Pleural effusions can be manifestations of heart failure, pulmonary embolism, infection, and malignancy. Symptom assessment, imaging, and pleural fluid analysis serve as key determinants in guiding clinical management. We present the case of a 75-year-old woman with a history of heart failure who developed worsening dyspnea and lower extremity edema. Initial imaging revealed a large unilateral pleural effusion, initially presumed to be secondary to heart failure exacerbation. Pleural fluid analysis demonstrated a discordant exudative effusion by Light's criteria. Following thoracentesis, the d

INTRODUCTION: Pleural effusion is present in half of the patients hospitalised with acute heart failure. The condition is treated with diuretics and/or therapeutic thoracentesis for larger effusions. No evidence from randomised trials or guidelines supports thoracentesis to alleviate pleural effusion due to acute heart failure. The Thoracentesis to Alleviate cardiac Pleural effusion Interventional Trial (TAP-IT) will investigate if a strategy of referring patients with acute heart failure and pleural effusion to up-front thoracentesis by pleural pigtail catheter insertion in addition to pharma

BACKGROUND: Serial thoracentesis is an established management strategy for hepatic hydrothorax (HH) patients who are intolerant to diuretics or not candidates for transjugular intrahepatic portosystemic shunt (TIPS) placement, but it carries a risk of complications. The role of indwelling pleural catheters (IPCs) in HH management remains controversial. This study aims to evaluate the safety and efficacy of IPCs compared with serial thoracentesis in patients with HH.
METHODS: We conducted a retrospective, propensity-matched cohort study using US population-based data, aged 18 to 89 years with

BACKGROUND: Pleural infection is a complex condition with a considerable healthcare burden. The average hospital stay for pleural infection is 14 days. Current standard of care defaults to chest tube insertion and intravenous antibiotics. There have been no randomised trials on the use of therapeutic thoracentesis (TT) for pleural fluid drainage in pleural infection.
AIMS AND OBJECTIVES: To assess the feasibility of a full-scale trial of chest tube vs TT for pleural infection in a single UK centre. The primary outcome was defined as the acceptability of randomisation to patients.
METHODS: Ad

BackgroundPleural effusion and ascites frequently cause refractory dyspnea and abdominal distension in patients with advanced cancer. Although ultrasound-guided thoracentesis and paracentesis are established procedures, accessing hospital- or outpatient-based drainage may impose substantial burdens on patients near the end of life. We evaluated the efficacy and safety of timely home-based ultrasound-guided thoracentesis and paracentesis for symptom relief in terminally ill patients with cancer receiving home hospice care.MethodsWe retrospectively analyzed prospectively collected clinical data

BACKGROUND: Acute pain management is a core ethical commitment to medical practice. However, there is evidence to suggest that sometimes infiltrative lidocaine (IL) is not used prior to thoracentesis and abdominocentesis due to the belief that two needles cause greater pain than one. However, topical anesthetics like lidocaine-prilocaine cream (LPC) are painless, easy to use, and have less systemic side effects. Therefore, LPC can be a suitable substitute for medical procedures.
OBJECTIVES: This study was designed to compare the analgesic effects of LPC with IL in thoracentesis and abdominoce

BACKGROUND: In patients with non-expandable lung, removal of pleural fluid can result in excessively negative pleural pressure, which is associated with chest discomfort, pneumothorax, and re-expansion pulmonary oedema. Pleural manometry is widely used to safeguard against pressure-related complications during thoracentesis despite little evidence to support the approach. We investigated whether monitoring of pleural pressure with manometry during thoracentesis could protect against complications compared with assessment of symptoms alone.
METHODS: We did a prospective randomised single-blind

BACKGROUND: Proficiency in performing thoracentesis is a key competency recommended in the Medical Schools Objective Project (MSOP) adopted by the American Association of Medical Colleges, USA, that medical students should possess prior to graduation, although they often do not practice it clinically until the later stages of their training. Thiel-embalmed cadavers, which offer a lifelike experience with less irritation than formalin-preserved cadavers, have been increasingly used as early as 1st year medical school to teach such procedures because of their feel-like and look-like real patient

Introduction The presence of pleural effusion impairs daily activity, including a decrease in exercise capacity. Aspiration of pleural fluid results in improvement of symptoms, but the impact, especially on exercise tolerance, has not been adequately studied. Methods Forty-eight patients with moderate to massive unilateral pleural effusion documented by chest radiograph were included in this prospective hospital-based study. The six-minute walk test (6-MWT), modified Borg dyspnoea scale, forced vital capacity (FVC), and forced expiratory volume in the first second (FEV1) were analysed before a

