Hair tourniquet syndrome of toes and fingers in infants.

OBJECTIVE: Hair tourniquet syndrome is an uncommon condition characterized by strangulation appendages by a hair or thread. The aim of this study was to evaluate the results of hair removal and antibiotic therapy in our patients with hair tourniquet syndrome. METHODS: Between January 2012 and August 2018, 16 patients (8 boys, 8 girls; mean age: 118.5 [range: 20 to 380] days) were treated surgically for hair tourniquet syndrome. All patients were treated surgically under local or general anesthesia in the pediatric emergency department or in the operating theater using magnifying loupes. The a

Utility of ultrasonography in hair-thread tourniquet syndrome.

A 2-year-old girl presented with a 2-month history of an erythematous, indurated plaque with well-defined borders on the third toe of the right foot. Bedside high-resolution ultrasonography demonstrated a thickened epidermis overlying a hyperechoic focus within the dermis. Her clinical and sonographic presentation was in keeping with a foreign body causing hair-thread tourniquet syndrome. The foreign body was surgically extirpated without neurovascular sequelae. Ultrasonography expedited accurate diagnosis and is a promising adjunct to clinical evaluation for radiolucent foreign bodies.

Hair-thread tourniquet syndrome in a preterm baby.

Hair-thread tourniquet syndrome is a rare disorder characterized by the encircling of an appendage by a hair or thread. It usually occurs in children under the age of one year. The tourniquet syndrome may occur in different parts of the body, particularly in toes, fingers, penis, clitoris, labia, neck and uvula. It is an emergency condition that induces progressive edema, ischaemia and tissue necrosis and can lead to autoamputation of digits or other strangulated structures. Emergency treatment is careful removal of the constricting fiber. We report a preterm newborn with hair-thread tournique

Metabolic effects of tourniquet application in burn patients.

BACKGROUND: Despite several studies investigating the pathophysiologic effects of tourniquet usage in extremity surgeries, there are not enough data about these effects in adhesion release surgeries of burn patients. In the light of numerous metabolic changes of burn tissues, we tried to determine whether there are any significant differences in metabolic responses of burn tissues to tourniquet ischemia in comparison to the findings of other studies in non burn tissue responses during tourniquet usage in extremity surgeries. METHODS: From March 2009 to April 2011, eighteen patients who were c

Penile hair tourniquet resulting in hypospadias failure.

Penile hair tourniquet (PHT) is a painless form of penile ischemia, typically seen in toddlers with long-haired mothers, caused by entanglement of hair on the balano-prepucial sulcus, normally associated with circumcision. Its association with hypospadias has been reported only once. A school-aged boy admitted for surgery to treat hypospadias failure was incidentally detected to have PHT and severe hourglass deformity of the penis. Urethral anastomosis and glanuloplasty were done after removal of the constricting ring, without complications. Normal erections were reported during follow up. Tre

The silicone ring tourniquet in orthopaedic operations of the extremities.

Tourniquets provide a bloodless field in limb operations and their introduction in orthopaedic operative technique has been considered as a landmark. A new tourniquet device, a silicone ring tourniquet (SRT) (HemaClear or S-MART, OHK Medical Devices, Haifa, Israel), was introduced into clinical practice a few years ago. A few clinical studies as well as comparative studies in volunteers have reported its use in a relatively small number of cases. The aim of this prospective study is to report the clinical use of this device in a large number of patients, including all possible applications of

TKA sans tourniquet: let it bleed: opposes.

The literature supports the routine use of a tourniquet during total knee arthroplasty (TKA). With tourniquet use, there is decreased intraoperative blood loss with subsequent improved visibility and a bloodless surgical field. This facilitates efficiency with the potential for decreased operating time. Increased operating time has been associated with an increase in the incidence of infection after TKA. Opponents of routine tourniquet use cite rare or theoretical concerns. Multiple authors have concluded that the incidence of deep vein thrombosis is not related to using a tourniquet. The rare

A hair tourniquet resulting in strangulation and amputation of the clitoris.

BACKGROUND: Hair tourniquet syndrome involves fibers of hair or thread wrapped around an appendage producing tissue necrosis. Appendages commonly involved include the toe, finger, and penis. We report a hair tourniquet resulting in amputation of the clitoris. CASE: An adolescent presented with a 4-year history of intermittent genital pain that increased in severity over the preceding 5 days. Physical examination revealed a necrotic clitoris surrounded by a black hair. During the examination, the tissue fell off resulting in immediate improvement in the patient's pain. CONCLUSION: We report a

The tourniquet revisited as an adjunct to lower limb revascularization.

