Video-Assisted Thoracic Surgery Thymectomy: Transpleural Approach.

There are several types of minimally invasive approaches for thymectomy, of which the transpleural approach by video-assisted thoracoscopic surgery is particularly useful. In this approach, thymectomy is performed from either side of the thoracic cage. Thoracic surgeons should be familiar with the principles of the procedure, the anatomy of the region, and surgical strategies for successful thymectomy. The details of transpleural thymectomy are discussed herein.

Video-assisted thoracic surgery for pleuroperitoneal communication.

BACKGROUND: Hydrothorax due to pleuroperitoneal communication (PPC) is a rare complication of continuous ambulatory peritoneal dialysis (CAPD). Approximately 50% of the patients need to convert to hemodialysis. CASE PRESENTATION: A 65-year-old man with chronic renal failure due to membranoproliferative glomerulonephritis underwent CAPD. Seven months after starting CAPD, a chest X-ray showed a right hydrothorax. For performing radioscintigraphy, 99mTc-macro-aggregated albumin was administered into the peritoneal cavity with dialysate, and a leakage point of the dialysate into the right pleural

Bypass surgery in arterial thoracic outlet syndrome.

Arterial thoracic outlet syndrome (TOS) causes ischemic symptoms; it is the rarest type, occurring in 5% of all TOS cases. This paper is a case report of a 38-year-old male patient diagnosed with arterial TOS, displaying symptoms of acute critical limb ischemia caused by thromboembolism. Brachial artery of the patient has been diffusely damaged by repeated occurrence of thromboembolism. It was thought to be not enough only decompression of subclavian artery to relieve the symptoms of hand ischemia; therefore, bypass surgery using reversed great saphenous vein was performed.

Anesthesia for awake video-assisted thoracic surgery.

Awake video-assisted thoracic surgery (VATS) has been increasingly employed in a variety of procedures involving pleura, lungs, and mediastinum. Adequate anesthesia and analgesia obtained from thoracic epidural anesthetic (TEA) allow VATS to be performed in awake patients. The potential general anesthesia-related adverse effects, such as intubation-related trauma, pneumonia, ventilator-associated lung injury, effects of neuromuscular blocking agents, and postoperative nausea and vomiting, can thus be avoided. Moreover, TEA holds the benefits of reducing pulmonary and cardiac morbidities and mo

Cardiac preload: hemodynamic physiology during thoracic surgery.

PURPOSE OF REVIEW: Operations in pleural cavity have circulatory effects both in pulmonary and systemic circulations. Nevertheless studies of these effects have yielded conflicting results. The importance of good understanding of hemodynamic changes during the operation in pleural cavity consists in fact that they are one of the factors influencing postoperative course of operated patients. RECENT FINDINGS: Dominating changes in the hemodynamics are represented by an increase of the cardiac output after opening the pleura. Changes in the arterial pressure are clinically unimportant and decrea

Pneumocephalus and pneumococcal meningitis after thoracic surgery.

A 62-year-old man with adenocarcinoma underwent complete resection with a right upper lobectomy and en-bloc resection of the chest wall, with metallic clips applied to the vertebral nerve roots. A sudden deterioration in neurological status occurred due to pneumocephalus and ascending bacterial meningitis resulting from a subarachnoid-pleural fistula. The neurological status normalized after thoracoplasty and ceftriaxone treatment.

Remote cerebellar hemorrhage following thoracic spinal surgery.

A 51-year-old man underwent surgery for ossification of the ligamentum flavum at the T9-T10 levels. Intraoperatively, the dura was opened unintentionally and a subcutaneous suction drain was placed. The patient complained of severe headache and nausea postoperatively. Brain computed tomography obtained 3 days after the surgery demonstrated remote cerebellar hemorrhage and hydrocephalus. Suboccipital decompression, C1 laminectomy, and ventriculostomy were performed and his symptoms subsided 2 months later. Remote cerebellar hemorrhage following spinal surgery is extremely rare, but may occur af

[Postoperative complications in thoracic surgery. Special aspects].

Complications are quite common in thoracic surgery due to the comorbidities of the mostly elderly patients and to the fact that the surgical procedure itself compromises the respiratory apparatus. Consequently, pneumonia and respiratory failure represent the most common causes of death. Postoperatively, early diagnosis and treatment of these complications are paramount while most other events can be anticipated and thus be avoided with corresponding operative experience.

[Surgery for thoracic metastases from urological malignancies].

