Influence of pectoralis minor and upper trapezius lengths on observable scapular dyskinesis.

OBJECTIVES: Although a relationship between short pectoralis minor and upper trapezius and scapular dyskinesis has been postulated, no studies have investigated this theory. Understanding the effect of these muscle lengths on observable scapular dyskinesis may aid in determining risks and therefore making treatment decisions. Being aware of the magnitude of this effect would help gauge the significance of risks involved. Our aim was to evaluate the influence of pectoralis minor and upper trapezius lengths on scapular dyskinesis. DESIGN: Cross-sectional study. SETTING: University research lab

Can the Use of Unstable Surfaces and Instruction for Conscious Abdominal Contraction Increase the EMG Activity of the Periscapular Muscles During the Dynamic Push-Up?

CONTEXT: It is believed that conscious abdominal contraction (CAC) during exercise encourages greater periscapular activation through existing myofascial connections. On the other hand, it is postulated that the use of unstable surfaces would promote greater neuromuscular demand. OBJECTIVE: To analyze the effect of CAC on periscapular muscle activity during push-up plus exercise on stable and unstable surfaces and to evaluate the correlation between electromyographic (EMG) activity of the serratus anterior (SA) and abdominal oblique muscles. DESIGN: Repeated-measures design in a single group

Boomerang-shaped extended latissimus dorsi flap for combined breast reconstruction and body contouring following massive weight loss.

We present a massive weight loss case who sought a correction for her left-sided implant-based breast reconstruction using an autologous approach. She had previously received a left-sided mastectomy as an infant. The initial implant-based breast reconstruction led to repeated corrective procedures. The cosmetic result was poor due to thin skin flaps, poor shape and inadequate volume; moreover, the patient experienced significant and persistent pain.An abdominal-based free flap was not feasible due to a prior vertical abdominoplasty, which had eliminated the availability of sufficient abdominal

Editorial Commentary: Arthroscopically Assisted Lower Trapezius Transfer: Promising Technique Still Searching for the Optimal Indications or Panacea for All Irreparable Rotator Cuff Tears?

Arthroscopic-assisted lower trapezius transfer (LTT) has gained in popularity as a technique to restore function and improve pain in large-to-massive irreparable posterosuperior rotator cuff tears. Surgeons who have adopted this technique have variable indications. While most agree that there is a limit of arthropathy, other factors appear to be contentious. Age, teres minor integrity, and degree of external rotation deficit remain debatable in terms of indications and contra-indications for LTT. The biomechanical narrative around LTT is sound and has been proven. Surgeons have recently raised

Morphometry of Latissimus Dorsi in Fetal Cadavers for Prenatal Spina Bifida Surgery.

AIM: To investigate latissimus dorsi morphometry, innervation, vascularization, and variational situations in fetuses. MATERIAL AND METHODS: Forty-nine fetuses, aged between 15 and 40 weeks of gestation, were examined for the morphological development of the latissimus dorsi. The horizontal lengths of the thoracolumbar fascia, the dimensions of the latissimus dorsi and its tendon were measured. Additionally, the attachments of the latissimus dorsi and the branching pattern of the thoracodorsal nerve were evaluated. The development of all measured parameters during gestational age (trimester)

Comparison of upper trapezius muscle stiffness between female assembly workers with and without myofascial trigger points.

BackgroundMuscle stiffness in the upper trapezius (UT) muscle is a significant parameter for evaluating myofascial trigger points (MTrPs). Because the measuring position influences UT muscle stiffness, investigating UT muscle stiffness in different working postures would be useful for evaluating and planning effective interventions for MTrPs.ObjectiveTo identify differences in UT muscle stiffness measured in the resting and working postures among female assembly workers, both with and without MTrPs.MethodsEighteen female workers with right (Rt) side UT MTrPs and 18 workers without MTrPs partic

Posterior intercostal flap: an anatomical study and best flap design.

BACKGROUND: Posterior intercostal arteries perforators (PICAPs) and lateral intercostal arteries perforators (LICAPs) are great vascular pedicles. Between the 4th and 11th spaces, they arise from the thoracic aorta. These are large perforators that can be the basis of many flaps. Yet, these perforators are underrated as they are poorly studied and scarcely utilized in plastic reconstructions. METHODS: Twenty (ten males and ten females) adult cadaveric dissections were done on both sides to study the types, locations, and sizes of posterior intercostal perforators to help design flaps based on

The Multiple Rib Osteomyocutaneous Split Latissimus Dorsi Flap for Calvarial Reconstruction: Indication, Operative Technique, and Review of Literature.

Major complex cranial defects may be challenging for the reconstructive microsurgeon. Affected patients often present with impaired soft tissues including dura exposure or fistulas. The lacking structural bony support may cause severe neurological issues and in select patients, there is a need for well-vascularized autologous tissue repair. The authors herein elucidate the role of the multiple rib osteomyocutaneous split latissimus dorsi flap for reconstruction of composite skull defects, providing an indication, an exemplary case, operation technique, and literature review. Ultrasound-guided five-point injection of botulinum toxin for patients with trapezius hypertrophy.

