Arthritis and Periodontitis: An Association Debated for Over Two Centuries.

The chronic diseases, rheumatoid arthritis (RA) and periodontal disease (PD) lead to confined destruction of soft and hard tissues as a result of inflammatory processes. Their pathogenesis is dictated by a network of inflammatory cells and its mediators. They also share some etiological risk factors and therapeutic alternatives. The evolution of focal infection theory is summarized in this review, with special reference to PD and its relationship to RA. Foci of chronic infections exist in the oral cavity and may result in anatomically distant disease in certain individuals. Recent cumulative e

[The role of thoracotomy for malignant mediastinal tumors (MMT)-- survey of consecutive 223 patients].

To elucidate the role of surgery for MMT, 214 patients during past 34 years were historically divided into the early (1960-1971, n = 80), intermediate (1972-1981, n = 79) and late series (1982-1993, n = 64). The thoracotomy rate for MMT, the rate of thoracotomy after preoperative diagnosis was done, and the number of postoperative survivors were significantly higher in the late series, compared with the early series. The intermediate series located between the early and late series. Nine patients in the late series undergoing chemo-radiation therapies obtained a good result after complete rese

Hyperbaric Oxygen Therapy for Cognitive Impairment Resulting from Neurological Disorders: Mechanisms of Action and Application Advances.

Cognitive impairment is a common and disabling consequence of various neurological disorders. Due to its high prevalence, high conversion rate, and significant burden, it has increasingly become a major challenge for aging societies worldwide. Existing treatment strategies have limited efficacy in halting disease progression, making effective intervention strategies urgently needed. As a non-invasive physical therapy, hyperbaric oxygen therapy (HBOT) exerts its effects by increasing arterial oxygen partial pressure and improving tissue oxygenation. Historically widely used for treating carbon

When Myeloma Escapes the Bone Marrow: Extramedullary Disease in the Immunotherapy Era.

Extramedullary disease (EMD)-the proliferation of clonal plasma cells in soft tissues without direct bone connection-represents one of the most challenging manifestations of multiple myeloma, associated with aggressive biology, treatment resistance, and poor outcomes. EMD is driven by distinct pathophysiologic mechanisms including downregulation of adhesion molecules, acquisition of high-risk cytogenetic abnormalities (del(17p), gain(1q)), activation of the RAS-MAPK pathway, epigenetic dysregulation such as EZH2 upregulation, and remodeling of the immune microenvironment toward an immunosuppre

Disrupting B and T-cell collaboration in autoimmune disease: T-cell engagers versus CAR T-cell therapy?

B and T cells collaborate to drive autoimmune disease (AID). Historically, B- and T-cell (B-T cell) co-interaction was targeted through different pathways such as alemtuzumab, abatacept, and dapirolizumab with variable impact on B-cell depletion (BCD), whereas the majority of patients with AID including rheumatoid arthritis, systemic lupus erythematosus, multiple sclerosis, and organ transplantation benefit from targeted BCD with anti-CD20 monoclonal antibodies such as rituximab, ocrelizumab, or ofatumumab. Refractory AID is a significant problem for patients with incomplete BCD with a greater

Pulmonary Arterial Hypertension: Recognition and Management in Primary and Acute Care Settings.

Pulmonary arterial hypertension (PAH) is a rare but life-threatening condition marked by progressive elevation in pulmonary vascular resistance and right ventricular failure. Because symptoms, most commonly progressive exertional dyspnea, overlap with more common cardiopulmonary disorders, diagnosis is frequently delayed. Primary and acute care clinicians are often the first to evaluate these patients, placing them in a key position to recognize early clues and initiate appropriate evaluation. This review provides practical, evidence-based guidance for the frontline assessment of suspected PAH

Successful Treatment of Upper Limb Superficial Lymphatic Malformation With Topical Sirolimus.

