Cleft hand.

Definition, incidence, genetics, and etiology of typical cleft hand are discussed in this paper. Cleft hand, central polydactyly, and osseous syndactyly were induced experimentally when the same teratogenic factors acted on the embryos at the same developmental stage. Typical cleft hand, central polydactyly, and syndactyly should be classified together as manifestations of the same entity, that is, failure of induction of finger rays. Typical cleft hand may be further subdivided into five types on the basis of the number of defective finger rays. Treatment of the typical cleft hand involves co

The vascularity of the wrist. Identification of arterial patterns at risk.

The intraosseous and extraosseous vascularity of carpal bones was studied in 75 cadaver limbs. Vascular patterns were correlated with the clinical incidence of avascular necrosis, and at-risk patterns of vascularity were identified. The carpal bones were found to fall into three groups. Group 1, which consisted of the scaphoid, the capitate, and eight percent of the lunates, had either vessels entering only one surface or large areas of bone that were dependent on a single vessel. This group was the most vulnerable to posttraumatic avascular necrosis. The bones of Group 2, the hamate and the t

Finite element evaluation of three methods of stable fixation of condyle base fractures.

The surgical treatment of mandibular condyle fractures currently offers several possibilities for stable internal fixation. In this study, a finite element model evaluation was performed of three different methods for osteosynthesis of low subcondylar fractures: (1) two four-hole straight plates, (2) one seven-hole lambda plate, and (3) one four-hole trapezoidal plate. The finite element model evaluation considered a load applied to the first molar on the contralateral side to the fracture. Results showed that, although the three methods are capable of withstanding functional loading, the lamb

Carpal fractures in children.

Carpal fractures in children are uncommon injuries, with the scaphoid being fractured most frequently. Thirty-three children with scaphoid fractures and one child each with fractures of triquetrum, trapezoid, hamate, and capitate were retrospectively reviewed. The mean age was 13 years, 4 months. The mechanism of injury was of low energy in most cases, with > 80% of injuries involving a fall. Localizing tenderness was present in 100% of cases. Of fractures, 97.3% were evident on the initial radiographs. One patient required a bone scan to confirm the diagnosis. All but one scaphoid fracture we

Growth of the nasopharynx and adenoid development from one to eighteeen years.

1. The dimensions of the nasopharynx, the adenoids and the nasopharyngeal airway were analyzed in twelve subjects selected from the longitudinal growth study of the Child Research Council of Denver. 2. The nasopharyngeal area was defined by four skeletally defined lines which formed a trapezoid. The nasopharyngeal area was divided into an adenoid-pharyngeal wall and airway areas which were measured using a polar planimeter. The trapezoid analysis proved to be a useful technique for quantification of nasopharyngeal dimensions. 3. The growth of the nasopharynx from nine months to 18 years was es

Analysis of carpal bones on MR images for age estimation: First results of a new forensic approach.

Current multifactorial age estimation methods are based on radiography, however, in the forensic field there is growing interest in using magnetic resonance imaging (MRI). With regard to the carpal region, MRI provides more information for defining the individual ossification nuclei and the cartilage surrounding single bones. During the phase of bone growth, the progressive reduction of the cartilage layer is accompanied by the development of a cartilage-bone interface. The aim of our study was to create a new model for age estimation, based on the ratio between the area occupied by the nucleu

Individualized herbal prescriptions for delayed union: A case series.

Background Bone fractures are important clinical events for both patients and professionals. Active treatment options are limited for delayed unions and for nonunions; surgery is common but not entirely risk-free. This report describes three cases of delayed union successfully treated with herbal decoction. Participants Three patients had trapezoid and 3rd metacarpal bone fractures, 2nd, and 5th metatarsal bone fractures, respectively. All three patients were diagnosed with delayed union by an independent orthopedic surgeon based on computed tomography (CT) scan/radiographic imaging and fractu

Can a day 4 bone scan accurately determine the presence or absence of scaphoid fracture?

STUDY OBJECTIVE: To evaluate the accuracy of day 4 bone scans in predicting the presence or absence of fracture in patients with "clinical scaphoid fracture." DESIGN: Prospective sensitivity study of ED patients with clinical scaphoid fractures. Each patient was immobilized in a thumb spica cast and had day 4 bone scans of both wrists and hands. Blinded day 4 bone scan results were ultimately compared with the diagnosis on day 14 when patients returned for repeat clinical examination and radiographs. In cases of equivocal radiographic or clinical examination results, a day 14 bone scan was pe

The anatomy of the ligaments of the wrist and distal radioulnar joints.

The ligaments of the wrist are responsible for guiding and constraining the complex motion of the carpal bones relative to the forearm bones, the metacarpals, and contiguous carpal bones. The majority of wrist ligaments are found within the joint capsule as organized thickenings composed of parallel collagen fascicles, small caliber nerves and blood vessels, and lined on their deep surfaces by synoviocytes. The palmar radiocarpal ligament complex is composed of the radioscaphocapitate, long radiolunate, radioscapholunate and short radiolunate ligaments. The ulnocarpal ligaments include the uln

A Thai mother and son with distal symphalangism, hypoplastic carpal bones, microdontia, dental pulp stones, and narrowing of the zygomatic arch: a new distal symphalangism syndrome?

A Thai mother and son with distal symphalangism and other associated abnormalities are reported. Distal and middle phalanges of fingers and toes 2-5 were either aplastic/hypoplastic or fused between the corresponding digits. The second fingers and fourth fingernails were most severely affected in both patients. The mother's hands were less severely affected; the middle and distal phalanges of her hands were malformed and fused. Besides the absence of fusion lines, the shape of the fused middle and distal phalanges was quite different from that of other types of fusion, i.e., fused bones in bot

[Early stages of trapezo-metacarpal joint arthritis and rotatory island transfer of the trapezo-metacarpal joint. DOOR procedure].

