What the practicing nurse should know about thiamine.

Thiamine (vitamin B1) is an essential nutritional component that acts as a coenzyme in the oxidative decarboxylation of alpha-keto acids. It also serves as the connection between the glycolytic cycle and the high energy-producing Krebs (or citric acid) cycle. Unlike other B vitamins, it activates the guanylate cyclase/cyclic guanosine monophosphate (GMP) system but not the adenylate cyclase system. The active coenzyme, thiamine pyrophosphate (TPP) is an antiberiberi substance. Thiamine itself is a pharmacologic antagonist of acetylcholine, which may explain the nerve lesions caused by thiamine

Can heatstable antithiamin factors inactivate thiamin in vitro and/or in vivo?

Aqueous solutions of thiamin hydrochloride were incubated with catechol for 24 hr, pH 7.4 at 50 degrees C. Additional solutions of thiamin and catechol were incubated separately. After incubation the remaining thiamin was analyzed by the thiochrome procedure which showed that the thiamin in the thiamin-catechol mixture was reduced by 82.5-90% and the thiamin incubated alone by 18-20%. The solutions were fed to groups of rats after they had been on a thiamin free diet for 17 days. Body weight gains, erythrocyte transketolase (ETKA), thiamin pyrophosphate effect (TPPE) and liver and urine thiami

Thiamine deficiency in fulminant hepatic failure and effects of supplementation.

Nine out of 24 patients with acute hepatocellular necrosis leading to fulminant hepatic failure showed biochemical evidence of thiamine deficiency early in the course of their illness, probably as a result of inadequate intake of the vitamin. This was corrected by twice daily administration of intravenous vitamin supplements containing thiamine hydrochloride (100 mg b.d.). These studies indicate that conversion of thiamine hydrochloride to its biologically active co-enzyme form, thiamine pyrophosphate, is possible even in the presence of severe acute hepatocellular necrosis, and it is suggeste

Can thiamine substitution restore cognitive function in alcohol use disorder?

AIMS: While clinical consequences of thiamine deficiency in alcohol use disorder (AUD) are severe, evidence-based recommendations on dosage, type of administration and duration of thiamine substitution (TS), and its' target levels remain sparse. This study aimed to compare the effect of two best practice TS regimens on thiamine blood levels (i.e. thiamine pyrophosphate, TPP) and cognitive function. METHODS: In 50 patients undergoing in-patient alcohol-withdrawal treatment, TPP levels were determined at baseline and end of weeks 1, 2 and 8 following administration of oral TS (3 × 100 mg/day fo

Thiamine deficiency in hematologic malignant tumors.

The case is reported of a patient with acute myeloid leukemia with severe right-sided congestive heart failure that responded to treatment with thiamine. Leukocytes cells contain relatively high concentrations of thiamine-dependent enzymes compared with erythrocytes. Because no other cause could be found, it was postulated that consumption of thiamine by blast cells was responsible for the deficiency. After studying this patient, the thiamine pyrophosphate (TPP) effect was measured in five other consecutive patients with fast-growing hematologic malignant tumors. In two patients, the TPP effec

Acute pernicious beriberi in a patient receiving parenteral nutrition. A case report.

Parenteral nutrition is being used increasingly in the treatment of the critically ill patient but it causes complications and metabolic derangement. A patient receiving parenteral nutrition in whom protracted vomiting from intestinal obstruction led to the development of acute cardiovascular beriberi (Shoshin) with severe metabolic acidosis--probably lactic--is described. The acidosis was refractory to bicarbonate infusion and inotropic support but the administration of intravenous thiamine 100 mg resulted in a dramatic recovery. Biochemical confirmation of thiamine deficiency was obtained by

High dose thiamine improves fatigue in multiple sclerosis.

The majority of the patients with multiple sclerosis (MS) experience fatigue. Some observations indicate that fatigue and related manifestations concomitant with MS could be associated with an intracellular mild thiamine deficiency. We recruited 15 patients with MS who also experience fatigue and assessed the severity of the fatigue using the Fatigue Severity Scale. Although blood thiamine and thiamine pyrophosphate levels were within normal limit in all the patients, high-dose thiamine therapy administered orally or parenterally led to an appreciable improvement of the fatigue. The absence of

Reappraisal of regional thiamine content in the central nervous system of the normal and thiamine-deficient mice.

The regional distribution of thiamine and its phosphate esters was measured in the central nervous system (CNS) of normal and thiamine-deficient mice. Twelve small areas were punched out from frozen sections and they were individually analyzed by high performance liquid chromatography (HPLC). Regional difference was noted in both the content and ratio of thiamine and its phosphate esters in the normal CNS. In pyrithiamine-induced thiamine deficiency, thiamine pyrophosphate (TPP) content in all the areas was reduced to less than 13% of the control values on day 10, when the neurological signs d

Metabolism of thiamin and its phosphoric esters in different regions of the nervous system: a new approach.

