore, our pr

Sympathetic nerves control bacterial clearance.

A neural reflex mediated by the splanchnic sympathetic nerves regulates systemic inflammation in negative feedback fashion, but its consequences for host responses to live infection are unknown. To test this, conscious instrumented sheep were infected intravenously with live E. coli bacteria and followed for 48 h. A month previously, animals had undergone either bilateral splanchnic nerve section or a sham operation. As established for rodents, sheep with cut splanchnic nerves mounted a stronger systemic inflammatory response: higher blood levels of tumor necrosis factor alpha and interleukin-

[Anatomo-surgical notes on splanchnicectomy: original research on 15 autopsy observations].

Surgical splanchnicectomy for the relief of neoplastic pain is a palliative strategy in cases of unremovable pancreatic cancer. The first step in the achievement of satisfactory and long-lasting relief of pain is the correct identification of semilunar ganglia and splanchnic nerves during laparotomy. In this light, we tried to estimate the exact location, number, shape, and length of splanchnic nerves and ganglia in 15 corpses (mean age 39.9 years, range 21-74, F/M/ = 5/10). Right and left splanchnic nerves always pierce the diaphragm laterally to the crus. On the right side, the splanchnic ne

Reflex control of inflammation by the splanchnic anti-inflammatory pathway is sustained and independent of anesthesia.

Following an immune challenge, there is two-way communication between the nervous and immune systems. It is proposed that a neural reflex--the inflammatory reflex--regulates the plasma levels of the key proinflammatory cytokine TNF-α, and that its efferent pathway is in the splanchnic sympathetic nerves. The evidence for this reflex is based on experiments on anesthetized animals, but anesthesia itself suppresses inflammation, confounding interpretation. Here, we show that previous section of the splanchnic nerves strongly enhances the levels of plasma TNF-α in conscious rats 90 min after they

Evidence for a non-classical splanchnic neural control of the feline pylorus.

Electrical stimulation of the divided splanchnic nerves proximal to the celiac ganglion in chloralosed cats induced a dilation of the pylorus in the majority of animals. However, in some cats a pure contractile motor response or a biphasic motor response (contraction followed by dilation) was elicited suggesting that the splanchnic nerves convey excitatory fibres to the pylorus as well. The pyloric and gastric contraction at such stimulation was sensitive to atropine, while the pyloric dilation was not sensitive to cholinergic, adrenergic or ganglionic blocking agents in contrast to the adrene

Electro-acupuncture stimulation effects on duodenal motility in anesthetized rats.

The effect of electro-acupuncture stimulation (EAS) on duodenal motility was examined in anesthetized, artificially ventilated rats. EAS was applied to the abdominal area or to a hindpaw for 30 s at stimulus intensities of 0.1-10.0 mA with a stimulus frequency of 20 Hz. The duodenal motility was measured using the balloon method at a position about 1.5 cm caudal from the pylorus. Duodenal motility was inhibited by EAS at intensities of more than 5.0 mA (suprathreshold of group IV afferent excitation) when applied to the abdominal area. The duodenal inhibitory response existed after bilateral v

[Hypoxemia, pleural and pericardial effusion after splanchnic neurolysis. A case report.].

INTRODUCTION: Alcohol neurolysis of splanchnic nerves is a valuable tool for treating visceral intractable pain from the upper abdomen in cancer patients. It is a safe and effective procedure, yet not risk free. It’s most common adverse effects are mild and self-limited. MATERIALS: We present a case of a 72-year old woman suffering from pancreatic cancer with intractable pain despite opioid use. Alcohol neurolysis of splanchnic nerves was indicated after hospital admission. RESULTS: After the procedure pain was subdued, yet hypoxemia, pleural and pericardial effusion developed. The extrinsic innervation of the rat kidney.

The anatomy of the extrinsic renal nerves is described making use of an in toto staining procedure for acetylcholinesterase activity. Nervous connections between the kidney and the celiac plexus, major and minor splanchnic nerves, the lumbar splanchnic nerves and the intermesenteric nerve plexus have been established. Bundles of nerve fibers enter or leave the kidney in the (peri-) hilar region. The occurrence of inter- and intraindividual variability is emphasized. Implications of these findings for the use of the rat as an experimental animal in denervation experiments are discussed.

