Improving laboratory test ordering can reduce costs in surgical wards.

Background and Aim Laboratory blood tests for hospitalized patients are often overused. Excessive costs and no proof of benefit suggest re-evaluating the current approach to laboratory test ordering. The aim of the study is to improve the decision-making process of test ordering and to investigate what effect a rational, evidence-based use of laboratory test ordering in surgical wards would have on costs and healthcare resources. Methods Three-phase experimental prospective study carried out at the tertiary referral teaching hospital of Parma. Phase 1 (baseline status). The baseline status of

[Estimation of surgical treatment capacity for managing mass casualty incidents based on time needed for life-saving emergency operations].

The surgical treatment capacity of a hospital constitutes a significant restriction in the capability to deal with critically injured patients from multiple or mass casualty incidents (MCI). With regard to the time needed for life-saving operative interventions there are no basic reference values available in the literature, which can aid in detailed planning for management of mass casualty incidents. The data of 20,815 trauma patients, recorded in the trauma registry hosted by the German Association for Trauma Surgery DGU, were analyzed to extract the median duration of life-saving surgical i

A history of cardiology in Jamaica.

The history of cardiology in Jamaica is conveniently considered in decades beginning in the 1950s. The decade of the 1950s was characterized by early descriptions of the pattern of cardiac disease in adults and children in Jamaica, the establishment of a cardiac clinic at the University Hospital of the West Indies and early cardiac surgical landmarks. Extensive preparatory experimental work in the canine laboratory with respect to cardiopulmonary bypass in the early to mid-1960s culminated in the successful completion of the first open heart surgical procedure in April, 1968. The Effect of Hurricane Maria on the Surgical Workload of the UPR-affiliated Hospitals.

OBJECTIVE: The Caribbean islands are regularly affected by hurricanes in a seasonal manner, but major (category 4 and 5) hurricanes are infrequent, and what happens in their aftermath is important for future planning. METHODS: We reviewed the surgical cases entered into the University of Puerto Rico (UPR) General Surgery Department database from January 1, 2016, through December 31, 2017. This database collects patient and procedural information from the surgical services of the UPR-affiliated hospitals. To analyze the impact that Hurricane Maria had on the surgical population, we compared ca

Clinical problem-solving: three rare conditions--one pediatric surgical patient.

The purpose of this report is to demonstrate a step-by-step process of clinical decision making. A case study is presented in stages to simulate the way such information emerges in clinical practice. The focus is on the diagnostic process of this pediatric surgical patient's presentation, with particular attention to the unusual sequence of events and the diagnostic dilemmas that emerged for the clinicians involved. This case also represents three very rare conditions related to the same patient that have never been presented in the literature occurring in combination or in relationship to one

Successful conversion from a linear accelerator-based program to a Gamma Knife radiosurgery program: the Cleveland Clinic experience.

From August 1989 to January 1997, 307 treatments in 293 patients were performed with a linear accelerator-based (LINAC) stereotactic radiosurgery system. Because of the program s success, the need for a dedicated radiosurgery unit in Ohio and the desire to treat functional disorders, the Cleveland Clinic Health System (CCHS) obtained the first Gamma Knife in the state of Ohio. Based on the previous volume of patients for radiosurgery, it was estimated that 75-100 patients would be treated during the first year of operation. However, during the first calendar year, 214 treatments were performed

Frozen section consultation. Utilization patterns and knowledge base of surgical faculty at a university hospital.

The authors studied the knowledge base of surgical faculty concerning frozen section consultations at a university hospital to determine whether it had any relationship to the appropriateness of frozen section requests. To accomplish this, the reasons for performing frozen sections during a 3-month period were analyzed, and those request that seemed ambiguous or inappropriate were identified. Simultaneously a 15-item questionnaire was distributed to faculty and housestaff dealing with factual information concerning the technique and limitations of frozen section diagnosis (Questions 1-8), as w

Hospital use of systemic antifungal drugs: a multi-center surveillance update from Germany.

BACKGROUND: The consumption of antifungal agents increased over the last decade, resulting in the development of resistant organisms and causing a significant pharmaco economic burden. Antifungal drugs are widely used for the treatment of systemic fungal infections and high-risk patients, especially with severe hematological or oncological conditions. Up to date, there are no reliable and systematically reported data on the consumption of antifungal substances on a nationwide level available. The presented study gives an update to the previously published multicenter study investigating antifu

Resource utilization associated with hospital and office-based insertion of a miniaturized insertable cardiac monitor: results from the RIO 2 randomized US study.

Background: Previous studies support operational benefits when moving insertable cardiac monitor (ICM) insertions outside the cardiac catheterization/electrophysiology laboratories, but this has not been directly assessed in a randomized trial or when the procedure is specifically moved to the office setting. To gain insight, the RIO 2 US study collected resource utilization and procedure time intervals for ICM insertion in-office and in-hospital and these data were used to calculate costs associated with staff time and supply use in each setting. Methods and results: The Reveal LINQ In-Office

Operative failures after parathyroidectomy for hyperparathyroidism: the influence of surgical volume.

OBJECTIVE: To determine whether surgical volume influences the cause of operative failures after parathyroidectomy for hyperparathyroidism. SUMMARY AND BACKGROUND DATA: The surgical success rate for hyperparathyroidism from high-volume centers exceeds 95%, but some patients have unsuccessful parathyroidectomies. Although operative failure can be due to hyperfunctioning parathyroid glands in ectopic locations, less experienced surgeons may be more likely to miss an abnormal parathyroid in normal anatomic locations, which we describe as "preventable operative failure." METHODS: We used 2 prosp

[Thyroid gland surgery in a teaching department of parenchymal surgery. Quality control and perspectives based on a 10-year material].

The complications after surgery for benign thyroid disease in a non-specialized department were evaluated in a retrospective study. Furthermore it was evaluated whether the new recommendations of The National Board of Health for referral of patients in need of thyroid surgery would influence the rate of operations and complications in the ward. Three hundred and seventy-four patients were operated on. The complication rate was at the same level as in earlier Danish reports. Permanent unilateral palsy of the recurrent laryngeal nerve was registered in 2.1% of the patients (i.e. in 1.4% of "nerv

Financial impact of emergency hand trauma on the health care system.

BACKGROUND: The US has a crisis of insufficient emergency coverage for hand trauma. One of the problems is the perceived financial loss associated with caring for this population. We evaluated the financial impact of treating emergency hand trauma patients on an academic medical practice and health care system. STUDY DESIGN: We examined billing records for 2,632 hand patients seen in the emergency department in 2005 at the University of Michigan. Financial data were separated into inpatient professional and facility revenues and costs. Professional net revenue was calculated by applying actua
