Haematopoietic stem cell transplants: principles and indications.

Haematopoietic stem cell transplantation was proposed as a treatment strategy just over 60 years ago. Owing to great advances in the field, haematopoietic stem cell transplantation has become an established method for the treatment of many haemato-oncological, immunological and hereditary conditions with the potential of cure. The number of haematopoietic stem cell transplants performed worldwide reached one million by 2012. This review provides an overview of autologous and allogeneic haematopoietic stem cell transplantation including disease indications, the individual steps of the procedure

Amniotic membrane transplants in the pediatric population.

PURPOSE: To investigate the indications for and results of amniotic membrane transplantation (AMT) for the treatment of ocular disease in pediatric patients at a single institution. METHODS: The medical records of patients <18 years of age who underwent AMT for ocular disease between January 1, 2003, and September 1, 2015, were reviewed retrospectively. Patients were determined to have reached a clinical endpoint if there was resolution of the ocular condition being treated after AMT placement, no additional surgery required for treatment of the ocular condition, and no active disease at most

The role of second transplants for leukemia.

Management of relapsed leukemia following allogeneic transplantation is challenging. Intensive chemotherapy, donor lymphocyte infusions (DLI), or second transplantation have some value, but most reported series describe only a limited number of patients surviving beyond 2 or 3 years following relapse. Additionally, understandable selection-bias of reports describing the outcomes of intensive management approaches for relapsed leukemia confound generalizability to a broader population. However numerous reports suggest that second allogeneic transplantation for relapsed leukemia following an ini

Solid organ transplants in HIV-infected patients.

There is a growing need for kidney and liver transplants in persons living with HIV. Fortunately, with the significant advances in antiretroviral therapy and management of opportunistic infections, HIV infection is no longer an absolute contraindication for solid organ transplantation. Data from several large prospective multi-center cohort studies have shown that solid organ transplantation in carefully selected HIV-infected individuals is safe. However, significant challenges have been identified including prevention of acute rejection, management of drug-drug interactions and treatment of r

Kidney transplants from infected donors: our experience.

Organ procurement from infected donors may transmit a disease to the recipient that could cause a graft loss and/or recipient morbidity. Retrospectively, all kidney transplants from infected donors at our center in the last 4 years were reviewed. A donor was considered infected in the presence of at least one positive culture before procurement. From January 1999 to 2003, 23 of 160 donors (14.5%) were infected: in 10 donors a positive blood culture; in 3, a urine culture; and in 13, a bronchial culture. In a further 12 (7%) donors, only the preservation solution was contaminated. Organisms iso

US of liver transplants: normal and abnormal.

Whole-liver transplantation is an accepted and successful method of treating end-stage liver disease. As a result of the shortage of cadaveric livers, split-liver transplantation and living donor liver transplantation are becoming more commonplace. Ultrasonography (US) is the initial imaging modality of choice for detection and follow-up of early and delayed complications from all types of liver transplantation. Vascular complications include thrombosis and stenosis of the hepatic artery, portal vein, or inferior vena cava, as well as hepatic artery pseudoaneurysms and celiac artery stenosis.

[Perforating keratoplasty with transplants from pseudophakic donors].

PURPOSE: Pseudophacia is considered to be a relative exclusion criterion for cornea donation. The aim of this retrospective study was to investigate the clinical outcome and prognosis of penetrating keratoplasty with grafts from pseudophacic donors. PATIENTS AND METHODS: From April 1996 to July 2000 16 patients received grafts from pseudophacic donors (group I). The control group (group II) comprised 26 patients with grafts from donors without a history of intraocular surgery. Graft survival, ratio of grafts without immune reactions and chronic endothelial cell loss were compared between both

Bone marrow transplants for paroxysmal nocturnal haemoglobinuria.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare clonal haematological disorder characterized by intravascular haemolysis and increased risk of thrombosis. PNH is associated with bone marrow failure syndromes including aplastic anaemia, myelodysplasia and leukaemia. Bone marrow transplants are sometimes used to treat PNH, but small series and reporting biases make assessment of transplant outcome difficult. The outcome of 57 consecutive allogeneic bone marrow transplants for PNH reported to the International Bone Marrow Transplant Registry (IBMTR) between 1978 and 1995 was analysed. The 2-

Adenovirus enterocolitis in human small bowel transplants.

This report describes two cases of pediatric small bowel transplant patients who developed diffuse adenovirus enterocolitis of their allografts. Based upon the presenting symptoms for this complication, in both patients a differential diagnosis of allograft rejection versus viral infection was clinically entertained. The clinical condition in both instances rapidly deteriorated and both patients died shortly after the development of the symptoms of fulminant septicemia. Autopsies were performed and histologic examination revealed extensive denudation of the gastrointestinal mucosa with edema a

Oral findings in children with liver transplants.

A report is presented of a prospective study into the oral manifestations of liver disease in children. Fifty-five children who had received a liver transplant were examined; their ages ranged from 8 months to 16 years 2 months, and 37 of them were under 5 years of age. The following information was noted: the teeth that were erupted, the presence and severity of enamel hypoplasia and of gingival hyperplasia, and the presence of intrinsic discoloration. Results showed that seven of the 17 children under 3 years of age had delayed eruption of teeth. Six children had enamel hypoplasia, three in

Our first 100 consecutive pediatric liver transplants.

Orthotopic liver transplantation (OLT) is nowadays accepted as the best therapy for end-stage liver disease. The difficulties involved are even greater in children than in adults, and it is debatable whether exclusively pediatric programs are warranted. The aim of this paper is to analyze our experience at the Children's Hospital "La Paz", with the first consecutive 100 OLT in children, 61% of whom weighed less than 20 kg. Since 1986, 220 pediatric patients were evaluated as candidates, 100 OLT were performed in 78 patients and 13 died on the waiting list, currently maintained below 15 cases.

[Outcome of kidney transplants with multiple arteries].

OBJECTIVES: To evaluate the morbidity of multiple artery renal transplants (> 1 artery) and to assess the value of this parameter as a risk factor for complications of renal transplantation in adults. METHODS: 140 renal transplants derived from brain dead donors with multiple arteries (2 arteries = 123, 3 arteries = 17) were reviewed with a mean follow-up of 3 years (6-120 months). Pedicle restoration was performed in 57 cases (end-to-side reimplantation of a polar artery = 17, composite aortic patch graft = 14, multiple procedures = 10, ligation of a polar artery = 8, arterial wound = 5, "gu

Large corneal transplants in corneal destructive disease.

PATIENTS AND METHODS: Forty nine keratoplasties were performed in 18 eyes of 17 patients with severe corneal destructive disease. RESULTS: One eye became phthisical and 5 others lost useful visual acuity but retained a satisfactory cosmetic result. No eyes were enucleated. Recurrence of the host disease in the graft was the largest cause of visual failure. Primary allograft rejection was rare in spite of the large size of the grafts and their proximity to the limbus and new vessels. CONCLUSION: For these grafts to succeed it is essential to bring the underlying systemic disease under control

Pediatric renal transplants--results with sequential immunosuppression.

Cyclosporine has improved the results of renal transplantation. In 1984, we began using it as part of a sequential immunosuppression protocol (MALG, AZA, P, and delayed administration of CsA) in our pediatric renal transplant recipients. We studied the outcome of the 131 pediatric renal transplants (less than or equal to 18 years of age at transplant) performed at our institution between June 1984 and March 1991. We compared these results with the 144 similar transplants performed since January 1980 that did not involve CsA immunosuppression. In the sequential immunosuppression group, there we
