Transforming the chemotherapy of human African trypanosomiasis.

SUMMARYPrior to 2019, when the orally available drug fexinidazole began its clinical use, the treatment of human African trypanosomiasis (HAT) was complex and unsatisfactory for many reasons. Two sub-species of the Trypanosoma brucei parasite are responsible for HAT, namely the rhodesiense form found in East and Southern Africa and the gambiense form found in Central and West Africa. Diseases caused by both forms manifest in two stages: stage 1 before and stage 2 after central nervous system involvement. Prior to 2019, different drugs were required for each of the two parasite sub-species at e

Generation of neuroinflammation in human African trypanosomiasis.

Human African trypanosomiasis (HAT) is caused by infection due to protozoan parasites of the Trypanosoma genus and is a major fatal disease throughout sub-Saharan Africa. After an early hemolymphatic stage in which the peripheral tissues are infected, the parasites enter the CNS causing a constellation of neurologic features. Although the CNS stage of HAT has been recognized for over a century, the mechanisms generating the neuroinflammatory response are complex and not well understood. Therefore a better understanding of the mechanisms utilized by the parasites to gain access to the CNS compa

Fexinidazole for the treatment of human African trypanosomiasis.

On November 15, 2018, Fexinidazole Winthrop received a positive opinion from the European Medicines Agency (EMA) (under Article 58) for treatment of first-stage (hemolymphatic) and second-stage (meningoencephalitic) human African trypanosomiasis caused by Trypanosoma gambiense (gHAT) in adults and children 6 years and older and weighing 20 or more kg. This is the first oral regimen for gHAT that is effective in treating both disease stages. Although fexinidazole has potential to simplify current therapies, it does not entirely eliminate the need for disease staging by lumbar puncture because p

Human African trypanosomiasis in travellers to Kenya.

BACKGROUND: Clinical scores such as quick sequential organ failure assessment (qSOFA), early warning score (EWS), and universal vital assessment (UVA) are commonly used as screening and prognostic tools in sepsis. However, consistent findings across different regions and hospital settings remain limited. This study aims to evaluate the performance of these clinical scores and identify predictors of survival in septic adults admitted to the ICU. METHODS: A prospective longitudinal study was conducted from October 10, 2023, to October 9, 2024, involving adults admitted to the intensive care uni

Towards the Atlas of human African trypanosomiasis.

BACKGROUND: Updated, accurate and comprehensive information on the distribution of human African trypanosomiasis (HAT), also known as sleeping sickness, is critically important to plan and monitor control activities. We describe input data, methodology, preliminary results and future prospects of the HAT Atlas initiative, which will allow major improvements in the understanding of the spatial distribution of the disease. METHODS: Up-to-date as well as historical data collected by national sleeping sickness control programmes, non-governmental organizations and research institutes have been co

Drugs and drug resistance in African trypanosomiasis.

Despite the many decades of use of most of the current trypanocides, we know little of their mode of action. This may in part be because most of these will act on multiple targets once inside the cell, and they derive their selective action on the parasite from selective accumulation by the pathogen. Loss of this capacity for drug uptake by the trypanosome would thus be a major cause for drug resistance. We here discuss the use of current drugs against human and veterinary African trypanosomiasis, the prevalence, causes and mechanisms of drug resistance and new developments in trypanosomiasis

Treatment and control of human African trypanosomiasis.

PURPOSE OF REVIEW: Access to treatment is a multi-step process and little progress has been made to improve treatments for sleeping sickness over the past 50 years. The current strategy is based on diagnostic tools developed in the 1960s while available drugs are still the same as those developed in the middle of the last century. Strategic opportunities can only be based on two achievements: improved diagnosis and safer drugs. This paper reviews the development of new diagnostic tools and drugs and the opportunity offered by new technologies for their further improvement. RECENT FINDINGS: Th

Eflornithine for the treatment of human African trypanosomiasis.

Eflornithine is the only new molecule registered for the treatment of human African trypanosomiasis over the last 50 years. It is the drug used mainly as a back-up for melarsoprol refractory Trypanosoma brucei gambiense cases. The most commonly used dosage regimen for the treatment of T. b. gambiensesleeping sickness consists of 100 mg kg(-1) body weight at intervals of 6 h for 14 days (150 mg kg(-1) body weight in children) of eflornithine given as short infusions. Its efficacy against Trypanosoma brucei rhodesiense is limited due to the innate lack of susceptibility of this parasite based on

Analysis of macrophage activation in African trypanosomiasis.

African trypanosomes cause a fatal disease of man and animals that is characterized by extensive functional, histological, and pathological changes in the lymphoid tissues of infected hosts, including an increase in the numbers and activation state of macrophages. Macrophage activation during infection is the result of exposure of these cells to parasite components and host-derived IFN-gamma, produced in response to parasite antigens. The balance of these different activation signals may determine the outcome of infection. In the experiments described here, we assessed the ability of the varia

The epidemiology and control of human African trypanosomiasis.

Human African trypanosomiasis is caused by Trypanosoma brucei gambiense in West and Central Africa, and by Trypanosoma brucei rhodesiense in East and southern Africa. In recent years there has been a dramatic resurgence of Gambian trypanosomiasis in Central Africa, especially in the Democratic Republic of Congo, Angola and Sudan. The disease is quiescent in most of West Africa, as is Rhodesian trypanosomiasis the other side of the continent. The epidemiology of Gambian trypanosomiasis is reviewed in detail. The long duration of infection in human hosts with cycles of intermittent parasitaemia,

[Diagnosis of human African trypanosomiasis in 2001].

Human African trypanosomiasis is characterized by a non-specific clinical presentation with no consistent, pathognomonic manifestations. However definite diagnosis is necessary to avoid unnecessary therapeutic risks with toxic drugs. Further complicating this situation is the frequent need to achieve field diagnosis in remote locations with limited facilities. Serological tests such as CATT (card agglutination trypanosomiasis test) are useful for initial population screening to identify suspects but are not sufficiently reliable for definitive diagnosis since the variations in sensitivity and

Perspectives in research on and control of African trypanosomiasis.

African trypanosomiasis affects both man and his domestic animals, and is fatal if untreated. The risk of epidemics makes the disease a major public health problem in 36 sub-Saharan African countries, where some 50 million people are at risk of contracting the disease. Continued suppression of the disease through medical surveillance is indispensable to prevent epidemics which are difficult and costly to control. Recent epidemics and flare-ups have occurred in certain countries due to breakdown in medical surveillance occasioned by political, social and economic factors. The development of new
