Osteochondral autograft from the second toe for complex proximal interphalangeal joint fracture-dislocations: a case report and literature review.

BACKGROUND: Managing complex fracture-dislocations of the proximal interphalangeal (PIP) joint poses significant clinical challenges, necessitating innovative treatment strategies. CASE DESCRIPTION: This case report highlights the successful treatment of a severe PIP joint fracture-dislocation in a 49-year-old male carpenter who sustained a traumatic laceration to the right index finger. The injury's complexity led to the pioneering use of an osteochondral autograft from the patient's second toe proximal phalanx, marking a novel approach in autologous tissue utilization for joint reconstructi

Biomechanical effects of an articulating prosthetic toe joint during stair navigation for individuals with unilateral, below-knee limb loss.

Stair navigation is an essential and demanding form of locomotion. During stair ascent and descent, persons with lower limb loss exhibit gait characteristics which may increase their risk of falls and joint degeneration of the intact limb. To reduce deviations from typically-able-bodied gait and overloading of the intact limb for this population, one potential intervention involves modifying passive prosthetic feet by incorporating a flexible toe joint that simulates the biological metatarsophalangeal joint. In this study, we aimed to assess the user preferences and biomechanical effects of a

Force Generation on the Hallux Is More Affected by the Ankle Joint Angle than the Lesser Toes: An In Vivo Human Study.

The structure of the first toe is independent of that of the other toes, while the functional difference remains unclear. The purpose of this study was to investigate the difference in the force generation characteristics between the plantar-flexion of the first and second-fifth metatarsophalangeal joints (MTPJs) by comparing the maximal voluntary plantar-flexion torques (MVC torque) at different MTPJs and ankle positions. The MVC torques of the first and second-fifth MTPJs were measured at 0°, 15°, 30°, and 45° dorsiflexed positions of the MTPJs, and at 20° plantar-flexed, neutral, and 20° do

An Extraordinary Reconstruction of a Postoperative Defect after Metatarsophalangeal Joint Arthrodesis of the Big Toe Using the "Chinese Forearm Flap".

BACKGROUND: We present an unusual technique for reconstruction of a postoperative tissue defect following hallux rigidus surgery. METHODS: A complicated course after left big toe arthrodesis resulted in a soft tissue defect with bone exposure of the first ray. Amputation of the big toe was categorically rejected by the patient. There was advanced arteriosclerosis with single-vessel supply to the foot via the posterior tibial artery. OUTCOME: To preserve the big toe, the defect was covered by a fasciocutaneous radial free flap with a 15 cm long vascular pedicle which was microanastomosed to t

The effect of subtendinous connective tissue restraints on extension of the vascularized toe joint transfer: a propensity score-matched comparison.

INTRODUCTION: Vascularized toe proximal interphalangeal (PIP) joint transfer restores motion after destructive finger injuries, yet outcomes are limited by a persistent extensor lag. We observed subtendinous connective tissue attachment beneath the extensor digitorum longus (EDL), from the proximal phalangeal neck to the PIP joint, that may tether the tendon to bone. We assessed whether releasing this tissue improves surgical outcomes without compromising perfusion. METHODS: We performed a retrospective matched cohort study with a 1:1 propensity score matching on sex, age at surgery, native t

Irreducible Dislocation of the Great Toe Interphalangeal Joint Secondary to an Incarcerated Sesamoid: A Retrospective Case Series of Six Patients.

INTRODUCTION: Irreducible interphalangeal (IP) joint dislocation of the hallux is a rare injury, most often caused by interposition of the sesamoid-plantar plate complex. Diagnosis may be delayed due to subtle radiographs, and closed reduction is typically unsuccessful. MATERIALS AND METHODS: This retrospective case series reports six patients (ages 25-60 years, 4 males and 2 females) who sustained dorsal or plantar hallux IP dislocations following sports injuries, falls, or crush trauma during 2 years. Four patients presented acutely, while one presented 3 weeks after injury. Closed reductio

Unilateral below-knee prosthesis users walking on uneven terrain: The effect of adding a toe joint to a passive prosthesis.

Lower limb prosthesis users cite uneven terrain as a challenging surface to walk on. We sought to determine whether adding a Flexible toe joint to a prosthetic foot would be preferred by unilateral below-knee prosthesis users relative to a Locked (non-articulating) toe joint for walking on uneven terrain. We also quantified lower limb joint kinetics for the Locked and Flexible toe joint configurations. Five of our nine participants preferred the Flexible toe joint when walking on uneven terrain, yet from a biomechanical standpoint, the reason for this is unclear. All participants exhibited red

Adding a toe joint to a prosthesis: walking biomechanics, energetics, and preference of individuals with unilateral below-knee limb loss.

