[A case of spinal tuberculosis in a young Filipino man].

A 19-year-old Filipino man was admitted to our hospital because of persisting fever and back pain. He had recognized his symptoms 6 months previously, but a definite diagnosis was not made. Image testing demonstrated a compressed fracture of the thoracic vertebrae accompanied with a perivertebral abscess. A biopsy specimen revealed granuloma compatible with tuberculosis (TB). Anti-TB drugs were initiated, and his clinical symptoms steadily improved. However, he developed neuropathic symptoms due to exacerbation of the abscess two months after starting the anti-TB drugs. An immediate laminectom

Surgical correction of kyphotic deformity in spinal tuberculosis.

PURPOSE: Approximately 5% of patients with spinal tuberculosis will develop a severe kyphotic deformity resulting in increased potential for pain, spinal cord compression, cardiopulmonary dysfunction, costopelvic impingement and cosmetic concerns. This manuscript reviews the evaluation and surgical management of tuberculous kyphosis. METHODS: This is a review article. RESULTS: Risk factors for the development of severe kyphosis include those who develop spinal tuberculosis as children, multiple vertebral body involvement and thoracic spine involvement. These complications can be prevented by

Unusual spinal tuberculosis with cord compression in an infant.

Spinal abscess is rare in children, especially in young infants. The most common etiology is bacteria, Staphylococcus aureus in particular. Mycobacterium tuberculosis is another cause. We report an unusual cervical spinal abscess with spinal cord compression in a 13-month-old child. The presenting symptoms were weakness in the right arm and, predominantly, the right leg for 1 month. Magnetic resonance imaging showed an abscess of the cervical spine, extension with bony destruction, and spinal cord compression. The patient underwent an emergency neurosurgical decompression and laminectomy. Path

Posterior listhesis of a lumbar vertebra in spinal tuberculosis.

The management of spinal tuberculosis, especially in children, is controversial. In children, vertebral destruction is more severe than adults because of the cartilaginous nature of their bone. Modern chemotherapy has significantly decreased mortality in spinal tuberculosis, but morbidity remains high. Without early surgery, patients can develop severe kyphosis leading to respiratory insufficiency, painful costopelvic impingement and paraplegia. Lumbar kyphosis results in early degenerative lumbar canal stenosis and is cosmetically unacceptable. We report a paediatric case of atypical spinal t

Current difficulties in the diagnosis and management of spinal tuberculosis.

BACKGROUND: The diagnosis of spinal tuberculosis (ST) is difficult and it commonly presents at an advanced stage. The management and follow up is complicated by a lack of guidance on the appropriate use and interpretation of spinal magnetic resonance studies (MR). AIMS: A retrospective study was performed at a UK centre to identify the demographic and presenting features of a spinal TB population, their response to treatment, and the value of follow up MR studies. PATIENTS AND RESULTS: Twenty one patients were identified with mean symptom duration of 11 (1.5-36) months having been assessed b

[Diagnostic capacities of laser biophotometry in spinal tuberculosis].

The capacities of laser biophotometry in the diagnosis of tuberculous spondylitis and its complications were studied in 42 patients with this disease and in controls. There was a definite significant trend of changes in the reflection coefficient of a laser signal in patients with an active inflammatory process in the vertebrae, which makes it possible to establish a differential diagnosis of the above conditions and to determine the time course of changes in the specific process in the vertebral column during therapy. Comparative analysis of the proposed method with the thermographic techniqu

Operative treatment of progressive deformity in spinal tuberculosis.

We report on 14 patients with tuberculosis of the spine treated by operation. All had disease of three or more vertebrae with involvement of both the anterior and posterior columns and a progressive kyphotic deformity in spite of conservative treatment. We consider such spines to be 'unstable' and have found that anterior as well as posterior fusion with instrumentation is necessary.

Spinal tuberculosis: a report of five cases and a review.

Spinal tuberculosis (TB) is an uncommon occurrence in developed countries. We present five cases of spinal TB illustrating some of the problems that can be encountered in clinical practice. Delay in diagnosis due to physicians unawareness of TB as a diagnostic possibility in patients with persistent back pain was observed in two patients. A high clinical index of suspicion is, therefore, needed for diagnosis. Tuberculin skin testing was positive in four patients. Computed tomography (CT) has become the examination of choice. It allows precise location of lesions and their extension to paraspin

A clinical study and classification of posterior spinal tuberculosis.

Tuberculous involvement of the posterior elements of the spine is uncommon. Review of the literature reveals little information and there is scant mention of this subject in standard textbooks of orthopaedics. A study of 27 cases of such lesions seen over 6 years is presented, drawn from two separate centres, 21 from Central India and 6 from South Eastern Iran. The clinical presentation, investigations and treatment are analysed. A new four point classification is described based upon the site of the lesion, the stage of the lesion, associated lesions and neurological deficit.

Mask R-CNN assisted diagnosis of spinal tuberculosis.

The prevalence of spinal tuberculosis (ST) is particularly high in underdeveloped regions with inadequate medical conditions. This not only leads to misdiagnosis and delays in treatment progress but also contributes to the continued transmission of tuberculosis bacteria, posing a risk to other individuals. Currently, CT imaging is extensively utilized in computer-aided diagnosis (CAD). The main features of ST on CT images include bone destruction, osteosclerosis, sequestration formation, and intervertebral disc damage. However, manual diagnosis by doctors may result in subjective judgments and

Paraplegia from spinal intramedullary tuberculosis: A case report.

BACKGROUND: Tuberculosis (TB) mostly attacks the lungs, and extrapulmonary TB involving the central nervous system is uncommon; among these cases, spinal intramedullary TB is even more rare. The clinical manifestations of spinal intramedullary TB are similar to those of intramedullary spinal cord tumors. Therefore, it is necessary to make a careful differential diagnosis of spinal intramedullary lesions to achieve the appropriate treatment and favorable prognosis. We report a rare case of a young male patient with paraplegia due to spinal intramedullary TB, which is uncommon and regrettable.

Spinal tuberculosis: magnetic resonance imaging and neurological impairment.

STUDY DESIGN: Retrospective descriptive study. OBJECTIVE: To evaluate the preoperative magnetic resonance imaging (MRI) findings in spinal tuberculosis and identify features that correlate with the neurologic status and outcome. SUMMARY OF BACKGROUND DATA: MRI plays an important role in the diagnosis of spinal tuberculosis with a high specificity and sensitivity. It allows demonstration of bony, soft tissue and neural pathology; however, the clinical correlation is not clear. METHODS: MRI scans of 82 consecutively managed spinal tuberculosis patients over a 4-year period were studied. Data
