Cementoenamel junction: microscopic analysis and external cervical resorption.

There is a lack of data regarding the shapes and distribution of the mineralized tissue that composes the cementoenamel junction. A sample of 198 permanent human teeth was analyzed by optical microscopy and scanning electron microscopy. Scanning electron microscopy showed three types of tissue interrelations: enamel overlapped by cementum; enamel and cementum edge-to-edge; and a gap, revealing a strip of exposed dentin. Using optical microscopy, a fourth type of cementoenamel junction was observed: cementum overlapped by enamel. The distribution of the hard tissues found at the cementoenamel j

CBCT Post-Processing Tools to Manage the Progression of Invasive Cervical Resorption: A Case Report.

This case report aimed to highlight the usefulness of cone beam computed tomography (CBCT) and its post-processing tools for the diagnosis, follow-up and treatment planning of invasive cervical resorption (ICR). A 16-year-old female patient was referred for periapical radiographic examination, which revealed an irregular but well demarcated radiolucency in the mandibular right central incisor. In addition, CBCT scanning was performed to distinguish between ICR and internal root resorption. After the diagnosis of ICR, the patient was advised to return shortly but did so only six years later. At

Esthetic evaluation of different approaches to treat gingival recession associated with non-carious cervical lesion treatment: a 2-year follow-up.

PURPOSE: To evaluate the esthetic outcome of four different approaches to treat gingival recession, associated with non-carious cervical lesion (combined defects) and the possible roles of patient-related factors in this esthetic outcome. METHODS: 78 combined defects were previously treated by: coronally advanced flap (CAF), CAF plus cervical restoration using resin-modified glass-ionomer material (CAF+R), connective tissue graft (CTG) and CTG+R. After a follow-up of 2 years, esthetic evaluations were performed using a modification of the Root Coverage Esthetic Score (MRES) and Qualitative Co

Surgical treatment options for grafting areas of gingival recession association with cervical lesions: a review.

STATEMENT OF THE PROBLEM: Gingival recession (GR) is a highly prevalent dental problem that may be associated with dentin hypersensitivity and aesthetic complaint. Furthermore, GR is often complicated by dental cervical wear of substance, such as erosion, abrasion, or abfraction lesions, as well as initial or more extensive carious involvement of the cervical area, which worsen the symptoms and make the treatment of this periodontal condition more challenging. THE PURPOSE OF THE STUDY: To provide an overview of surgical treatment options presented in the literature for sites affected by GR wi

A familial pattern of multiple idiopathic cervical root resorption in a father and son: a 22-year follow-up.

BACKGROUND: The etiology of idiopathic cervical root resorption has not been elucidated clearly. However, the process has been linked to trauma, intracanal bleaching, and partial-thickness connective tissue grafts. METHODS: This study describes a familial pattern of multiple idiopathic cervical root resorption in a father and son. RESULTS: The father was a healthy 63-year-old white male who presented with the first resorption lesion in 1983. Twenty-seven additional lesions were identified on 16 teeth over 22 years. Five teeth were lost as a result of extensive resorption. The son was a healt

External cervical resorption: part 2 - management.

Effective management of external cervical resorption (ECR) depends on accurate assessment of the true nature and accessibility of ECR; this has been discussed in part 1 of this 2 part article. This aim of this article was firstly, to review the literature in relation to the management of ECR and secondly, based on the available evidence, describe different strategies for the management of ECR. In cases where ECR is supracrestal, superficial and with limited circumferential spread, a surgical repair without root canal treatment is the preferred approach. With more extensive ECR lesions, vital p

Multiple idiopathic cervical resorption: a diagnostic dilemma.

Multiple idiopathic cervical resorption is an uncommon condition, the etiology of which remains unknown. The case of a 36-year-old woman with idiopathic resorption in the cervical areas of multiple teeth is presented. Her medical history was noncontributory, but her dental history was valuable in reaching a diagnosis. Hematologic and endocrinal investigations were unremarkable, but a computed tomography scan revealed relevant information. The progression of the lesion and involvement of multiple teeth without any pattern or chronology are challenges faced when diagnosing and treating this cond

Playing wind instruments as a potential aetiologic cofactor in external cervical resorption: two case reports.

AIM: To present two cases of external cervical resorption (ECR) on maxillary incisors, in which the primary aetiologic factor is suggested to be pressure trauma by frequently playing wind instruments. SUMMARY: The exact aetiological spectrum of ECR is still poorly understood. For resorption to occur, a defect in the cementum layer (trigger) is a likely prerequisite. Whilst the mechanism for continuation (stimulus) is still unclear, knowledge of potential predisposing factors is important in assessing patients at risk. Pressure generated by playing wind instruments could present an aetiologica

Enamel pearls and cervical enamel projections on 2 maxillary molars with localized periodontal disease: case report and histologic study.

Presented here is an unusual case of teeth with aberrant enamel. One small and 3 large enamel pearls were identified in 2 maxillary third molars of a patient. One long enamel projection was connected to one of the large enamel pearls, along with 2 short cervical enamel projections. The ectopic enamel was associated with advanced localized periodontal destruction. After extraction, the molars were investigated radiographically and under the scanning electron microscope. The 3 large pearls contained distinct dentinal cones. The enamel of the pearls and the long cervical projection exhibited most

Successful application of atelocollagen for treatment of perforated teeth.

