Little Patients Big Discoveries: Potential Pediatric to Adult Neurogastroenterology Translation.

Pediatric neurogastroenterology conditions, including disorders of gut-brain interaction and motility disorders, affect millions of children worldwide. Owing to limited pediatric data, reference ranges and management are often extrapolated from adult studies. We review 4 pediatric neurogastroenterology areas where clinical science may translate and inform adult gastroenterology. Sucrase-isomaltase deficiency can be diagnosed through disaccharidase enzyme testing from duodenal mucosal biopsies. Dietary restriction and sacrosidase supplementation are effective, based on randomized controlled tri

Duodenal disaccharidase activities in the follow-up of villous atrophy in coeliac disease.

BACKGROUND: In active coeliac disease, mucosal atrophy is associated with a marked decrease in intestinal disaccharidase enzyme activities. We investigated the value of duodenal mucosal disaccharidases to predict the severity of mucosal villous atrophy and its recovery in 50 patients with coeliac disease. METHODS: Duodenal mucosal histology and disaccharidase activities were studied at least twice with a mean interval of 9 months. Histology of specimens from all patients was examined by the same pathologist blinded to the data on disaccharidase activities. Mucosal damage was scored into four

Early or pre-coeliac mucosa: development of gluten enteropathy.

Duodenal mucosa showed normal morphology, interepithelial lymphocytes, alkaline phosphatase, and sucrase in a girl with growth retardation and iron deficiency, but normal absorption of lactose and xylose after two years of abnormal stools. Mucosal lactase was low. Fourteen months later mucosal damage consistent with coeliac disease was evident, and gluten intolerance was subsequently confirmed by gluten challenge. It is probable that, in some children, the mucosal lesion occurs very gradually, so that at an early stage with normal morphology, suppression of lactase activity and possibly interf

Jeju Citrus (Citrus unshiu) Leaf Extract and Hesperidin Inhibit Small Intestinal α-Glucosidase Activities In Vitro and Postprandial Hyperglycemia in Animal Model.

Citrus fruits are widely distributed in East Asia, and tea made from citrus peels has demonstrated health benefits, such as a reduction in fever, inflammation, and high blood pressure. However, citrus leaves have not been evaluated extensively for their possible health benefits. In this study, the α-glucosidase-inhibitory activity of Jeju citrus hot-water (CW) and ethyl alcohol (CE) extracts, along with hesperidin (HP) (a bioactive compound in citrus leaf extracts), was investigated, and furthermore, their effect on postprandial blood glucose reduction in an animal model was determined. The Microbiota Contributes to CD8+ T Cell Activation and Nutrient Malabsorption following Intestinal Infection with Giardia duodenalis.

Giardia duodenalis is a noninvasive luminal pathogen that impairs digestive function in its host in part by reducing intestinal disaccharidase activity. This enzyme deficiency has been shown in mice to require CD8(+) T cells. We recently showed that both host immune responses and parasite strain affected disaccharidase levels during murine giardiasis. However, high doses of antibiotics were used to facilitate infections in that study, and we therefore decided to systematically examine the effects of antibiotic use on pathogenesis and immune responses in the mouse model of giardiasis. We found

Rapid Communication: The relationship of enterocyte proliferation with intestinal morphology and nutrient digestibility in weaning piglets.

Understanding the regulatory mechanisms of intestinal morphology and function is essential for improving postweaning growth in pigs. The objective of this study was to identify the relationships of enterocyte proliferation with intestinal villus height, crypt depth, and nutrient digestibility in piglets. Sixty-four 21-d-old weaned piglets were used. Gastrointestinal cell proliferation was evaluated via Ki-67 immunohistochemistry. Villus height and crypt depth were measured using hematoxylin and eosin (H&E)-stained sections. The apparent total tract digestibility (ATTD) of CP and GE was determi

The relationship between villous height and growth performance, small intestinal mucosal enzymes activities and nutrient transporters expression in weaned piglets.

Intestinal villi are important structures for digesting and absorbing nutrients. It was hypothesized that intestinal villous height (VH) is related to growth performance, and that VH is associated with digestive and absorptive capabilities in piglets. Sixty 21-d-old weaned piglets were fed identical diets for 28 days. The piglets were grouped according to jejunal or ileal VH at the end of the experiment, including short, middle and high VH groups. The final BW (quadratic, p = .003), ADG (liner, p = .052; quadratic, p = .030) and G: F (liner, p = .074; quadratic, p = .005) were greater in highe

Oral administration of deuterium-labelled polyamines to sucking rat pups: luminal uptake, metabolic fate and effects on gastrointestinal maturation.

Non-physiological amounts of oral polyamines have been reported to induce precocious gut maturation in rat pups. The aim of the present study was to investigate organ distribution and metabolic fate of orally administered stable-isotopically labelled polyamines in rat pups. Pups received tetradeuterium-labelled putrescine (Pu-d4; 3 mumol), spermidine (Sd-d4; 5 mumol), spermine (Sp-d4; 3 mumol), or physiological saline twice daily on postnatal days 7-10 or 12-15. They were killed on days 10 and 15. We determined activities of ileal lactase (EC 3.2.1.23), maltase (EC 3.2.1.20), sucrase (EC 3.2.1

Design, synthesis of novel methyl-glycosyl furanose based sulfonium type α-glucosidase inhibitors with potent antihyperglycemic activity.

Developing effective α-glucosidase inhibitors is critical for managing hyperglycemia in individuals with high-carbohydrate diets; however, current drugs exhibit limitations in both efficacy and tolerability. In the present study, 25 derivatives of neoponkoranol, an active sulfonium type α-glucosidase inhibitor identified from Genus Salacia were synthesized. These compounds containing a methyl-glycosyl furanose scaffold modified with various hydrophobic groups at the 5'-O-position, were characterized using HRMS and NMR spectroscopy. Biological evaluations demonstrated a selectively enhanced mal

[Disaccharidase activity as a diagnostic criterion for the mucosa of the small intestine].

Activities of lactase, sucrase, and maltase were determined in small intestinal biopsies of 125 children with coeliac disease, cow's milk protein intolerance, transient gluten intolerance, nonspecific enteropathies, and controls. Four cases of primary disaccharidase deficiencies could be identified. In the enteropathies, morphometric data were more closely correlated to the degree of the mucosal lesion (r = -0.92 for crypt depth) than were disaccharidase activities (r = 0.61 for lactase). In a stepwise discriminant analysis of the patient groups, based upon immunological, morphometric, and bio
