Primary Anterior Thoracic Wall Abscess with Osteomyelitis by Salmonella paratyphi A : Case Report.

Salmonella enterica serovar Typhi and Paratyphi have been imputed in the causation of enteric fever. Cardiovascular and extraintestinal Salmonella infections have been documented among immunocompromised individuals. Rarely these pathogens are ascribed in the causation of extraintestinal infections among immunocompetent hosts due to hematogenous seeding. We report a case of anterior chest wall abscess with osteomyelitis in an immunocompetent adult by Salmonella paratyphi A without any prior predisposing conditions or gastrointestinal symptoms. The patient underwent incision and drainage of the

Effect of simulated obstructive hypopnea and apnea on thoracic aortic wall transmural pressures.

Preliminary evidence supports an association between obstructive sleep apnea (OSA) and thoracic aortic dilatation, although potential causative mechanisms are incompletely understood; these may include an increase in aortic wall transmural pressures, induced by obstructive apneas and hypopneas. In patients undergoing cardiac catheterization, mean blood pressure (MBP) in the thoracic aorta and esophageal pressure was simultaneously recorded by an indwelling aortic pigtail catheter and a balloon-tipped esophageal catheter in randomized order during: normal breathing, simulated obstructive hypopn

Clinical evaluation of the anterior chest wall deformity in thoracic adolescent idiopathic scoliosis.

STUDY DESIGN: A retrospective radiographical study of pre- and postoperative anterior chest wall shape in thoracic adolescent idiopathic scoliosis. OBJECTIVE: To assess the anterior chest wall deformity and define its relationship with other deformed components in adolescent idiopathic scoliosis. The surgical influence on the deformed chest wall contour was also evaluated to improve the clinical recognition of the risk factors associated with aggravated chest wall shape postoperatively. SUMMARY OF BACKGROUND DATA: Anterior chest wall deformity is one of the major cosmetic concerns that may b

[Interdisciplinary management of thoracic wall tumors: indication, technique, and results of interscapulothoracic amputation].

Interscapulothoracic amputation of the upper extremity, described for the first time by Berger in 1887, has undergone a change of indication. Originally performed in traumatic injuries of the shoulder or arm, it has been done in radical resection of malignancies of the upper extremity, axilla, or the surrounding thoracic wall. Particularly it is performed in tumors which cannot be controlled by neoadjuvant therapies or locally by radiation. The indication, technique, and results of such amputation are discussed by presenting our patients operated on with the procedure. Although this treatment

Variations in the anterior thoracic wall with sternalis muscle and accessory pectoralis major muscle.

PURPOSE: The new type of the sternalis muscle needs to be recognized, and the accessory pectoralis major muscle (AcPM) was found between the pectoralis major and minor muscle. It needs to be acknowledged those two different variations can exist in one case. METHODS: The muscle was found on a 73-year-old male cadaver during the dissection class for the anterior thoracic wall. It was proceeded to identify the adjacent structures with precise dissection. RESULTS: The cadaver had sternalis muscle bilaterally. Both side sternalis muscles had a medial and lateral belly and attached to pectoral fas

Erector Spinae Plane Block for Surgery of the Posterior Thoracic Wall in a Pediatric Patient.

OBJECTIVE: Historically, regional anesthesia for surgery on the posterior thoracic wall has been limited to neuraxial and paravertebral nerve blocks. The erector spinae plane (ESP) block is a novel technique that anesthetizes the dorsal rami of the spinal nerves innervating the posterior thoracic wall. We report the use of the ESP block for this clinical application in a pediatric patient. CASE REPORT: A healthy 3-year-old girl was scheduled for resection of a giant paraspinal lipoma extending over the T4-T7 dermatomes. She received a preoperative single-shot ESP block at the level of the T1

Apico-aortic conduit for aortic stenosis in a patient with chronic thoracic wall infection.

We report a case of an apico-aortic bioprothesis-valved conduit for a 70-year-old-woman with symptomatic, severe aortic stenosis and severe calcification of the ascending aorta. She had a history of mastectomy and radiation therapy for breast cancer and was suffering from radiodermatitis and chronic thoracic wall infection. Transthoracic echocardiography showed severe aortic valve stenosis with heavy calcification and high aortic valve pressure gradients. In patients with a chronically infected thoracic wall, median sternotomy is considered to be a high risk procedure, resulting in postoperati

[Plastic surgery of the thoracic wall (malformations and tumors) using a sliding splint-stapler].

Chest wall instability, created surgically by correction of a chest deformity (funnel chest or, more rarely, pigeon chest) or by resection of a tumour of the chest wall, is serious as it can jeopardize the immediate vital prognosis because of paradoxical respiration and, in the long term, the functional and aesthetic prognosis due to progressive impaction of the unstable segment of the wall. The polyvalent and adaptable material which we have developed (sliding splint-staple) and which we also use in thoracic traumatology (thoracic flaps), has allowed us to perform audacious corrections for de

Problems caused by the unstable thoracic wall and by cardiac injury due to blunt injury.