Toe joints play an important functional role in able-bodied walking; however, for prosthesis users, the effect of adding a toe joint to a passive prosthetic foot remains largely unknown. The current study explores the kinematics, kinetics, rate of oxygen consumption and user preference of nine individuals with below-knee limb loss. Participants walked on a passive prosthetic foot in two configurations: with a Flexible, articulating toe joint and with a Locked-out toe joint. During level treadmill gait, participants exhibited a decrease in Push-Off work when using the Flexible toe joint prosthe

Low multiscale entropy of ankle joint position during toe gait in children with cerebral palsy is related to contracture development.

BACKGROUND: Toe walking in children with cerebral palsy is typically less stable and less energy-efficient than plantigrade walking in typically developing children. We hypothesized that children with cerebral palsy may, nevertheless, show more stable toe walking due to the development of contractures compared to age-matched typically developing children. METHODS: Multiscale entropy of the vertical position of the ankle joint was used to measure ankle joint stability during the stance phase of walking. Kinematic data were obtained from 37 children with spastic cerebral palsy and habitual toe

Design of 3D printable prosthetic foot to implement nonlinear stiffness behavior of human toe joint based on finite element analysis.

Toe joint is known as one of the critical factors in designing a prosthetic foot due to its nonlinear stiffness characteristic. This stiffness characteristic provides a general feeling of springiness in the toe-off and it also affects the ankle kinetics. In this study, the toe part of the prosthetic foot was designed to improve walking performance. The toe joint was implemented as a single part suitable for 3D printing. The various shape factors such as curved shape, bending space, auxetic structure, and bending zone were applied to mimic human foot characteristics. The finite element analysis

Effectiveness of Flexor Tenotomy With a Transpositional Skin Flap for the Treatment of Severe Claw Toe Deformity With Mild-to-Moderate Joint Contracture.

Flexor tenotomy is the most effective for achieving healing and for the prevention of toe ulcer resulting from claw toe deformity. Although flexor tenotomy might be effective for a flexible claw toe, it might not provide benefits for severe claw toe deformity involving joint contracture. We devised a method involving the transfer of a flap to the skin defect caused by tenotomy, as severe claw toe deformity is associated with skin contracture. Although transpositional skin flap might increase the postoperative complication risks, it can be effectively used for severe claw toe deformity involvin

Free neurovascular toe-(joint)-transfers compared to alternative reconstructive procedures for amputation injuries of two and tripartite fingers with substance loss.

BACKGROUND: Complex injuries of the hand frequently result in loss of essential functional features. Common reconstructive procedures for soft tissue defects of the thumb or phalanges are locoregional flaps like Moberg-, Foucher-, Cross-Finger- or Littler flaps. Microneurovascular toe (joint-) transfers complete the arsenal of operative reconstructive procedures and allow for most detailed reconstructions. Our experiences with free toe transplants are reported and diversely discussed regarding contending procedures. METHODS: From 2010 until 2019 14 patients who received emergency or elective

Children with idiopathic toe walking display differences in lower limb joint ranges and strength compared to peers: a case control study.

BACKGROUND: Idiopathic toe walking (ITW) is an exclusionary diagnosis. There has been limited exploration of lower limb active range of motion and strength measures in children with ITW. This researched aimed to determine any differences in lower limb muscle active range of motion and strength in children who have ITW, compared to normative data collected from children who displayed typical gait. METHODS: Children were recruited with had a diagnosis of ITW, aged between 4 and 10 years, and no recent treatment. Data collected included parent reported data such as time spent toe walking, percen

Free microvascular transfer of second toe ray and serratus anterior muscle for management of thumb loss at the carpometacarpal joint level.

A 50-year-old machine operator suffered traumatic amputation of the thumb, ring, and small fingers. The thumb had been amputated at the carpometacarpal joint and was reconstructed 6 months later by microneurovascular transplantation of the second toe including the second metatarsal. Opposition was provided and the first web space was released at a second stage by reconstruction with the lowest two digitations of the serratus anterior muscle covered with a split-thickness skin graft.

Traumatic Fifth Toe Proximal Interphalangeal Joint Dislocation in a Pediatric Patient: Management with Open Reduction Internal Fixation and Kirschner Wire Fixation.

Traumatic dislocations of the proximal interphalangeal joint (PIPJ) of the lesser toes are rare injuries in the pediatric population, with limited literature available to guide management decisions. We report the case of a 5-year-old male who presented with irreducible dorsolateral dislocation of the right fifth toe PIPJ following trauma during play activities. Clinical examination revealed swelling, tenderness, and restricted movement of the affected digit. Plain radiographs confirmed the dislocation without associated fractures. Initial attempts at closed reduction under procedural sedation
