Valproic acid sensitizes human glioma cells for temozolomide and γ-radiation.

Temozolomide (TMZ) is given in addition to radiotherapy in glioma patients, but its interaction with the commonly prescribed antiepileptic drug valproic acid (VPA) is largely unknown. Induction of DNA demethylation by VPA could potentially induce expression of the O(6)-methylguanine-DNA-methyltransferase (MGMT) protein, causing resistance to TMZ and thereby antagonizing its effect. Therefore, this study investigates the interaction between VPA, TMZ, and γ-radiation. Two glioma cell lines were used that differ in TMZ sensitivity caused by the absence (D384) or presence (T98) of the MGMT protein

Selective lymphopenia and opportunistic infections in neuroendocrine tumor patients receiving temozolomide.

Temozolomide is utilized as a treatment for a variety of solid tumors and has been associated with the development of selective lymphopenia. We evaluated the incidence of lymphopenia and opportunistic infections during treatment and up to 12 months following treatment discontinuation in a cohort of 39 patients receiving temozolomide for advanced neuroendocrine tumors. The incidence of Grade 3-4 lymphopenia was 46 percent after 4 months of therapy and remained at 30 percent or greater for 12 months following treatment discontinuation. The overall incidence of opportunistic infections was 10 per

Phase I trial of temozolomide using an extended continuous oral schedule.

Temozolomide, a methylating imidazotetrazinone, has antitumor activity against gliomas, malignant melanoma, and mycosis fungoides and is presently administered as a 5-day oral schedule every 4 weeks. This Phase I study aimed to determine the maximum tolerated dose of temozolomide administered as a single oral daily dose for a continuous 6- or 7-week period, evaluate the plasma pharmacokinetics on this schedule, and compare total plasma exposure over 7 weeks with the conventional 5-day regimen. Twenty-four patients with varying tumor types (17 of 24 gliomas) received temozolomide. All had clini

CD13 identifies S100A4+ immunosuppressive monocytes and predicts temozolomide benefit in glioblastoma.

BACKGROUND: Glioblastoma (GBM) is an immunologically "cold" tumor in which S100A4-driven immunosuppressive myeloid cells promote progression and resistance. However, the specific immune subpopulation that intrinsically expresses S100A4 and the corresponding cell surface targets remain poorly defined. MATERIALS AND METHODS: S100A4 expression was profiled across immune subsets in healthy human bone marrow and peripheral blood by flow cytometry. TCGA GBM datasets were used to identify S100A4-correlated surface markers, and CD13 (encoded by ANPEP) was validated across the Human Protein Atlas, Imm

Evaluating temozolomide for pediatric adamantinomatous craniopharyngiomas using microspheroid-based drug screening.

PURPOSE: Pediatric adamantinomatous craniopharyngioma (ACP) is the most common tumor of the diencephalic-pituitary axis in children. Although histologically benign, pediatric ACP is frequently associated with substantial endocrine dysfunction and neurological complications. The gold-standard treatment is complete surgical resection; however, because of the tumor's location, total removal is often not feasible. Consequently, several authors recommend radiotherapy as an adjuvant option. Nevertheless, partial resections are frequently followed by recurrence, and repeated surgical interventions in

β-Lapachone sensitizes glioblastoma to Temozolomide by inhibiting NF-κB signaling.

Temozolomide (TMZ) constitutes the first-line chemotherapeutic regimen for glioblastoma (GBM), yet its clinical efficacy remains suboptimal. β-lapachone, a naturally occurring NQO1 inhibitor, has demonstrated potent antitumor activity across multiple malignancies. Based on the critical involvement of nuclear factor-κB (NF-κB) in GBM pathogenesis, this study investigated whether β-lapachone can enhance the antitumor effect of TMZ by targeting the NF-κB pathway using both in vitro and in vivo models. We found that β-lapachone acts synergistically with TMZ to inhibit GBM cell proliferation, epith

Case Report: Temozolomide Treatment of Refractory Prolactinoma Resistant to Dopamine Agonists.

Therapeutic agents for refractory prolactinomas that are resistant to dopamine agonists (DAs) are troublesome, and surgery often only removes a large part of the tumor without complete remission. Among the various second-line treatment regimens, the treatment effect of the alkylating agent temozolomide (TMZ) is only effective for approximately half of patients; however, complete remission is rare. Here we report a patient with prolactinoma who was resistant to high-dose cabergoline (CAB) treatment, demonstrating a continuous increase in both the tumor volume and the prolactin (PRL) level. Antitumor action of temozolomide, ritonavir and aprepitant against human glioma cells.

In the effort to find better treatments for glioblastoma we tested several currently marketed non-chemotherapy drugs for their ability to enhance the standard cytotoxic drug currently used to treat glioblastoma- temozolomide. We tested four antiviral drugs- acyclovir, cidofovir, maraviroc, ritonavir, and an anti-emetic, aprepitant. We found no cytotoxicity of cidofovir and discussed possible reasons for discrepancy from previous findings of others. We also found no cytotoxicity from acyclovir or maraviroc also in contradistinction to predictions. Cytotoxicity to glioma cell line GAMG for temoz

Treatment-related myelodysplastic syndrome after temozolomide for recurrent high-grade glioma.

