[Current development in therapy of congenital funnel chest].

OBJECTIVE: To review the current development in therapy of congenital funnel chest. METHODS: Recent literature concerning the development of the treatment method for congenital funnel chest was extensively reviewed and summarized. RESULTS: The main therapies for congenital funnel chest are thoracoplasty (Ravitch sternum elevation procedure and minimal invasive Nuss procedure) and prosthesis implantation. The magnetic mini-mover procedure and the vacuum bell are still in the research phase. CONCLUSION: Besides the improvement in function, the requirement in appearance after surgery is also i

Negative Pressure Wound Therapy for Residual Cavity Reduction After Open-Window Thoracostomy Following Pneumonectomy.

Postpneumonectomy empyema with a bronchopleural fistula (BPF) usually requires open-window thoracostomy, which can leave a large difficult-to-close residual cavity when muscle or omental flaps are unavailable. An 81-year-old man who developed empyema with a BPF after completion left pneumonectomy underwent open-window thoracostomy. The BPF closed with granulation, and the large pleural cavity remained. Negative-pressure wound therapy (-50 to -75 mm Hg) achieved >50% cavity reduction within 10 days, enabling definitive thoracoplasty. Negative-pressure wound therapy appears a safe and effective

Chronic expanding hematoma in the chest.

We herein report the successful surgical treatment of 2 cases of chronic expanding hematoma in the chest. The first patient, who had undergone thoracoplasty 42 years earlier due to tuberculosis, became aware of a slowly growing mass protruding in the lateral thoracic wall. The second patient, who had tuberculous pleurisy 36 years earlier, was referred to our department because of a slowly expanding intrathoracic mass revealed by a roentgenogram. The tumors, which were encapsulated chronic hematomas, were both surgically resected. These cases are rare because of the development of a very large

The Contemporary Utility of the Eloesser Flap for Management of Complex Pleural Space Infections.

A 27-year-old man with hyperimmunoglobulin E syndrome and remote left upper lobectomy for refractory hemoptysis presented with recurrent hemoptysis and a defunctionalized left lung. He underwent a reoperative thoracotomy and completion pneumonectomy complicated by a bronchopleural fistula. A 45-year-old man with a prior left upper lobectomy for adenocarcinoma presented with progressive cavitary Mycobacterium avium refractory to third-line antibiotics. These cases of chronic pleural space pathology in the immunosuppressed, reoperative setting were successfully addressed through a staged operati

A Novel Method of Chest Wall Reconstruction Succeeding Pediatric Heart Transplantation Donor-recipient Size Mismatch: 25-year Follow-up.

Pediatric heart transplantation can present with many obstacles, including size mismatch of the donor heart with the thoracic cavity of the recipient. For the patient to have vitality, they may need to have surgical interventions to address these conditions. We investigate the case of a 23-month-old infant who underwent orthotopic heart transplantation for nonischemic dilated cardiomyopathy. Given the large size of the donor heart relative to the infant, a subsequent modified thoracoplasty was performed to allow for chest wall closure. This report provides potential surgical techniques that ca

[Subtotal sternal agenesis with ectopia cordis: surgical management and long-term outcome (a case in Benin)].

Sternal agenesis as well as ectopia cordis are extremely rare congenital malformations. We here report a single case treated in the Department of Paediatric Surgery in Benin. The study involved a 3-year-old girl with congenital sternal agenesis associated with ectopia cordis; firstly, she underwent controlled healing. Then thoracoplasty was performed with favourable outcome. Long-term results are good. Now, she is 13 years old, is attending school and has a satisfactory clinical condition. This is one of the few cases reported in the literature. Optimal therapeutic management has been keeping

[Surgical approach for destructive pneumonia associated with COVID-19].

There were over 400 million people with COVID-19 pneumonia worldwide and over 12 million in the Russian Federation after 2020. Complicated course of pneumonia with abscesses and gangrene of lungs was observed in 4% of cases. Mortality ranges from 8 to 30%. We report 4 patients with destructive pneumonia following SARS-CoV-2 infection. In one patient, bilateral lung abscesses regressed under conservative treatment. Three patients with bronchopleural fistula underwent staged surgical treatment. Reconstructive surgery included thoracoplasty with muscle flaps. There were no postoperative complicat

Basing on the literature data analysis the authors suppose the insufficient studying of issues, concerning simultant interventions for pulmonary tuberculosis, coinciding with chronic phthysic pleurytis. Surgical treatment of such processes was performed in 181 patients, total pleurectomy--in 172 (95%) of them, partial--in 9 (5%). Various kinds of pulmonary resection as the additional intervention were performed in 172 (95%) patients, extrapleural thoracoplasty--in 15 (8.3%), diaphragmatic plasty--in 15 (8.3%). In early postoperative period the tuberculosis micobacteria have had disappeared in

[Differentiated approach to the method of sternum stabilization in surgical treatment of funnel chest in children].

