Ultrasound as the New Stethoscope: A Journey From Just Locating Fluid to Assessing Haemodynamics and Venous Congestion.

Point-of-care ultrasound (POCUS) has evolved from a simple tool for fluid localization to a comprehensive modality for hemodynamic assessment and real-time clinical decision-making. The Bedside Lung Ultrasound in Emergency (BLUE) protocol enables clinicians to diagnose causes of cardiorespiratory failure more accurately than clinical examination alone. In patients presenting with shock and hypotension, POCUS facilitates rapid identification of the underlying aetiology. This allows for targeted treatment in these critically ill patients. In complex patients presenting with concurrent cardiac fa

[Ultrasound stethoscope in the rapid assessment of left ventricular systolic function and valvular regurgitation--personal experience].

BACKGROUND: Recent technological advances enabled miniaturization of echocardiographic machines and construction of portable "ultrasound stethoscopes", which enable rapid two-dimensional and color-Doppler imaging and supplement clinical examination of the heart. We present the results of systematic assessment of these devices in comparison with standard echocardiographic machines. METHODS: We prospectively examined with both ultrasound stethoscopes and standard echocardiographic machines 60 patients referred to our laboratory for routine echocardiographic examination. Image quality (score: 1

Spectral analysis of bruits with an electronic stethoscope enhances screening of carotid stenosis and plaques beyond conventional auscultation.

BACKGROUND: Stroke is the second leading cause of death worldwide, with carotid stenosis being a primary contributor. Therefore, stroke prevention would benefit from accessible carotid stenosis screening tools. Historically, acoustic stethoscopes were used to listen to the carotid artery, but this method is now outdated due to its subjectivity and inconsistent sensitivity and specificity in detecting stenosis. In contrast, electronic stethoscopes record audio, enabling precise and objective analysis. To overcome traditional auscultation limitations, our study introduces a signal analysis schem

Automated Detection of Reduced Ejection Fraction Using an ECG-Enabled Digital Stethoscope: A Large Cohort Validation.

BACKGROUND: Asymptomatic left ventricular systolic dysfunction (ALVSD) affects 7 million globally, leading to delayed diagnosis and treatment, high mortality, and substantial downstream health care costs. Current detection methods for ALVSD are inadequate, necessitating the development of improved diagnostic tools. Recently, electrocardiogram-based algorithms have shown promise in detecting ALVSD. OBJECTIVES: The authors developed and validated a convolutional neural network (CNN) model using single-lead electrocardiogram and phonocardiogram inputs captured by a digital stethoscope to assess

Automated Assessment of Test of Masticating and Swallowing Solids Using a Neck-Worn Electronic Stethoscope: A Pilot Study.

BACKGROUND: The Test of Masticating and Swallowing Solids (TOMASS) is a validated screening tool for assessing masticatory and swallowing functions. However, the conventional TOMASS relies on operator-dependent methods, which limit its objectivity and efficiency. The neck-worn electronic stethoscope (NWES), a contact sensor positioned on the back of the neck, has recently been developed to automatically detect and monitor swallowing actions through deep learning-based analysis of collected sound data. OBJECTIVE: This study piloted a semi-automated assessment approach using a NWES to objective

Measurement and estimation of human forced expiratory noise parameters using a microphone with a stethoscope head and a lapel microphone.

OBJECTIVE: The time signature of tracheal noise in the 200-2000 Hz frequency band recorded during a forced expiratory manoeuvre (FETa) is a promising tool for diagnosis of bronchial obstructions. FETa is typically registered using a microphone with a stethoscope head placed on the neck over the trachea (stethoscope sensor). We studied basic time and spectral parameters of forced expiratory tracheal noises with a microphone placed near the mouth (lapel microphone, outside the flow of exhaled air) and compared these with measurements via stethoscope sensor, with the aim of providing patient home

Comparison between Two Endotracheal Tube Cuff Inflation Methods; Just-Seal Vs. Stethoscope-Guided.

INTRODUCTION: The Endotracheal Tube (ETT) cuff performs a critical function of sealing the airway during positive pressure ventilation. There is a narrow range of cuff pressure required to maintain a functionally safe seal without exceeding capillary blood pressure. AIM: We aimed to compare Just-Seal (JS) and Stethoscope-Guided (SG) method of ETT cuff inflation with respect to the volume of air required to inflate the cuff, the manometric cuff pressure achieved and also to assess for the occurrence of postoperative sore throat after extubation in both the groups. MATERIALS AND METHODS: It wa

Deep learning-driven automated detection of canine cardiac murmurs via digital wireless stethoscope auscultation.

A heart murmur is a key indicator of cardiovascular disease, making auscultation essential. Nonetheless, its accuracy is influenced by the clinician’s experience and subjective interpretation. This study developed a deep learning–based algorithm (CNN6) for automated detection of heart murmurs using phonocardiogram (PCG) data acquired via a digital wireless stethoscope. A total of 2,269 recordings (over 20 h) from 406 dogs were used for model development and validation, and 297 recordings from 60 dogs were reserved for independent testing. The model achieved 89.9% sensitivity, 92.7% specificity

Lung auscultation using smartphone built-in microphone versus digital stethoscope: a comparative early feasibility study.

