BACKGROUND: Trauma navigation applications employ conventional mechanical surgical instruments for the simulation of drilling trajectories. Few complex mechanical targeting instruments, such as guides with fixed angles or with multiple parallel cannulas, have been adapted into trauma navigation systems. We have integrated a complex mechanical tool, the parallel drill guide (PDG) for the minimally invasive treatment of femoral neck fractures, into a trauma navigation module. The combined advantage of a complex yet commonly used mechanical tool with the benefits of fluoroscopic navigation was ev

Combined component placement of cup and stem is closely correlated to stability, functionality and wear in total hip replacement (THA). Computer-navigated orthopedic surgery offers a reliable control method for a complex three-dimensional situation. Imageless navigation systems without the need of preoperative or intraoperative image acquisition and exposure to radiation have been proven to increase the accuracy of positioning the acetabular component and measure intraoperative leg length and offset changes precisely. A new development in this field is the noninvasive external femoral referenc

Chondroma in the area of the spheno-ethmoidal junction is very rare. A 29-year-old male patient with chronic rhinosinusitis with nasal polyps was arranged for a preoperative computed tomography scan, and a lesion was accidentally found in his spheno-ethmoidal junction and involved the skull base. Combined with MRI, the lesion was misdiagnosed as fungal sinusitis. However, no fungal lesions were found during the operation, and cartilage tissue was confirmed only after some bone was ground away under the guidance of a surgical navigation system. Our case indicates that chondroma is easily misdia

Spinal navigation systems improve pedicle screw placement accuracy, but their reliance on supine preoperative imaging can introduce errors due to positional differences between preoperative and intraoperative spinal alignment. These misalignments may compromise surgical outcomes, particularly in lumbar spine procedures. This study investigates how key lumbar and lumbopelvic parameters differ between prone and supine positions, aiming to refine imaging workflows and surgical navigation practices. A retrospective cohort study analyzed paired prone and supine CT images from 85 adult patients in t

Orbital fracture and orbital floor reconstruction surgery is characterised by limited surgical exposure and high risks. Before the advent of digital technology, the design and implementation of the surgical scheme for orbital floor reconstruction surgery mainly depended on the intuitive understanding of imaging and the clinical experience of surgeons, with strong unpredictability and experience dependence. Surgical navigation systems with real-time positioning and imaging functions, when used in orbital reconstruction surgery, can detect the real-time intraoperative position, avoid risks, and

Computer-aided systems have been developed recently to improve the precision of implantation of unicompartmental knee replacement (UKR) or total knee replacement. Minimal invasive techniques have been developed to decrease the surgical trauma related to the prosthesis implantation. However, there might be a concern about the potential of minimal invasive techniques for a loss of accuracy. Navigation systems might address this issue. We are currently using routinely a nonimage-based navigation for total knee replacement. We developed a modified system for UKR, suitable for either a conventional

INTRODUCTION: Computer-aided surgery (CAS) systems integrate endoscopic visualization with real-time localization based on preoperative imaging. One short-coming of the current systems is the lack of real-time, radiographic assessment of surgical changes. We describe the potential applications of xCAT, a new intraoperative mobile volume CT scanner and its utility in providing surgical navigation updates in the operating room.
METHODS: A case report is presented describing how intraoperative CT updates were used in a complicated, revision endoscopic frontal sinus surgery.

BACKGROUND: Limitation in performing restoration of orbital structures is the narrow, deep, and dark surgical field, which makes it difficult to view the operative site directly. To avoid perioperative complications from this limitation, the authors have evaluated the usefulness of computer-assisted navigation techniques in surgical treatment of blowout fracture.
METHODS: Total 37 patients (14 medial orbital wall fractures and 23 inferior orbital wall fractures) with facial deformities had surgical treatment under the guide of navigation system between September 2012 and January 2015.

Pelvic Kirschner wire (K-wire) insertion is a challenging surgical task requiring interpretation of complex 3D anatomical shape from 2D projections (fluoroscopy) and delivery of device trajectories within fairly narrow bone corridors in proximity to adjacent nerves and vessels. Over long trajectories (~10-25 cm), K-wires tend to curve (deform), making conventional rigid navigation inaccurate at the tip location. A system is presented that provides accurate 3D localization and guidance of rigid or deformable surgical devices ("components" - e.g., K-wires) based on 3D-2D registration. The patien
