[Three-phalangeal thumbs].

Many teratogenic mechanisms converge to create a three-phalangeal thumb: duplication, fusion and deletion. Fine observation of the position of growth centres and of associated hand malformations suggests that the intermediate delta phalanx of the three-phalangeal thumb results of the fusion of a three-phalanx ray with a two-phalanx thumb. The ulnar or radial side of the deviation is determined by the radial or ulnar position of this three-phalangeal ray fusing with the thumb. An opposable three-phalangeal thumb ulnarly deviated with a radial intermediate delta phalanx is the result of the fusi

Triplicated thumbs: a rarity?

Triplication of the thumb is supposed to be a rare condition and a complex form of radial polydactyly. However, we encountered an unusually high number of triplicated thumbs at our unit. Is triplication of the thumb indeed a rare condition? In our study, 121 patients with radial polydactyly were recorded between 1933 and 2005. In nine patients of this group, triplicated thumbs, either unilateral or bilateral, were identified. This complex type of radial polydactyly occurs in various forms. A total of 11 triplicated thumbs were found, and all cases were combined with triphalangeal components. O

Treatment of the triphalangeal thumb.

Triphalangeal thumb is a thumb with 3 phalanges and has an estimated incidence of 1 in 25,000 live births. Clinical presentation of triphalangeal thumb can vary considerably. Most strikingly is the long finger-like thumb with clinodactyly, in the same plane as the fingers and may or may not present with an extra thumb. Anatomically, the extra phalanx can have different shapes, from wedge to rectangular. Furthermore, the involved joints, ligaments, muscles, and tendons of the first ray, from distal interphalangeal joint to radiocarpal joint, can be hypoplastic, malformed, or absent with varying

[Apparent pseudarthrosis of the thumb].

Pseudarthrosis in the hand are defined as a non-union of a fracture after 6 month. Reasons might be biological disorders, mechanical instability or infection after fracture treatment. Pseudoarthrotic bone demands stable osteosynthesis and often bone grafting. The presented case is about a dislocated osseous fragment attached to the Aponeurosis dorsalis mimicking clinical and radiological a Pseudarthrosis of the proximal phalanx of the thumb. We would like to draw attention that osseous fragments may present very similarly to a pseudoarthrosis of a phalanx.

An uncommon injury of the thumb.

Ulnar collateral ligament rupture of the thumb is a relatively common injury that is often missed in the emergency department. This in combination with an interphalangeal joint dislocation of the ipsilateral thumb is rare and we report such a case. The importance of looking specifically for an associated ulnar collateral ligament laxity in any injury to the thumb is highlighted. The force producing a combination of ulnar collateral ligament rupture with ipsilateral simultaneous injury to the thumb is often severe enough to cause complete rupture of the ligament, necessitating open repair.

Disorders of the thumb sesamoids.

The subsesamoid joints of the thumb are a common site of arthritis, but their small size makes diagnosis of disorders challenging. The sesamoid and subsesamoid joints may also be injured acutely with the volar plate complex during hyperextension injuries, and may sometimes produce mechanical dysfunction. Simple excision of chronically painful sesamoids provides excellent relief.

Reconstruction of the hypoplastic thumb.

Blauth has classified congenital hypoplasia of the thumb into five grades. Grade I requires no treatment. Grade II can be reconstructed by widening the first web space with various flaps, stabilizing the metacarpophalangeal joint, and performing an opponensplasty. In such cases, a pollex abductus configuration of the flexor pollicis longus should be sought, and if found, corrected. Grades III, IV, and V require pollicization, a procedure that has become more sophisticated by attention to skin flap design, intrinsic transfer, and positioning of the head of the second metacarpal. Partial aplasia

Polydactyly and the triphalangeal thumb.

Polydactyly associated with triphalangeal thumb is not rare, as among the records of 1,269 patients with congenital deformation of the upper extremity at the University of Iowa there were 21 patients with 32 hands with such involvement. More detailed classification based on Wassel's work was made. Treatment should be early, with removal of the radial digit if equal in size, excision of all accessory parts including delta bone when present, reconstruction of joint ligaments or joint fusion and osteotomy for correction of deviations. Care should be taken to avoid leaving a narrow first web space

Second Toe-to-thumb Transfer Using Intramedullary Screw Fixation after Traumatic Thumb Amputation.

