Shin-splints: common exercise-related syndromes affecting the lower leg.

Lower leg pain is a common complaint of athletically active individuals, often limiting physical activities. As such, the group of lower leg conditions related to athletic pursuits and physical exercise confer considerable operational implications for the military. Whilst acute injuries to the lower limb are commonly encountered and are clearly of significance, this article focuses instead on chronic conditions related to physical activity. These include insults to bone such as stress fractures and medial tibial stress syndrome, and those related to the soft tissues such as chronic exertional

Temporomandibular joint vibration before and after exercises and occlusal splints.

Temporomandibular joint (TMJ) sounds are frequent in patients. The aim of this study was to analyze patients with clicking at the end of opening and at the beginning of closing their mouths treated by muscular exercises through chewing and by occlusal splints. Fifteen patients with clinically verified clicking and TMJ and 15 patients without sounds were selected by the Research Diagnostic Criteria for Temporomandibular Disorders. They were submitted to electrovibratography at consultation and 60 and 120 days of treatment by occlusal splints and exercises. Patients demonstrated significant redu

[Treatment of Barton fractures with external fixation with paper splints].

OBJECTIVE: To study the control of instability of paper splints for the treatment of Barton fractures. METHODS: From 1998 to 2007, 30 patients with Barton fractures were treated with manual reduction and external fixation with paper splints. Among the patients, 20 patients were male and 10 patients were female, ranging in age from 20 to 68 years, averaged 45.3 years. The course of the disease ranged from 1 to 15 hours, with a mean of 4.2 h. All the patients had a history of trauma. The X-rays showed palm-side distal radius fractures, fractured fragments displacing toward proximal palm and the

Overuse injuries: tendinopathies, stress fractures, compartment syndrome, and shin splints.

Approximately 50% of all sports injuries are secondary to overuse and result from repetitive microtrauma that causes local tissue damage. Injuries are most likely with changes in mode, intensity, or duration of training and can accumulate before symptoms appear. Intrinsic factors contributing to injuries are individual bio-mechanical abnormalities such as malalignments, muscle imbalance, inflexibility, weakness, and instability. Contributing extrinsic (avoidable) factors include poor technique, improper equipment, and improper changes in duration or frequency of activity. Injuries are often re

Femur fracture immobilization with traction splints in multisystem trauma patients.

OBJECTIVE: To evaluate the frequency of concomitant injuries that can complicate and/or contraindicate the use of traction splints (TSs) for femur fracture immobilization (FFI) in a population of multisystem trauma patients. METHODS: This was a descriptive, prospective study utilizing a data collection tool to identify patients with multisystem trauma for which a TS was in place for FFI. Patient care records and follow-up diagnoses were reviewed to identify patients with positive femur fracture(s) who concurrently had injuries that can complicate and/or contraindicate TS use. Injuries conside

Splints and scar management for acute and reconstructive burn care.

Because patients assume a position of comfort during acute burn management, affected joints or regions must be splinted in positions of function to avoid contractures. Particularly with the increasing trend towards ambulatory burn care, close monitoring by the burn team and patient education is required to achieve the best functional result. Despite adequate initial care, contractures sometimes occur, requiring a shift in splinting tactics to correct the deformity. If secondary reconstruction is required, the affected region is once again maintained in the position of function. Although the ph

Transient peroneal nerve palsies from injuries placed in traction splints.

Two patients thought to have distal femur fractures presented to the emergency department (ED) of a level 1 trauma center with traction splints applied to their lower extremities. Both patients had varying degrees of peroneal nerve palsies. Neither patient sustained a fracture, but both had a lateral collateral ligament injury and one an associated anterior cruciate ligament tear. One patient had a sensory and motor block, while the other had loss of sensation on the dorsum of his foot. After removal of the traction splint both regained peroneal nerve function within 6 hours. Experiences with resin-bonded bridges and splints--a retrospective study.

In a joint evaluation of 496 resin-bonded bridges and splints anchored by various retention principles and composites, the initial experiences of seventeen clinicians were compiled. The main indications for treatment were congenital anodontia and loss of teeth due to caries and trauma. After 3 months, 95% of the bridges were still in place without need of reattachment procedure. After 6 months the figure was 91%, after 1 year 81.5% and after 1 1/2 years 73%. Seventy-five per cent of the loosened bridges were attached successfully a second time. The success rate for splints was significantly lo

Management of mandibular fractures using biphasic pins and mandibular splints.

Often the otolaryngologist is asked to manage the patient with facial trauma. Mandibular fractures make up 20 to 39% of all facial trauma cases, and thus necessitate a diversified and complete set of management tools. We have selected two seldom discussed modes of therapy for the complicated jaw fracture to present in this paper: 1. external fixation of Joe-Morrison Hall biphasic pins, and 2. mandibular dental appliances--splints. The advantage of the biphasic pin apparatus is that the fracture can be repaired and stabilized at a point remote from the infected fracture site. The high risk pati

Prophylactic antibiotics in septoplasty with intranasal septal splints: A comparative analysis.

