Genomic epidemiology of Streptococcus pyogenes across multiple centres in China.

BACKGROUND: Streptococcus pyogenes causes a broad spectrum of invasive and non-invasive infections globally, associated with high morbidity and mortality. In mainland China, distribution of the emm genotype has shifted markedly over recent decades, with emm12 emerging as the dominant type. Meanwhile, resistance to macrolides, lincosamides, and tetracyclines has reached extremely high levels, severely limiting clinical treatment options and leading to the designation of S. pyogenes as a medium-priority pathogen by the World Health Organization. METHODS: A total of 133 non-duplicate S. pyogenes

Understanding carriage of Streptococcus pyogenes directly from the human host.

Using genomic approaches, Faiola et al.1 evaluated the Streptococcus pyogenes carriage state, and pre- and post-treatment pharyngitis, directly from human subjects. This work suggests that carriage isolates may evolve with host-specific mechanisms rather than a unified regulatory phenotype.

Endothelial NLRP3 inflammasome activation drives immunothrombosis in Streptococcus pyogenes infection.

Streptococcus pyogenes (Strep A), a formidable human pathogen, is notorious for causing life-threatening diseases such as necrotizing fasciitis and streptococcal toxic shock syndrome, often complicated by thrombosis and coagulation abnormalities. While macrophage inflammasome activation has been widely studied in Strep A pathogenesis, the contribution of vascular endothelial cells-key regulators of immunity and coagulation-remains largely unexplored. This study aimed to determine whether endothelial NLRP3 inflammasome activation drives immunothrombosis during invasive Strep A infection and to

Streptococcus pyogenes emm Type 3.93 Emergence, the Netherlands and England.

A global increase in the incidence of invasive group A Streptococcus (iGAS) infections was observed after lifting of COVID-19 related restrictions in 2022, and type M1UK dominated in many countries. After seasonal declines in iGAS incidence during the summer of 2023, simultaneous, rapid expansion of a previously rare emm type 3.93 was seen beginning in November, increasing to 20% of all cases in England and 60% of all cases in the Netherlands within 4 months. emm3.93 was associated with iGAS in children 6-17 years of age and with increased risk for pneumonia or pleural empyema and meningitis i

A Rapid Molecular Detection Tool for Toxigenic M1UK  Streptococcus pyogenes.

BACKGROUND: The gradual replacement of the Streptococcus pyogenes M1global genotype by a newly emergent M1UK variant is a global public health threat warranting increased surveillance. M1UK differs from progenitor M1global genotype by 27 single-nucleotide polymorphisms and is characterized by increased speA superantigen expression in vitro. METHODS: An allele-specific real-time polymerase chain reaction assay was developed for the rapid detection of M1UK strains. The assay was used in combination with whole genome sequencing to determine emm (sub)type distribution for 51 invasive (n = 9) and

Volatile profiling distinguishes Streptococcus pyogenes from other respiratory streptococcal species.

Sore throat is one of the most common complaints encountered in the ambulatory clinical setting. Rapid, culture-independent diagnostic techniques that do not rely on pharyngeal swabs would be highly valuable as a point-of-care strategy to guide outpatient antibiotic treatment. Despite the promise of this approach, efforts to detect volatiles during oropharyngeal infection have yet been limited. In our research study, we sought to evaluate for specific bacterial volatile organic compounds (VOC) biomarkers in isolated cultures in vitro, in order to establish proof-of-concept prior to initial cli

Characterization of emergent toxigenic M1UK Streptococcus pyogenes and associated sublineages.

Streptococcus pyogenes genotype emm1 is a successful, globally distributed epidemic clone that is regarded as inherently virulent. An emm1 sublineage, M1UK, that produces increased levels of SpeA toxin was associated with increased scarlet fever and invasive infections in England in 2015/2016. Defined by 27 SNPs in the core genome, M1UK is now dominant in England. To more fully characterize M1UK, we undertook comparative transcriptomic and proteomic analyses of M1UK and contemporary non-M1UK emm1 strains (M1global). Just seven genes were differentially expressed by M1UK compared with contempor

Molecular emm typing of Bulgarian macrolide-resistant Streptococcus pyogenes isolates.

Group A streptococcus (GAS) is a human pathogen causing a broad range of infections, linked with global morbidity and mortality. Macrolide resistance rates vary significantly in different parts of the world. Driving factors of the emergence and spread of resistant clones are not clearly understood. We investigated 102 macrolide-resistant GAS strains collected during the period 2014-2018 from various clinical specimens from Bulgarian patients. Strains were characterized by the presence of mefA/mefE, ermA, and ermB using polymerase chain reaction and sequencing for mefA/mefE. Resistant strains w

[Streptococcus pyogenes infection in paediatrics: from pharyngotonsillitis to invasive infections].

