Lumbar spine stability after combined application of interspinous fastener and modified posterior lumbar interbody fusion: a biomechanical study.

INTRODUCTION: Posterior lumbar interbody fusion (PLIF) and internal fixation are commonly performed for the treatment of lower back pain due to lumbar spinal degeneration. We have developed a novel interspinous fixation device, the interspinous fastener (ISF) for potential use in the surgical management of degenerative spinal disease. The aim of this study was to assess the in vitro biomechanical characteristics of calf lumbar spine specimens after ISF fixation with modified PLIF. MATERIALS AND METHODS: Ten lumbar spine (L3-L6) specimens from ten fresh calf cadavers (8-10 weeks of age) were u

AbsorbaTack™ vs. ProTack™ vs. sutures: a biomechanical analysis of cervical fixation methods for laparoscopic apical fixations in the porcine model.

PURPOSE: Treatment of pelvic organ prolapse (POP) often requires the use of synthetic mesh. In case of a novel and standardized bilateral apical fixation, both uterosacral ligaments are replaced by polyvinylidene-fluoride (PVDF) tapes. One of the main problems remains the fixation method, which should be stable, but also simple and quick to use. The current study evaluated biomechanical differences between the cervical tape fixation with sutures (group 1), non-absorbable tacks (group 2) and absorbable tacks (group 3) in an in vitro porcine model. METHODS: A total of 28 trials, conducted in th

Intramedullary nailing in pertrochanteric fractures of the proximal femur.

INTRODUCTION: Pertrochanteric fractures of the proximal femur should be treated surgically, unless the medical condition of the patient does not allow it. Currently, there are two ways to fix these fractures; either with a sliding hip screw or with an intramedullary nail. However, there is much debate over which implant is the best for pertrochanteric fracture fixation. The sliding hip screw has been used over time with good clinical results. While it was true that with first generation intramedullary nails the risk of complications was higher, there is evidence supporting the superiority of i

Current perspectives in the management of scaphoid nonunions.

When left untreated, scaphoid fractures follow a downward spiral resulting in carpal collapse and arthritis. The purpose of intervention is to allow a cartilage-wrapped bone to heal and maintain a smooth articular surface. Proposed treatment options for scaphoid nonunions have varied success rates. Using the Mack-Lichtman classification for nonunions, a plan can be formulated for individual patients based on motion loss, degenerative changes, carpal collapse, and fracture instability manifested by bone loss. Stable nonunions may benefit from bone grafting and internal fixation. Unstable nonuni

Retrospective cohort study analyzing temporal bone cortical thickness and perioperative complication rate, in pediatric cochlear implantation.

BACKGROUND: Cochlear implant fixation in pediatric patients can be challenging due to the thin cranial bone. The dura matter can be exposed by drilling a bony recess leading to possible complications. A minimally invasive newer fixation method might avoid such risks. OBJECTIVES: The study focus is to assess the feasibility of drilling a bony well adequate for cochlear implant receiver/stimulator device embedment in pediatric patients of different age groups. We also aim report the occurred complications and device failure rates using different surgical techniques for cochlear approach and fix

[Treatment guidelines and results in severe open tibial fractures].

27 patients with compound fractures of the lower leg were treated according to the following scheme: Exact excision and surgical cleaning of the wound, reduction of the fragments without further exposure of the bone by additional incision, minimal osteosynthesis, stabilization by means of external fixation device and secondary closure of the accidental wound. During the last year we preferred the V-shaped arrangement in contrast to the tent shaped arrangement of the external fixation device as this minimizes the trauma to the extensor muscles of the foot. After external fixation combined with

Osteoporotic pertrochanteric hip fractures: management and current controversies.

The treatment of intertrochanteric hip fractures has evolved over the past 80 years because of a better understanding of fracture anatomy, application of biomechanical principles, and novel technologic advances. Surgical treatment of intertrochanteric hip fractures is the current standard of care, with short-term goals of fracture stabilization and early patient mobilization and the long-term objective of restoring patients to their previous level of independence and function. Treatment for stable intertrochanteric hip fractures includes use of percutaneous intramedullary devices and open redu

Role of bone grafting in correction of protrusio acetabuli by total hip arthroplasty.

A study of 27 hip arthroplasties derived from a pool of more than 200 total hip arthroplasties performed between 1976 and 1981 was conducted to assess the need for and benefits of bone grafting in the surgical management of protrusio acetabuli. The objective of surgery and the reason for augmentation by bone graft was to lateralize the acetabular component, normalize the center of rotation of the hip, and strengthen the deficient medial wall. Based on this study, the authors recommend: when protrusion is less than 5 mm in either direction and the medial wall is reasonably strong, bone graft is

Triceps Repair and Restoration of Triceps Footprint With Anchorless Suture Fixation.