Experience in surgical treatment of 186 patients with funnel chest deformity (FCD) is generalized. With consideration for the great variety of the forms of deformity of the plastron, simple and complicated forms of FCD are distinguished. The simple forms include isolated, symmetrical, low (beginning from the level of the fourth rib) II-III degree deformities, the complicated forms include wide, II-III degree deformities beginning from the level of the second rib, and flat asymmetrical deformities. The use of a metal plate as a stabilizer in complicated forms of FCD is suggested. The optimal me

Surgical treatment efficacy in 172 cases of tuberculosis-destroyed lungs.

OBJECTIVES: Compared to the use of anti-TB drugs for the treatment of tuberculosis (TB), surgery is destructive and is associated with higher risk. Whether surgery is needed for the treatment of TB-destroyed lungs (for short: destroyed lungs) is still controversial and unresolved in the clinic. METHODS: Retrospective analysis of treatment efficacy was performed on 172 cases of destroyed lungs (176 surgeries) in the Thoracic Surgery Department of our hospital from April 1992 to June 2010. RESULTS: A total of 83 male and 89 female patients was analyzed. The youngest patient was 7 years old, an

[Tuberculosis sequelae in Japan].

Tuberculosis had been the leading cause of death in Japan until 1950, and in these days there were about 3 million patients with active tuberculosis every year. From about 1950 to 1960 surgery was the treatment of choice if there were cavities and the lesions were regional. The number of patients who had thoracoplasties and/or pulmonary resections at national sanatoriums during the period of 1954 to 1961 was about 200,000. Since national sanatoriums had about 25% of the total beds for tuberculosis in Japan at that time, the total number of surgically treated patients would be around four times

Spinal hydatid cyst as cause of neurological injury in a patient from Brazilian amazon region.

Echinococcus granulosus infection is the primary cause of spinal hydatidosis. We describe the case of a 22-year-old man from the Brazilian Amazon region with crural spastic paraparesis and back pain. Radiological examinations showed multilocular lesions involving compression of the thoracic spine and rib injury. The patient underwent vertebrectomy with spinal stabilization and thoracoplasty with resectioning of the costal arch. Subsequently, the patient was prescribed oral treatment with albendazole. Marked recovery of the neurological status was achieved. Bone hydatid disease is rare, account

Anterior transpericardial closure of a main bronchus fistula after pneumonectomy.

A young woman sustained a penetrating wound to the right anterior chest during a vehicular accident. Septic complications led to emergency pneumonectomy followed by infection of the pleural space and disruption of the right bronchus closure. Her condition improved after creation of a pleural window for dependent drainage and gauze packing of the pleural space. Subsequently, the open bronchial stump was closed utilizing a transpericardial approach through a median sternotomy incision which permitted eventual closure of the pneumonectomy space without thoracoplasty. When the length of the bronch

Risk factors for pulmonary complications after posterior spinal instrumentation and fusion in the treatment of congenital scoliosis: a case-control study.

BACKGROUND: Although surgery prevents the progression of deformity and maintains the overall balance of the spine in congenital scoliosis (CS) patients, it is associated with a high risk of perioperative complications. Pulmonary complication is one of the most common complications. This retrospective study aimed to investigate the risk factors for pulmonary complications in CS patients after posterior spinal instrumentation and fusion. METHODS: Analysis of consecutive patients who underwent posterior spinal instrumentation and fusion for congenital scoliosis was performed. Preoperative clinic

Chest Surgery for Transgender and Gender Nonconforming Individuals.

Chest surgery can greatly facilitate the experience of living in a gender role. For transfeminine chest surgery, most surgeons recommend a 12-month period of feminizing hormone therapy prior to breast augmentation. For those who already have some breast volume due to hormone treatment, lipofilling can be a good option. Transmasculine chest surgery includes mastectomy and creation of a male chest. Preoperative parameters to be evaluated include breast volume, degree of excess skin, nipple-areola complex size and position, and skin elasticity. The algorithm the authors developed and modified can
