[Oculopalpebral and facial synkinesis associated with ptosis: epidemiological, clinical, and therapeutic features].

Synkinetic movements of the upper eyelids may be noted in association with movements of either extraocular muscles or other muscles of the face. Patients with oculopalpebral or facial-palpebral synkinesis may also have ptosis of the involved eyelid. The clinical and therapeutic features of this association are specific. We mainly distinguish two forms of synkinetic movements, characterized either by the elevation of the ptotic eyelid such as in Marcus Gunn phenomenon and in the Fuchs sign or by the falling of the upper lid such as in Marin Amat syndrome. Many surgical techniques have been used

Hereditary bimanual synkinesis combined with hypogonadotropic hypogonadism and anosmia in four brothers.

A new genetic syndrome of the combined occurrence of hypogonadotropic hypogonadism, anosmia (Kallmann syndrome) and congenital mirror movements in four brothers is presented. Mirror movements were manifest only within the distal parts of the upper extremities and resembled congenital mirror movements described for isolated or familial cases or those occurring in combination with other genetic defects. The hypothesis is supported, that a midline fusion disorder with preponderance of uncrossed pyramidal tract fibers is a major pathogenetic factor for the occurrence of congenital mirror movements

Does Electrical Stimulation Therapy Influence Synkinesis in Facial Paralysis? A Systematic Review.

OBJECTIVE: Facial paralysis (FP) has aesthetic, social, and functional consequences. While electrical stimulation (ES) shows promise for peripheral nerve injury, its role in FP remains controversial due to concerns about increasing synkinesis. DATA SOURCES: A literature review was conducted using Medline/PubMed, Cochrane, and Web of Science databases. REVIEW METHODS: Included studies evaluated the incidence or severity of synkinesis after ES in FP patients. Extracted data included demographics, ES parameters, FP etiology, synkinesis outcomes, and treatment duration. RESULTS: Of 601 identifi

Physiological oculo-auricular-facial-mandibular synkinesis elicited in humans by gaze deviations.

Integrated motor behaviors involving ocular motion-associated movements of the head, neck, pinna, and parts of the face are commonly seen in animals orienting to a visual target. A number of coordinated movements have also been observed in humans making rapid gaze shifts to horizontal extremes, which may be vestiges of these. Since such integrated mechanisms point to a nonpathological coactivation of several anatomically separate cranial circuits in humans, it is important to see how the different pairs of integrative motor behaviors with a common trigger (i.e., ocular motion) manifest in rela

Stimulation of denervated muscle promotes selective reinnervation, prevents synkinesis, and restores function.

OBJECTIVES/HYPOTHESIS: Previously, electrical stimulation of denervated canine laryngeal muscle was shown to promote reinnervation by native over foreign motoneurons. The goal of this study was to assess the effect of different stimulus paradigms on reinnervation quality and functional recovery. STUDY DESIGN: A prospective study of six canines over 8 to 20 months. METHODS: A clinical model of laryngeal paralysis was used, where recurrent laryngeal nerves of the animals were sectioned and ventilation compromised. The abductor, posterior cricoarytenoid (PCA) muscles were implanted bilaterally

Local neurotoxins for prevention of laryngeal synkinesis after recurrent laryngeal nerve injury.

OBJECTIVES: Persistent vocal fold motion impairment after recurrent laryngeal nerve (RLN) injury is not characteristically due to absent reinnervation, but often results from spontaneous aberrant reinnervation (synkinesis). We administered local neurotoxins to selected laryngeal muscles after RLN injury to determine whether aberrant reinnervation could be selectively inhibited. METHODS: Unilateral RLN transection was performed in 24 male rats. Three weeks later, the denervated laryngeal adductor complex was injected with phenol, high- or low-dose vincristine sulfate (VNC), or saline solution.

Abducens-oculomotor synkinesis associated with internuclear ophthalmoplegia and acquired abducens nerve palsy.

We report a case of acquired abducens-oculomotor synkinesis, internuclear ophthalmoplegia, and abducens nerve palsy following brainstem hemorrhage. Weakening surgery on the medial rectus muscle did not eliminate the synergistic movement. The mechanism responsible for the abducens-oculomotor synkinesis is discussed.

Depressor Anguli Oris Resection for Postparalysis Facial Synkinesis: Who Stands to Benefit?

