Theophylline disposition in patients with hepatic cirrhosis.

To determine the role of liver dysfunction in theophylline toxicity, we administered single intravenous doses of the drug to nine patients with cirrhosis and observed its disposition over a period of 24 to 48 hours. As compared to 19 normal subjects, these patients had a prolonged plasma half-life (mean, 25.6 vs. 6.7 hours) and a decreased plasma clearance (mean, 0.042 vs. 0.062 liter[kg-1]hr-1). Volumes of distribution of theophylline in the cirrhotic patients (central-compartment volume of 0.330, and steady-state volume of distribution of 0.785 liter per kilogram) did not substantially diffe

Advanced aptamer-based sensors for monitoring theophylline.

Theophylline is a widely used bronchodilator for treating asthma-related symptoms like shortness of breath and chest tightness. However, its narrow therapeutic plasma range (20-200 μM) necessitates careful monitoring of blood levels to prevent toxicity. Various clinical laboratory techniques have been developed for detecting theophylline, including spectroscopy, high-performance liquid chromatography (HPLC), fluorescence polarization immunoassay, and radioimmunoassay. Despite their utility, these methods are limited by complex sample preparation, long processing times, large sample volumes, an

Theophylline for Spinal Cord Injury Associated Bradycardia.

Background: Spinal cord injury (SCI) related bradycardia occurs frequently in patients with high cervical spine injuries. In patients with SCI-related symptomatic bradycardia, a variety of agents have been used to improve heart rate and reduce the need for vasopressor therapy. The literature concerning the use of theophylline in this disease state is sparse. Objective: The primary objective of this study was to evaluate and describe the use of theophylline for SCI-related symptomatic bradycardia. Methods: This was a retrospective case series of patients with SCI-related symptomatic bradycardia

Severe amnestic syndrome due to theophylline toxicity.

We report a case of severe amnestic syndrome following theophylline overdose. A woman in her early 30s was admitted to hospital where she developed status epilepticus following an intentional overdose of theophylline and lansoprazole. She developed a profound acidosis and required intubation in the intensive care unit. Following extubation the patient was noted to have a severe amnestic syndrome with poor short-term memory. A work-up to exclude infectious, autoimmune and paraneoplastic causes for encephalitis was undertaken. Cerebrospinal fluid analysis was normal and autoimmune encephalitis t

Neonatal theophylline intoxication: pharmacokinetics and clinical evaluation.

The pharmacokinetics of theophylline and its metabolites, caffeine, 1,3-dimethyluric acid, 1-methyluric acid and 3-methylxanthine were studied in a 5-day old 1.3 kg premature neonate who accidentally received 180 mg theophylline in 26 h during treatment for bradycardia. Tachycardia, hyperventilation, increased diuresis, central nervous system excitation, an increase in blood glucose concentrations followed by a prolonged decrease and hypercalcaemia were the predominant clinical and laboratory manifestations. The patient responded to supportive care and survived without sequelae. The concentrat

Massive theophylline overdose with atypical metabolic abnormalities.

We report a case of fatal theophylline overdose in a 16-year-old asthmatic boy who presented with seizures, respiratory arrest, and a theophylline concentration of 117 mg/L in serum. His hospital course was complicated by refractory hypotension and severe ischemic necrosis of skeletal muscle, bowel, and liver. The metabolic abnormalities observed early in his hospital course included severe hyperkalemia, hyperphosphatemia, hypermagnesemia, hypocalcemia, and profound metabolic acidosis. These metabolic abnormalities differ from those previously reported in cases of massive theophylline overdose

Adverse effects of theophylline-beta agonist interactions.

Beta-adrenergic agonists, when administered alone or concomitantly with methylxanthines are capable of producing lesions of myocardial necrosis and an increased incidence of cardiac arrhythmias in several species of animals. These drugs, including "beta 2-specific" agonists, also have the potential to produce adverse cardiovascular effects when used clinically. In addition, beta agonists and methylxanthines can interact to potentiate or lessen effectiveness and theoretically increase the risk to the patient. As a result, the extent to which the concomitant administration of these drugs may con

Theophylline-induced seizures: clinical and pathophysiologic aspects.

The clinical features and management of theophylline-induced seizures are not well appreciated in spite of their unique aspects. These seizures tend to occur in neurologically intact patients and leave no or only minor neurologic sequelae if controlled early. They begin with focal motor seizures with or without secondary generalization and are followed by stupor or coma. They are responsive only to adjustment of theophylline dosage. Should the motor phenomenon persist, it takes the form of epilepsia partialis continua. Extensive workup for a structural brain lesion may be unrewarding. The elec

Theophylline magnesium salicylate, a new xanthine compound.

