Swallowing in Alzheimer's disease.

Using a prospective case series design, we examined the incidence of oropharyngeal swallowing abnormalities in 25 patients with moderate or severe Alzheimer's disease by videofluoroscopy. Aspiration occurred in 6 of 25 (28.6%). Only four patients showed unequivocally normal performance. Exploratory statistical analyses showed that swallowing abnormalities associated significantly with duration of dementia, eating dependency, and abnormal oral praxis. We observed a trend toward a higher incidence of aspiration in patients with more severe dementia. We conclude that oropharyngeal swallowing abno

Swallowing disorders in the elderly.

The classic symptoms of dysphagia, heartburn, and chest pain are the presenting manifestation of oropharyngeal and esophageal disorders in the elderly as well as the young. Several unique conditions, including Zenker's diverticulum and vascular compression of the esophagus, may occur in the elderly. Certain disorders, such as oropharyngeal dysphagia due to neurologic disorder, increase in frequency with age. Treatment approaches are similar in elderly and younger patients, but the potential for adverse drug effects and interaction is greater in the elderly than in the young.

Cervical auscultation using Doppler ultrasound for swallows of different consistencies by young and older healthy adults.

AIM: The use of a stethoscope for cervical auscultation (CA) has been reported to be useful in detecting potential swallowing ailments. Nevertheless, stethoscopic CA suffers several constraints including poor signal-to-noise ratio. The current study examined the signal characteristics of CA using Doppler ultrasound obtained when swallowing liquids of different consistencies by both older and younger healthy individuals. METHOD: Thirty healthy younger and older adults participated in the study. Doppler ultrasound CA was carried out when they swallowed 5-mL liquids of different consistencies ac

Optimizing Assessment of Contraction Reserve in Ineffective Esophageal Motility: A Study of Upright Multiple Rapid Swallows.

BACKGROUND/AIMS: Multiple rapid swallows (MRS) is a provocative test during high-resolution esophageal manometry (HRM) to evaluate contraction reserve (CR). This study aims to determine the prevalence of CR in patients with ineffective esophageal motility (IEM) and MRS performed in the upright position, and to assess the ideal number of MRS sequences. METHODS: We enrolled adult patients diagnosed with IEM according to the Chicago classification version 4.0 who underwent HRM with 3 MRS sequences. Presence of CR was identified as either: the distal contraction integral (DCI) of the post-MRS seq

How provocative tests in addition to wet swallows during high-resolution manometry can direct clinical management.

PURPOSE OF REVIEW: Provocative tests have been advocated to address limitations of high-resolution manometry (HRM) with wet swallows. We describe the commonly used provocative manometric manoeuvres [rapid drink challenge (RDC), multiple rapid swallows (MRS), solid swallows and the solid test meal (STM)], compare their diagnostic yield and accuracy to wet swallows, and outline their role in directing clinical management. RECENT FINDINGS: Provocative testing with RDC and STM identifies a proportion of achalasia cases missed by standard testing, and RDC can play a similar role to radiology in th

Esophageal Contractions After Wet and Dry Swallows in Patients With Esophagitis, Chagas' Disease and Idiopathic Achalasia.

BACKGROUND: In normal subjects the distal esophageal response to dry swallows differs from that of wet swallows. Our aim in this investigation was to compare the esophageal contractions of the proximal and distal esophageal body to wet and dry swallows. METHODS: We studied the esophageal contractions of eight patients with idiopathic achalasia, 37 patients with Chagas' disease, 28 patients with esophagitis, and 31 normal volunteers using manometric examination with continuous perfusion. The esophageal contractions were measured at 2 cm (proximal) and 22 cm (distal) from the upper esophageal s

Conformal Swallowing Accelerometry: Reimagining the Acquisition and Characterization of Swallowing Mechano-Acoustic Signals.

(1) Background: Non-invasive instrumental measurement of swallowing acoustic signals has rested upon the assumptions of signal symmetry and reproducibility along the cervical region and has hence taken the form of single-point acquisition on optimal sites. This study aimed to (i) revisit such assumptions by adopting a conformal array of accelerometers, and hence (ii) lay the foundation for the future design of swallowing accelerometry. (2) Methods: Thirteen young healthy individuals, including eight females (mean age ± SD = 24.38 ± 0.92) and five males (mean age ± SD = 24 ± 3.74), were recruit

A revised version of the Seirei Swallowing Questionnaire for people with cognitive decline (Swallow-10).

