Metronome-paced tachypnea test cutoff value for detecting dynamic hyperinflation in COPD: A diagnostic accuracy study.

BACKGROUND: Dynamic hyperinflation (DH) is a key mechanism of exertional dyspnea in COPD, but cardiopulmonary exercise testing (CPET) is not universally available. OBJECTIVE: To determine the diagnostic accuracy and optimal absolute cutoff value of the metronome-paced tachypnea (MPT) test for detecting exercise-induced DH in patients with moderate COPD, using constant-load CPET as the reference standard. METHODS: This diagnostic accuracy study included 18 patients with moderate COPD. Patients underwent pulmonary function testing, symptom-limited incremental CPET, and two paired sessions, 21

Evaluation of the Effects of Lipoxin A4 and Resolvin D1 on the Severity of Transient Tachypnea of the Newborn: A Prospective Study.

Objective: Transient tachypnea of the newborn (TTN) is a common condition observed in neonates. Since its management often requires intensive care and leads to maternal-infant separation, it is a major source of parental concern. The present study aimed to evaluate the effects of lipoxin A4 and resolvin D1 on the clinical course of TTN and to determine whether complete blood count parameters could serve as predictors of disease severity. Materials and Methods: A total of 62 neonates admitted to the neonatal intensive care unit with a diagnosis of TTN were included. According to Silverman scori

Association between thiol-disulfide hemostasis and transient tachypnea of the newborn in late-preterm and term infants.

BACKGROUND: Transient tachypnea of the newborn (TTN), which is the most common respiratory disease in the neonatal period, increases respiratory workload in newborns. We purposed to evaluate the oxidative stress (OS) status and thiol disulfide hemostasis in late preterm and term newborns with TTN in this study. METHODS: The study was carried out in a single-centre neonatal intensive care unit to investigate the effect of continuous airway positive pressure (CPAP) on the oxidative system in newborns with TTN. Thiol (native and total) and disulfide levels, total antioxidant and oxidant status (

Thoracic Fluid Content in Neonates Presented with Respiratory Distress as a Predictive Tool for Transient Tachypnea of Newborn.

INTRODUCTION: Respiratory conditions are the most common reason for admission of newborns to a neonatal care unit. The index of contractility (ICON) can be used to measure the thoracic fluid content (TFC) in neonates which is a significant parameter in cases presented with transient tachypnea of newborn (TTN). OBJECTIVE: The objective was to compare TFC between newborn infants with TTN compared with other causes of respiratory distress (RD). We tested the hypothesis that TFC would be higher in infants with TTN. STUDY DESIGN: In total, 105 newborns were enrolled at the delivery room and were

Assessment of intrahepatic cholestasis in pregnancy and the effect of disease severity on transient tachypnea in the newborn in uncomplicated fetuses.

OBJECTIVES: Considering the effects of bile-acid levels on fetal lungs and pulmonary surfactants, we hypothesized that in the presence of intrahepatic pregnancy cholestasis (ICP), poor neonatal respiratory problems are observed in relation to the severity of the disease. Delivery timing with the presence of ICP is scheduled during late-preterm and early term gestational weeks. The aim of this study was to assess ICP and disease severity effects on transient tachypnea of the newborn (TTN) in uncomplicated fetuses. METHODS: This study comprised 1,097 singleton pregnant women who were separated

Continuous Positive Airway Pressure and the Burden of Care for Transient Tachypnea of the Neonate: Retrospective Cohort Study.

OBJECTIVES: Transient tachypnea of the neonate (TTN) is the commonest neonatal respiratory disorder. Given TTN physiopathology, continuous positive airway pressure (CPAP) could be indicated for its treatment, but no data are available. Our aim is to clarify if CPAP might reduce the TTN burden of care. DESIGN: Retrospective multicenter cohort study enrolling 42 full-term TTN babies treated with CPAP and 40 with oxygen supplementation. RESULTS: CPAP-treated infants show shorter intensive care unit stay (CPAP, 2.5 ± 2 vs. Oxygen, 4.4 ± 2.6 days; adjβ, -2.1 [95% confidence interval (CI): -3.1; -

Fluid handling and blood flow patterns in neonatal respiratory distress syndrome versus transient tachypnea: a pilot study.

BACKGROUND: Cardiovascular and renal adaptation in neonates with Respiratory Distress Syndrome (RDS) and Transient Tachypnea of the Newborn (TTN) may be different. METHODS: Neonates ≥32 weeks were diagnosed with RDS or TTN based on clinical, radiologic and lung sonographic criteria. Weight loss, feeding, urine output, and sodium levels were recorded for the first 3 days, and serial ultrasounds assessed central and organ Doppler blood flow. A linear mixed model was used to compare the two groups. RESULTS: Twenty-one neonates were included, 11 with TTN and 10 with RDS. Those with RDS showed le

A 50-year-old woman with haemoptysis, cough and tachypnea: cholesterol pneumonia accompanying with pulmonary artery hypertension.

Lipoid pneumonia is an uncommon disease caused by the presence of lipid in the alveoli. Here we described a case of a 50-year-old woman with haemoptysis, cough and tachypnea, who was diagnosed with cholesterol pneumonia accompanying with pulmonary artery hypertension. The extremely high pulmonary artery pressure achieved, in this case, is alarming and should alert the physicians that the cholesterol pneumonia may be one of the underlying causes of pulmonary artery hypertension. After a treatment of methylprednisolone, her clinical symptoms were significantly improved, which suggested that ster

Sleep-related stridor due to dystonic vocal cord motion and neurogenic tachypnea/tachycardia in multiple system atrophy.

Sleep-disordered breathing and sleep-related motor phenomena are part of the clinical spectrum of multiple system atrophy (MSA). Stridor has been attributed to denervation of laryngeal muscles or instead to dystonic vocal cord motion. We studied 3 patients with nocturnal stridor in the setting of MSA. All patients underwent nocturnal videopolysomnography (VPSG) with breathing and heart rate, O(2) saturation and intra-esophageal pressure recordings, and simultaneous EMG recordings of the posterior cricoarytenoid, cricothyroid, and thyroarytenoid muscles and continuous vocal cord motion evaluati

Diagnostic performance of lung ultrasound and procalcitonin in differentiating early neonatal pneumonia from transient tachypnea of the newborn.

BACKGROUND: Early neonatal pneumonia and transient tachypnea of the newborn (TTN) often present with overlapping clinical features in late preterm and term neonates, making early etiological differentiation challenging. OBJECTIVE: To evaluate and compare the diagnostic performance of lung ultrasound alone and in combination with serum procalcitonin for distinguishing early neonatal pneumonia from TTN in neonates born at or beyond 34 weeks' gestation. METHODS: This retrospective observational study included neonates ≥ 34 weeks' gestation admitted with respiratory distress within the first 72

New Risk Factors for Transient Tachypnea of the Newborn and Childhood Asthma: A Study of Clinical Data and a Survey of Parents.

OBJECTIVES: It is established that transient tachypnea of the newborn (TTN) is associated with an increased risk of early childhood asthma. However, the question remains whether both asthma and TTN have common risk factors as well as the same underlying etiology. This study aims to determine possible risk factors for TTN as well as early childhood asthma. METHODS: This study was carried out in two phases. While the first phase included medical records of 1318 newborns, the second phase consisted of a phone survey. RESULTS: Elective cesarean section (ECS), maternal asthma, gestational age (GA