The treatment of flail chest remains highly controversial. In the literature convincing arguments can be found to support any therapeutic procedure. Newer concepts of mechanical ventilation such as SIMV and CPAP, as well as the use of epidural analgesia, have resulted in a significant reduction in the duration of artificial ventilation. Although the mechanical problems are generally overestimated in this situation, the use of a ventilator is indicated in many cases because of the associated lung damage. Internal fixation of the unstable thoracic wall is restricted to special, selected cases wh

Co-existence of the abdominal wall and thoracic endometriosis: case report and literature review.

Extra-pelvic endometriosis always presents complex diagnostic challenges due to its non-specific symptoms. We describe a rare case of coexisting iatrogenic abdominal wall endometriosis (AWE) and thoracic endometriosis (TE) in a 33-year-old woman without pelvic involvement. The patient, with a history of two cesarean sections, presented with chronic abdominal pain and an asymptomatic pulmonary nodule. Both lesions were removed through surgical resection and histologic examination confirmed endometriosis. One year after the surgery, the patient experienced a recurrence of the abdominal pain and

A pictorial essay of thoracic wall diseases: multiple pathologies in the same anatomical site.

OBJECTIVES: To describe diagnostic and radiological features of the main pathologies affecting the thoracic wall, providing a pictorial atlas based on several clinical cases extracted from our archive. MATERIALS AND METHODS: A wide variety of pathologies affect the tissues of the thoracic cage; these conditions are often encountered by radiologists during examinations performed for unrelated clinical questions. Modern imaging techniques enable the detection of these pathologies and allow definitive diagnoses to be achieved. RESULTS: Pathological processes that involve the chest wall may be c

Acute Presentation of a High-Grade Myxofibrosarcoma Originating in the Thoracic Wall: A Case Report.

We report the first case of a patient with myxofibrosarcoma (MFS) who presented acutely with a rib fracture and developed a rapidly expanding loculated hemothorax after chest trauma. The patient was taken to the operating room for evacuation of hemothorax, and samples and biopsy specimens were taken for cytologic and pathologic examination. Final report with immunohistochemical staining showed a high-grade MFS. After the procedure, there was clinical and radiological improvement, and the patient was followed up as an outpatient. Myxofibrosarcoma is a very rare and aggressive connective tissue

Surgery for Deformities of the Thoracic Wall: No More than Strengthening the Patient's Self-Esteem?

In most cases, correction of a pectus deformity is performed for cosmetic reasons. The main indication for corrective therapy is impaired body image and self-esteem due to the deformity. In these instances, patients suffer from a lower quality of life that is not correlated with the severity of the chest wall deformity. After correction of a pectus excavatum (PE) or pectus carinatum (PC) patients experience a significant improvement in body image and self-esteem. For many, corrective surgery is life changing as it leaves patients feeling satisfied with the appearance of their chest, and this i

Respiratory insufficiency in a newborn with mesenchymal hamartoma of the chest wall occupying the thoracic cavity.

The authors describe a newborn patient with mesenchymal hamartoma of the chest wall associated with pulmonary hypoplasia. A massive thoracic tumor was diagnosed by prenatal ultrasonography and magnetic resonance imaging at the 28th week of gestation. She was delivered through cesarean delivery at the 36th gestational week. Respiratory distress because of pulmonary hypoplasia necessitated neonatal intensive care. The tumor extensively involved the left hemithorax including all 12 ribs and the first 10 thoracic vertebrae, resulting in marked deformity of the thorax. At 5 days of age, she underwe

Thoracic epidural morphine in the palliation of chest wall pain secondary to relapsing polychondritis.

Relapsing polychondritis is a rare disease of unknown etiology characterized pathologically by degeneration of the chondrocyte and replacement with fibrous connective tissue. The following case report presents the pain management of a 34-yr-old man suffering from intractable pain secondary to relapsing polychondritis. Systemic narcotic analgesics, adjunctive drugs, and peripheral nerve blocks with local anesthetic and steroid failed to adequately control the patient's pain. Thoracic epidural morphine was used to provide excellent relief of pain. Factors in the selection of an implantable narco

[Surgical treatment of deformities of the anterior thoracic wall by frontal chondrotomy: an original technic].

According to the author, the frequency of recurrence after sternochondroplasty is related to the poor quality of the chondral callus which is of limited thickness. In addition, it is subjected to shearing force at each respiratory movement. The author proposes frontal chondrotomies allowing anterior displacement in funnel chests or posterior cartilages are sutured. The correction obtained is stable without any supplementary fixation. The plane of section being perpendicular to respiratory movement, the sutures are no longer submitted to shearing strain but to alternate compression and traction