In patients with recurrent malignant glioma, treatment-related myelodysplastic syndrome (t-MDS) and acute leukemia are rare adverse effects because the median survival after relapse is limited. We report a 44-year-old woman with t-MDS (refractory anemia with excess blasts) following treatment of recurrent anaplastic astrocytoma with temozolomide (TMZ). A cytogenetic study showed del (3)(q11.1). MDS was diagnosed 8.4 months after beginning TMZ. The disease rapidly evolved into acute leukemia within 1 month after the onset of MDS, and the patient died 1 month later during induction chemotherapy.

Lactate-associated H3K18 lactylation and SYT8 in the temozolomide response of glioblastoma cells.

Lactate and histone H3K18 lactylation (H3K18la) have been linked to glioblastoma biology. We examined their relationship with synaptotagmin 8 (SYT8) and the response to temozolomide (TMZ). U251MG and U87MG cells were cultured under normoxia or hypoxia and exposed to TMZ, sodium lactate or both. H3K18la enrichment at the SYT8 promoter was assessed by chromatin immunoprecipitation. The effects of SYT8 knockdown were evaluated using CCK-8, colony formation, flow cytometry, wound healing and Transwell assays. Lactate and protein abundance were measured, including H3K18la normalized to total H3 in

EGFR inhibition down-regulates MGMT and enhances responsiveness to temozolomide in glioblastoma.

Glioblastoma (GBM) is a devastating cancer with a dismal prognosis. Current treatment includes temozolomide (TMZ), which is more effective in about 50% of GBMs that have O6-methylguanine DNA methyltransferase (MGMT) promoter methylation. MGMT is a DNA repair protein that reverses TMZ-induced DNA damage. EGFR is a prime oncogene in GBM. Here, we report that EGFR inhibition induced the down-regulation of MGMT in GBM cells, revealing a previously unidentified link between EGFR signaling and response to TMZ. EGFR inhibition led to activation of two transcription factors, activator protein-1 (AP-1)

Prolonged sequential temozolomide in glioblastoma: A systematic review with exploratory quantitative synthesis.

BACKGROUND: In patients with glioblastoma (GBM), the optimal number of adjuvant temozolomide (TMZ) cycles following combined radio-chemotherapy remains uncertain. This study aimed to synthesize the available randomized evidence addressing treatment duration. METHODS: Following PRISMA 2020 recommendations, we conducted a systematic review with exploratory quantitative synthesis of prospective randomized trials in newly diagnosed GBM. Studies were critically appraised with particular attention to survivorship bias, post-randomization selection, molecular heterogeneity, and trial design. Quantit

Aberrant PLAC8 expression characterizes glioblastoma with temozolomide resistance and an immunosuppressive microenvironment.

Glioblastoma (GBM), Isocitrate Dehydrogenase-wildtype (IDH-WT) represents the most prevalent and clinically aggressive subtype of adult diffuse gliomas, typically associated with poor prognosis. Temozolomide (TMZ) remains the first-line chemotherapeutic agent for GBM; however, the emergence of TMZ resistance represents a major therapeutic obstacle in clinical practice. This study identifies placenta-specific 8 (PLAC8) as a novel mediator of TMZ resistance in IDH-WT GBM. Elevated PLAC8 expression was strongly correlated with poorer survival rates, higher tumor grades in glioma, establishing it

Albumin-Based Microneedles for Spatiotemporal Delivery of Temozolomide and Niclosamide to Resistant Glioblastoma.

Glioblastoma (GBM) is the most common and aggressive malignant brain tumor. Standard therapy includes maximal surgical resection, radiotherapy, and adjuvant temozolomide (TMZ) administration. However, the rapid development of TMZ resistance and the impermeability of the blood-brain barrier (BBB) significantly hinder the therapeutic efficacy. Herein, we developed spatiotemporally controlled microneedle patches (BMNs) loaded with TMZ and niclosamide (NIC) to overcome GBM resistance. We found that hyaluronic acid (HA) increased the viscosity of bovine serum albumin (BSA) and evidenced that concen

Treatment of Extraneural Metastases of Myxopapillary Ependymomas With Dose-Dense Temozolomide and Lapatinib.

Myxopapillary ependymomas (MPEs) are rare tumors of the central nervous system, and outcomes are generally worse with recurrent disease. These tumors can rarely metastasize outside the neuraxis. We present a case of a 35-year-old female with a history of MPEs who developed extraneural metastases 11 years after her initial gross total resection. Sites of metastases included multiple bilateral intrapulmonary and pleural-based masses with pleural effusion and a pelvic mass. The patient was treated with dose-dense TMZ and lapatinib and had a mixed radiographic response after 12 cycles of treatment
