Rang retractors for sciatic notch exposure during pelvic osteotomy.

We present simple but effective retractors used in pairs to expose the sciatic notch during Salter innominate osteotomy. We have found them to be useful for a wide range of procedures requiring similar exposure. We present them here in tribute to the memory of the designer Mercer Rang.

The rehabilitative management of the traumatic brain injury patient with associated femoral neuropathy.

The initial diagnoses of associated injuries in patients with traumatic brain injury (TBI) are often overlooked because of the priority given to life-sustaining measures. Pelvic and abdominal injuries comprise less than 5% each of the concurrent injuries associated with TBI and multiple trauma. This report describes a 32-year-old man who sustained a moderate TBI with facial, pelvic, and extremity fractures secondary to a fall. His hospital course was complicated by sepsis, acute renal failure, and retroperitoneal hemorrhage. Admitted to the rehabilitation service 6 weeks after the fall, the pa

Meningitis in the neurosurgical patient.

In the patient with a basilar skull fracture and CSF leak, the risk of meningitis is greatly increased. The diagnosis of both leak and infection can be obscured by the patient's other injuries, and requires aggressive investigation of symptoms that suggest infection. Although the diagnosis is made with CSF cultures, when clinically suspected, treatment should begin after appropriate cultures have been obtained. Treatment should be directed against the most likely organisms, Streptococcus pneumoniae, Haemophilus influenzae, and the other organisms common to the upper respiratory tract. There ar

There Is No Difficult Patient.

Patients receiving chronic hemodialysis frequently experience significant physical, psychological, and emotional stress that often contributes to patients' dysregulation, treatment resistance, and behaviors labeled as "difficult." Traditional responses to these situations can unintentionally escalate the distress the patient is experiencing and negatively affect the staff-patient relationship. This article explores patient behavior through the lens of emotional intelligence, trauma-informed care, and nervous system regulation. Examples from real-world clinical experiences are used to demonstra

Exploring altered oscillatory activity in the anterior cingulate cortex after nerve injury: Insights into mechanisms of neuropathic allodynia.

While neural oscillations play a critical role in sensory perception, it remains unclear how these rhythms function under conditions of neuropathic allodynia. Recent studies demonstrated that the anterior cingulate cortex (ACC) is associated with the affective-aversive component of pain, and plasticity changes in this region are closely linked to abnormal allodynic sensations. Here, to study the mechanisms of allodynia, we recorded local field potentials (LFPs) in the bilateral ACC of awake-behaving rats and compared the spectral power and center frequency of brain oscillations between healthy

Nose-to-brain translocation and nervous system injury in response to indium tin oxide nanoparticles of long-term low-dose exposures.

Indium tin oxide (ITO) is a semiconductor nanomaterial with broad application in liquid crystal displays, solar cells, and electrochemical immune sensors. It is worth noting that, with the gradual increase in worker exposure opportunities, the exposure risk in occupational production cannot be ignored. At present, the toxicity of ITO mainly focuses on respiratory toxicity. ITO inhaled through the upper respiratory tract can cause pathological changes such as interstitial pneumonia and pulmonary fibrosis. Still, extrapulmonary toxicity after nanoscale ITO nanoparticle (ITO NPs) exposure, such a

Gradual stretching of the proximal nerve stump induces the growth of regenerating sprouts in rats.

We investigated the effect of direct gradual stretching on the proximal nerve stump morphologically. A 10-mm-long nerve segment was resected from the sciatic nerve of a rat. The end of the proximal nerve stump was fixed to a small ring and the marking suture was placed at a point 1 mm proximal to the ring. Then, the nerve stump was lengthened at a rate of 1 mm/day via a stretching of the ring using an original external device. After a stretching of 20 days, the distance from the ring to the marking suture became 12 mm. Whereas large mature myelinated axons were observed in the proximal part of

Neurovascular injury: avoiding catastrophe.

