Tube Thoracostomy at the Time of Congenital Diaphragmatic Hernia Repair: Reassessing the Risks and Benefits.

PURPOSE: Postoperative pneumothorax and effusion remain a concern following congenital diaphragmatic hernia (CDH) repair. Despite a recent trend away from intraoperative thoracostomy, few studies have actually compared outcomes with and without a chest tube. Rationale commonly cited for the more minimalistic approach include the presumed low likelihood of postoperative complications, potential risk of patch infection, and prolonged intubation. We evaluate these theories, as well as the implications of intraoperative chest tube (IOCT) placement. METHODS: We performed a retrospective chart revi

The impact of a chest tube management protocol on the outcome of trauma patients with tube thoracostomy.

OBJECTIVE: to investigate the effect of standardized interventions in the management of tube thoracostomy patients and to assess the independent effect of each intervention. METHODS: A chest tube management protocol was assessed in a retrospective cohort study. The tube thoracostomy protocol (TTP) was implemented in August 2012, and consisted of: antimicrobial prophylaxis, chest tube insertion in the operating room (OR), admission post chest tube thoracostomy (CTT) in a hospital floor separate from the emergency department (ED), and daily respiratory therapy (RT) sessions post-CTT. The inclus

Horner's syndrome as a rare complication of tube thoracostomy: case reports and review of literature.

Horner's Syndrome (HS), caused by the interruption of the oculosympathetic pathway, is a rare yet morbid complication of tube thoracostomy. However, literature regarding HS secondary to tube thoracostomy is limited to case reports, with little comprehensive information available. We report two cases and review all cases from the published literature to assess the outcome of this complication. HS secondary to tube thoracostomy leaves sequelae in 45.8% of patients. Immediate removal or repositioning of the tube does not affect prognosis. Therefore, precautions must be taken to avoid this complic

Horner's syndrome after treatment of tension pneumothorax with tube thoracostomy in a newborn infant.

A premature infant who developed respiratory distress syndrome required artificial ventilation. A right-sided tension pneumothorax at the age of 78 h was treated by insertion of a P.V.C. chest tube reaching deep into the right upper chest. At discharge on day 51 a Horner's syndrome of the right eye was noticed. Detailed examination at the age of one year revealed a typical peripheral preganglionar Horner's syndrome. Because other causes could be ruled out we assume that the lesion of the sympathetic nerve fibers was caused by the tip of the chest tube near the first thoracic intervertebral spa

Beyond light's criteria: pleural lactate predicts need for tube thoracostomy in complicated parapneumonic effusions.

BACKGROUND: Differentiation of pleural effusions and identification of complicated parapneumonic effusions remain important challenges in clinical practice. Although Light's criteria are routinely applied, additional pleural fluid parameters may provide complementary information in selected patients. AIMS: To evaluate the diagnostic performance of pleural fluid lactate and the pleural fluid-to-blood lactate ratio in pleural effusions and to examine their association with complicated parapneumonic effusions and drainage approach. METHODS: This study included 110 patients who underwent pleural

Pediatric Emergency Tube Thoracostomy: Complications and Independent Risk Factors in a Five-Year Cohort.

OBJECTIVES: Tube thoracostomy is a frequently performed emergency procedure in pediatric patients; however, complication patterns and risk stratification in emergency settings remain incompletely defined. This study aimed to evaluate 30-day complication rates and identify independent predictors of adverse outcomes in pediatric patients undergoing tube thoracostomy in emergency settings. METHODS: This retrospective observational cohort study included 451 consecutive pediatric patients (0-18 years) who underwent tube thoracostomy between June 2020 and June 2025 at a tertiary referral center. Negative Pressure Wound Therapy for Residual Cavity Reduction After Open-Window Thoracostomy Following Pneumonectomy.

Postpneumonectomy empyema with a bronchopleural fistula (BPF) usually requires open-window thoracostomy, which can leave a large difficult-to-close residual cavity when muscle or omental flaps are unavailable. An 81-year-old man who developed empyema with a BPF after completion left pneumonectomy underwent open-window thoracostomy. The BPF closed with granulation, and the large pleural cavity remained. Negative-pressure wound therapy (-50 to -75 mm Hg) achieved >50% cavity reduction within 10 days, enabling definitive thoracoplasty. Negative-pressure wound therapy appears a safe and effective

Serratus Anterior Plane Block for Procedural Anesthesia for Pigtail Tube Thoracostomy: A Case Series.

INTRODUCTION: Pneumothoraces are frequently treated by emergency physicians. Tube thoracostomy, the definitive treatment for a spontaneous pneumothorax, is associated with significant pain. Analgesia prior to tube thoracostomy often involves the administration of opioids and local infiltration of anesthetics. Thus far, regional anesthesia prior to pigtail tube thoracostomy in the emergency department (ED) has not been well described; it offers promise in alleviating pain associated with this procedure. Due to its ability to anesthetize all or most of the structures associated with tube thoraco

Small versus large-bore thoracostomy for traumatic hemothorax: A systematic review and meta-analysis.