Lymphatic malformation (LM) is a congenital anomaly of the lymphatic system that can affect various anatomical sites, most commonly skin and subcutaneous tissues. Treatments that were historically used, including surgery, laser therapy (pulsed dye laser [PDL] and CO2 laser), and sclerotherapy, can be associated with complications such as bleeding and lesion recurrence. Oral sirolimus is effective in treating LMs but can be associated with systemic side effects, including immunosuppression and metabolic disturbances. Targeting the mTOR pathway, topical sirolimus effectively treats superficial L

Advances in preclinical models for pediatric diffuse intrinsic pontine glioma.

Diffuse intrinsic pontine gliomas (DIPG) are highly aggressive brainstem tumors, representing the leading cause of pediatric cancer-related mortality despite their rarity. DIPGs are predominantly characterized by the H3K27M mutation, which drives tumorigenesis through epigenomic reprogramming and dysregulated gene expression. A major barrier to therapeutic advancement has been the scarcity of representative preclinical models, historically limited by the rarity of tumor tissue samples. Recent advancements in biopsy safety and model development have accelerated progress in understanding DIPG bi

Eosinophil diversity and function: emerging insights into subtypes and therapeutic targets.

Eosinophils are multifunctional polymorphonuclear granulocytes garnering attention for their roles in disease and maintaining physiological homeostasis. Historically, these immune cells were known to mediate allergic responses and defend against parasitic infections. Recent evidence further links eosinophils to several eosinophil-associated disorders, including eosinophilic esophagitis, eosinophilic granulomatosis with polyangiitis, and hypereosinophilic syndrome; however, their pathogenic contribution appears to be disease-specific and remains incompletely defined in some contexts. The advanc

Testosterone Therapy Among Prostate Cancer Survivors.

INTRODUCTION: The use of testosterone in men with a history of prostate cancer remains controversial in light of established findings linking androgens to prostate cancer growth. However, hypogonadism significantly affects quality of life and has negative sequelae, and the risks and benefits of testosterone therapy might be worthwhile to consider in all men, even those with a history of high-risk prostate cancer. AIM: To discuss the effects of testosterone on the prostate and the use of testosterone therapy in hypogonadal men with a history of prostate cancer. METHODS: Review of the literatu

Editorial Commentary: Arthroscopic Treatment of Mild Hip Dysplasia Can Result in Excellent Outcome and Avoid More Invasive Periacetabular Osteotomy.

Several mechanisms either support or decrease stability of the hip joint. Primary stability of the hip comes from bony coverage of the femoral head, influenced by acetabular version and femoral antetorsion. In addition, soft tissue structures, such as the acetabular labrum, the ligamentum teres, and the hip capsule play a significant role in maintaining joint stability. Untreated hip instability may lead to pathological force transmission between the acetabular socket and femoral head, and subluxation resulting in osteoarthritis. Historically, pelvic and/or femoral osteotomies have been perfor

Endoscopic techniques for full thickness intestinal biopsy.

PURPOSE OF REVIEW: Accurate diagnosis of gastrointestinal neuromuscular diseases requires full thickness biopsy for adequate histologic evaluation of the enteric neuromuscular and ancillary cells. Historically, this has been achieved by surgical approaches. An overview of procedure evolution and current techniques of endoscopic full thickness biopsy (EFTB) for diagnosis of gastrointestinal neuromuscular disorders will be presented. RECENT FINDINGS: Emergence and advancement of endoscopic full thickness resection techniques has offered a less invasive, nevertheless an effective modality of tis

[Relapsing polychondritis: What's new in 2017?].

Relapsing polychondritis (RP) is a rare condition characterized by recurrent inflammation of cartilaginous tissue and systemic manifestations. Data on pathophysiology are scarce and suggest an autoimmune mechanism. Recently, the possibility of dividing patients with RP into three distinct clinical phenotypes has been suggested: the hematological form representing less than 10% of patients, essentially older men with associated myelodysplasia and poor prognosis, the respiratory form representing about 25% of patients with predominant tracheobronchial involvement, and the mild and most frequent