INTRODUCTION: Trapezium dysplasia and trapezo-metacarpal joint instability induce TMC-joint arthritis. To correct these two factors, we propose a surgical technique associating a double oblique osteotomy of the trapezium and of the first metacarpal basis, and an island rotator transfer of the TMC-joint harvested in block on the radial pedicle. The articular block is rotated at 180° on the longitudinal axis of the thumb. METHOD: Since 2000, 41 cases were performed on 38 patients (35 women, three men). Mean age was 47 (from 17 to 63). All the patients complained of a painful thumb and sometimes

Attention-guided jaw bone lesion diagnosis in panoramic radiography using minimal labeling effort.

Developing a deep-learning-based diagnostic model demands extensive labor for medical image labeling. Attempts to reduce the labor often lead to incomplete or inaccurate labeling, limiting the diagnostic performance of models. This paper (i) constructs an attention-guiding framework that enhances the diagnostic performance of jaw bone pathology by utilizing attention information with partially labeled data; (ii) introduces an additional loss to minimize the discrepancy between network attention and its label; (iii) introduces a trapezoid augmentation method to maximize the utility of minimally

Mechanism of scaphoid waist fracture: finite element analysis.

The mechanism of scaphoid waist fracture is not completely understood. We used finite element analysis to study the formation of scaphoid waist fractures. Clinical computed tomography scans of 12 wrists were used to create models for finite element analysis. The points of application of load were at different sites along the scaphotrapeziotrapezoid joint distally, and the scaphoid fossa plus the area underlying the radioscaphocapitate ligament was regarded as the fixed proximal support. A fracture was produced at the scaphoid waist in all cases. The location of failure of bone at its anterior

Headless screw fixation through the dorsal rough surface for proximal-pole scaphoid-nonunion: a report of 15 patients.

Clinical outcomes of the dorsal-retrograde headless screw-fixation technique in 15 patients with proximal scaphoid nonunion are presented. In this technique, screws are inserted from the dorsal rough surface of the scaphoid, located between the dorsal ridge and scaphoid-trapezium-trapezoid joint. Fifteen patients underwent osteosynthesis with this technique with iliac bone graft. Seven patients required primary surgery, and eight patients with a history of failed operation required revision surgery. Among 15 patients, 13 achieved union and two with persistent nonunion were asymptomatic with av

Assessing the Feasibility of Selective Piezoelectric Osteotomy in Transorbital Approach to the Middle Cranial Fossa: Anatomical and Quantitative Study and Surgical Implications.

OBJECTIVE: To verify the feasibility and discuss advantages and disadvantages of a piezoelectric orbitotomy during superior eyelid endoscopic transorbital approach (SETOA). An illustrative case demonstrating the application of this novel technique is also presented. METHODS: Exoscopic/endoscopic SETOA to middle cranial fossa was performed on 5 adult specimens. The surgical corridor was created via piezoelectric orbitotomy by performing 3 selective and safe micrometric bone cuts providing a 1-piece trapezoid bone flap, which was repositioned and secured at the end of the procedure. Effects of dividing the transverse carpal ligament on the mechanical behavior of the carpal bones under axial compressive load: a finite element study.

Transecting the transverse carpal ligament (TCL) is a routine procedure to surgically treat carpal tunnel syndrome; yet, its mechanical consequences on carpal bones are unclear. In this study, our intent was to perform a computational analysis of carpal biomechanics resulting from TCL release. A three-dimensional finite element model of the wrist was constructed, which included all the carpal bones, the distal ulna and radius, the proximal metacarpals and the interosseous ligaments. Cartilage layers of each bone were modeled manually according to anatomic visualization software. The TCL was al

A Novel Surgical Technique for Fixation of Recurrent Acromioclavicular Dislocations: AC Dog Bone Technique in Combination with Autogenous Semitendinosus Tendon Graft.

Various surgical techniques have been described for the fixation of acromioclavicular (AC) dislocations. However, recurrent dislocation is one of the main complications associated with the majority of these techniques. We report a case of postoperative AC joint redislocation. In order to overcome recurrent dislocation after revision surgery, a reconstruction of the conoid and trapezoid ligament with the use of a free tendon graft in combination with a FiberTape was provided within a novel surgical technique. After 12 months, the patient was very satisfied with the functional outcome. The patie

[Translated article] Influence of the scaphotrapeziotrapezoid joint on the results of total trapeziometacarpal prostheses.

INTRODUCTION: Scaphotrapeziotrapezoid (STT) joint osteoarthritis may influence the outcome after trapeziometacarpal prosthesis (TMP) implantation. The literature regarding its clinical and radiological assessment is unclear. The aim of our study was to determine by means of a pre- and post-intervention CT study whether the degree of STT involvement influences the clinical-functional or radiological results after TMP implantation, and to establish whether or not STT osteoarthritis could be a contraindication for the use of these implants. METHODS: Prospective study of 60 patients with trapezio

Acute Traumatic Pseudo-pointing Index Finger Due to an Isolated Second Metacarpal-Trapezoid Dislocation: A Case Report.

INTRODUCTION: An isolated second metacarpal dislocation is one of the least reported injuries of the hand. This particular injury assumes clinical significance as they occur subsequent to high energy direct trauma and are usually missed with associated carpal and metacarpal bone fractures. We describe this unique case of an isolated metacarpal-trapezoidal disruption with clinical presentation as the pseudo-pointing index finger and also describe a novel radiological sign to ascertain the rotational deformity of metacarpals on plain radiographs.