PURPOSE: The purpose of this study was to evaluate the role and efficacy of the tourniquet in lower limb revascularization. METHODS: During a 3-year period, 195 patients underwent 205 infrainguinal reconstruction operations in the lower extremity. These patients underwent bypass with a tourniquet and inflow occlusion (group 1) or bypass without a tourniquet (group 2). The type of infrainguinal reconstruction, tourniquet ischemia time, blood loss, and complications related to tourniquet use were recorded. A subset of patients underwent serial muscle biopsies. [The hair-thread tourniquet syndrome of the toes and penis].

We present eight cases of the hair-thread tourniquet syndrome which affected the toes in 6 cases and presented strangulation of the penis in two. All patients were infants and the mechanism of injury was the strangulation by hair or thread wrapped around an appendage. Neither lesions were promptly recognized by their physicians and the patients were seen in the emergency room with swelling, erythema and a circumferential scar with the typical distal edema. The lesions had ischemic signs, but without tissue necrosis. All patients were treated by immediate removal of the constricting fibers and

Evaluation of the tourniquet test as a predictor of diabetic retinopathy.

Medical practitioners often have difficulty in assessing the presence or severity of diabetic retinopathy. The tourniquet test is a method of assessing diabetic capillary fragility that has been felt to reliably correlate with background and proliferative diabetic retinopathy. We studied 100 consecutive diabetic patients and 50 age-matched controls in a masked fashion, using fundus photographs and fluorescein angiography to correlate the amount of capillary fragility with the presence and severity of background and proliferative diabetic retinopathy. Although the severity of diabetic capillary

The use of a tourniquet when plating tibial fractures.

Sixty closed fractures of the tibia were treated by open reduction and internal fixation with plates and screws. Half the operations were performed with a thigh tourniquet and half without. In the tourniquet group, there were six cases with erythema and induration of the wound; in the other group there were no such complications. Despite negative bacterial cultures, superficial infection of the inflamed wounds was suspected. It is suggested that a tourniquet may predispose tissues to infection, and its use is not recommended during operations for internal fixation of the tibia.

Changes of leukotrienes in rabbits subjected to tourniquet shock.

Rabbits were used as a traumatic shock model, and the right femoral root was fastened tightly with a tourniquet for human children for 24 hours. The amounts of LTB4, LTD4 and LTE4 in the blood and muscle were measured just before unfastening and 3, 6 and 12 hours after unfastening. After unfastening, the rabbits showed a gradual decline of blood pressure, and one died 7 hours after unfastening. LTB4 levels in the blood and muscle were within the levels of the control rabbits throughout the experimental period. On the contrary, LTD4 levels in the blood increased just before and 6 and 12 hours a

Unrecognised compartment syndrome in a patient with tourniquet palsy.

We report a case in which compartment syndrome and tourniquet paralysis occurred simultaneously. This is a previously unreported combination which presents a diagnostic problem. We recommend that electrophysiological studies and continuous monitoring of compartment pressures should be used to minimise morbidity in high-risk cases.

Tourniquet-induced nerve ischemia: an experimental investigation.

This experimental investigation has demonstrated that an inflated pneumatic tourniquet causes slowing in conduction velocity of the sicatic nerve. The amount of slowing and recovery time vary according to inflation pressure and to the duration of applied pressure. The conduction velocity always returned to normal provided the tourniquet was inflated for 2 hours or less, and pressures were below 500 mm Hg. The nerve directly under the tourniquet was most susceptible to injury. Inflation pressure should be as low as possible and be determined according to systolic blood pressure. An Esmarch band

Tourniquet infusion chemotherapy in extremities with malignant lesions.

Tourniquet infusion chemotherapy involves the direct injection of a chemotherapeutic drug into the main artery of an extremity with prior application of an external tourniquet proximally on this extremity set at above the level of systolic pressure for ten minutes. Thus, the drug is not diluted and pushed by the blood into the venous circulation before diffusion into the tissues has occurred. This technique, applied in seven patients for a total of 40 instances, proved to be safe. Skin erythema and blisters, which are reversible, occur in the treated area. Complete clinical regression was achi

Use of the Fogarty embolectomy catheter as an "internal tourniquet".

The Fogarty embolectomy catheter has been found useful in minimizing blood loss in large surgical procedures about the hip and pelvis. Its utility lies in the ability to achieve temporary intraluminal occlusion of the common iliac artery while the proposed surgical procedure is being carried out. The technique of instrumentation is simple, rapid, and can be carried out using no special instruments other than the catheter itself. The instrumentation adds little, if any, additional time to the surgical procedure, and in fact may significantly shorten it by achieving earlier definitive hemostasis
