What Evidence Do We Need to Improve the Attainment of Bladder and Bowel Control in Children and Prevent Negative Sequelae in Adolescence and Adult Life? ICI-RS 2025.

AIMS: Nocturnal enuresis (NE), daytime urinary incontinence (DUI), and fecal incontinence (FI) and functional constipation (FC) are common functional disorders in childhood, which can persist into adolescence and adulthood. The aim of this paper is to review evidence of risk factors that impede the attainment of bladder/bowel control and contribute towards the persistence of incontinence, and to outline protective factors towards minimizing long-term negative sequelae. METHODS: A think tank group consisting of multidisciplinary experts at the International Consultation on Incontinence Researc

Decision-making for a successful bowel continence program.

Bowel continence is one of the most difficult challenges for patients with spina bifida. Incontinence acts as a social stigma for children and a barrier for adults seeking employment. We present an algorithm for stepwise decision-making in construction of personalized continence programs for greater likelihood of success. The protocol contains 13 assessment points including; stool consistency, frequency and amount; mobility; level of paraplegia: diet; medication; anal/rectal canal tone; prior programs attempted; family routines; age; accessibility; and learning issues. Based on outcomes of the

[Enuresis in childhood: what should one know? What should one do?].

Bladder control is a developmental process which is determined by somatic, individual psychological and psychosocial factors. It depends on the maturation of bladder capacity and of adequate neuromuscular coordination, on the quantity of urine and on appropriate recognition of bladder expansion. The latter especially is clearly correlated to healthy individuation and psychosocial integration of the child. Statistically, 80-90% of children have successfully developed bladder control at the age of 4-6 years. In the remaining 10 or 20% who do not attain dryness during daytime or at night, wetting

Bowel function and lower urinary tract symptoms in females with anterior anus treated conservatively: Controlled outcomes into adulthood.

PURPOSE: Anterior anus (AA) is a low type of anorectal malformation that is seen exclusively in females, characterized by an essentially normal appearing anus in an abnormally anterior position. In AA, the anal canal is located mostly within the voluntary sphincter complex, differentiating it from a perineal fistula in females, and allowing for a nonoperative approach to management. This study aimed to characterize the controlled, long-term bowel functional outcomes and lower urinary tract symptoms (LUTS) in females with conservatively treated anterior anus. METHODS: A single-institution, cro

An evaluation of the modified endorectal procedure for Hirschsprung's disease.

During the past 4 yr, 60 patients with Hirschsprung's disease have been treated at our institution. Twenty-eight patients within this group have undergone a modified endorectal pull-through procedure. There are 18 males and 10 females in the series. There are 5 children with total colonic involvement, 3 patients with aganglionosis to the mid-transverse colon, and one child with Hirschsprung's disease extending to the distal jejunum. The associated anomalies have consisted of four cases of Down's syndrome, one of whom also has transposition of the great vessels. There has been no operative-rela

Selective strategy for urethral catheterization in febrile young girls to confirm urinary tract infection diagnosis.

BACKGROUND: Urinary tract infections (UTIs) are a common source of bacterial infections in childhood. Making a proper diagnosis is important but requires invasive urine collection techniques. We aimed to derive a clinical decision rule to identify non-toilet-trained febrile girls at high risk for UTIs to restrict urethral catheterizations (UCs) to this high-risk group of patients. METHODS: We included all non-toilet-trained girls with a positive microscopic urinalysis from urine collected by sterile bag in a prospective cohort study to derive a model to predict UTI assessed by urine culture f

Urinary function after Snodgrass repair of distal hypospadias: comparison with the Mathieu repair.

PURPOSE: To evaluate urinary function in patients with distal hypospadias undergoing repair by the tubularized incised-plate urethroplasty (TIP or Snodgrass), compare the results with those in patients treated by the Mathieu technique, and show the potential issues inherent to the evaluation of such results. PATIENTS AND METHODS: A cross-sectional assessment was performed of uncomplicated distal hypospadias operated on during a 3-year period, already toilet trained, and able to void volitionally. Evaluation included clinical assessment urinary symptoms and urinary stream, and uroflowmetry. R

Adherence to follow-up ten years after hypospadias repair.

INTRODUCTION: Periodic follow-up prior to and after puberty to evaluate for long-term sequalae following hypospadias repair is commonly recommended. Few studies have evaluated this follow-up, especially into adulthood. This study aimed to evaluate adherence to routine postoperative follow-up appointments over 10 years following elective hypospadias repair. MATERIAL AND METHODS: Retrospective review of all patients undergoing hypospadias repair at our institution under the age of 10 from January-December 2012 was performed. Data were extracted including demographics, meatal location, type of h

Can a complete primary repair approach be applied to cloacal exstrophy?