BACKGROUND: Lung auscultation using a smartphone built-in microphone is promising for home monitoring of pediatric respiratory diseases. Our aim was to compare respiratory sounds recorded by a smartphone and a digital stethoscope by assessing the proportion of quality recordings and adventitious sounds detected by each device. METHODS: A comparative early feasibility study with children from a public school in Northern Portugal was conducted. Lung auscultation was performed at 7 locations (trachea, anterior superior, posterior superior, and posterior inferior regions of both hemithoraces). Tw

Swallowing sound evaluation using an electronic stethoscope and artificial intelligence analysis for patients with amyotrophic lateral sclerosis.

BACKGROUND AND PURPOSE: Non-invasive, simple, and repetitive swallowing evaluation is required to prevent aspiration pneumonia in neurological care. We investigated the usefulness of swallowing sound evaluation in patients with amyotrophic lateral sclerosis (ALS) using our new electronic stethoscope artificial intelligence (AI) analysis tool. METHODS: We studied patients with ALS who provided written informed consent. We used an electronic stethoscope, placed a Bluetooth-enabled electronic stethoscope on the upper end of the sternum, performed a 3-mL water swallow three times, and remotely id

Design of ear-contactless stethoscope and improvement in the performance of deep learning based on CNN to classify the heart sound.

Cardiac-related disorders are rapidly growing throughout the world. Accurate classification of cardiovascular diseases is an important research topic in healthcare. During COVID-19, auscultating heart sounds was challenging as health workers and doctors wear protective clothing, and direct contact with patients can spread the outbreak. Thus, contactless auscultation of heart sound is necessary. In this paper, a low-cost ear contactless stethoscope is designed where auscultation is done with the help of a bluetooth-enabled micro speaker instead of an earpiece. The PCG recordings are further com

Prediction of Neonatal Respiratory Distress in Term Babies at Birth from Digital Stethoscope Recorded Chest Sounds.

Neonatal respiratory distress is a common condition that if left untreated, can lead to short- and long-term complications. This paper investigates the usage of digital stethoscope recorded chest sounds taken within 1 min post-delivery, to enable early detection and prediction of neonatal respiratory distress. Fifty-one term newborns were included in this study, 9 of whom developed respiratory distress. For each newborn, 1 min anterior and posterior recordings were taken. These recordings were pre-processed to remove noisy segments and obtain high-quality heart and lung sounds. The random unde

Fully portable continuous real-time auscultation with a soft wearable stethoscope designed for automated disease diagnosis.

Modern auscultation, using digital stethoscopes, provides a better solution than conventional methods in sound recording and visualization. However, current digital stethoscopes are too bulky and nonconformal to the skin for continuous auscultation. Moreover, motion artifacts from the rigidity cause friction noise, leading to inaccurate diagnoses. Here, we report a class of technologies that offers real-time, wireless, continuous auscultation using a soft wearable system as a quantitative disease diagnosis tool for various diseases. The soft device can detect continuous cardiopulmonary sounds

Design and development of a digital stethoscope encapsulation for simultaneous acquisition of phonocardiography and electrocardiography signals: the SmartHeart case study.

The stethoscope is a major symbol of modern medicine. It is used for diagnosis of different conditions and enables physicians to listen to internal body sounds. Electrocardiography was only introduced in medicine in the beginning of the twentieth century. Today measuring heart's electrical activity is also fundamental cardiac diseases diagnosis. Although performed with independent devices, requiring physician and patient presence in the same physical space, in combination they enhance cardiovascular assessment. In this paper, a digital stethoscope encapsulation was designed, adding new functio

A urologic stethoscope? Urologist performed sonography using a pocket-size ultrasound device in the point-of-care setting.

PURPOSE: Ultrasound is commonly used in urology. Technical advances with reduced size and cost led to diffusion of small ultrasound devices to many clinical settings. Even so, most ultrasound studies are performed by non-urologists. We aimed to evaluate the utility of a pocket-size ultrasound device (Vscan™ GE Healthcare) and the quality of urologist performed study. METHODS: Three consecutive studies were performed: (1) a urologist using the pocket ultrasound, (2) a sonographist using the pocket ultrasound, and (3) a sonographist using a standard ultrasound device. Thirty-six patients were e

"Absolute Necessity": The Discovery of the Fetal Heartbeat with the Stethoscope, and Its Impact on Obstetric Practice in Dublin and Edinburgh, 1820-1840.

Many people now take knowledge of the fetal heartbeat for granted. Despite this, it wasn't until 1818, following the invention of the stethoscope and popularization of the technique of auscultation, that the fetal heartbeat was first discovered. Listening to the fetal heartbeat enabled practitioners to confirm the existence of pregnancy, gain information on the internal positions of the fetus and the placenta, and determine the life or death of the fetus in utero. Additionally, signs from the stethoscope provided guidance for practitioners when dealing with long or difficult labors. This artic

Application of a reverse stethoscope to overcome communication barriers: A case report of an elderly patient with illiteracy and profound hearing loss.

RATIONALE: As a common clinical diagnostic tool, stethoscopes have the ability to amplify sound, isolate ambient noise, and locate the source of sound. However, we reversed the stethoscope innovatively. This changed its standard use, delivering sound directly to the patient's ear. It successfully overcame communication barriers between clinicians and patients who have both illiteracy and severe hearing loss (HL). PATIENT CONCERNS: This paper reports a case of a 74-year-old elderly male patient with illiteracy combined with very severe HL requiring a hearing decibel of 82 dB, living alone, who
