Functional near-infrared spectroscopy (fNIRS) biomarkers of tinnitus severity within tinnitus subtypes.

Tinnitus, the perception of sounds not present externally, can significantly impair quality of life. Currently there are no clinically available objective measures to enable monitoring of tinnitus-related changes in brain activity. Our previous work demonstrated sensitivity of a non-invasive imaging technique called functional near-infrared spectroscopy (fNIRS) to changes in brain activity associated with tinnitus presence and severity. In this study we aimed to assess whether fNIRS features could quantify tinnitus severity on a continuous scale, a requirement for effective monitoring of treat

Individualized Treatment of Tinnitus during Sleep Using Combined Tinnitus Signal and Music.

INTRODUCTION: Tinnitus is a widely seen otological symptom that interferes with daily activities and causes discomfort. Tinnitus treatments can be classified into 4 main groups: pharmacological treatments, cognitive and behavioral therapy, psychological treatments, and combined treatment approaches made up of at least 2 of these 3 treatment methods. OBJECTIVE: The aim of this study was to assess whether it would be possible to develop an individualized treatment method of tinnitus by application of a combined tinnitus signal and music during sleep. METHODS: Forty-three ears of 30 patients wh

Comparison of Tinnitus Handicap Inventory and Tinnitus Functional Index as Treatment Outcomes.

BACKGROUND: Tinnitus is an audiological disorder for which there are no objective measuring tools. Thus, many self-report questionnaires have been proposed to assess its severity. These questionnaires have been judged for their capacity to assess the tinnitus severity at baseline, their sensitivity to treatment-related changes (responsiveness), and their resolution. METHODS: The most widely used questionnaires for clinical and research studies are the Tinnitus Handicap Inventory (THI) and the Tinnitus Functional Index (TFI). While both questionnaires have been recognized as good evaluators of

Tinnitus and tinnitus disorder: Theoretical and operational definitions (an international multidisciplinary proposal).

As for hypertension, chronic pain, epilepsy and other disorders with particular symptoms, a commonly accepted and unambiguous definition provides a common ground for researchers and clinicians to study and treat the problem. The WHO's ICD11 definition only mentions tinnitus as a nonspecific symptom of a hearing disorder, but not as a clinical entity in its own right, and the American Psychiatric Association's DSM-V doesn't mention tinnitus at all. Here we propose that the tinnitus without and with associated suffering should be differentiated by distinct terms: "Tinnitus" for the former and "T

Why Is Tinnitus a Problem? A Qualitative Analysis of Problems Reported by Tinnitus Patients.

Tinnitus is a prevalent complaint, and people with bothersome tinnitus can report any number of associated problems. Yet, to date, only a few studies, with different populations and relatively modest sample sizes, have qualitatively evaluated what those problems are. Our primary objective was to determine domains of tinnitus problem according to a large clinical data set. This was a retrospective analysis of anonymized clinical data from patients who attended a U.K. Tinnitus Treatment Center between 1989 and 2014. Content analysis was used to code and collate the responses of 678 patients to t

Positive and Negative Thinking in Tinnitus: Factor Structure of the Tinnitus Cognitions Questionnaire.

OBJECTIVES: Researchers and clinicians consider thinking to be important in the development and maintenance of tinnitus distress, and altering thoughts or thinking style is an object of many forms of psychological therapy for tinnitus. Those working with people with tinnitus require a reliable, psychometrically robust means of measuring both positive and negative thinking related to it. The Tinnitus Cognitions Questionnaire (TCQ) was designed as such a measure and its authors showed it to be reliable, with good psychometric properties. However, no research teams have yet carried out independen

Polish Translation and Validation of the Tinnitus Handicap Inventory and the Tinnitus Functional Index.

Objective: The need for validated measures enabling clinicians to classify tinnitus patients according to the severity of tinnitus and screen the progress of therapies in our country led us to translate into Polish and to validate two tinnitus questionnaires, namely the Tinnitus Handicap Inventory (THI) and the Tinnitus Functional Index (TFI). Design: The original English versions of the questionnaires were translated into Polish and translated back to English by three independent translators. These versions were then finalized by the authors into a Polish THI (THI-Pl) and a Polish TFI (TFI-Pl

A scientific cognitive-behavioral model of tinnitus: novel conceptualizations of tinnitus distress.