Ipsilateral seminal emission generated by electrostimulation of the lumbar sympathetic nerve during nerve sparing laparoscopic retroperitoneal lymph node dissection for testicular cancer.

PURPOSE: To confirm nerve preservation during laparoscopic retroperitoneal lymph node dissection we perform electrostimulation of the lumbar sympathetic nerves relevant to ejaculation. In recent cases we monitored seminal emission using endoscopy of the posterior urethra to observe the response to stimulation. MATERIALS AND METHODS: Six patients with testicular tumors underwent intraoperative stimulation of the lumbar sympathetic nerves during laparoscopic retroperitoneal lymph node dissection with a unilateral modified template. A long handled pair of bipolar electrodes was inserted through

Nerve-preserving aortoiliac reconstruction surgery: anatomical study and surgical approach.

OBJECTIVE: Dysfunctional ejaculation and, to a lesser extent, dysfunctional erection caused by disruption of efferent sympathetic pathways is a common complication after aortoiliac reconstruction surgery. The aim was to give an anatomic motivation for a nerve-preserving approach on the basis of right-sided unilateral disruption of lumbar splanchnic nerves. METHODS: Anatomic and microscopic analysis of preaortic and para-aortic retroperitoneal regions in human cadavers was performed. Anatomic analysis was conducted of two aortoiliac reconstruction operations performed on human cadavers; one wa

Laparoscopic fundoplication and new aspects of neural anatomy at the oesophagogastric junction.

BACKGROUND: In fundoplication, mobilization of the distal oesophagus and proximal stomach is essential to obtain a sufficient tension-free intra-abdominal oesophageal length for creation of an efficient antireflux barrier. Most surgical literature and anatomical illustrations do not describe nerve branches running from the diaphragm to the stomach. After observing small nerve branches at laparoscopic fundoplication, penetrating the left crus of the diaphragm lateral to the hiatus and apparently running into the stomach, an anatomical cadaver study was undertaken to identify the origin and targ

The Lumbar Sympathetic Trunk and Its Branching Variability: Relevance for Clinical and Interventional Strategies.

Precision medicine relies on a thorough understanding of lumbar sympathetic anatomy and its branches to elucidate related pathophysiology and improve treatment of conditions such as low back pain, lumbopelvic pain, and vascular autonomic disorders affecting the lower limbs. This study aims to expand knowledge of fetal lumbar sympathetic anatomy by providing a detailed description and systematic classification of the communicating branches, their specific distribution to each lumbar spinal nerve, and the origin of lumbar splanchnic nerves. The lumbar and retroperitoneal regions of 25 human fetu

Morphometric aspects of peripheral nerves in adults and the elderly.

There have been inconsistencies among reports of age-related differences in human peripheral nerves (PNs). For such studies, normal control values are necessary. Moreover, the diversity of methods employed makes it difficult to compare results. We used the same histological procedures and methods to measure 12 PNs: 8 in the cranial nerves, 2 motor nerves in the lower limb, and 2 nerves in the autonomic system. We performed a morphometric analysis of nerve fibers and estimated the change in the total number (TN) and average transverse area (ATA) of myelinated axons from adulthood to old age. Th

Branches of the thoracic sympathetic trunk in the human fetus.

The segmental organization of the thoracic sympathetic trunk and all its ramifications was studied in 6 human fetuses (16-22 weeks) by means of the acetylcholinesterase in toto staining method. Each trunk was divided into 12 sympathetic segments. A segment is defined as that part of the sympathetic trunk which is connected via its rami communicantes with one spinal nerve, without discriminating between grey and white rami. The diameter of the rami communicantes and their direction towards the spinal nerves are variable. The number of peripheral segmental ramifications of the trunk is much larg

Extrinsic innervation of the pelvic organs in the lesser pelvis of human embryos.

Realistic models to understand the developmental appearance of the pelvic nervous system in mammals are scarce. We visualized the development of the inferior hypogastric plexus and its preganglionic connections in human embryos at 4-8 weeks post-fertilization, using Amira 3D reconstruction and Cinema 4D-remodelling software. We defined the embryonic lesser pelvis as the pelvic area caudal to both umbilical arteries and containing the hindgut. Neural crest cells (NCCs) appeared dorsolateral to the median sacral artery near vertebra S1 at ~5 weeks and had extended to vertebra S5 1 day later. Onc