BACKGROUND: Several studies report smile improvements after depressor anguli oris (DAO) resection for patients with postparalysis facial synkinesis. However, outcomes postprocedure are variable, and it is difficult to predict which patients will derive the greatest benefit. METHODS: The medical records of all patients who underwent DAO resection from January of 2021 to June of 2022 were retrospectively reviewed. Preprocedure and postprocedure photographs were analyzed using the automated Emotrics software program, and a blinded clinician-graded assessment using the Electronic Facial Paralysis

Electroneurography cannot predict when facial synkinesis develops in patients with facial palsy.

The objective of this study is to clarify when facial palsy patients with lower value of Electroneurography (ENoG) should begin the rehabilitation to prevent the development of facial synkinesis. For this purpose, we examined the relationship between the value of ENoG measured 10-14 days after facial palsy onset and the onset day of the development of oral-ocular synkinesis. Sixteen patients with facial palsy including 11 with Bell's palsy and 5 with Ramsay Hunt syndrome (7 men and 9 women ; 15-73 years old ; mean age, 41.6 years) were enrolled in this study. There was no correlation between E

Arm-diaphragm synkinesis: electrodiagnostic studies of aberrant regeneration of phrenic motor neurons.

Arm-diaphragm synkinesis may occur after injury to the proximal portion of the brachial plexus or cervical nerve roots. Regenerating axons of phrenic motor neurons are misdirected to supply limb muscles. Electrodiagnostic investigations of three patients with symptoms referable to upper brachial plexus or cervical roots revealed motor units in either biceps or triceps muscles that discharged during inspiration. These units did not fire during forced or passive expiration or Valsalva maneuver. Activation of these units could not be achieved by volitional contraction of the appropriate arm muscl

Familial Bell's Palsy: Patient With Facial Synkinesis and Bilateral Recurrence of Facial Palsy.

We present a case of Bell's palsy involving bilateral recurrence in a family history of multiple affected individuals. Bell's palsy, also referred to as facial palsy (FP), has been associated with infectious, ischemic, and familial cluster occurrences, yet its etiology remains largely unclear. Familial clustering, also referred to as familial Bell's palsy, may indicate an underlying anatomical abnormality of the facial canal or an immune-mediated mechanism contributing to a hereditary predisposition. However, this potential link remains understudied. Regardless of the underlying cause, the dia

Kinematic and diffusion tensor imaging definition of familial Marcus Gunn jaw-winking synkinesis.

BACKGROUND: Marcus Gunn jaw-winking synkinesis (MGJWS) is characterized by eyelid ptosis, which disappears during jaw movement. Familial MGJWS is an extremely rare condition. Some authors suggested that MGJWS is due to neural misdirection in the brainstem whereas others suggested that aberrant reinnervation or ephapse may be responsible for synkinetic activity. Pathogenesis of this condition is therefore still unclear. METHODOLOGY/PRINCIPAL FINDINGS: To investigate pathogenetic mechanism in familial MGJWS we performed neurophysiological (EMG, Blink Reflex, Recovery cycle of the R2 component o

Botulinum toxin A treatment in facial palsy synkinesis: a systematic review and meta-analysis.

BACKGROUND: Synkinesis is defined as involuntary movements accompanying by voluntary movements and can occur during the aftermath of peripheral facial palsy, causing functional, aesthetic and psychological problems in the patient. Botulinum toxin A (BTX-A) is frequently used as a safe and effective treatment; however, there is no standardized guideline for the use of BTX-A in synkinesis. The purpose of this article is to review and summarize studies about the BTX-A treatment of synkinesis in patients with a history of peripheral facial palsy; including given dosages, injection sites and time i

A Novel Long-term Therapy of Facial Synkinesis with Botulinum Neurotoxins Type A and Fillers.

Facial synkinesis is the involuntary movement of facial muscles accompanying a voluntary movement. It is a common sequelae to Bell's palsy. The authors present a 50-year-old patient who developed facial synkinesis after Bell's palsy. Pain, discomfort, and facial asymmetry accompanied the condition. She was treated on multiple occasions with neurotoxins and fillers over a period of six years. During this time she reported great improvement in the pain of the affected area, along with an acceptable improvement in the cosmetic appearance of her face. The hope is that in the future, fillers as wel