Major neurovascular injury is the least common, but most distressing, complication of total hip arthroplasty (THA). The keys to minimizing the incidence of these complications are recognizing patients at risk and knowledge of the relevant anatomy. Partial sciatic palsy is the most common nerve injury. At least partial recovery can be expected in 70% to 80% of cases, with the remainder frequently displaying dissatisfaction with their surgery. Vascular injury is most frequently associated with the use of screws for fixation of structural grafts, acetabulur components, and protrusio rings or cage

Avoidance and management of neurovascular injuries in total hip arthroplasty.

Neural injuries that occur after total hip arthroplasty (THA) can be classified as involving either the central nervous system or peripheral nerves. Central nervous system changes after THA may be attributed to increased appreciation of fat embolism syndrome associated with THA. Certain maneuvers such as impacting the acetabulum, femoral reaming, and cement pressurization can force marrow fat into the venous system. When there is an associated right to left shunt, paradoxical embolization can occur, which may account for previously unexplained cases of confusion and mental status changes after

Opioid-responsive and opioid-non-responsive pain in cancer.

Cancer pain in general responds in a predictable way to analgesic drugs and drug therapy is the mainstay of treatment, successfully controlling pain in 70-90% of patients. The two major problem areas are pain associated with nerve damage, and 'incident' (movement-related) bone pain. Nerve damage pain tends not to respond well to morphine or other opioids. The difficulty with severe incident pain is that if the dose of opioid is titrated sufficiently to relieve the pain on weight-bearing or on movement and is then given regularly at this level, it is too much for the patient at rest. The patien

Prevention of central nervous system infections in patients at risk.

Infections of the central nervous system in patients at risk are relatively uncommon when compared with pneumonia, bacteremia, and soft tissue infection. However, they carry serious morbidity and are frequently fatal. Each of the diverse conditions that can place a patient at risk for central nervous system infection is associated with a fairly predictable spectrum of etiologic organisms. Various forms of trauma (including blunt and penetrating injuries and neurosurgery, especially when a cerebrospinal fluid shunt is implanted) predispose to infection with common pathogenic bacteria. Defects o

Autonomic Dysfunction and Postural Orthostatic Tachycardia Syndrome Secondary to Abuse.

Postural orthostatic tachycardia syndrome (POTS) and other forms of autonomic dysfunction are increasingly recognized in patients with histories of physical or psychological trauma. Emerging evidence suggests that trauma-related dysregulation of the autonomic nervous system may manifest as POTS, with hypervigilance and hyperadrenergic states proposed as potential mechanisms. However, the relationship between sexual abuse and subsequent autonomic dysfunction remains unclear. We present a series of patients referred to cardiology for suspected POTS, highlighting clinical features, potential mech

Experimental therapies for repair of the central nervous system: stem cells and tissue engineering.

Several stem cell-based therapeutic tools are currently being investigated for the regeneration of central nervous system (CNS) injuries. This review focuses on innovative approaches for CNS tissue repair via the use of implantable cellular devices. These devices are supported by biopharmaceuticals and conventional physiotherapy for the restoration of lost neuronal circuits and CNS function. This paper further reviews new and promising tools currently in pre-clinical and clinical tests for the treatment of CNS diseases where substantial loss of cellular and extracellular components of neural t

Recovery of colonic transit following extrinsic nerve damage in rats.

INTRODUCTION: Injury to pelvic sympathetic and parasympathetic nerves from surgical and obstetrical trauma has long been cited as a cause for abnormal colorectal motility in humans. Using a rat model, acute transaction of these extrinsic nerves has been shown to effect colorectal motility. The aim of this study is to determine in a rat model how transection of these extrinsic nerves affects colonic transit over time. METHODS: Eighty-two Sprague-Dawley rats underwent placement of a tunneled catheter into the proximal colon. Bilateral hypogastric, pelvic nerves (HGN and PN) or both were transec