Triceps tendon rupture is an uncommon yet potentially devastating injury affecting patients over a broad demographic. Surgical treatment is essential to restore upper extremity functional status, and a vast array of techniques has been implemented with different fixation devices including suture buttons, intraosseous anchors, and suture repairs. Outcomes of distal triceps tendon repair have demonstrated nearly full return of functional capacity. Complications include infection, ulnar nerve neuropathy, arthrofibrosis, flexion contracture, hardware irritation, and most commonly, repair failure.

A Novel Approach to Sternal Fracture Repair With the Implementation of a Compression/Distraction Device.

Sternal fractures are common following blunt traumatic injury. Most sternal fractures can be managed successfully nonoperatively; however, surgical fixation should be considered in certain scenarios. Specifically, surgery may be indicated in cases of severe pain, respiratory failure or dependency on mechanical ventilation, cosmetic deformity, malunion, disunion, and compression of the heart. A variety of surgical approaches to sternal fracture fixation have been documented (steel wire, suture materials, a seven-hole aluminum plate, an eight-holed Sternolock X plate, sternum-osteosynthesis plat

Posterior cervical fixation for fracture and degenerative disc disease.

There are numerous newer techniques that have been developed for the internal fixation of the cervical spine in recent years. Wiring techniques are still appropriate for posterior stabilization of the cervical spine. The halo vest is still widely used for the conservative management of cervical fractures and for postoperative external immobilization. The authors stress that the surgical indications for more modern rigid implants should be adhered to strictly. These implants also should be selected by weighing their advantages versus potential risks. In the upper cervical spine, the surgeon may

Epidemiology and treatment outcome of surgically treated mandibular condyle fractures. A five years retrospective study.

Surgical management of mandibular condyle fractures is still controversial. Although it provides better outcome than closed treatment questions still remain about the surgical approach and the osteosynthesis devices to be used. Between 2005 and 2010, we managed 168 mandibular condyle fractures with open treatment. Two surgical approaches were used in this study, a pre-auricular and a high submandibular approach (one or the other or as a combined approach). Internal fixation was performed using TCP(®) plates (Medartis, Basel, Switzerland) or with two lag screws (15 and 17 mm). Delta plates were

Le Fort III advancement with gradual distraction using internal devices.

The purpose of this report is to demonstrate a practical clinical method for advancement of the midface using a combination of Le Fort III osteotomy and gradual distraction. Use of internal, submergible distraction devices and modification of the Ilizarov distraction protocol are presented. Case reports show the effect of departing from the Ilizarov protocol by eliminating the latency period, initiating distraction intraoperatively, and rapidly distracting the segments postoperatively. A method of controlling fragment distraction rate by regulating pressures within the osteotomy site is descri

Alignment of displaced or impacted teeth with the traction chain.

To align those teeth in the dental arch that cannot erupt or are displaced, they must be surgically exposed. After the traction device is attached, the tooth is again covered with the mucoperiosteal flap to protect the tissues involved. For this reason, the loss of this traction device must be absolutely avoided in order to prevent another surgical intervention. Here at the University of Ulm, we have developed and tested an optimized traction chain for that purpose. The chain is attached to the tooth with a laser-roughened pad applying the acid-etching technique. The chain links serve as fixat

Surgical fusion of the posttraumatic spine: a radiologic assessment.

Radiological assessment of surgical fusion of fractures and dislocations of the cervical, thoracic, and lumbar spine are an integral part of management of spinal cord injury patients. A meaningful assessment of such fusions can be done properly only when there is adequate knowledge of the types of surgical fusions, the biological material used, and metallic devices applied. Early surgical reduction and internal fixation of the traumatized spine facilitate rapid restoration of spinal alignment, make nursing care less difficult, and sometimes lead toward improvement of neurological function. Early Results of a Novel Intramedullary Fixation Device for Proximal Fifth Metatarsal Fractures.

BACKGROUND: Proximal fifth metatarsal fractures are commonly treated surgically due to their poor healing capacity. While intramedullary screws may be the most popular operative treatment choice, newer fixation methods continue to develop. We present a case series utilizing a novel intramedullary fixation device for proximal fifth metatarsal fractures. To our knowledge, no other study in the literature has assessed the safety and efficacy of this fixation method. METHODS: A retrospective analysis was performed for 16 patients with proximal fifth metatarsal fractures who underwent fixation wit