PURPOSE: Surgical reconstruction for children with cloacal exstrophy remains challenging. The operative approach to cloacal exstrophy has expanded with the addition of the complete primary exstrophy repair. We assessed the safety and efficacy of complete primary exstrophy repair for this complex condition. MATERIALS AND METHODS: We performed a retrospective review of children treated from birth for cloacal exstrophy between March 1, 1994 and January 1, 2003 at our institution. We evaluated associated anomalies, method of closure, complications and urinary continence. RESULTS: Seven patients

Diaper dermatitis. Factors to consider in diagnosis and treatment.

Diaper dermatitis is a common problem in infants and young children. Although the pathogenesis of diaper dermatitis is not well defined, many associated factors have been identified, including individual predisposition to atopic or seborrheic dermatitis, occlusion and friction caused by the diaper, and overgrowth of bacteria and yeast. General treatment goals involve keeping the diaper area as dry as possible, washing the area as infrequently as possible, and avoiding tight-fitting diapers. Petrolatum, talc, baking soda, and fluorinated topical steroidal medications should be avoided. If all e

Achieving fecal continence in patients with spina bifida: a descriptive cohort study.

PURPOSE: Fecal incontinence is a major problem in patients with myelomeningocele. We evaluate the results of a stratified approach aimed at obtaining fecal pseudo-continence in patients with myelomeningocele. MATERIALS AND METHODS: We conducted a cross-sectional descriptive study of last file data in 80 patients 5 to 18 years old with myelomeningocele followed at our center. Beginning at birth patients with myelomeningocele were seen at least annually by the pediatric gastroenterologist, a member of the multidisciplinary "spina team." Constipation was treated with diet and osmotic laxatives.

Nonpharmacologic treatment of urinary incontinence.

Standard therapy for urinary incontinence often includes pharmaceutical agents that carry a risk of side effects or interactions with other drugs. As an alternative, several nonpharmacologic management approaches are available. Scheduled voiding regimens are effective for patients who have uninhibited detrusor contractions, even when cognitive impairment is present. In motivated patients, pelvic muscle exercise is effective for stress incontinence. These methods are safe and inexpensive, and studies indicate that they can be as effective as or more effective than pharmaceutical agents. Other n

Short-term catheterization after TIP repair in distal hypospadias: who are the best candidates?

Over the last decade, tubularized incised plate (TIP) urethroplasty has become the first choice of surgical technique in patients with distal hypospadias. Despite the excellent cosmetic and functional results, prolonged catheterization (7-14 days) remains the main disadvantage of the TIP repair. In this study, we investigated the outcomes of the short-term catheterization in children with distal hypospadias in order to elucidate any relationship between the length of catheterization and the patients' age, meatal localization and postoperative complication rates. The charts of 183 patients who

Outcome analysis of tubularized incised plate repair in hypospadias: is a catheter necessary?

OBJECTIVE: Tubularized incised plate (TIP) urethroplasty is performed by many pediatric urologists and has been widely accepted for repair of distal and mid-shaft hypospadias. However, the role of urethral catheter placement in TIP repair remains controversial. In this study, the surgical outcomes of indwelling urethral catheter and non-catheter TIP repairs in boys with hypospadias were compared. METHODS: A total of 254 patients with primary distal and mid-shaft hypospadias underwent TIP repair and were evaluated retrospectively. A urethral catheter was placed in 103 patients (group A) and no

Urethral Triplication Without Bladder Duplication: Endourologic Diagnosis and Management.

Urethral triplication is a rare congenital anomaly of the lower urinary system, with <15 cases reported so far. We present a 24-month-old boy with accessory hypoplastic urethra ending in glans. At the beginning of toilet training, urine output was observed along the rectum. Rigid cystoscopy shows a perineal urethra starting in the posterior urethra. Subsequently, flexible cystoscopy showed entry of epispadic urethra in the bladder immediately superior to the bladder neck. It was running parallel to primary urethra. Then, we observed two most frequent types of urethral duplication in the sagitt

Bladder Dysfunction in Children with Primary Vesicoureteral Reflux: Do We Need to Change Indications for Surgery?

PURPOSE: Vesicoureteral reflux (VUR) is commonly seen in children. However, in Indian literature, not much has been discussed about bladder dysfunction (BD) in VUR. The present study aims to study BD in children with primary VUR. METHODS: In this prospective study, 86 children with primary VUR were enrolled over a 2-year duration with at least 1 year of follow-up. BD was studied using noninvasive (history, ultrasound kidney ureter bladder, and Dysfunctional Voiding Scoring System) and invasive (urodynamic study [UDS]) methods and was classified into (1) underactive bladder, (2) overactive bla