BACKGROUND: Streptococcus pyogenes or Group A Streptococci (GAS) cause many infections in infancy. Changes in its epidemiology have been described in recent years, including an increase in invasive infections (iGAS). METHODS: A retrospective-descriptive study was conducted on children less than 15 years old, with GAS infections, in particular iGAS, and their complications from February 2004-April 2014. RESULTS: A total of 2,192 positive cultures were obtained of which 92.7% were pharyngeal cultures. Twenty-nine patients were admitted to hospital: 4 with suppurative complications, 7 post-infe

Status of research and development of vaccines for Streptococcus pyogenes.

Streptococcus pyogenes is an important global pathogen, causing considerable morbidity and mortality, especially in low and middle income countries where rheumatic heart disease and invasive infections are common. There is a number of promising vaccine candidates, most notably those based on the M protein, the key virulence factor for the bacterium. Vaccines against Streptococcus pyogenes are considered as impeded vaccines because of a number of crucial barriers to development. Considerable effort is needed by key players to bring current vaccine candidates through phase III clinical trials an

[Epidemiology of Streptococcus pyogenes invasive diseases in France (2007-2011)].

Group A Streptococcus (GAS) is a human pathogen responsible for a wide range of clinical manifestations. An increase of GAS invasive infections has been described since the mid 1980s. To study the French epidemiology of invasive infections (i) we characterized all GAS invasive strains received at the French National Reference Center for streptococci (CNR-Strep) between 2007 and 2011; (ii) we analyzed the epidemiological data on the corresponding strains. For each strain, emm genotype, superantigen genes and antibiotics susceptibility were determined. Among the 2 603 non redundant invasive GAS

Spread of a highly mucoid Streptococcus pyogenes emm3/ST15 clone.

BACKGROUND: Hyaluronic acid capsule plays a key role in Streptococcus pyogenes virulence. Circulation of mucoid or highly encapsulated strains has been related to rheumatic fever epidemics and invasive disease in several countries. In 2009, an outbreak of mucoid S. pyogenes isolates was detected in northern Spain. The aim of the study was to describe clinical and molecular characteristics of mucoid strains causing this outbreak and to compare them with a sample of non-mucoid S. pyogenes isolates obtained during the same period of time. METHODS: All S. pyogenes isolates with a mucoid colony mo

Rapid screen for epithelial internalization of Tn917-mutagenized Streptococcus pyogenes.

Group A streptococci (GAS) cause a number of human diseases ranging from pharyngitis to necrotizing fasciitis. GAS are hypothesized to escape killing by either the immune system or beta lactam antibiotics by internalization into epithelial cells. A Tn917 library of transposon mutants was screened for capacity to invade and survive in human epithelial cells using a novel blood agar overlay method. Although the screen revealed that a majority of Tn917 insertions occurred within a 10 kb region of the genome, GAS genes identified as essential for internalization into epithelial cells included ABC

NrdI essentiality for class Ib ribonucleotide reduction in Streptococcus pyogenes.

The Streptococcus pyogenes genome harbors two clusters of class Ib ribonucleotide reductase genes, nrdHEF and nrdF*I*E*, and a second stand-alone nrdI gene, designated nrdI2. We show that both clusters are expressed simultaneously as two independent operons. The NrdEF enzyme is functionally active in vitro, while the NrdE*F* enzyme is not. The NrdF* protein lacks three of the six highly conserved iron-liganding side chains and cannot form a dinuclear iron site or a tyrosyl radical. In vivo, on the other hand, both operons are functional in heterologous complementation in Escherichia coli. The

Susceptibility of Streptococcus pyogenes to two macrolides in northern Israel.

In the present study, the minimal inhibitory concentration (MIC) of azithromycin and roxithromycin for 200 Streptococcus pyogenes isolates from outpatients with tonsillopharyngitis were determined using Etest. All but one (99.5%) of the isolates were sensitive to both antibiotics; the MIC of the resistant isolate being 12 mg/l to azithromycin and 32 mg/l to roxithromycin. In this region, macrolides remain the drug of choice for the treatment of patients with S. pyogenes tonsillitis who present allergy to penicillin. The routine testing of susceptibility of S. pyogenes to macrolides in northern

Remarkable attachment of lactoferrin to Streptococcus pyogenes during acute pharyngotonsillitis.

The purpose of this study was to establish whether lactoferrin (hLf) attached to Streptococcus pyogenes, one causative agent of acute pharyngotonsillitis (AT), during the course of the disease. Bacterial samples were obtained from the tonsillar surfaces of 7 patients (6 females, 1 male; median age 26 years; range 16-50 years) suffering from AT who were culture-positive for S. pyogenes and from 5 healthy adult controls who were culture-negative for this pathogen. Using gold-labelled antiserum against S. pyogenes and hLf, this pathogen and other bacteria on the tonsillar surfaces coated with hLf