The Seirei dysphagia screening questionnaire (namely, Seirei dsq) has been used to screen for oropharyngeal dysphagia (OD). Some of the questions in the questionnaire are difficult for people with cognitive decline to answer. We selected 10 items from the Seirei dsq that could be assessed by others, such as care workers (named Swallow-10). This study aims to verify the validity of the scoring method used in Swallow-10. The dataset used in this study was the one used in the development of Seirei dsq : 50 patients with cerebrovascular disease who were diagnosed with OD, 145 patients with cerebro

Swallowing Dynamics and Breathing-Swallowing Coordination Are Altered in Patients with Mild Cognitive Impairment.

INTRODUCTION: Postinspiratory activity, which is essential for laryngeal closure during swallowing to prevent aspiration of food into the airways, is reduced in a mouse model of tauopathy. Therefore, we hypothesized that patients at the stage of mild cognitive impairment (MCI) exhibit alterations in swallowing dynamics and coordination between swallowing and breathing. METHODS: We examined breathing-swallowing coordination in patients with MCI. Patients who scored ≥24 on the Mini-Mental State Examination and <26 on the Japanese version of the Montreal Cognitive Assessment were recruited at Su

The Effect of the Cervical Orthosis on Swallowing Physiology and Cervical Spine Motion During Swallowing.

Cervical orthosis is used to immobilize the neck in various disorders such as trauma and post-operation. However, it is still uncertain how cervical orthosis restricts the degree of movement of the cervical spine during swallowing and how they affect swallowing physiology. The purpose of this study was to evaluate these issues using the Philadelphia(®) Collar. We conducted videofluorography of swallowing in 39 healthy subjects (23 men, 16 women; mean age of 34.3 years) with and without cervical orthosis. To compare the two conditions regarding the cervical spine motion, we determined the angul

Effects of Moderate Hyperbilirubinemia on Nutritive Swallowing and Swallowing-Breathing Coordination in Preterm Lambs.

BACKGROUND: Hyperbilirubinemia (HB) occurs in 90% of preterm newborns. HB induces acute neurological disorders (somnolence, abnormal tone, feeding difficulties, auditory dysfunction) and alterations in respiratory control. These findings suggest brainstem neurotoxicity that could also affect swallowing centers. OBJECTIVE: To test the hypothesis that HB impairs nutritive swallowing (NS) and swallowing-breathing coordination. METHODS: Two groups of preterm lambs (born 14 days prior to term), namely control (n = 6) and HB (n = 5), were studied. On day 5 of life (D0), moderate HB (150-250 µmol/l

[Fiberoptic endoscopic evaluation of swallowing--the Tel Aviv Voice and Swallowing Disorders Clinic].

Fiberoptic endoscopic evaluation of swallowing (FEES) involves passing a fiberoptic laryngoscope transnasally to visualize the hypopharynx, larynx, and proximal trachea in order to assess swallowing disorders. FEES has been compared with the modified barium swallow (MBS) (the presumed "gold standard"). To date, reports have demonstrated that FEES is as sensitive as, or even more sensitive, for use as a tool in swallowing assessment compared with the MBS. FEES provides the clinician with a safe, portable, effective, and valid means of evaluating individuals with swallowing disturbances. FEES al

Relationship Between Swallowing Frequency and Swallowing-Related Muscle Mass in Older Adults.

BACKGROUND: Older adults have decreased swallowing-related muscle mass, which may lead to decreased swallowing function. One of the causes of this decrease in muscle mass in older adults is a decrease in swallowing frequency. OBJECTIVE: The purpose of this study was to evaluate the relationship between swallowing frequency and swallowing-related muscle mass. METHODS: The study included 84 older adults residing in long-term care beds or elderly care facilities. The correlation between swallowing frequency measured by detecting swallowing sounds from laryngeal sounds and the coronal cross-sect

Normal values for swallow events during endoscopic evaluation of swallowing: a preliminary study.

PURPOSE: The current investigation aimed to establish preliminary normative data for endoscopic swallow studies (FEES). The investigators collected data for three timing measures (time to whiteout, duration of whiteout, and total swallow time), three swallowing outcomes (safety, efficiency, and number of swallows per bolus), and one physiologic event (glottal response), for both healthy young and older adults using two liquid volumes, one pureed bolus and a solid bolus. METHODS: Blinded raters retrospectively analyzed 65 randomly selected, deidentified videos of endoscopic swallowing examinat