Surgery of thoracic metastases from urological malignancies essentially concerns renal carcinoma and non seminomatous testicular germ cell tumors (NSGCT). Complete resection of renal cell cancer lung metastases can be done with low mortality and an appreciable long survival rate, especially for single lesion with a long free interval. For NSGCT, resection of all pulmonary lesions and mediastinal residual masses after chemotherapy affords a very high long term survival rate. In the case of multiple lesions, surgical approaches must be carefully chosen.

Limited upper sternotomy in general thoracic surgery.

PURPOSE: To evaluate the status of limited upper sternal split in general thoracic surgery. METHODS: We reviewed the clinical files of 100 consecutive patients operated on through limited upper sternotomy at a hospital in Italy during the 10 years between January 1995 and December 2004. RESULTS: Thymus surgery represented the main indication for this approach (n = 51): for myasthenia without thymoma in 28 patients, for thymus neoplasms with or without myasthenia in 22, and for intrathymic parathyroid adenoma in 1. Thyroid surgery constituted the second main indication for upper sternal split

Video-assisted thoracic surgery for spontaneous haemopneumothorax.

BACKGROUND AND OBJECTIVE: The aim of this study was to review the treatment options for spontaneous haemopneumothorax (SHP) by video-assisted thoracoscopic surgery (VATS). METHODS: Records from 16 patients (14 male, age 16-38 years, mean age 26.1 years) with prominent SHP (blood loss over 400 cc in the first 24 h) undergoing VATS from July 1994 to December 2005 and treated by one thoracic surgeon in four medical centres or community hospitals of North and Mid-Taiwan were reviewed retrospectively. RESULTS: Thirteen patients (81.3%) were identified to have a prominent bleeding source intraoper

Clinical outcomes in "complex" thoracic aortic surgery.

Although the term "complex aortic surgery" has come into increasing use, it has not been defined. We propose the following definition: replacement or remodeling (not resuspension of commissures) of the aortic root, together with either an intracardiac procedure or a replacement of more than 1 segment of aorta, all of which require cerebral protection. We retrospectively analyzed data pertaining to 152 patients (mean age, 56 +/- 12 years) who underwent surgery for thoracic aortic disease with aid of cardiopulmonary bypass from October 2000 through December 2005. The replaced segment was the asc

[Video-assisted thoracic surgery for lung cancer].

Video-assisted thoracic surgery (VATS) has been applied to treat patients with various pulmonary diseases. Recent advances in video endoscopic instruments have been enabled radical resection of pulmonary malignancies as minimally invasive surgery. Forty patients with primary lung cancer underwent several types of lung resection in the past 3 years at out unit. Of these, twenty-seven anatomical pulmonary resections were performed using VATS. Peripherally located, early-stage lung cancer is thought to be a potential candidate for curative surgery with video-assisted lobectomy. Although there are

[Video-assisted thoracic surgery: indications and limitations].

From November 1990 to September 1992, 72 patients underwent a video-assisted thoracic surgery (VATS), 47 men and 25 women, of mean age 49 years. Video-assisted thoracic surgery was indicated for the following: 44 spontaneous pneumothorax, 6 benign tumors of the lung or the mediastinum, 4 pleural effusions, one broncho-pleural fistula and one sympathectomy; 17 diagnostic procedures (lung or mediastinal lymph-nodes biopsies). In one patient, it was necessary to associate a mini-thoracotomy and 5 patients went on to a formal thoracotomy. There was no mortality and the morbidity rate was 5.5%. Som

[After-care following heart and thoracic surgery].

The paper is directed towards those practitioners charged with the responsibility of delivering care pertinent to patients after cardiac valve replacement, coronary artery bypass grafting and surgery for lung cancer. In addition to the postpericardiotomy-syndrome the following complications associated with valve replacement are discussed: thrombotic occlusion or systemic thromboembolism, prosthetic valve endocarditis, mechanical valve dysfunction, postoperative arrhythmias. The patency of aorto-coronary bypass grafts decreases progressively over time and relates directly to improvement in symp

Antenatal diagnosis of congenital thoracic malformations: early surgery, late surgery, or no surgery?

Today, congenital thoracic malformations (CTMs), which comprise a spectrum of anomalies rather than separate entities, are frequently detected on routine antenatal ultrasound. However, with similar appearances of different congenital lung and nonpulmonary lesions, a definitive diagnosis cannot usually be established antenatally with absolute certainty. The natural history of CTMs is extremely variable. Large lesions may cause serious complications in the fetus, necessitating treatment in utero. Such prenatal therapy, however, is only required in a small minority of fetuses. Many lesions decrea
