[A persistent inflammatory adenopathy… think about tularemia].

In recent years, we have observed a geographic expansion of tularemia cases recorded in Europe. In Switzerland, the incidence of reported cases almost tripled between 2014 and 2023. The infection is transmitted via a cutaneous, oral, or respiratory route from an animal reservoir, and arthropod vectors are one route of contamination. In Europe, the ulcero-glandular form is the most common. Complications may arise if treatment is inadequate or delayed. Treatment is based on targeted antibiotic therapy. A number of simple preventive measures can be taken to limit transmission. This article provid

Oculoglandular Tularemia From Crushing an Engorged Tick.

We report on an unusual case of oculoglandular tularemia acquired after crushing a tick removed from a dog. As a droplet sprayed into the patient's eye the eyelids became inflamed, and on the fourth day, a high fever started. Prompt antibiotic treatment prevented serious complication.

Pediatric Oropharyngeal Tularemia Cases: Challenges in Management.

Background: Tularemia is one of the most prevalent zoonoses across the world. Patients in Turkiye mostly contract the oropharyngeal form, acquired through drinking, or contact with microorganism-contaminated water. Methods: Patients with oropharyngeal tularemia aged under 18 years and diagnosed between January 01, 2017, and December 31, 2020, were evaluated retrospectively. Tularemia was diagnosed in patients with compatible histories, symptoms, clinical presentations, and laboratory test results. Results: The mean age of 38 children was 12.1 ± 3.4 years, and the female/male ratio was 0.58 (14

Extended-Interval Gentamicin Dosing for Pulmonic Tularemia.

Francisella tularensis is a Gram-negative coccobacillus that is rarely encountered in clinical practice. Patients can present with cutaneous, pulmonary, cardiac, mucous membrane, or gastrointestinal involvement. A clinician should have a heightened suspicion in endemic areas or when outbreaks appear. Diagnosis is achieved through serological testing or polymerase chain reaction assays. Although historically the treatment of choice was streptomycin, gentamicin is now preferred due to its availability and relatively safer side effect profile with extended-interval dosing. Limited published evide

Biopolymer-Antigen Conjugate Vaccines for Prevention of Tularemia.

Francisella tularensis, the causative agent of tularemia, is a facultative intracellular pathogen categorized as a tier I bioterrorism agent. The current live vaccine strain (LVS) vaccine poses safety and virulence concerns, advocating the need for a safer alternative. In this study, we demonstrated a combination of antigens FopA, Tul4, SucB, and DnaK conjugated to biopolymer particles (BPs) as a multivalent subunit vaccine that not only induces LVS and SchuS4-specific antibody titers but also confers protective immunity with differential cytokine profiles upon LVS challenge. This multivalent

Neck Mass: Tularemia Misdiagnosed as a Possible Malignancy.

BACKGROUND: Tularemia is a zoonotic disease of the Northern Hemisphere caused by Francisella tularensis. Given its diverse animal reservoir and wide range of transmission modes, a thorough history of potential exposure is paramount. In the right context, fever combined with skin eruption and lymphadenopathy should raise the possibility of tularemia. Additionally, health care providers should be mindful that F. tularensis is a risk group 3 pathogen and a category A bioterrorism agent and presents a risk for laboratory personnel due to its aerosolization potential, low infectious dose, and fatal

Typhoidal tularemia with pancytopenia and pericarditis in a georgian adolescent.

We report a rare case of typhoidal tularemia in an 18-year-old male from Kvemo Kartli, southern Georgia. The patient presented with a two-week history of high-grade fever, severe malaise, anorexia, and laboratory evidence of pancytopenia with hypoglycemia. The clinical course was complicated by pericarditis. Hematologic malignancy was initially suspected, but bone marrow aspiration revealed no blasts or dysplasia. Serologic testing confirmed Francisella tularensis infection. The patient responded to targeted antimicrobial therapy with full recovery. This case highlights the importance of consi

A case of pulmonary tularemia mimicking lung cancer.

Tularemia is a zoonotic infectious disease caused by Francisella tularensis. The main reservoir for F. tularensis is lagomorphs, rodents, arthropods, and the hydrotelluric environment. It also can be transmitted by infected animals or by drinking contaminated water. Pulmonary tularemia is a rare form of tularemia mostly transmitted by inhalation. In this report, we present a 51-year-old male patient who was admitted to the hospital with fever, cough, sputum, and chest pain. Biopsy of the lesion compatible with mass on chest radiography revealed granulomatous inflammation. The diagnosis of pulm

Preclinical testing of a vaccine candidate against tularemia.

Tularemia is caused by a gram-negative, intracellular bacterial pathogen, Francisella tularensis (Ft). The history weaponization of Ft in the past has elevated concerns that it could be used as a bioweapon or an agent of bioterrorism. Since the discovery of Ft, three broad approaches adopted for tularemia vaccine development have included inactivated, live attenuated, or subunit vaccines. Shortcomings in each of these approaches have hampered the development of a suitable vaccine for prevention of tularemia. Recently, we reported an oxidant sensitive mutant of Ft LVS in putative EmrA1 (FTL_068

The potential of liposome-encapsulated ciprofloxacin as a tularemia therapy.

Liposome-encapsulation has been suggested as method to improve the efficacy of ciprofloxacin against the intracellular pathogen, Francisella tularensis. Early work with a prototype formulation, evaluated for use against the F. tularensis live vaccine strain, showed that a single dose of liposomal ciprofloxacin given by the intranasal or inhalational route could provide protection in a mouse model of pneumonic tularemia. Liposomal ciprofloxacin offered better protection than ciprofloxacin given by the same routes. Liposomal ciprofloxacin has been further developed by Aradigm Corporation for Pse

Current status of vaccine development for tularemia preparedness.

Tularemia is a high-risk infectious disease caused by Gram-negative bacterium Francisella tularensis. Due to its high fatality at very low colony-forming units (less than 10), F. tularensis is considered as a powerful potential bioterrorism agent. Vaccine could be the most efficient way to prevent the citizen from infection of F. tularensis when the bioterrorism happens, but officially approved vaccine with both efficacy and safety is not developed yet. Research for the development of tularemia vaccine has been focusing on the live attenuated vaccine strain (LVS) for long history, still there
