A small study of nursing home owners, managers, residents and relatives found effective management depended on strong leadership skills, high bed occupancy, enthusiastic staff and low staff turnover. Funding problems were a key issue for homes, creating massive insecurity for residents, relatives and staff. There was a feeling that nursing homes could be used more effectively by the NHS, but there were concerns about capacity and competence. The viability of homes would be improved by annual block booking by the NHS. This would also facilitate the development of intermediate care. A national d

The clinical ladder program has evolved with the ever-changing environment of nursing at one academic medical center. The process has developed over time while adhering to the foundational tenets of recognizing and rewarding excellent bedside nursing practice. Nurses on the clinical ladder at this institution represent the ideal performance of mentoring and motivating other nurses and interdisciplinary team members. This article describes how the program has evolved over the last two decades, the challenges that have been met, and the successful outcomes that endure.

Anorexia nervosa (AN), which primarily affects adolescent girls and young women, is characterized by excessive dieting, leading to severe weight loss and a pathological fear of becoming overweight, or unhealthy behaviours that hinder weight gain. Caring for patients who have AN is particularly challenging for nursing staff, who are on the front line and have the closest contact with patients. However, little is known about how nursing staff care for adolescent patients with AN in Taiwan. This study aimed to explore nurses' perceptions on and experiences in conflict situations in caring for ado

BACKGROUND: Social care staff shortages are having a detrimental impact across the health and care system. There are reports of care homes closing, stopping nursing services and not admitting new residents because of challenges with staff shortages.
AIM: To develop an explanatory framework of strategies used to attract, recruit, and retain registered nurses and care workers working in care homes. Explain how and why strategies work, for whom, the conditions needed and the costs involved.
DESIGN AND METHODS: A realist synthesis approach was used. In step 1, strategies were identified and init

BACKGROUND: Participatory action research (PAR) shifts power to communities as decision-makers in research. Community health workers (CHWs) are valuable partners for implementing PAR.
OBJECTIVES: Using the community engagement continuum, this paper describes the PAR approach used by a team to conduct a study with CHWs in California. We present lessons learned for meaningfully including CHWs in interdisciplinary research teams.
METHODS: A collective of CHW and university researchers attended reflection sessions to describe the four phases of the research project, classify steps across the com

BACKGROUND: Effective teaching in health professions education is essential for preparing competent graduates and promoting safe, high-quality clinical care. Faculty development is a key strategy for enhancing teaching competencies and fostering change in educational practices. It is increasingly recognized that peer learning can contribute to the intended outcomes in faculty development. However, it remains unexplored how and in what ways interprofessional collaboration can further strengthen these programs. This study aims to examine how participation in interprofessional faculty development

Although most managers can recognize an off-kilter leader (consider the highly supportive boss who cuts people too much slack), it's quite difficult to see overkill in yourself. Unfortunately, that's where leadership development tools such as 360-degree surveys fail to deliver, say Kaplan and Kaiser. Dividing qualities into "strengths" and "weaknesses" and rating them on a five-point scale will not account for strengths overplayed. The authors suggest several strategies, based on their years of consulting experience and research, for figuring out which attributes you've employed to excess and

Hospital clinical staff routinely confront challenging behaviors in patients with dementia with limited training in prevention and management. The authors of the current article conducted a survey of staff on a chronic care hospital unit concerning knowledge about dementia, perceived educational needs, and the care environment. The overall mean score for a 27-item knowledge scale was 24.08 (SD = 2.61), reflecting high level of disease knowledge. However, staff indicated a need for more information and skills, specifically for managing behaviors nonpharmacologically (92.3%), enhancing patient s

INTRODUCTION: Neonatal intensive care is a traumatic environment for parents, infants and staff. Services need to collaborate with their users to develop acceptable and sustainable support. Music therapy can have positive impacts on physiological and psychological outcomes for infants and has the potential to help create a supportive service environment. This study aimed to co-design a culturally sensitive music therapy intervention to support the development of continuing bonds for parents with infants with uncertain futures on neonatal units.
METHODS: An Experience-based Co-design approach

Behavioural models identify the actions of others as an important factor in the development and maintenance of challenging behaviours. In the present study, 109 care staff were asked about their immediate and longer-term intervention strategies for a fictitious young man's challenging behaviour. Staff descriptions of long-term interventions were largely consistent with the aims of psychological interventions. However, their immediate intervention strategies were similar to the counter-habilitative strategies identified in previous observational and self-report research. The implications of the

Staff reactions play an important role in the development and maintaining of clients' challenging behaviour. Because there is a paucity of research on staff reactions in naturalistic settings, this study examined sequential associations between challenging behaviour and staff reactions by means of a descriptive analysis. We analysed video recordings of the reactions of 10 staff members towards challenging behaviour of clients with severe or profound intellectual disabilities who displayed self-injurious behaviour, stereotyped behaviour and/or aggressive/destructive behaviour. As expected, the

OBJECTIVE: We implemented a comprehensive strategy to track and reduce adverse events.
STUDY DESIGN: We incrementally introduced multiple patient safety interventions from September 2004 through November 2006 at a university-based obstetrics service. This initiative included outside expert review, protocol standardization, the creation of a patient safety nurse position and patient safety committee, and training in team skills and fetal heart monitoring interpretation. We prospectively tracked 10 obstetrics-specific outcome. The Adverse Outcome Index, an expression of the number of deliveries
