Have there been improvements in Alzheimer's disease drug discovery over the past 5 years?

Alzheimer's disease (AD) is the most important neurodegenerative disorder with a global cost worldwide of over $700 billion. Pharmacological treatment accounts for 10-20% of direct costs; no new drugs have been approved during the past 15 years; and the available medications are not cost-effective. Areas covered: A massive scrutiny of AD-related PubMed publications (ps)(2013-2017) identified 42,053ps of which 8,380 (19.60%) were associated with AD treatments. The most prevalent pharmacological categories included neurotransmitter enhancers (11.38%), multi-target drugs (2.45%), anti-Amyloid age

[Treatment Expectations in the Perioperative Setting - Influence on Postoperative Pain Treatment].

Treatment expectations are relevant modulators of postoperative pain and recovery. In perioperative care, patients' expectations are shaped by previous experiences, verbal information, social learning, and the therapeutic context. Positive expectations may enhance analgesic effects via placebo mechanisms, whereas negative expectations may increase pain and reduce treatment efficacy via nocebo mechanisms. For anaesthetists, this offers a practical opportunity to improve perioperative pain management. During the preoperative consultation, patients can be asked about the intensity of pain they exp

Implementing sodium-glucose co-transporter 2 inhibitors in cardiovascular-kidney-metabolic syndrome: a multidisciplinary expert perspective.

AIMS: Cardiovascular-kidney-metabolic (CKM) syndrome defines a pathophysiological continuum driven by reciprocal dysfunction across the cardiovascular, renal and metabolic systems. Although sodium-glucose co-transporter 2 inhibitors (SGLT2i) provide consistent, organ-protective benefits across this spectrum, clinical implementation remains suboptimal. This expert opinion, developed by a multidisciplinary Italian board, aims to translate current evidence and guideline recommendations into practical, integrated strategies for the early and effective implementation of SGLT2i in patients at risk o

Managing Arterial Hypertension in Chronic Renal Failure: Myths, Mechanisms, and Therapeutic Realities.

Hypertension is highly prevalent among patients with chronic kidney disease (CKD), contributing significantly to cardiovascular morbidity and progressive renal decline. This overview explores the intricate pathophysiologic mechanisms driving hypertension in renal insufficiency, including volume overload, renin-angiotensin-aldosterone system (RAAS) activation, sympathetic overactivity, and vascular dysfunction. Diagnostic challenges such as white-coat hypertension and the underuse of ambulatory monitoring are discussed, along with the importance of volume assessment and target organ evaluation.

Toward a Unified Framework in Molecular Neurobiology of Alzheimer's Disease: Revisiting the Pathophysiological Hypotheses.

Despite decades of research, Alzheimer's disease (AD) remains without a curative therapy. While amyloid- and tau-centered approaches have dominated the field, failures of monotherapeutic strategies underscore the need for a broader system-level understanding. Here, this review critically revisits the principal hypotheses of AD pathogenesis, including the amyloid cascade, tauopathy, neuroinflammation, cholinergic dysfunction, oxidative and mitochondrial stress, metal dyshomeostasis, autophagy-lysosomal failure, genetic susceptibility, and infectious triggers. This review synthesizes molecular a

Optimizing pharmacological management of the febrile child.

INTRODUCTION: Pediatric fever often causes significant parental anxiety, or 'fever phobia,' leading to the inappropriate use of antipyretics. This narrative review synthesizes current evidence to support clinical decision-making, framing fever as a regulated physiological host defense mechanism, not a disease. The primary therapeutic goal is improving the child's overall comfort, rather than simply normalizing body temperature. AREAS COVERED: This review dispels common misconceptions, clarifying that fever height and response to antipyretics are poor predictors of illness severity. We cover e

Assessment of Knowledge, Perception, Experience, and Phobia Toward Corticosteroids Among the General Public in Two Southeast Asian Countries.

The widespread use of corticosteroids, particularly during the COVID-19 pandemic, has emphasized their therapeutic benefits while also raising public concerns regarding their safety and side effects. This study aims to assess the public's knowledge, perceptions, experiences, and corticophobia related to corticosteroid use in Indonesia and Malaysia. A cross-sectional study was conducted using a pre-validated online questionnaire distributed to the general public. Descriptive statistics, knowledge and corticophobia score computations, and linear regression analysis were performed to analyze the

Delivering information about medication assisted treatment to individuals who misuse opioids through a mobile app: a pilot study.