The importance of psychological factors in tinnitus distress has been formally recognized for almost three decades. The psychological understanding of why tinnitus can be a distressing condition posits that it becomes problematic when it acquires an emotive significance through cognitive processes. Principle therapeutic efforts are directed at reducing or removing the cognitive (and behavioral) obstacles to habituation. Here, the evidence relevant to a new psychological model of tinnitus is critically reviewed. The model posits that patients' interpretations of tinnitus and the changes in beha

The Tinnitus Handicap Inventory as a screening test for psychiatric comorbidity in patients with tinnitus.

BACKGROUND: Psychiatric comorbidity is common in patients who seek help for tinnitus. The perceived severity of tinnitus correlates closer to psychological and general health factors than to audiometrical parameters. Audiologists need valid screening tools in order to identify patients with psychiatric disorders and to tailor treatment in a multidisciplinary setting. The tinnitus handicap inventory (THI) has gained widespread acceptance as a self-report measure of tinnitus handicap. In several studies, THI test score correlates with the level of psychopathologic distress. The aim of our study

Tinnitus and acoustic neuromas: analysis of the effect of surgical excision on postoperative tinnitus.

In this retrospective review of the 160 acoustic neuroma cases treated at The University of Iowa between 1980 and 1991, follow-up information was obtained for 82% by telephone interview. Postoperative tinnitus was a symptom reported by 75% of the acoustic neuroma patients. Overall, among patients with tinnitus preoperatively, the condition postoperatively was absent in 45%, lessened in 17%, unchanged in 30%, and greater in 8%. Detailed results are compared to other series and significant differences in the depth of comparison described.

Tinnitus-induced weight loss in rats. An animal model for tinnitus research.

Growing rats were exposed to noise and/or tinnitus-producing drugs (sodium salicylate, quinine, gentamycin). Growth inhibition or weight loss was absent or significantly lower in the noise-only and salicylate-only/quinine-only groups than in the groups exposed to both noise and drugs. In the gentamycin groups, initially the same trend was found, but general toxicity soon prevailed, leading to heavy weight losses in both groups. The findings clearly favor the hypothesis that tinnitus-induced stress may be the cause for this effect. The experimental set-up thus could serve as an animal model for

Patients with tinnitus use more primary healthcare compared to people without tinnitus.

Tinnitus is a heterogeneous condition not only in terms of nature of the sound, but also in co-morbidities such as mental health issues. Prevalence number range widely between 5 and 43%. Even though the etiologic pathway between tinnitus and its comorbidities remains unclear, in this study we aim to assess whether people with tinnitus use more primary health care than people without tinnitus. To compare primary healthcare consumption between patients with tinnitus and people without tinnitus. In this cross-sectional study, data on number of consultations with the general practitioner or nurse

An adaptive short form of the tinnitus handicap inventory (THI) applicable for slight and mild tinnitus, facilitating online tinnitus treatment triage.

OBJECTIVE: This study developed an adaptive short form of the Tinnitus Handicap Inventory (THI) using a Genetic Algorithm (GA) to facilitate online tinnitus treatment triage. The full 25-item THI can increase patient burden, highlighting the need for a shorter, effective alternative. DESIGN: A genetic algorithm (GA) selected THI items in a three-stage scheme (THI-12/8/5). The GA was trained on Dutch THI datasets and tuned to minimise the difference between projected and full THI-25 scores below the mild threshold. STUDY SAMPLE: The GA was trained on 1,121 questionnaires and validated on a se

Subtyping patients with somatic tinnitus: Modulation of tinnitus and history for somatic dysfunction help identify tinnitus patients with temporomandibular joint disorders.

OBJECTIVE: Determine in a cohort of patients with normal hearing and chronic tinnitus if self-reported history for temporomandibular joint (TMJ) dysfunction and a positive modulation of tinnitus in the TMJ region could be suggestive of an underlying TMJ disorder. PATIENTS AND METHODS: The study included 226 patients presenting to the Head and Neck Service of our University Hospital. Following audiological and somatic tinnitus evaluation, patients were divided into two groups. The study group (n = 134) included subjects that met both the following criteria: A) a self-reported history for TMJ d

Change in Tinnitus Severity After an Online Self-Paced Tinnitus Course: A Retrospective Cohort Study in Acute and Chronic Tinnitus Patients.

Background: Tinnitus can significantly impact a patient's quality of life. As no evidence-based curative treatments exist, therapies such as cognitive behavioral therapy, tinnitus retraining therapy, acceptance and commitment therapy, and mindfulness-based interventions aim to minimize tinnitus severity and have been shown effective. Since traditional delivery can be costly and time-consuming and often has limited accessibility, therapies might also be provided via eHealth. This study investigates the change in tinnitus severity measured by the Tinnitus Functional Index (TFI) score after parti