Loss of the thumb secondary to trauma leads to significant disability due to the digit contributing 40% of overall hand function. Toe-to-thumb transfer has been a well-studied reconstructive method as a means to restore function after thumb amputation. First described in 1969 by Cobbett and colleagues, toe-to-thumb transfers have undergone several modifications in technique to better improve functional and aesthetic outcomes, including toe wrap-around flaps, trimmed great toe transfer, and second toe transfers. Although these methods have allowed patients to retain use of the thumb, traditiona

[Post-traumatic palmar instability of the thumb metacarpophalangeal joint. The "other skier's thumb"].

"Passive" and "active" stabilizers give the thumb metacarpophalangeal (MP) joint the necessary palmar stability. It is possible to temporarily stabilize the joint by means of the flexor muscles in cases where there is an injury to the palmar ligaments due to hyperextension trauma. However, sudden severe or prolonged strain may lead to palmar instability and/or stress-related pain. In an experimental study, the MP joints of 40 specimen thumbs were hyperextended to varying degrees. The extent of tissue damage was evaluated during dissection. Between 1984 and 1992, in 72 cases with acute hyperext

The Dorsal Aponeurosis of the Thumb.

PURPOSE: To evaluate the thumb dorsal aponeurosis anatomy. Consideration of structural differences between the fingers and thumb will provide an improved clinical understanding of the thumb dorsal aponeurosis anatomy. METHODS: Ten fresh cadaver hands from 5 patients with an average age of 31.6 (range, 22-41) years were dissected. The thenar muscles were identified and insertion sites were documented. The fibers of the dorsal aponeurosis and contributions were identified. RESULTS: The flexor pollicis brevis superficial head contributed to the radial fibers of the dorsal aponeurosis in 8 speci

Crossbow injuries to the thumb.

Archery injuries resulting in significant digital impairment are uncommon. The purpose of this study was to review seven patients treated at our institution for injuries of the left thumb sustained during recreational use of a crossbow. We reviewed all thumb injuries caused by crossbow use treated at Geisinger Health System between August 2011 and November 2012. The electronic medical records were reviewed to document mechanism of injury, injury pattern, patient characteristics, treatment, and outcomes. Seven patients were identified with injury to their left thumb, six males and one female. T

[Myxoid neurofibroma of the thumb].

UNLABELLED: Myxoid neurofibroma (MN) is a benign tumor of poorly identified perineural cell origin. We report a case of NM of left thumb and discuss the main differential diagnosis. CASE REPORT: A 34-year-old woman presented with a painless, slowly progressive tumor of the left thumb of 6 months duration. This tumor had recurred after resection done 26 years before. Clinical examination revealed a tumor on the anterior surface of the left thumb, painless, firm, 4×3cm, ulcerated and covered with thin telangiectasia. The X-ray of the phalanx showed no alteration to the underlying bone. The hist

Necrotising fasciitis of the thumb.

Necrotising fasciitis is a rare condition that should be diagnosed early and managed aggressively in order to avoid death. We present a case of necrotising fasciitis of the thumb and discuss the assessment and management of this serious condition. A 44-year-old woman presented with a painful and erythematous right thumb and progressive swelling of her hand following a minor injury to the tip of her thumb 3 days previously. A diagnosis of necrotising fasciitis was made and the patient underwent urgent debridement of non-viable tissue. She required 1 week of supportive therapy in intensive treat

[Osteoarthritis of the thumb and fingers].

Most commonly affected joints of the hand in osteoarthritis include the carpometacarpal joint of the thumb (CMC 1) and the distal (DIP) and proximal (PIP) interphalangeal joints. Ageing, female gender, genotype, heavy work causing pressure on the hands, and injuries predispose to osteoarthritis in the hand. The pain is likely to be due to secondary synovitis caused by molecules released from the joint cartilage. Initial treatment of osteoarthritis is always conservative: analgesic medication, splint and physiotherapy. Surgery is considered for severe symptoms. The most common procedures includ