In the nervous system thiamin could have two main functions: coenzymatic, as thiamin pyrophosphate, and/or non-coenzymatic, but related to the nervous activity. In order to investigate the thiamin role we studied the turnover and metabolism of thiamin and its phosphoesters (mono-, pyro-, and tri-phosphate) in different regions (cerebellum, brain stem, cerebral cortex and sciatic nerve) of the nervous system, in a steady state condition. Practically, we determined the regional radioactivity of thiamin and its phosphoesters at different times (up to 240 hrs) from labeled thiamin administration,

Repurposing extracellular phosphatases to enhance thiamine biosynthesis and refine a genetic tool in Schizosaccharomyces pombe.

Thiamine (Vitamin B1) and its active form, thiamine pyrophosphate (TPP), are essential cofactors in all living organisms. However, industrial thiamine production in prokaryotes has been hindered by tight feedback regulation, such as that imposed by TPP riboswitches. This study explores the fission yeast Schizosaccharomyces pombe-which naturally lacks TPP riboswitches-as an alternative production host. We identified the extracellular phosphatases Pho1 and Pho4 as key redundant enzymes that hydrolyze thiamine monophosphate to support the growth of thiamine auxotrophic strains. Intracellular rete

Emerging role of thiamine therapy for prevention and treatment of early-stage diabetic nephropathy.

Thiamine supplementation may prevent and reverse early-stage diabetic nephropathy. This probably occurs by correcting diabetes-linked increased clearance of thiamine, maintaining activity and expression of thiamine pyrophosphate-dependent enzymes that help counter the adverse effects of high glucose concentrations-particularly transketolase. Evidence from experimental and clinical studies suggests that metabolism and clearance of thiamine is disturbed in diabetes leading to tissue-specific thiamine deficiency in the kidney and other sites of development of vascular complications. Thiamine supp

Thiamine deficiency and oxalosis.

Type I hyperoxaluria results from reduced activity of alpha-ketoglutarate: glyoxylate carboligase, which is necessary for the synergistic decarboxylation of glyoxylate and alpha-ketoglutarate to alpha-hydroxy-beta-keto-adipate. Since thiamine pyrophosphate is a cofactor in the reaction, thiamine deficiency might be expected to result in tissue oxalosis. However, there was no significant increase in the incidence of renal oxalosis in 15 patients with Wernicke's encephalopathy at necropsy compared with controls. It is possible that hyperoxaluria was present in these thiamine-deficient patients b

Erythrocyte transketolase activity in adult Ghanaian subjects.

The erythrocyte transketolase activity, and the effect of thiamin pyrophosphate stimulation (TPP effect) on this enzyme, was determined in 263 adult Ghanaian subjects who included normal males and females, male labourers known to consume a diet high in carbohydrate, diabetics and pregnant women. Using the guideline that a TPP effect of greater than 25% indicates thiamin deficiency, it was found that a significant proportion of apparently healthy subjects (36%-47%) may be regarded as thiamin deficient. The results are discussed in the light of similar studies carried out by other workers.

Erythrocyte transketolase activity in protein-energy malnutrition.

Erythrocyte transketolase activity (ETKA) and the percentage thiamine pyrophosphate (TPP) effects have been measured in a group of 10 healthy children and 50 malnourished subjects (23 oedematous and 27 non-oedematous cases). The observed ETKA values and percentage TPP effects for the two malnourished groups of subjects differed significantly (P less than 0.001) from the values for the control group. These values reverted almost to normal in 24 subjects who were re-tested after 6 weeks of thiamine therapy. Suggestions are made for this apparent biochemical thiamine deficiency without manifestat

Erythrocyte transketolase activity in the Wernicke-Korsakoff syndrome.

Erythrocyte transketolase activity and the effect of adding thiamine pyrophosphate (% TPP effect) were measured in subjects suffering from Wernicke-Korsakoff syndrome both before and during treatment with thiamine and/or thiamine tetrahydrofurfuryldisulphide (TTFD). Transketolase activity was significantly lower in untreated patients than in healthy volunteers. Treatment with either thiamine or with TTFD restored enzyme levels to control values but TTFD produced a greater increase than thiamine in enzyme activity. In a group of seven patients there was no correlation between duration of TTFD t

Effect of thiamine deprivation on thiamine metabolism in mice.

Mice were made deficient by thiamin deprivation. Thiamin pyrophosphokinase and thiamin pyrophosphatase activities were measured, and their changes were compared to transketolase (TK), pyruvate and alpha-ketoglutarate dehydrogenase (DG) activities and thiamin pyrophosphate (TPP) level in the same tissues. Thiamin pyrophosphokinase activity was increased on day 10 of thiamin deficiency and remained unchanged within the next period of investigation. Thiamin pyrophosphatase activity decreased after 5 days and was enhanced in 10, 15 and 20 days of deficiency. This fact may be related to the regulat

Alcohol intakes and deficiencies in thiamine and vitamin B6 in black patients with cardiac failure.

Thiamine and vitamin B6 status was evaluated in 73 consecutive black patients with cardiac failure at Baragwanath Hospital. They consumed moderate to large amounts of traditional as well as Western-type beer and liquor. Thirty per cent had erythrocyte thiamine concentrations below the reference range. The transketolase response to thiamine pyrophosphate (TPP effect) suggested thiamine deficiency in 32.4%, of whom 13.2% were classified as severely deficient. Vitamin B6 deficiency was present in 21.4%, with a further 42.9% in the very low normal range. Only one patient had beriberi heart disease
