An Integrative Model Accounting for the Symptom Cluster Triggered After an Acoustic Shock.

Acoustic shocks and traumas sometimes result in a cluster of debilitating symptoms, including tinnitus, hyperacusis, ear fullness and tension, dizziness, and pain in and outside the ear. The mechanisms underlying this large variety of symptoms remain elusive. In this article, we elaborate on the hypothesis that the tensor tympani muscle (TTM), the trigeminal nerve (TGN), and the trigeminal cervical complex (TCC) play a central role in generating these symptoms. We argue that TTM overuse (due to the acoustic shock), TTM overload (due to muscle tension), and ultimately, TTM injury (due to hypoxi

High-fidelity, inexpensive surgical middle ear simulator.

HYPOTHESIS: A high-fidelity, inexpensive middle ear simulator could be created to enhance surgical training that would be rated as having high face validity by experts. BACKGROUND: With rapid prototyping using additive manufacturing technology (AMT), one can create high-resolution 3-dimensional replicas of the middle ear at low cost and high fidelity. Such a simulator could be of great benefit for surgical training, particularly in light of new resident training guidelines. METHODS: AMT was used to create surgical middle ear simulator (SMS) with 2 different materials simulating bone and soft

Clinical behavior and pathogenesis of secondary acquired cholesteatoma with a tympanic membrane perforation.

CONCLUSION: The pathogenesis of secondary acquired cholesteatoma with a tympanic membrane (TM) perforation is very different from that of other types of cholesteatoma. This disease should be clearly categorized as a different type of cholesteatoma. OBJECTIVE: Primary cholesteatoma generally arises from retraction of the squamous epithelium of the TM. However, in rare cases, epithelial invasion occurs from the edge of the TM perforation and migrates to the medial surface of the TM and middle ear cavity. The aim of this study was to evaluate the clinical features of secondary acquired cholestea

Tic-Like Repetitive Ear-Popping Behavior With Objective Eustachian Tube Opening in an Adult With Tourette Syndrome.

Tourette syndrome presents with diverse motor and vocal tics, yet involvement of Eustachian tube dynamics has not been previously described. We report the case of a 31-year-old woman who developed an unusual symptom she described as an "ear-popping tic," characterized by a strong premonitory urge to release air from the ears and clicking sounds. Her long clinical history included childhood-onset tics, emotional instability, psychiatric hospitalizations, and fluctuating tic severity. At age 31, she reported repetitive ear-popping sensations accompanied by voluntary suppression to some extent. E

[Direct observation of the tympanic cavity by superfine fiberscopy through the eustachian tube].

It was impossible to observe directly the tympanic cavity with the ear drum intact. The superfine fiberscope, developed by the Department of Otolaryngology, Tokyo Medical College, makes it possible to observe the tympanic cavity through the eustachian tube without any surgical intervention such as myringotomy. This fiberscope is flexible and very fine, having an outer diameter of 0.6-0.8mm. Using this new fiberscope, the following structures can be observed: the handle of the malleus, the long process of the incus, the incudostapedial joint, the head of the stapes, the crus of the stapes, the

Inner ear basic fibroblast growth factor in CBA/J, C3H/HeJ, and autoimmune Palmerston north mice.

Basic fibroblast growth factor (bFGF) has a mitogenic effect on fibroblasts and osteoblasts for matrix proliferation and on endothelial cells for neovascularization. Because otic capsule osteogenesis in autoimmune disease subjects often involves abnormal matrix and vascular changes, bFGF may serve as a potential mediator for such bone disorders. To investigate this relationship, bFGF was evaluated in the Palmerston North autoimmune strain mouse, which develops otic capsule sclerotic lesions during the progression of its systemic disease. Inner ears from PN mice, along with control CBA/J and C3

The acoustic middle ear muscle reflex in albino rats.

The acoustic middle ear muscle reflex was studied in albino rats anesthetized with chloralose. The best frequency of the reflex and the threshold at this frequency were on average about 3 kHz and 57 dB SPL, respectively. The threshold increased as frequency increased above, and decreased below, the best frequency at a rate of about 20 dB/octave. Above about 12 kHz, the muscular response showed instability and habituation. Thresholds were similar between stapedius and tensor tympani reflexes and between ipsilateral and contralateral reflexes. The middle ear transmission loss due to the reflex w

Inflammatory chronic otitis media and the anterior epitympanic recess.

OBJECTIVE: To demonstrate the important role of the anterior epitympanic recess (AER) in the surgery of noncholesteatomateous chronic inflammatory middle ear disorders. To establish selective criteria as to the indication of surgical intervention on the AER, aiming to create a permanent anterior aeration pathway for the attic. In addition, to point out the mandatory role of preoperative temporal bone computed tomography (CT) demonstrating whether the AER is involved and thus contributing, within the clinical context, to the indication for this surgery and its appropriate approach. Adam Politzer-Father of Modern Otology.

