Treatment of urticaria and angioedema: low-sedating H1-type antihistamines.

H1-type antihistamines are considered the therapeutic agents of choice for treating urticaria and angioedema. The use of traditional H1 antihistamines is limited by their side effects. In recent years low-sedating H1 antihistamines with reduced sedative and anticholinergic side effects have become popular choices for the treatment of urticaria and angioedema. Terfenadine and astemizole are currently available in the United States, and cetirizine and loratadine, currently under review at the Food and Drug Administration, are available in other countries. Terfenadine, cetirizine, and loratadine

Effects of H1- and H2-antihistamines on platelet-activating factor and bradykinin-induced inflammatory responses in human skin.

Previous studies show that oral antihistamines affect the weal and flare response to intradermal injections of the inflammatory mediators platelet-activating factor (PAF) and bradykinin (BK). The aim of this study was to compare the effects of terfenadine (an H1-antagonist) and cimetidine (an H2-antagonist) on weal and flare responses to PAF and BK in healthy non-atopic human volunteers. The effects of doxepin on PAF responses were investigated, as there is evidence that doxepin may have direct anti-PAF effects in addition to its known antihistaminic actions. Terfenadine significantly reduced

hERG block potencies for 5 positive control drugs obtained per ICH E14/S7B Q&As best practices: Impact of recording temperature and drug loss.

According to the ICH S7B guideline, drug candidates are screened for hERG block prior to first-in-human testing to predict the likelihood of delayed repolarization associated with a rare, but life-threatening, ventricular tachyarrhythmia. The new ICH E14 Q&As guideline allows hERG results to be used in later clinical development for decision-making (Q&As 5.1 and 6.1). To pursue this path, the hERG assay should be conducted following the new ICH S7B Q&A 2.1 guideline, which calls for best practice considerations of the recording temperature, voltage protocol, stimulation frequency, recording/da

Characterization of antihistamines using biphasic cutaneous reaction in BALB/c mice.

Effects of 11 histamine H1 receptor antagonists on IgE-mediated biphasic cutaneous reaction in mice were examined. The immediate phase reaction (IPR) assessed at 1 hour after antigen application was significantly inhibited by all antihistamines examined. The inhibition of IPR by cetirizine and mequitazine were potent, but those by cyproheptadine and diphenhydramine were weak. The later phase reaction (LPR) assessed at 24 hours after antigen application was inhibited by chlorpheniramine, oxatomide, ketotifen, mequitazine, emedastine, terfenadine and azelastine. The inhibition of LPR by emedasti

Daytime sleepiness and antihistamines.

A daytime nap procedure was used to evaluate the daytime sleepiness associated with antihistamines, as well as to assess their hypnotic potential. Healthy, normal subjects received diphenhydramine (150 mg), terfenadine (120 mg), and placebo and went to bed at 900, 1100, 2000, and 2200 h with the instruction to try to fall asleep. The remained in bed for 60 min while standard sleep recordings were made. Across all conditions latency to stage 1 sleep increased significantly from nap 1 to nap 4 and the amount of sleep (all nonstage 1 sleep) decreased significantly. Over the four naps the mean lat

An oxygen-permeable, stirrer-pump-integrated liver-heart microphysiological system enables time-resolved analysis in PK-PD relationships of metabolism-dependent cardiotoxicity.

Predicting metabolism-dependent drug toxicity encompassing both the bioactivation and detoxification processes remains a critical challenge in preclinical safety evaluations. Conventional cardiomyocyte-only assays frequently misclassify compounds such as terfenadine, which require hepatic metabolism to attenuate cardiotoxicity. Current microphysiological systems (MPS) have practical limitations, such as insufficient hepatocyte oxygenation and complex external fluidic setups. We developed a liver-heart co-culture system that integrates three-dimensional human iPS cell-derived engineered heart t

Pharmacological assessment of zebrafish-based cardiotoxicity models.