PURPOSE: To compare the disparities between the paracenteses and thoracenteses performed by radiologists with those performed by nonradiologists over time. Variables included the volume of procedures, the days of the week, and the complexity of the patient's condition.
MATERIALS AND METHODS: Using carrier claims files for a 5% national sample of Medicare beneficiaries from 2004 to 2016, paracentesis and thoracentesis examinations were retrospectively classified by physician specialty (radiologist vs nonradiologist), day of the week (weekday vs weekend), and the complexity of the patient's con

BACKGROUND: Malignant pleural effusion (MPE) indicates advanced disease and imposes a significant symptomatic burden to patients. Current guidelines recommend stepwise investigation and management.
RESEARCH QUESTION: Is it feasible and safe to combine ultrasound (US)-guided pleural biopsy and indwelling pleural catheter (IPC) insertion as the initial diagnostic and therapeutic procedure for patients with high preprocedural probability of MPE?
STUDY DESIGN AND METHODS: We retrospectively analyzed patients who underwent pleural procedures between March 1, 2021, and September 30, 2022. Sixteen

OBJECTIVES: Recurrent symptomatic effusions can be durably managed with pleurodesis or placement of indwelling pleural catheters. Recent pleurodesis trials have largely relied on lung re-expansion on post-thoracentesis radiograph as an inclusion criterion rather than pleural elastance as determined by manometry, which is an important predictor of successful pleurodesis. We investigated the association between lung re-expansion on post-pleural drainage chest imaging and pleural physiology, with particular attention to pleural elastance over the final 200 mL aspirated.
DESIGN: Post-hoc analysis

BACKGROUND: Tuberculous pleural effusions (TBEs) and parapneumonic pleural effusion (PPEs) have similar clinical presentations and fluid biochemistry. A pleural biopsy is usually required to diagnose TBE but complete fluid evacuation may not be necessary, contrasting with complicated PPE (CPPE). A point-of-care test that distinguishes between TBE and CPPE enables the appropriate procedures to be performed during the initial diagnostic thoracentesis. Lactate is a metabolic product measurable by a blood-gas analyzer. This study measured pleural fluid (Pf) lactate levels in TBE and compared them

BACKGROUND: Malignant pleural effusion (MPE) is a common cancer complication. Clinical and economic implications of different recurrent MPE treatment pathways have not been evaluated fully.
RESEARCH QUESTION: What clinical outcomes, complications, health care resource use, and costs are associated with various rapidly recurrent MPE treatment pathways?
STUDY DESIGN AND METHODS: This retrospective cohort study using Surveillance, Epidemiology and End Results Medicare data (2011-2015) included patients 66 to 90 years of age with rapidly recurrent MPE. Rapid recurrence was defined as receipt of

BACKGROUND: Non-expandable lung (NEL) has severe implications for patient symptoms and impaired lung function, as well as crucial implications for the management of malignant pleural effusion (MPE). Indwelling pleural catheters have shown good symptom relief for patients with NEL; hence, identifying patients early in their disease is vital. With the inability of the lung to achieve pleural apposition following thoracentesis and the formation of a hydropneumothorax, traditionally, chest X-ray and clinical symptoms have been used to make the diagnosis following thoracentesis. It is our aim to in

BACKGROUND: Pleural effusions remain a common postoperative complication following lung transplantation, occurring in 10% to 26% of cases. We aimed to explore potential clinical or radiographic features associated with clinically significant post-lung transplant pleural effusions requiring repeat interventions for their management.
METHODS: Lung transplantation recipients who underwent thoracentesis at our institution between June 2012 and October 2022 were reviewed. In total, 77 patients were included. Data were collected via the electronic health record and adjudicated through direct chart

Although eosinophilic pleural effusion (EPE) has been a subject of numerous studies, its clinical significance still remains unclear. The aim of our study was to evaluate: 1) the relative incidence and aetiology of EPE; 2) the predictors of malignancy in patients with EPE; and 3) the relationship between repeated thoracentesis and pleural fluid eosinophilia. A retrospective analysis of 2,205 pleural fluid samples from 1,868 patients treated between 1995 and 2007 was performed. We identified 135 patients with EPE (7.2% of all patients with pleural effusion) and 153 EPE samples. The most common
