[Pulmonary function and exercise tolerance in patients treated with bone marrow transplantation (BMT)].

We assessed pulmonary function and exercise tolerance in 10 BMT patients. Their underlying disorders were as follows; chronic myeloid leukemia 5 cases, acute lymphoblastic leukemia 2 cases, aplastic anemia, acute myeloid leukemia and non-Hodgkin's lymphoma one case each. Their mean age was 26 +/- 9 years old. When the patients were healthy and free of serious complications and anemia, arterial blood gas examination, pulmonary function tests and incremental treadmill exercise test were examined repeatedly. Although %VC and FEV1.0% kept within normal range, PaO2 at rest, %DLCO, VO2max, VO2max/kg

Mixed chimerism and permanent specific transplantation tolerance induced by a nonlethal preparative regimen.

The use of allogeneic bone marrow transplantation as a means of inducing donor-specific tolerance across MHC barriers could provide an immunologically specific conditioning regimen for organ transplantation. However, a major limitation to this approach is the toxicity of whole body irradiation as currently used to abrogate host resistance and permit marrow engraftment. The present study describes methodology for abrogating host resistance and permitting marrow engraftment without lethal irradiation. Our preparative protocol involves administration of anti-CD4 and anti-CD8 mAbs in vivo, 300-rad

Epigenetic regulation in transplant rejection and tolerance: mechanisms and therapeutic prospects.

Epigenetics modulates gene expression and plays a pivotal role in numerous immune disorders. Over the past two decades, increasing evidence has indicated that epigenetic regulation contributes to transplant rejection and immune tolerance. Four primary epigenetic mechanisms-DNA methylation, histone modifications, chromatin remodeling, and non-coding RNAs (ncRNAs)-exert distinct effects on rejection after transplantation. Therefore, understanding the epigenetic mechanisms of rejection provides novel avenues for inducing immune tolerance and developing strategies to prolong graft survival and pat

The innate natural killer cells in transplant rejection and tolerance induction.

PURPOSE OF REVIEW: The roles of adaptive immune cells in transplant models have been extensively studied, but very little is known about the role of innate immune cells in the allograft response, especially in tolerance induction. In this review, we summarized the latest developments in the study of the role of natural killer cells in mediating graft rejection and tolerance induction. RECENT FINDINGS: Natural killer cells are potent cytolytic cells; they also induce tissue inflammation by producing powerful proinflammatory cytokines. Thus, natural killer cells can act as effector cells in tra

Mycophenolate mofetil in organ transplantation: focus on metabolism, safety and tolerability.

Mycophenolate mofetil (MMF) received its first approval for the prevention of renal allograft rejection in 1995 and has now become the most frequently used antiproliferative agent in maintenance immunosuppressive therapy for kidney, pancreas, liver and heart transplantation. In addition, its use for the treatment of autoimmune diseases steadily increases. This review focuses on the miscellaneous pharmacodynamic properties of the drug, its pharmacokinetics in healthy subjects, recipients of different organ transplants and combination therapy with other pharmaceuticals, as well as its safety pro

Radiation tolerance of the transplanted liver. A histopathologic study in three cases.

Three patients with multifocal recurrence of hepatocellular carcinoma following liver transplantation received palliative irradiation. Hyperfractionated irradiation (150 cGy/fraction b.i.d.) was delivered in two cases to the entire liver using parallel opposed oblique portals to a total dose of 30 Gy. Conventional irradiation (180 cGy/fraction) totaling 45 Gy was administered to the liver hilus with concomitant infusional 5-fluorouracil chemotherapy in the third case. Clinicopathologic correlations were performed. At autopsy all patients had massive tumor burden within the liver. Veno-occlusiv

Power to the Tregs in transplantation tolerance in LATAM: From the basics to the bedside.

T regulatory cells (Tregs) come in different flavors, but they all share suppressive function and the role in the generation and maintenance of immune tolerance. Due to this essential activity, Tregs are targets for immunotherapy: Tregs promotion is wanted in pathological conditions such as autoimmunity, allergies and transplantation, whereas their elimination is the main goal in the context of cancer. Transplantation is the only treatment available when organ failure is detected but unfortunately transplant rejection has not been fully achieved regardless of all efforts made, such as the iden

Safety and Tolerability of SGLT-2 Inhibitors Following Lung Transplantation.

Sodium-glucose cotransporter-2 inhibitors (SGLT2i's) are frequently prescribed for T2DM control, with additional efficacy in congestive heart failure therapy and preserving renal function in CKD. Despite their potential to mitigate comorbidities, prescribing of SGLT2i's following solid organ transplantation has been limited due to safety concerns regarding infection, renal function, and diabetic ketoacidosis. SGLT2i prescription following transplantation of other solid organs has been evaluated, but only one study included a limited number of lung transplant recipients. We performed a retrospe

Treg Therapy for the Induction of Immune Tolerance in Transplantation-Not Lost in Translation?