Adam Politzer (1835-1920) was born in Alberti near the city of Budapest in Hungary. He studied medicine at the University of Vienna and obtained his Doctorate degree in 1859. Some of his teachers belonged to the famous second "Vienna School" such as Joseph Skoda, Karl Rokitansky, Von Hebra, Josef Hyrtil, Johann Von Oppolzer and famous physiologist Carl Ludwig -who took special interest in him and was influential in his subsequent career. Politzer showed unusual interest in diseases of the ear and started to work in Carl Ludwig's laboratory. His interest at that time was mainly the physics of t

Histological study of the external, middle and inner ear of horses.

Clinical, anatomical and histological aspects of the equine acoustic organ have been poorly investigated and illustrated in literature so far. It is understood that an intact acoustic organ and hearing function are of vital importance for the well-being of flight animals like horses. The knowledge of the acoustic organ is usually transferred analogously from other mammals to horses. The purpose of this study was to provide a detailed and complete histological description of the healthy equine auditory organ, and to determine its congruity to other mammalians. Anatomical dissections and histolo

Effect of central depressants on the acoustic middle ear reflex in rabbit. A method for quantitative measurements of drug effect on the CNS.

The respective effects of pentobarbital-sodium, enibumal-sodium, urethane, urethane-chloralose and lidocaine on the function of the acoustic middle ear reflex in the rabbit were studied. The response of the middle ear muscles was measured by recording changes in both ears' acoustic impedance when the reflex was elicited by applying pure tone stimuli (2,000 Hz) to the two ears one at a time. In that way both the crossed and the uncrossed reflexes were studied. All the drugs were found to depress the reflex in such a way that a higher sound intensity was required after administration to achieve

A comparative morphologic and morphometric study about geniculate ganglion development in man and in rat.

CONCLUSIONS: Human-rat geniculate ganglion (GG) have multiple origins: (1) An initial proximity (20 μm) to the endocranial foramen of the IAM, suggests neural crest induction; and (2) The influence of epibranchial placodes: the tensor tympani muscle (TTM) and the otic apical coil. OBJECTIVES: This study was undertaken to determine the comparative development of human-rat GG. MATERIALS AND METHODS: A light microscopic study of the GG in human material obtained from spontaneous abortions at 9, 13, 14, 17, 18, and 30 weeks, and one neonate was done. This study examined Webster rat embryos and a

Anatomical and morphometrical approaches on the auditory ossicles in European ground squirrel (Spermophilus citellus).

INTRODUCTION: The study of the middle ear and especially the ossicular chain in different species can bring interesting insights into the biomechanics of hearing. The ground squirrel's middle ear anatomy has not been studied as thoroughly as its other systems and organs. METHODS: Our study describes the ear ossicles in this little-studied species, providing the morphological and morphometrical characteristics and a series of comparative data, in an attempt to characterise also some functional anatomy of the incus, malleus and stapes. RESULTS AND DISCUSSION: The malleus comprises the typical

[Measurement of facial nerve blood flow in rabbits using laser Doppler flowmetry].

OBJECTIVE: To study the microcirculation disorder of the facial nerve entrapment in rabbits. METHODS: Laser Doppler Flowmetry (LDF) was used in the blood flow measurement of the horizontal segment of the facial nerve and other adjacent structures in 32 New Zealand White rabbits. The repeated test was used to evaluate the stability of LDF. RESULTS: An animal model was established to study the microcirculation of the facial nerve in rabbits. The record of repeat test showed that LDF is a stable method. LDF of the horizontal segment of the facial nerve is higher in the proximal end but lower in

Human efferent adaptation of DPOAEs in the L1,L2 space.

The adaptive properties of distortion product otoacoustic emissions (DPOAEs) at 2f(1)-f2 were investigated in 12 ears of normally hearing adults aged 18-30 years using long-lasting 1-s primary-tone on-times. In this manner, DPOAE adaptation at a single f2 of 1.55 kHz (f2/f1=1.21) was evaluated as a function of the levels of the primary tones in a matrix of L1, L2 settings, which varied from 45 to 80 dB SPL, in 5-dB steps. DPOAEs were elicited under both monaural and binaural stimulus-presentation conditions. Adaptation was defined as the difference in DPOAE levels between the initial 92-ms bas

["Irritative" pathology of the external auditory canal and decreased sound sensation].

We have not found in the literature an explanation for the intermittent and transient decreased lesser hearing sensation in patients with dysesthesia of the external auditory canal (EAC). In this paper we offer a possible explanation for it. Our hypothesis is that the stimulation of the sensory fibers of the trigeminal and facial nerves in the EAC is able to increase the stiffness of the ossicular chain by means of a reflex stimulation of malleus and stapes muscles. This intermittent and transient increase of the ossicular stiffness could explain the intermittent and transient decrease of hear

Vascularization of the long process of the malleus: surgical implications.

Knowledge of the vascularization of the malleus may help to better understand some unwanted effects occurred in those surgical techniques using the handle as an attachment site of ossicle-replacement prostheses. Our hypothesis is that vascular damage occurred in the long process of the malleus during tympanic membrane detachment before fastening a malleovestibulopexy (MVP) prosthesis promotes the lysis of this ossicle at the attachment site. To confirm this premise, we carried out an evolutive and descriptive study of ten MVP procedures as well as a morphological analysis of ten cadaveric mall