OBJECTIVES: Our study aimed to explore the clinical therapeutic effects of ultrasound-guided five-point injection of botulinum toxin type A for patients with trapezius hypertrophy. METHODS: Twenty female patients diagnosed with trapezius hypertrophy were enrolled in this study. The thicknesses of the trapezius muscle were measured by using the ultrasound scanner to locate the thickest point of trapezius, followed by labelling the other four points around the first point. Botulinum toxin type A was injected bilaterally (50 IU/side, 5 points/side) in the trapezius muscle of these patients. The

[Neuromuscular dynamic scapular winging: Clinical, electromyographic and magnetic resonance imaging diagnosis].

Dyskinesia of the scapula is a clinical diagnosis and includes all disorders affecting scapula positioning and movement whatever its etiology. Scapular winging is a subtype of scapular dyskinesia due to a dynamic prominence of the medial border of the scapula (DSW) secondary to neuromuscular imbalance in the scapulothoracic stabilizer muscles. The two most common causes of DSW are microtraumatic or idiopathic lesions of the long thoracic nerve (that innerves the serratus anterior) or the accessory nerve (that innerves the trapezius). Diagnosis of DSW is clinical and electromyographic. Use of m

Reverse turnover pedicled latissimus dorsi muscle flap for lower back reconstruction: Two case reports.

RATIONALE: Large soft tissue defects on the lower back represent a treatment challenge. Among a variety of reconstructive procedures, the latissimus dorsi (LD) muscle or musculocutaneous flap is one of most frequently used pedicled flaps. However, the pedicled LD flap carries a bulky pivot point and a short arc of rotation. If a pedicled LD muscle flap is transferred using a reverse turnover pattern, theses drawbacks can be avoided. PATIENT CONCERNS: The first patient was a 56-year-old man with hepatocellular carcinoma and multiple bone metastases involving D11-L4 vertebral bodies. A chronic

Effects of C3/C4 Manual Decompression on Scapular Posterior Tilt, Lower Trapezius Muscle Activity, and Strength in Individuals With Lower Trapezius Weakness.

BACKGROUND AND PURPOSE: Lower trapezius (LT) weakness-induced anterior scapular tilt is a key mechanism of shoulder dysfunction. Although the primary motor innervation of the trapezius originates from the spinal accessory nerve, cervical nerve branches (C3/C4) contribute sensory and accessory motor fibers, leading to hypotheses that C3/C4 segmental sensitivity may be associated with altered LT performance. We aimed to investigate the immediate effects of C3/C4 manual decompression on scapular posterior tilt (SPT) kinematics, LT activity, and maximal isometric strength. METHODS: Twenty-seven p

Editorial Commentary: Lower Trapezius Transfer Delivers Durable, Meaningful Recovery at 5 Years: Thresholds Tell Only Part of the Story.

Arthroscopic-assisted lower trapezius tendon transfer has emerged as a compelling joint-preserving solution for massive irreparable posterosuperior rotator cuff tears, yet the field has lacked a robust framework for interpreting what constitutes a meaningful patient outcome after this procedure. Establishing procedure-specific minimal clinically important difference and patient acceptable symptom state thresholds in this setting is both timely and necessary. When such thresholds are derived, however, they must be understood as cohort-specific constructs that cannot be universally applied acros

Editorial Commentary: Arthroscopically Assisted Posterior Latissimus Dorsi and Teres Major Transfers Show Improvement for Irreparable Posterosuperior Rotator Cuff Tears but Are Rarely the Best Choice.

Tendon transfers have shown good clinical results in a numerous reviews of treatment options for unrepairable posterosuperior rotator cuff tears. Combined latissimus dorsi and teres major transfer have been described as an option to improve external rotation strength. Although outcomes have been acceptable, other options for treatment of this patient population may be preferable in terms of outcomes and perioperative morbidity.

Techniques for Lower Trapezius Transfer Using Achilles Allograft in Irreparable Posterosuperior Rotator Cuff Tears.

The management of irreparable rotator cuff tears presents significant challenges, particularly in active individuals experiencing functional limitations, such as reduced forward elevation and deficits in both external and internal rotation. Traditional latissimus dorsi (LD) tendon transfer has shown effectiveness in reducing pain associated with posterosuperior cuff tears but often yields inconsistent functional outcomes. This is largely due to the LD's primary role as an internal rotator, which limits its capacity to restore normal shoulder biomechanics. To address these limitations, the lowe

Immunohistochemical properties of motoneurons supplying the porcine trapezius muscle.

The trapezius muscle (TRAP) belongs to the scapulothoracic group of muscles, which play a crucial role in the integrity and strength of the upper limb, trunk, head, and neck movements and, thus, in maintaining balance. Combined retrograde tracing (using fluorescent tracer Fast Blue, FB) and double-labelling immunohistochemistry were applied to investigate the chemical coding of motoneurons projecting to the porcine TRAP. FB-positive (FB+) motoneurons supplying the cervical (c-TRAP) and thoracic part (th-TRAP) of the right (injected with the tracer) TRAP were located within the IX-th Rexed lami

Chronic fistulas are a dreaded complication to surgery. They are often persistent to treatment and involve several negative side effects and a significant reduction in quality of life. We present a case report of a 36-year-old male with a deep chronic fistula in the pelvis after pouch surgery. More than 60 procedures were performed under general anaesthesia without treatment effect. Through a multidisciplinary approach a complete rectum extirpation and reconstruction with a free latissimus dorsi flap was performed. The patient was seen at followup 18 months after the surgery; pain free and wit