OBJECTIVES: Postoperative antibiotic therapy is a common practice following septoplasty with intra-septal splints placement (ISS), even though there is a lack of evidence to support it. We sought to investigate the role of antibiotic therapy in septal surgeries with the placement of ISS. DESIGN: A retrospective comparative study was conducted using the electronic charts of adult patients who underwent septoplasty with the placement of ISS with or without turbinate reduction. Nasal cultures were taken routinely during surgery as part of the department's protocol for monitoring infectious disea

Ergonomic design of a computer mouse for clients with wrist splints.

OBJECTIVE: We explored effects of cutaneous feedback and hump position on efficiency and comfort in mouse use with a splint. We also analyzed the relationship between anthropometric measurements (width of hand and length of hand, palm, and index) and the task performance. METHOD: Thirty participants performed a computer task with two forms of mice (front hump and rear hump) and two kinds of wrist splints (dorsal and volar). Movement time and satisfaction scores were recorded. RESULTS: No interaction effect (Hump Position × Splint Type) was found on movement time. Movement time was shorter fo

Adductor insertion avulsion syndrome (thigh splints): spectrum of MR imaging features.

OBJECTIVE: "Thigh splints," also known as the adductor insertion avulsion syndrome, is a painful condition affecting the proximal to mid femur at the insertion of the adductor muscles of the thigh. Scintigraphic findings in this syndrome have been described; we report a spectrum of MR imaging abnormalities involving this portion of the femur in a group of patients presenting with hip, groin, or thigh pain. CONCLUSION: Symptoms of vague hip, groin, or thigh pain may be associated with stress-related changes in the proximal to mid femoral shaft (thigh splints). When interpreting MR imaging stud

The soleus syndrome. A cause of medial tibial stress (shin splints).

Radionuclide bone scans have demonstrated linear uptake along the posterior medial border of the tibia in patients with shin splints. This area was investigated by anatomical dissection (14 human cadavers), electromyographic (EMG) and muscle stimulation studies (10 patients), and open biopsy (1 patient). Histologically, the increased metabolic activity manifested on the radionuclide scan is due to a periostitis with new bone formation. The soleus muscle and its investing fascia are anatomically and biomechanically implicated in the production of these stress changes, particularly when the heel

Patient preference for light-cured composite bite splint compared to heat-cured acrylic bite splint.

BACKGROUND: Heat-cured acrylic has been the most commonly used material for construction of bite splints. Although effective, its processing involves several steps and is time consuming. Furthermore, acrylic splints distort easily if not kept in water when not worn for long periods of time. A newly developed light-cured composite material is now being used for bite splint fabrication. The composite material offers benefits in ease and speed of construction, has minimal warping and distortion, and has proven patient acceptance. The aim of this study was to determine if patient satisfaction with

A comparison between horizontal splint and repositioning splint in the treatment of 'disc dislocation with reduction'. Literature meta-analysis.

We reviewed the literature from 1985 to 1996 concerning the preliminary treatment of disc dislocation with reduction (DDWR) by means of occlusal splints, pinpointing two main methods. The conflicting results of many papers induced us to make a literature meta-analysis of those articles with the necessary requirements in order to verify the effectiveness of the two therapeutic means more frequently used in DDWR; the repositioning splint and the bite plane. A statistical comparison between the two kinds of treatment has demonstrated that the repositioning splint is more effective both in the res

Digital workflow in oral splint manufacturing.

AIM: The objective of the present article is to demonstrate the digital workflow used to manufacture an adjusted oral splint in a patient case. MATERIALS AND METHODS: A 25-year-old female patient presented for management of her bruxism. Therefore, an adjusted oral splint was manufactured. A computer-aided motion analysis of the patient was conducted (JMA Optic). Full-arch scans of the maxilla and mandible, a biocopy of the maxilla with a bite fork, and buccal scans of the centric jaw relation (Primescan) were performed. The jaw relation was determined beforehand by ballistic closing on a chai

Novel Custom Maxillary Disimpaction Splint.

LeFort I, II, and III osteotomies are commonly used in complex craniofacial reconstruction. Patients requiring these procedures typically have a craniofacial cleft, other congenital craniofacial deformities, or severe facial trauma. Both the cleft and traumatized palate have poor bony support, which leads to possible complications when the disimpaction forceps are used during the downfracture of the maxilla. Such potential complications include trauma or formation of a fistula of the palatal, oral, or nasal mucosa; trauma to adjacent teeth; and fracture of the palate and alveolar bone. To help