BACKGROUND: Digital therapeutic tools (e.g. mobile applications) can be accessible, low-cost interventions that counter misconceptions about medication assisted treatment (MAT) and/or improve deficits in MAT knowledge that are common barriers to treatment entry among individuals with opioid dependence. The purpose of this pilot study was to examine the preliminary effectiveness of a mobile application, 'uMAT-R', that includes health information about OUD recovery supported by science and MAT benefits. METHODS: Twenty-six adult participants with OUD recruited via social media completed all mod

Pilot trials in thrombosis: Purpose and pitfalls.

Randomized controlled trials provide important evidence to guide clinical practice. These full-scale trials are expensive, time consuming and many are never successfully completed. Well conducted pilot studies help with full-scale trial design, assessment and optimization of feasibility, and can avoid the waste of resources associated with starting a full-scale trial that will not succeed. They also provide an opportunity for capacity growth and mentorship of new investigators. It is important to appreciate that the usual goal of a pilot trial is assessment of feasibility and refinement of tri

Transcultural headache medicine: A framework for integrating cultural contexts into headache science and care - a call to action from the International Headache Society.

Headache disorders are among the most disabling neurological conditions, affecting over 1.5 billion people globally. Despite advances in pharmacological therapies, major inequities persist due to underdiagnosis, undertreatment and limited access to effective care, particularly in low- and middle-income countries. Social determinants of health, including cultural meanings, language and health beliefs, are increasingly recognized as key drivers of disparities in burden, diagnosis and treatment outcomes. Traditional medicine, used by more than 80% of the global population, remains first-line care

Intestinal Permeability, Irritable Bowel Syndrome with Constipation, and the Role of Sodium-Hydrogen Exchanger Isoform 3 (NHE3).

Increased intestinal permeability has been identified as one of the many pathophysiological factors associated with the development of irritable bowel syndrome (IBS), a common disorder of gut-brain interaction. The layer of epithelial cells that lines the intestine is permeable to a limited degree, and the amount of paracellular permeability is tightly controlled to enable the absorption of ions, nutrients, and water from the lumen. Increased intestinal permeability to macromolecules can be triggered by a variety of insults, including infections, toxins from food poisoning, or allergens, which

Exploring Service Users' Experiences of Activities Used During Their Occupational Therapy Treatment in South Africa.

Background/Objectives: Occupational therapy theories, models, and frameworks appear to be developed in Western countries. The philosophical framework for occupational therapy in South Africa appears to have relied upon various models, predominantly stemming from Western and European origins, with vague and fleeting references to African knowledge systems and philosophies. The objectives of the study were to explore how service users understand occupational therapy following their engagement in occupational therapy services; service users' perceptions of the activities used during occupational

Adherence to Antipsychotics, COVID-19 and Stigma: A Rainbow After the Storm?

Poor adherence to antipsychotic treatment is the main reason for relapse and admission in patients with severe mental illness (SMI). In addition to apprehensions about side effects, factors for non-adherence include the stigma of mental illness, which also extends to its treatment and which is further compounded by negative perceptions of antipsychotics within the medical profession itself. By reinforcing misconceptions around antipsychotics, warnings at the outbreak of pandemic associating antipsychotic use with coronavirus disease 2019 (COVID-19) related risks could have discouraged antipsyc

World Small Animal Veterinary Association Global Dental Guidelines.

Dental, oral, and maxillofacial diseases are some of the most common problems in small animal veterinary practice. These conditions create significant pain as well as localized and potentially systemic infection. As such, the World Small Animal Veterinary Association (WSAVA) believes that un- and under treated oral and dental diseases pose a significant animal welfare concern. Dentistry is an area of veterinary medicine which is still widely ignored and is subject to many myths and misconceptions. Effective teaching of veterinary dentistry in the veterinary school is the key to progression in

[Pharmacotherapeutical possibilities in mental disorders: current status in Serbia].

The new therapeutical approaches have direct implications on living in order to accomplish remission, stop further progression of illnesses or improve the quality of life. The life expectancy has been increased up to 10 years in the last 55 years, probably owing to the innovative drugs. The innovative drugs application in our everyday clinical practice should be analyzed, related to economical aspects and to the transitional status of a country, but also in the context of fears and misconceptions of therapists. The cost of therapy is not equal to the cost of prescribed drugs, however it also i

A new look at bladder neck obstruction by the food and drug administration regulators: guide lines for investigation of benign prostatic hypertrophy.

A basic assumption of current guide lines to test drug therapy for benign prostatic hypertrophy is that the clinical disease is caused by bladder neck obstruction and its sequelae and complications. Asymptomatic non-prostatic hypertrophy, no matter how large the adenoma, is considered to be a pre-clinical phase of the disease as long as bladder trabeculation, impairment of the flow rate and residual urine are absent. The guide lines recognize that histological changes and gross anatomical enlargement of the prostate are the essence of the disease but objective signs of current therapeutic reli
