Current rat model of rotator cuff (RC) tear could not mimic the suture anchor (SA) repair technique in the clinical practice. We designed a novel SA for RC repair of rats to establish a clinically relevant animal model. Small suture anchors that fit the rat shoulder were assembled. 60 rats were assigned to the transosseous (TO) repair group or SA repair group (n = 30/group). Micro-computed tomography (Micro-CT) scanning, biomechanical test and histological analysis were implemented at 2, 4, and 8-week post-repair. The failure load and stiffness in the SA group were significantly higher than th

We describe a new technique of using titanium alloy anchors to perform internally rotating capsulorrhaphy during open reduction and stabilization procedure and obliteration of pseudoacetabulum when one exists. We routinely use anterior approach via bikini incision (modified Smith-Peterson approach) to perform open reduction and internally rotating capsulorrhaphy. We describe the technique of using barbed titanium alloy anchors preloaded with nonabsorbable sutures and two needles, to accomplish a secure and easy capsulorrhaphy, in selected cases. We have used this technique in 21 hips, since 20

All-suture anchors require smaller drill holes (often under 2.0 mm) than comparable solid glenoid anchors (e.g., Gryphon: 2.5-mm drill). A smaller drill allows closer anchor approximation, but there is no indication that this improves repair biomechanics. In fact closely associated multiple fixation points are associated with glenoid fractures, and the same multiple fixation points can be achieved with double- or triple-loaded conventional anchors. All-suture anchors require deployment immediately adjacent to intact cortical bone. Without this, slack and pistoning of the suture ball anchor occ

PURPOSE: To report outcomes in 6 patients with the terrible triad of the elbow treated with our modified protocol.
METHODS: 6 men aged 26 to 54 years underwent surgery for the terrible triad of the elbow by a single surgeon. All the patients had a displaced comminuted fracture of the radius, posterior dislocation of the elbow, and Morrey type-I fracture of the coronoid. They all underwent replacement of the radial head and repair of the lateral collateral ligament to the isometric part of the lateral condyle using suture anchors. Five had an additional capsular fixation to the anterior corono

This study evaluated the resistance to gapping and the mode of failure for 2 knotless suture anchor systems used for rotator cuff repair compared with the performance of a conventional titanium anchor system. Eight matched pairs of fresh-frozen humeri were dissected free of all soft tissues and scanned to measure bone mineral density (BMD). The suture anchor systems tested were the TwinFix 5.0 Titanium (Smith & Nephew, Andover, MA), Bioknotless RC (DePuy Mitek, Norwood, MA), and Magnum (Opus Medical, San Juan Capistrano, CA), and each was inserted into each humerus. Cyclic, tensile loading was

Repair of soft tissue to bone is increasingly frequently performed using absorbable suture anchors. If a repair fails clinically, it is often impossible to identify the cause of failure at repeat surgery. We report on 2 cases of recurrence of instability after arthroscopic Bankart repair. In reoperation in these cases, all sutures were correctly knotted around the labrum but were intact and torn out of the anchor eyelets. No sign of anchor displacement (3 anchors in each patient) was seen. This is the first clinical report of unambiguous structural suture anchor failure. These observations emp

Rotator cuff tears are a common cause of shoulder pain and dysfunction. After surgical repair, there is a significant re-tear rate (25%-90%). The aim of this study was to determine the primary mode of mechanical failure for rotator cuffs repaired with suture anchors at the time of revision rotator cuff repair. We prospectively followed 342 consecutive torn rotator cuffs, repaired by a single surgeon using suture anchors and a mattress-suturing configuration. Of those shoulders, 21 (6%) subsequently underwent a revision rotator cuff repair by the original surgeon, and 1 underwent a second revis

Eighty-seven consecutive patients were treated with an open Bankart repair for traumatic, recurrent anterior instability of the shoulder. Forty-six of these patients underwent a transosseous suture technique. Forty-one patients were operated on with suture anchors. After follow-ups that ranged from 18 to 85 months, 7 of the 87 patients showed signs of recurrence and another 14 patients reported apprehension. Compared with the 66 patients without residual instability, these 21 patients were more likely to have been operated on with suture anchors (P < .05), had a greater incidence of hypermobil

BACKGROUND: Several biomechanical studies comparing open and arthroscopic rotator cuff repair methods have shown inferior performance of arthroscopic repairs. Suture anchor-augmented transosseous repairs and suture bridge repairs have shown superior biomechanical performance when compared with other methods, but these 2 repair methods have not been directly compared.
HYPOTHESIS: There will be no difference in the biomechanical performance of the transosseous-suture anchor and suture bridge techniques.
STUDY DESIGN: Controlled laboratory study.
METHODS: Eight paired cadaveric shoulder specim

BACKGROUND: Extensor mechanism tendon ruptures, including the quadriceps and patellar tendons, are devastating injuries that most often require acute surgical repair to restore knee mechanics and function. There are 2 well-described categories of quadriceps repair techniques, including superior pole suture anchor repairs and transosseous repairs, with recent studies reporting similar overall outcomes. However, failure rates have been reported to be as high as 18% and 21%, respectively.
INDICATIONS: Extensor mechanism ruptures.
TECHNIQUE DESCRIPTION: The described technique utilizes 2 transos

UNLABELLED: The suture anchor-enhanced medial capsulorrhaphy of the great toe is utilized as an adjuvant procedure to proximal and distal osteotomies for the treatment of hallux valgus. In traditional open techniques, hallux valgus repair requires both osseous correction along with shortening of the capsule on the medial side of the metatarsophalangeal joint. Osseous correction typically corrects the intermetatarsal angle, whereas capsular correction maintains the hallux valgus angle1.
DESCRIPTION: A standard medial approach to the 1st metatarsophalangeal joint is performed. A medial midline

Operative treatment of quadriceps and patellar tendon ruptures with transosseous bone tunnels remains the gold standard, but potential benefits exist with the use of suture anchor fixation for these injuries. Such benefits include stronger biomechanical fixation, reduced soft-tissue disruption, smaller incision, reduced postoperative pain, shorter operative time, lower knot burden, lack of prepatellar bursa scarring, and avoidance of some transosseous repair risks. In this investigation, we present the reproducible technique and outcomes of using suture anchors for repair of quadriceps and pat

Osteochondritis dissecans (OCD) is a subchondral bone abnormality, in which subchondral bone and the overlying articular cartilage detach from the bony bed. Multiple techniques for OCD fixation have been described, including metallic, bioabsorbable implants and biological fixation. We describe a surgical technique for OCD lesions including bony bed preparation with curettage and microfracture, anatomic reduction, and fixation using a suture anchor to provide stability and healing of the lesion.

BACKGROUND: Mesh suture used in high-tension wound closures produces large knots susceptible to increased palpability, infection, and foreign body response; yet has superior tensile strength and increased resistance to cutting through tissue compared to standard suture. This study investigates mesh suture fixation in abdominal tissue with a knotless novel, low-profile anchor-clip.
METHODS: Single and double end fixation of mesh suture in swine rectus abdominus fascia with an anchor-clip, a knot, and predicate device fixation underwent cyclic testing followed by pull-to-failure testing.