OBJECTIVE: Cervical or furcal root perforation is a serious clinical problem and one of its treatment modalities is perforation repair with composite resin. However, many cases still progress in inevitable extraction. When primary teeth are affected, early tooth loss can cause problems related to the eruption space for the permanent successors. The aim of the present study was to evaluate a novel clinical treatment method for perforated teeth. STUDY DESIGN: Atelocollagen was applied to perforated furcal and cervical areas of 13 primary teeth in 13 children aged 4-9 years and 8 permanent teeth

A radiographic and histological study of modern human lower first permanent molar root growth during the supraosseous eruptive phase.

There is increasing focus on the relationship between root growth and the eruptive process in studies of primate dental development, and the first permanent molar (M1) is regarded as a key tooth in many of these comparative studies. In this study of modern human M(1)s, histological and radiographic data were compared. Rates of root extension were determined histologically in 20 M(1)s from individuals of known sex using data for daily incremental markings and the orientation of accentuated lines in root dentine. Mean values at the mesiobuccal enamel cervix were 4.3-5.4 microm per day and then r

Dentinal dysplasia type I: review of the literature and report of a family.

A family is reported with dentinal dysplasia type I affecting both dentitions. Presenting features included unusual mobility of the teeth, followed by early exfoliation; normal clinical shape of the crowns of the teeth, but with an amber color without any sign of attrition or abnormal loss of enamel. Radiographic findings showed pulp-chamber and root-canal obliteration, poor root formation, radiolucent linear appearance of the pulp chamber parallel to the cementoenamel junction and frequent periapical radiolucencies. Histological studies have reported large masses of calcified tubular dentin,

The relationship between cervical enamel projection and class II furcation defects in humans.

The purposes of the present study were to examine the frequency of cervical enamel projection in mandibular molars with class II furcation defects and to examine the response of these teeth to guided tissue regeneration procedures. Probing depth and vertical and horizontal periodontal attachment levels were recorded with a constant-force electronic periodontal probe. Following surgical exposure of the bifurcation and prior to membrane placement, the cervical enamel projection was examined and then graded clinically; photographs were taken to allow further examination of the bifurcation. [The cervical third of deciduous teeth. An ultrastructural study of the heard tissues by SEM].

As information on amelocemental junction of deciduous teeth is limited, this topographical area was investigated by scanning electron microscopy (SEM) to verify differences from that of permanent teeth. Twenty-six carious and non-carious human maxillary and mandibular primary teeth were placed in a fixative immediately after extraction. Pulpal tissue was removed from the pulpal chambers and root. The primary teeth blocked onto stubs and all specimens were platinum coated and examined by SEM. In these specimens an overlapping of cementum onto to enamel and an edge to edge relationship was domin

Morphological comparative study between a fifth-generation adhesive system and a universal system in total-etch mode applied at the cervical region.

For both direct and indirect restorations at the cervical level, the preparation aims to ensure only enamel bonding, due to its proven performance over time. This study sought to evaluate the adhesion to the cervical tooth structure. The aim of the study was to morpho-structurally compare a two-step adhesive system with a universal system in total-etch mode, starting from the hypothesis of the presence of only enamel as the bonding substrate at the level of cavities prepared to the standard depth of 0.4 mm, located 2 mm coronal to the cementoenamel junction. Each adhesive system was assigned a

Identification of Candidate Genes of Familial Multiple Idiopathic Cervical Root Resorption.

INTRODUCTION: Multiple idiopathic cervical root resorption (MICRR) is a disease with an unknown etiology that causes invasive cervical root resorption in multiple teeth. Although previous MICRR genomic studies have identified candidate gene variants, the etiology of the condition remains poorly understood. In the present study, we investigated the genetic causality of MICRR to explore candidate variants. METHODS: Saliva samples from a family containing 2 affected and two unaffected subjects with the dominant transmission of MICRR were subjected to whole-exome sequencing. RESULTS: As a result

Association Between Residual Pericervical and Apical Dentine and Vertical Root Fracture in Endodontically Treated Molars: A Case-Control Study.

OBJECTIVE: Vertical root fracture (VRF) in endodontically treated molars (ETMs) is a multifactorial condition. However, the relationship between residual pericervical and apical dentine in ETMs and VRF has yet to be fully assessed. This study aimed to investigate the association between residual pericervical and apical dentine and VRF in ETMs. MATERIAL AND METHODS: ETMs with VRFs (44 cases) and those without VRFs (92 controls) were included. Residual dentine at pericervical level and apical terminus of root canal filling (RCF) were assessed based on the ratio between the mesiodistal widths of

Force distribution is more important than its intensity!

A common question about root resorption is raised in orthodontic practice: What is more important, the intensity of force or its distribution along the root, periodontal and alveolar structures? Diffuse distribution of forces applied to periodontal tissues during tooth movement tends not to promote neither extensive areas of cell matrix hyalinization nor significant death of cementoblasts that lead to root resorption. However, focal distribution or concentration of forces within a restricted area--as it occurs in tipping movements, even with forces of lower intensity--tend to induce extensive
