Fatigue analysis of tympanic membrane after ossiculoplasty.

CONCLUSION: A tentative conclusion was made that the finite element method can be used to investigate the fatigue life of the tympanic membrane after ossiculoplasty; the main reason of the extrusion of the tympanic membrane may not be blamed to the fatigue mechanism under normal sound pressure. OBJECTIVE: This study was to investigate the extrusion of the prosthesis from the tympanic membrane at post-ossiculoplasty by finite element method. METHODS: A finite element model of the human middle ear has been constructed and used as the model of the constructed middle ear at post-ossiculoplasty.

Wrinkling of Tympanic Membrane Under Unbalanced Pressure.

Mechanics of tympanic membrane (TM) is crucial for investigating the acoustic transmission through the ear. In this study, we studied the wrinkling behavior of tympanic membrane when it is exposed to mismatched air pressure between the ambient and the middle ear. The Rayleigh-Ritz method is adopted to analyze the critical wrinkling pressure and the fundamental eigenmode. An approximate analytical solution is obtained and validated by finite element analysis (FEA). The model will be useful in future investigations on how the wrinkling deformation of the TM alters the acoustic transmission funct

[Granuloma on tympanic membrane - a case report].

Granuloma on tympanic membrane is a rare complication after ventilation drainage of tympanic cavity. The paper presents a case of a child for chronic otitis media with effusion with ventilation drainage of the right tympanic cavity, retained for 24 months, and granuloma on tympanic membrane growing all over the ventilation drain, imitating acute inflammation, causing conductive hearing loss. The patient was treated with the vent tube removal together with granulation inflammatory. Spontaneous healing of the perforation of the tympanic membrane and improve hearing were obtained after a four-mon

Ossicular calisthenics: Pneumomassage of the tympanic membrane.

OBJECTIVES/HYPOTHESIS: Throughout the latter portion of the 19th and early 20th centuries, pneumomassage devices were widely used by otologists to treat a variety of ear diseases. The so-called eardrum massagers produced a regular, repetitive, oscillatory movement through modifying the air pressure in the ear canal. The goal of this study was to trace the invention, clinical use, technological diversification, abandonment, and ultimate resurrection of tympanic pneumomassage. METHOD: Review of the 19th- and early 20th-century medical journals, texts, and trade catalogs concerning the tympanic

Tympanic membrane perforation with squamous epithelial ingrowth.

The squamous epithelium of the tympanic membrane and external auditory canal exhibits an inherent migration pattern to facilitate the exfoliation of keratinizing squamous cells as part of a natural cleansing mechanism.

Functional correlations of tympanic membrane perforation size.

OBJECTIVE: The correlation between tympanic membrane perforations and hearing loss was studied. STUDY DESIGN: Prospective data from 220 patients, who underwent primary surgery for simple chronic otitis media with a perforated eardrum, were analyzed. SETTING: Tertiary referral center. PATIENTS: One hundred fifty-one patients with 155 eardrum perforations, which were checked for correct diagnosis, normal middle-ear status, and integrity of the ossicular chain, were included. INTERVENTIONS: All patients underwent primary myringoplasty. MAIN OUTCOME MEASURES: Preoperative conductive hearing l

Scaffolds for tympanic membrane regeneration in rats.

Tympanic membrane (TM) perforations lead to significant hearing loss and result in possible infection of the middle ear. Myringoplasty is commonly performed to repair chronic perforations. Although various grafts and materials have been used to promote TM regeneration, all have associated limitations. The aim of this study was to evaluate the efficacy and feasibility of two graft materials, silk fibroin scaffold (SFS) and porcine-derived acellular collagen type I/III scaffold (ACS), compared with two commonly used graft materials (paper patch and Gelfoam) for the promotion of TM regeneration.

Natural history of pediatric tympanic membrane perforation.

OBJECTIVE: To investigate the natural history of pediatric tympanic membrane perforation using an existing clinical database, with the aim of defining the time beyond which a perforation is unlikely to close naturally. STUDY DESIGN: Retrospective cohort study. SETTING: Tertiary center. PATIENTS AND INTERVENTIONS: A database of pediatric ear, nose, throat, and audiology consultations containing 20 years of data was analyzed. A total of 2865 episodes of perforation were followed, with all cases of surgical reconstruction excluded. Cases of perforation after ventilation tube extrusion were tag

Traumatic tympanic membrane perforation: an aetiological profile.

BACKGROUND: Traumatic perforation of the tympanic membrane may be due to direct or indirect source. The aim of the study is to profile the various aetiologies of traumatic tympanic membrane perforation in Ilorin, north central Nigeria.A retrospective review of 64 patients seen at the University of Ilorin Teaching hospital, Ilorin, Nigeria over a ten year period (January 1998 to Dec 2007) with history of traumatic tympanic membrane perforation from various causes, these also included multiply injured patients with bleeding from middle ear as part of their presentations. Chemical closure of chronic tympanic membrane perforations.

BACKGROUND: The purposes of closing chronic dry perforations of the tympanic membrane are to improve hearing and prevent middle ear infections. Before the use of the operating microscope and improved instrumentation, tympanic membrane perforations were closed by application of chemicals or with a temporary patch. Today, although surgery is the treatment of choice, small dry perforations may be successfully closed by chemical means in an office setting. METHODS: Twenty-eight of the 69 patients with chronic dry tympanic membrane perforations on the surgical waiting list at Hawke's Bay Regional

Ventilation tubes and persisting tympanic membrane perforations.

Surgical management of otitis media with effusion and recurrent acute otitis media includes myringotomy and the use of ventilation tubes. Since this procedure was reintroduced by Armstrong in 1954, it has become one of the most commonly performed operations in otolaryngology. In most series perforation of the tympanic membrane in some patients has been reported after spontaneous extrusion or removal of the tympanostomy tubes. We present a retrospective review designed to examine the incidence of persisting perforations of the tympanic membrane in our series of 2604 operated ears. The study als

Use of sclera in tympanic membrane reconstruction.

Because of its high density, high tensile strength, and low antigenicity, sclera appears to be an appropriate grafting material for tympanic membrane reconstruction. I have used preserved donor sclera in 100 such cases over a three-year period with an overall success rate for complete closure of the perforation of 96%. Most of the procedures were done without hospitalization. The adults had local anesthesia; the children, general.

Air caloric stimulation with tympanic membrane perforation.

Warm air caloric stimulation in an ear with tympanic membrane perforation or mastoidectomy cavity often causes contralateral nystagmus. Secondary nystagmus is common. Our evidence with squirrel monkeys and patients indicates that the primary "inversion" results from endolymph cooling due to evaporative cooling due to evaporative cooling of the mucus lining the middle ear cavity, by the dry air stimulus. Disconjugate horizontal nystagmus was found in a patient with large eardrum perforation, after cold air caloric stimulation. The effect probably resulted from stimulation of the anterior or pos
