A machine learning model (Sol_ME) assisted development of a unit-dose lipid formulation for apalutamide: formulation, pharmacokinetics, and pharmacodynamics evaluation.

PURPOSE: The development of a novel lipid-based formulation for apalutamide, a potent androgen receptor inhibitor for non-metastatic castration-resistant prostate cancer (nmCRPC), is explored in this study. METHOD: To address its poor solubility and high dose requirement, a single-dose lipid-based soft gelatin capsule delivering 240 mg of apalutamide was developed using machine learning (ML)-assisted excipient selection. The ML model, Sol_ME, predicted cinnamon oil as the optimal solubilizer, enhanced further by vanillin. The optimized formulation showed improved dispersion, stability, and bi

PSMA expression assessed by [18F]PSMA-1007 PET/CT imaging in metastatic hormone-sensitive prostate cancer patients treated with apalutamide.

BACKGROUND: PSMA PET/CT (Prostate-specific membrane antigen positron emission tomography / computed tomography) has revolutionized prostate cancer imaging. While PSMA expression is influenced by androgen receptor pathway inhibitors (ARPI), few studies have examined the specific effects of apalutamide, a widely used ARPI for hormone-sensitive metastatic prostate cancer. This study evaluated the impact of apalutamide on PSMA radioligand uptake. METHODS: We evaluated 17 patients with metastatic hormone-sensitive prostate cancer (mHSPC) who underwent [18F]PSMA-1007 PET/CT imaging prior to (PET1) (

Falls and fractures in men with prostate cancer taking second-generation androgen receptor antagonists: A retrospective chart review.

IntroductionSecond-generation androgen receptor antagonists, including enzalutamide, apalutamide, and darolutamide, are commonly used to treat metastatic and non-metastatic prostate cancer. Although these medications are typically well-tolerated, falls were reported in 4.2-15.6% of patients and fractures in 4.2-11.7% of patients enrolled in the phase III clinical trials evaluating these drugs in prostate cancer. However, post-marketing studies have reported a lower incidence than reported in clinical trials. The objective of this study is to determine the prevalence of falls and fractures in p

Association of race and survival in patients treated with apalutamide: Pooled analysis of two phase 3 trials.

BACKGROUND: Clinical studies have shown that outcomes of patients with prostate cancer could vary depending on race. In this study, the authors sought to determine if the treatment effect of apalutamide, an androgen receptor pathway inhibitor (ARPI), on overall survival (OS) varies depending on the race of the patient. METHODS: This pooled analysis includes individual patient data from two phase 3 trials, TITAN and SPARTAN, which randomized patients to androgen deprivation therapy (ADT) ± apalutamide in metastatic hormone-sensitive and nonmetastatic castration-resistant prostate cancer, respe

The comparison of outcomes between neoadjuvant novel and classical hormonal therapy in patients with locally advanced prostate cancer: a retrospective study.

OBJECTIVE: This study aimed to compare the outcomes of neoadjuvant novel hormonal therapy (NHT) versus classical hormonal therapy (CHT) before radical prostatectomy (RP) in patients with locally advanced prostate cancer. METHODS: Propensity score matching (2:1) was performed to minimize the effect of confounders at our center. We retrospectively analyzed 99 patients who received neoadjuvant hormonal therapy for 3-6 months from March 2019 to April 2023. The novel hormonal agents included apalutamide, darolutamide, or enzalutamide, whereas the classical hormonal agent was bicalutamide. The prim

Phenotypic Plasticity and Androgen Receptor Bypass Drive Cross-Resistance to Apalutamide in Castration-Resistant Prostate Cancer Cell Models.

The treatment of choice for prostate cancer is androgen deprivation (ADT) and novel hormonal agents such as Abiraterone, Enzalutamide, or Apalutamide. Initially, this therapy is highly effective, but a significant challenge arises as most patients eventually develop resistance, resulting in castration-resistant prostate cancer (CRPC). Furthermore, the sequential use of these drugs can lead to cross-resistance, diminishing their efficacy. Tumor heterogeneity plays a pivotal role in the development of resistance to different treatments. This study utilized cellular models of CRPC to assess the r

Association between medication burden and acute care use in older metastatic prostate cancer patients on androgen receptor signaling inhibitors.

BACKGROUND: Management of metastatic prostate cancer often requires combining androgen deprivation therapy (ADT) with novel androgen receptor signaling inhibitors (ARSIs). Although these agents improve survival, older patients may face acute care utilization from medication burden, reflected in polypharmacy and nonadherence. METHODS: Using SEER-Medicare data, the authors identified patients ≥66 years old with de novo metastatic prostate cancer prescribed abiraterone, enzalutamide, or apalutamide (2010-2017). Polypharmacy was defined by the Youden index (≥8 medications). ARSI adherence was mea

Fibrin deposition following bile duct injury limits fibrosis through an αMβ2-dependent mechanism.

Coagulation cascade activation and fibrin deposits have been implicated or observed in diverse forms of liver damage. Given that fibrin amplifies pathological inflammation in several diseases through the integrin receptor αMβ2, we tested the hypothesis that disruption of the fibrin(ogen)-αMβ2 interaction in Fibγ(390-396A) mice would reduce hepatic inflammation and fibrosis in an experimental setting of chemical liver injury. Contrary to our hypothesis, α-naphthylisothiocyanate (ANIT)-induced liver fibrosis increased in Fibγ(390-396A) mice, whereas inflammatory cytokine expression and hepatic n

Long-term trends in androgen-directed therapy in advanced prostate cancer: a 29-year national analysis.

BACKGROUND: Androgen deprivation therapy (ADT) has been pivotal in advanced prostate cancer (PCa) for decades. The introduction of androgen-receptor pathway inhibitors (ARPIs) has transformed treatment paradigms. However, their real-world adoption over time has not been comprehensively characterised. We examined prescribing trends for androgen-directed therapies in Australia over a 29-year period. METHODS: A retrospective population-based analysis of Pharmaceutical Benefits Scheme (PBS) dispensing data (1996-2024) was performed. PBS-listed anti-androgen therapies-luteinising hormone-releasing

Prostate-Specific Antigen Response at Six Months Predicts Progression in Metastatic Hormone-Sensitive Prostate Cancer Treated With Apalutamide.

BACKGROUND: PSA response to apalutamide combined with androgen deprivation therapy (ADT) in metastatic hormone-sensitive prostate cancer (mHSPC) has been linked to prognosis. Post hoc analyses from clinical trials suggest that PSA levels at 6 months are critical for predicting radiographic progression-free survival (rPFS) and overall survival (OS). Real-world evidence (RWE) is needed to confirm these findings. MATERIALS AND METHODS: This multicentre, retrospective study included patients with mHSPC treated with apalutamide plus ADT from May 2018 to January 2025 across 18 Spanish centers. Pati
