Arytenoid adduction and type I thyroplasty in the treatment of aphonia.

Arytenoid adduction is a procedure used to medialize the paralyzed vocal fold, closing the posterior glottis. Isshiki type I thyroplasty allows medialization of the anterior membranous vocal fold. Using the arytenoid adduction, in combination with Isshiki type I thyroplasty as needed, five patients were treated for aphonia. Surgical results were evaluated with voice recordings, electroglottography, and photoglottography. Jitter ratio, shimmer ratio, and signal-to-noise ratio were measured. Laryngeal stroboscopy and glottography were used to assess the mucosal wave and vibratory nature of the v

Influence of thyroid surgery on voice function and laryngeal symptoms.

A comprehensive examination of voice function and laryngeal symptoms was applied systematically to 20 patients undergoing thyroid surgery. Patients were excluded from the series if there had been previous neck surgery or if postoperative indirect laryngoscopy showed abnormal cord mobility. Examination before operation showed that patients with sporadic non-toxic goitre or medically pre-treated toxic goitre had impaired voice function based on stroboscopy, electro-glottography, phono-oscillometry and determination of voice range, phonation time, pitch, and peak flow. Three months after thyroid

Synchronous primary parotid tuberculosis and EBV-associated primary lymphoepithelial carcinoma.

A 48-year-old gentleman presented with complaints of neck swelling in the parotid region for 2 years. Diagnostic nasal endoscopy and video laryngeal stroboscopy were normal, and positron emission tomography scan showed no other primary lesion. The lesion with tail of parotid was excised and sent for histopathological examination. Part of the tissue was also sent for diagnosis of tuberculosis by polymerase chain reaction (PCR) as it is a very common disease in South Asian countries. At this point, differentials considered were undifferentiated primary carcinoma, metastatic undifferentiated naso

Update: laryngeal reinnervation for unilateral vocal cord paralysis with the ansa cervicalis.

Eight patients underwent ansa cervicalis anastomosis to the adductor branch of the recurrent laryngeal nerve for unilateral vocal cord paralysis. They were followed long enough (at least 1 year) to determine if the procedure was successful. All cases have been subjected to preoperative and postoperative voice recording, acoustic analysis, and videolaryngoscopy. Some of them have been subjected to stroboscopy and electromyography (EMG). Data from these cases indicate that satisfactory phonatory quality may be achieved after the procedure. The reinnervated vocal cord neither abducted nor adducte

Quality analysis of laryngography in a busy hospital ENT voice clinic.

The Portable Laryngograph provides a non-acoustic plot of wet mucosal contact at the glottis over the vocal fold vibrational cycle called the Lx waveform in Laryngograph terminology. Although well-equipped clinics gain most ENT requirements for investigation from the video endoscope, the nature of stroboscopy prevents the investigation over single glottal periods and many transitory problems may be missed. A high-quality Lx signal waveform will reflect any irregularities in mucosal contact and substantially complements the endoscopy examination. Yield, however, is a problem in the environment

Laryngotracheal paratubal air envelope: a useful sign in voice and airway reconstructive surgery.

Aphonia, dysphonia, and laryngotracheal stenosis with upper airway compromise are diagnosed with increasing frequency. Endoscopic surgical reconstructive techniques accompanied by tracheal T-tube stenting successfully restore voice, airway function, and structure. Timing of tracheal T-tube removal remains controversial, particularly when the tip of the upper arm of the optimally sized T-tube is placed in a critical supraglottic position. The appearance of an air envelope, manifested as linear air densities between the outside wall of the T tube and the inside wall of the larynx and trachea, ha

Complications of laser surgery for recurrent respiratory papillomatosis.

During an 8-year period, 66 patients with recurrent respiratory papillomatosis underwent 890 laser microlaryngoscopy procedures. A retrospective analysis of immediate and delayed complications occurring with laser microlaryngoscopy in the treatment of these patients revealed that complications secondary to anesthetic technique and laser methodology were remarkably low (0.3%). Delayed complications of tissue injury (36% of pediatric patients and 17% of adult patients) were more frequent and included anterior glottic webbing, interarytenoid scarring and arytenoid fixation, vocal fold fibrosis, a

Vocal fold nodules in children.

PURPOSE OF REVIEW: Vocal nodules are a common presentation in children, representing the most common cause of dysphonia. RECENT FINDINGS: Children with siblings, ADHD, and of male sex are most likely to be affected. Female children, however, tend to have more likely progression into adolescence. Quality-of-life indices can aid in screening for vocal nodules whereas stroboscopy, and possibly, ultrasound are required for definitive diagnosis. SUMMARY: Management has not significantly changed over time and should begin with conservative behavioral and environmental modifications along with voic

Evaluation of clinical value of videokymography for diagnosis and treatment of voice disorders.

This study aimed at determining the clinical value of videokymography (VKG) as an additional tool for the assessment of voice disorders. 105 subjects with voice disorders were examined by an experienced laryngologist. A questionnaire was used to specify diagnosis, diagnostic confidence, and treatment recommendations before and after VKG. The first part of questionnaire was filled by the laryngologist for each patient after routine ear-nose-throat evaluation, including stroboscopy, the second part after the subsequent VKG examination. In 31% of subjects VKG confirmed the stroboscopic diagnosis,

Objective measures of laryngeal imaging: what have we learned since Dr. Paul Moore.

Dr. Paul Moore pioneered the use of high-speed cinematography for observation of normal and abnormal vocal fold vibrations during phonation. His analysis of the glottal area waveform, opening and closing speed index, and open quotient from the high-speed films were labor intensive but relevant today. With advances in digital image capture and automated image extraction techniques, stroboscopy and high-speed images of vocal fold vibration may be analyzed with objective measures. Digital high-speed image capture in color is now clinically practical at high resolution. Digital kymography now allo

The inspiratory maneuver: a simple method to assess the superficial lamina propria during endoscopy.

We describe a simple means of assessing the condition of the superficial lamina propria during endoscopy, which we use in our practice as an adjunct to careful stroboscopic examination. An intake of breath against partially adducted vocal folds serves to draw the mucosa inferomedially into the glottis, which reveals areas of diminished mobility, the relationship of mucosal lesions to deeper tissues of the vocal folds, and subtle submucosal lesions. This is possible because of the unique geometry of the glottis that renders the vector forces of airflow different at inspiration and expiration. B

Laryngeal neuroma in a MEN-2B paediatric patient.

Multiple endocrine neoplasia type 2B (MEN-2B) syndrome is a genetic disorder that predisposes to the development of tumours in the mouth, eyes and endocrine glands. It is associated with mucosal neuromas; however, laryngeal mucosal neuromas are very rare. We describe an adolescent with MEN-2B who presented with dysphonia, nocturnal stridor and exertional dyspnoea following a total thyroidectomy for an invasive medullary thyroid carcinoma with right recurrent laryngeal nerve sacrifice.Following initial assessment, laryngeal stroboscopy was performed, followed by a microlaryngobronchoscopy under