BACKGROUND: Traumatic hemothorax (HTX) is common, and while it is recommended to drain it with a tube thoracostomy, there is no consensus on the optimal catheter size. We performed a systematic review to test the hypothesis that small bore tube thoracostomy (SBTT) (≤14 F) is as effective as large-bore tube thoracostomy (LBTT) (≥20F) for the treatment of HTX. METHODS: Pubmed, EMBASE, Scopus, and Cochrane review were searched from inception to November 2022 for randomized controlled trials or cohort studies that included adult trauma patients with HTX who received a tube thoracostomy. Dyspnea following thoracostomy closure after right pneumonectomy: An uncommon echocardiographic diagnosis and therapeutic approach.

Dyspnea and hypoxemia are common postoperative problems after pneumonectomy. One of the rarer causes of respiratory distress after right pneumonectomy is the development of a significant right-to-left shunt across a patent foramen ovale (PFO), which can evolve at a variable interval of time after the operation. We report here our experience with a patient who underwent right pneumonectomy, followed by several complications, and who presented severe dyspnea 7 months later, after the closure of a right thoracostomy. This report outlines the management of this challenging clinical condition; tran

Repair of diaphragmatic injury and placement of tube thoracostomy during right upper quadrant peritonectomy.

OBJECTIVE: Patients with advanced or recurrent ovarian cancer often have metastatic disease in the upper abdominal region, especially to the right hemidiaphragm, which requires diaphragmatic resection in order to achieve optimal cytoreduction. The aim of this surgical video is to demonstrate repair of a diaphragmatic injury and placement of tube thoracostomy during right upper quadrant peritonectomy in a patient with recurrent ovarian cancer. METHODS: This is the case of a 45-year-old woman presented with platinum sensitive recurrent ovarian cancer. Abdomen computed tomography also confirmed

Closed Tube Thoracostomy Morbidity Rate in a Philippine Tertiary Government Hospital: 6-Year Review.

BACKGROUND AND OBJECTIVE: Closed tube thoracostomy is one of the most common hospital procedures known for its effectivity and safety; however, complications may occur, leading to poor patient outcomes. To date, the burden of morbidity among patients who underwent closed tube thoracostomy in the Philippines remains unknown. Therefore, this study aims to determine the in-hospital morbidity rate among patients who underwent closed tube thoracostomy in a tertiary government hospital. METHODS: This is a descriptive cross-sectional study of admitted patients who underwent closed tube thoracostomy

Clinical presentation of tension pneumothorax among patients undergoing prehospital thoracostomy: A retrospective cohort study.

BACKGROUND: Tension pneumothorax (TPTX) is a life-threatening condition requiring rapid diagnosis and management. However, accurate diagnosis is challenging in the prehospital environment. We sought to investigate the clinical presentation of TPTX in the prehospital environment. Secondary objectives were to investigate if any vital signs are independently associated with TPTX, and if any differences in vital signs of TPTX exist between patients based on ventilatory status. METHODS: We conducted a retrospective cohort study of adult patients > 18 years of age, not in cardiac arrest, who receiv

Prehospital portable ultrasound for safe and accurate prehospital needle thoracostomy: a pilot educational study.

BACKGROUND: Simulated needle thoracostomy (NT) using ultrasound may reduce potential injury, increase accuracy, and be as rapid to perform as the traditional landmark technique following a brief educational session. Our objective was to determine if the use of an educational session demonstrating the use of handheld ultrasound to Emergency Medical Services (EMS) staff to facilitate NT was both feasible, and an effective way of increasing the safety and efficacy of this procedure for rural EMS providers. METHODS: A pre/post-educational intervention on a convenience sample of rural North Americ

Reexpansion Pulmonary Edema following Tube Thoracostomy in a Pediatric Patient with Anterior Mediastinal Mass.

Reexpansion pulmonary edema (RPE) is an exceedingly rare and potentially fatal complication of a rapidly reexpanded lung following evacuation of air or fluid from the pleural space secondary to conditions such as a mediastinal mass, pleural effusion, or pneumothorax. Clinical presentations can range from mild radiographic changes to acute respiratory failure and hemodynamic instability. The rapidly progressive nature of the disease makes it important for clinicians to appropriately diagnose and manage patients who develop RPE. We present a case of a child with a large malignant pleural effusio

Clinical Characteristics of Patients Undergoing Needle Thoracostomy in a Canadian Helicopter Emergency Medical Service.

Objective: Needle thoracostomy (NT) can be a life-saving procedure when used to treat tension pneumothorax. However, there is some question regarding the efficacy of NT in the prehospital setting. Failure to treat tension pneumothorax in a helicopter emergency medical service (HEMS) setting may prove especially deleterious to the patient due to gas expansion with increasing altitude. This study's objective was to identify the characteristics of patients treated with NT in a Canadian HEMS setting and the factors that may influence outcomes following NT use. Methods: This was a retrospective char

Management of emergency department patients with primary spontaneous pneumothorax: needle aspiration or tube thoracostomy?

STUDY OBJECTIVE: The emergency management of primary spontaneous pneumothorax is controversial. This evidence-based emergency medicine review evaluates the existing evidence about the efficacy and safety of needle aspiration in comparison to tube thoracostomy for management of primary spontaneous pneumothorax. METHODS: We searched MEDLINE, EMBASE, the Cochrane Library, and other databases. We selected studies for inclusion in the review if the authors stated that they had randomly assigned hemodynamically stable patients with no underlying lung disease to needle aspiration or tube thoracostom