Interaction of 1 mole of magnesium salicylate and 2 moles of theophylline in water precipitated a crystalline compound, identified as theophylline magnesium salicylate pentahydrate from analytical and supportive physicochemical data. Similarly, barium salicylate and theophylline produced theophylline barium salicylate. No precipitates were formed with calcium salicylate or strontium salicylate under the same conditions. Theophylline magnesium salicylate is not a mixture of components and differs in composition from the known theophylline calcium salicylate dihydrate. Unlike the latter compound

First-line therapy for theophylline-associated seizures.

Theophylline-associated seizures (TAS) are considered a neurologic emergency, as they can sometimes be intractable and difficult to stop with standard treatments such as intravenous administration of diazepam. As a consequence, a proportion of patients who experience status epilepticus while receiving theophylline will require endotracheal intubation. The optimal first-line therapy for TAS has not yet been fully investigated. We compared 54 cases of TAS with 779 cases of non-TAS, that had presented at a single institution between 1991 and 2002. Among the 54 cases of TAS, 36 experienced general

Metabolic abnormalities associated with intentional theophylline overdose.

To verify the frequency and severity of metabolic abnormalities occurring as a result of intentional theophylline overdose, we retrospectively analyzed admission laboratory data from 22 cases of theophylline poisoning. Hypokalemia, hyperglycemia, and leukocytosis were frequent findings, and the magnitude of these abnormalities was related to the corresponding serum theophylline level. Although measurements were available for a smaller number of patients, respiratory alkalosis, hypophosphatemia, and hypomagnesemia were also common findings. Given the potential contribution of these abnormalitie

Theophylline and Caffeine as Alternatives During an Aminophylline Shortage.

The latest aminophylline shortage has prompted a need for alternative reversal agents for pharmacological stress testing. Cardiac stress testing is common for diagnosis and prognosis in patients with coronary heart disease. Options for pharmacological stress test agents include adenosine, regadenoson, dipyridamole, and dobutamine, whereas aminophylline is the recommended reversal agent. Adenosine and dobutamine can be used as alternatives to regadenoson and dipyridamole to decrease or eliminate the use of aminophylline. Alternatives to aminophylline include theophylline and caffeine. It is imp

Caffeine versus theophylline for apnea in preterm infants.

BACKGROUND: Recurrent apnea is common in preterm infants, particularly at very early gestational ages. These episodes of loss of effective breathing can lead to hypoxemia and bradycardia, which may be severe enough to require resuscitation including use of positive pressure ventilation. Two forms of methylxanthine (caffeine and theophylline) have been used to stimulate breathing in order to prevent apnea and its consequences. OBJECTIVES: To evaluate the effect of caffeine compared with theophylline treatment on the risk of apnea and use of mechanical ventilation in preterm infants with recurr

The pharmacologic stability of 35-year old theophylline.

The propensity to preserve and to hoard drugs over the years at home is a well-known phenomenon and offers the possibility for intentional and accidental drug poisoning in man. We report a case of acute theophylline poisoning in an 80-year old women after ingestion of 'Asthmo-Kranit', a 35-year old combined preparation containing theophylline and aminopyrine as the main ingredients. The patient developed the typical clinical picture of a symptomatic theophylline poisoning with flush, tremor, tachycardia, hyperventilation, hypotonia, and hyperglycaemia. The clinical course after treatment with

Effect of passive smoking on theophylline clearance in children.

The author investigated the effects of passive tobacco smoking on the metabolism of theophylline in a pediatric population. In a retrospective analysis of 201 children admitted to a pediatric unit for asthma, 31 were identified with an acute exacerbation of asthma of noninfectious origin in which environmental exposure to tobacco smoke could be established. The parents were known smokers with a minimum 1-pack-per-day habit. An age-and gender-matched control population of children was then identified who had an acute exacerbation of asthma without any environmental exposure to tobacco smoke. In

Theophylline or caffeine: which is best for apnea of prematurity?

Apnea of prematurity is a common problem of the premature infant under 30 weeks gestation. Theophylline and caffeine, two methylxanthines, are widely used to treat this condition. The drugs are equally effective in preventing apnea in the premature infant. Caffeine citrate has many advantages over theophylline, however, including once-a-day dosing, more predictable plasma concentrations, earlier onset of action, and minimal side effects. Caffeine is therefore the initial drug of choice for apnea of prematurity.