Cardiotoxicity remains the most common reason for failure during drug development. Recently, the zebrafish (Danio rerio) model has emerged for the evaluation of drug-dependent cardiotoxicity and for the identification of cardioprotective molecules. However, it remains unknown how closely the zebrafish-based results may be translated to humans. To tackle this issue, we established embryonic zebrafish models of doxorubicin-, adrenaline- and terfenadine-induced cardiotoxicity with unified dosing regimen which eventually enabled head-to-head comparison of the drugs. Subsequently, we determined whe

Effects of nonsedating histamine H1-antagonists on EEG activity and behavior in the cat.

The central effects of the newly-developed antihistamines (H1-receptor antagonists) loratadine, astemizole, mequitazine and terfenadine were evaluated by studying brain electrical activity (EEG), sleep-waking patterns and behavior in the cat. The different stages of the sleep-waking cycle, i.e., wakefulness (W), spindle sleep (SS), slow wave sleep (SWS) and REM sleep (REM) were evaluated. The power spectrum analysis of the EEG was obtained by a computerized technique. For comparison, the sedating agent diphenhydramine was examined. Given at 3 mg/kg orally, a dose slightly above that effective

Blockade of cardiac potassium and other channels by antihistamines.

The use of terfenadine and astemizole, two long-acting nonsedating histamine H1 receptor antagonists, has been associated with prolongation of the QT interval, development of ventricular arrhythmias, particularly torsade de pointes, and sudden cardiac death. Both drugs block the rapidly activating component of the delayed rectifier channel, I(Kr). At much higher concentrations, they also block several other cardiac channels (Na+, Ca2+, K+). Since many other antihistamines can also block one or other of the cardiac ion currents (e.g. loratadine blocks the human cardiac K+ channel, hKv1.5, with

Hay fever treatment with combined antihistamine and cyclooxygenase-inhibiting drugs.

Twenty-eight ragweed-allergic patients with hay fever participated in a clinical trial that examined the pattern of symptom relief resulting from addition of a cyclooxygenase-inhibiting drug to standard antihistamine therapy. In the first week antihistamine use by the subjects was standardized, and subjects were oriented to use of the hay fever symptom diary. They were then allocated, according to symptom severity, to two treatment groups. One group received 120 mg of terfenadine and 300 mg of flurbiprofen (a cyclooxygenase-inhibiting drug) per day. Members of the other group received terfenad

Inhibitory effects of the methanol extract of Ganoderma lucidum on mosquito allergy-induced itch-associated responses in mice.

Recently, we showed that a methanol extract of Ganoderma lucidum inhibits scratching, an itch-related response, induced by intradermal injections of some pruritogens in mice. The present study investigated whether G. lucidum extract would inhibit allergic itch. In mice sensitized with an extract of salivary gland of mosquito (ESGM), an intradermal injection of ESGM elicited scratching, which was suppressed by oral administration of G. lucidum extract (100 and 300 mg/kg). The scratching was inhibited by the H₁ histamine-receptor antagonist azelastine, but not by the peripherally acting H₁-antag

Acute antihistamine ingestion does not affect muscle strength and endurance.

Twelve subjects (three females, nine male, age 32 +/- 1.3 yr) performed tests of isokinetic muscle strength and endurance under each of three treatment conditions. The three conditions, administered in double-blind, randomized order, were single oral administrations of placebo, diphenhydramine (50 mg), and terfenadine (60 mg). Tests, separated by 1 wk, began 2 h after ingestion of the drug or a placebo. Subjects performed a velocity spectrum test (VST) consisting of peak torque measurements at five velocities (90, 150, 210, 270, and 330 degrees.s-1) and a 45 s muscle endurance test (MET) at a

Effects of antihistamines on inflammatory mediators.

The nasal antigen challenge model has proved useful in assessing the roles of inflammatory mediators in the clinical allergic response. Studies using this model have revealed that the acute allergic response is associated with increased concentrations of histamine, prostaglandin D2 (PGD2), leukotrienes, tryptase, and kinins, and with increased TAME-esterase activity. The effects of antihistamines on the clinical response and inhibition of mediator release have also been examined with this model. Premedication with terfenadine caused a marked reduction in sneezing as well as decreased histamine

Telemetered common marmosets is useful for the assessment of electrocardiogram parameters changes induced by multiple cardiac ion channel inhibitors.