The clinical success of solid organ transplantation is still limited by the insufficiency of immunosuppressive regimens to control chronic rejection and late graft loss. Moreover, serious side effects caused by chronic immunosuppressive treatment increase morbidity and mortality in transplant patients. Regulatory T cells (Tregs) have proven to be efficient in the induction of allograft tolerance and prolongation of graft survival in numerous preclinical models, and treatment has now moved to the clinics. The results of the first Treg-based clinical trials seem promising, proving the feasibilit

Role of the hepatic parenchyma in liver transplant tolerance: a paradigm revisited.

Unlike other solid organs, liver transplants are spontaneously accepted in a wide range of animal models. In the clinic, transplanted livers also display privileged immunological properties allowing weaning of immunosuppression therapy in up to 20% of selected patients. To explain this phenomenon, many studies have focused on the role of donor-derived 'passenger' leukocytes that are thought to induce antigen-specific tolerance by migrating from the graft into recipient secondary lymphoid tissues. Although convincing evidence exists that these cells are able to elicit antiallograft T cell hypor

Pharmacokinetics and tolerability of intravenous busulfan in hematopoietic stem cell transplantation.

Intravenous (IV) busulfan has been developed to overcome variable absorption of oral busulfan and tested in several trials. We tested its pharmacological properties and tolerability in 16 Korean stem cell transplantation (SCT) patients. IV busulfan was administered at 0.8 mg/kg every six h for a total of 16 doses (days -7 to -4), which was followed by cyclophosphamide administration at 60 mg/kg every 24 h for two d (days -3 and -2). The median AUC(inf) values (at the first dose) and AUC(ss) (at the steady state) were 1060.4 microM.min (range: 511.1-1812.7) and 1092.5 microM.min (range: 539.7-1

Donor Macrophage Depletion Permits Post-Transplant Tolerance Induction in a Murine Islet Transplant Model.

Current strategies for experimental tolerance induction for allogeneic transplantation typically require recipient preparation days to weeks prior to transplantation, making them not applicable to deceased donor transplantation. Developing tolerance strategies feasible for deceased donor transplantation would greatly increase the pool of eligible patients for tolerance induction. Here, we aimed to induce tolerance with post-transplant only interventions in a murine pancreatic islet transplant model. We demonstrated that transplant tolerance induction by recipient infusions of ethylcarbodiimide

Total body irradiation of donors can alter the course of tolerance and induce acute rejection in a spontaneous tolerance rat liver transplantation model.

Liver transplantation is an established therapy for end-stage liver diseases. Graft rejection occurs unless the recipient receives immunosuppression after transplantation. This study aimed to explore the mechanism of acute rejection of liver allografts in rats pre-treated with total body irradiation to eliminate passenger lymphocytes and to define the role of CD4(+)CD25(+) regulatory T cells in the induction of immunotolerance in the recipient. Male Lewis rats were used as donors and male DA rats were recipients. Rats were randomly assigned to the following four groups: control group, homogene

TIGIT+ B cells: emerging immunoregulatory players in autoimmunity and transplantation tolerance with therapeutic potential.

T cell immunoreceptor with Ig and ITIM domains (TIGIT) is an inhibitory immune checkpoint receptor classically studied in T cells and natural killer (NK) cells. Recent evidence demonstrates that TIGIT is also expressed on distinct B cell subsets exhibiting potent immunoregulatory properties. TIGIT+ B cells are proposed to contribute to immune homeostasis through anti-inflammatory cytokine secretion, modulation of antigen-presenting cell function, regulation of T cell differentiation, and bidirectional crosstalk with regulatory T cells and myeloid populations. The strongest direct evidence for

IL35 Production After Living-donor Renal Transplantation: Relevance to Local, Exosome-mediated, Clinical Tolerance.

BACKGROUND: We previously reported that 100% of HLA-identical siblings and 50% of HLA-haploidentical donor-recipient pairs exhibited bidirectional immunoregulation (BDIR) before living-related renal transplantation. BDIR predicted successful transplant outcomes. However, at the time, we were unaware of the role of the cytokine interleukin-35 (IL35) in immunoregulation, and we now appreciate exosomes as a means of confining immunoregulation to the transplanted tissue. Here we show that HLA-identical sibling transplants use IL35 in posttransplant BDIR. METHODS: We analyzed the posttransplant IL

Cooperative role of MDSCs and Tregs in alum-induced alloimmune tolerance in corneal transplantation.

Alum, a widely used adjuvant, has recently been recognized for its capacity to modulate immune responses beyond classical activation. In this study, we investigated whether alum-induced expansion of myeloid-derived suppressor cells (MDSCs) and their interaction with regulatory T cells (Tregs) promote immune tolerance in corneal transplantation. C57BL/6 mice received repeated intraperitoneal alum injections to induce MDSCs, which were subsequently characterized and tested in suppression assays. Corneal allograft recipients were treated with alum, adoptive transfer of alum-induced MDSCs, or comb