The objective of this study is to assess the response of telemetered common marmosets to multiple cardiac ion channel inhibitors and to clarify the usefulness of this animal model in evaluating the effects of drug candidates on electrocardiogram (ECG). Six multiple cardiac ion channel inhibitors (sotalol, astemizole, flecainide, quinidine, verapamil and terfenadine) were orally administered to telemetered common marmosets and changes in QTc, PR interval and QRS duration were evaluated. Drugs plasma levels were determined to compare the sensitivity in common marmosets to that in humans. QTc pro

Antihistamines: topical vs oral administration.

The pathogenesis of allergic rhinitis is complex, involving not only histamine and mast cell-derived tryptase, but also eosinophil- and neutrophil-derived mediators, cytokines, and intercellular cell adhesion molecules (ICAM-1). It is surprising that antihistamines, which block only one component of the process, have proved so effective in the management of allergic rhinitis. Research has therefore focused on whether antihistamines have additional pharmacological activities. In vitro studies have shown that high concentrations of second generation antihistamines can block inflammatory mediator

Characterization of daytime sleepiness and psychomotor performance following H1 receptor antagonists.

BACKGROUND: While first generation H1-receptor antagonists are widely used, there are relatively few data describing their comparative effects on subjective daytime sleepiness and psychomotor performance. OBJECTIVE: To compare the effects of first generation H1 receptor antagonists on subjective daytime sleepiness and psychomotor performance. METHODS: We conducted two single-dose, cross-over studies. In the first, we validated our methodology in 18 healthy subjects by examining the response to diphenhydramine (50 mg), terfenadine (60 mg), and placebo. In the second trial, we evaluated the re

Effects of anti-inflammatory drugs upon nitrate and myeloperoxidase levels in the mouse pleurisy induced by carrageenan.

The effects of several drugs (terfenadine, bradykinin B2 receptor antagonists: HOE 140, NPC 17731, diacerein, indomethacin, meloxicam, nabumetone, and dexamethasone) upon myeloperoxidase and nitrate levels were analyzed in an inflammation model characterized by biphasic peaks (4 and 48 h) of cell migration and of fluid leakage. Myeloperoxidase levels were significantly higher only in the first phase (4 h; median and range; 537.5; 323.6-683.7 mU/ml; P < 0.01), whereas increased mean nitrate levels were detected in both phases (4 h: 19.0; 6.2-32 microM and 48 h: 13.7; 8.9-17.8 microM; P < 0.01).

Improving prediction of torsadogenic risk in the CiPA in silico model by appropriately accounting for clinical exposure.

Any adverse event is reliant on three properties: the appropriate pharmacology to trigger the event, the appropriate exposure of compound, and intrinsic patient factors. Each alone is necessary but insufficient to predict the event. The Comprehensive in vitro Proarrhythmia Assessment (CiPA) initiative attempts to predict the risk of torsade de pointes (TdP) by focusing on an in-silico model with thresholds determined at modest multiples of the therapeutic exposure for the parent molecule. This emphasizes the pharmacologic properties necessary for TdP but does not account for situations where c

Interferences of the autonomic nervous system with drug induced QT prolongation: a point to consider in non-clinical safety studies.

INTRODUCTION: QT interval assessment by telemetry has become one of the most useful models in testing strategies adopted for detection of drug induced QT prolongation in non-clinical safety pharmacology studies. This study reports experimental data showing that the autonomic nervous system might influence drug induced QT prolongation. METHODS: Animals were instrumented with telemetric transmitters and epicardial ECG leads. Effects on QT interval of reference drugs such as thioridazine and terfenadine were analysed with different approaches, the Holzgrefe's probabilistic method, the QT shift m
