The author has described one of the typical combinations of injuries in the elbow joint area in children, i.e. fractures of the neck of the radius with a simultaneous fracture of the ulna. The study is based on 37 cases observed by the author. Two types of the above-mentioned fractures have been distinguished: abduction (32) and extension (5) fractures. The methods of closed reposition of each type have been presented. These methods allow to reach the desired results even in major dislocations (60-90 degrees) of the head of the radius.

BACKGROUND: Wrist fractures comprise nearly 30% of all fractures in children. Fractures that damage the distal radius and/or distal ulnar growth plates may require epiphysiodesis to prevent skeletal deformity and/or length discrepancy. Epiphysiodesis is commonly used to correct leg-length discrepancies in children but has not been extensively studied for its use on the distal radius and ulna. The purpose of this study is to present the outcomes of epiphysiodesis procedures about the distal radius and/or distal ulna.
METHODS: A retrospective review identified pediatric patients (<18 years) who

Focal fibrocartilaginous dysplasia (FFCD) of the distal ulna is a rare benign lesion resulting in progressive radiocapitellar dislocation, limb deformity and limitation in function. This study reported our experience with 10 cases and it aimed to determine a reasonable strategy for diagnosis and treatment. Ten cases treated from 2010 to 2018 in our department were retrospectively reviewed. The diagnosis was based on imaging features. All patients underwent lesion excision in the early stage and five patients required ulna lengthening in the second stage. The radiographic and functional outcome

INTRODUCTION: Hand laterality, an important ability to determine the orientation of a limb is common to get affected after short term immobilization. Distal radius and/or ulna fracture is a commonly encountered fracture resulting from upper-limb trauma. Conservative treatment using closed reduction and plaster cast application to immobilize the joint remains choice of treatment over surgery in the treatment of these fractures. There is a paucity of literature reporting impairment in hand laterality after long term immobilization as commonly performed in patients with distal radius and/or ulna

Fractures of the proximal ulna range from simple olecranon fractures to complex Monteggia fractures or Monteggia-like lesions involving damage to stabilizing key structures of the elbow (i.e. coronoid process, radial head, collateral ligament complex). In complex fracture patterns a computerized tomography scan is essential to properly assess the injury severity.Exact preoperative planning for the surgical approach is vital to adequately address all fracture parts (base coronoid fragments first). The management of olecranon fractures primarily comprises tension-band wiring in simple fractures as

Dedifferentiated parosteal osteosarcoma is a variant in which a high grade osteosarcoma coexists with a parosteal osteosarcoma. We report the case of a 20-year-old female patient who presented with six months of evolution of pain and functional limitation of the right forearm, with no apparent cause; radiographs were performed, observing a tumoral lesion in the diaphysis of the right ulna. Physical examination showed pain upon palpation in the diaphysis of the ulna and limitation of prone-supination. Axial computed tomography of the thorax revealed metastatic disease in the upper lobe of the l

BACKGROUND: Isolated fractures of the shaft of the ulna, which are often sustained when the forearm is raised to shield against a blow, are generally treated on an outpatient basis. This is an update of a Cochrane review first published in 1998 and last updated in 2009.
OBJECTIVES: To assess the effects of various forms of treatment for isolated fractures of the ulnar shaft in adults.
SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (April 2012), the Cochrane Central Register of Controlled Trials (The Cochrane Library 2012, Issue 3), MEDLINE (

The objectives of this work were to explore a methodology that combines static and dynamic finite element (FE) analysis, linear elastic fracture mechanics (LEFM) and experimental methods to investigate a worst-case scenario in which a previously damaged bone plate system is subjected to an impact load. Cadaver ulnas with and without midshaft dynamic compression plates are subjected to a static three-point bend test and loaded such that subcritical crack growth occurs as predicted by a hybrid method that couples LEFM and static FE. The plated and unplated bones are then unloaded and subsequentl

A 6-month-old child developed cutaneous mucormycosis of the forearm 5 weeks after liver transplantation, which progressed to osteomyelitis of the proximal ulna. Aggressive treatment, which included serial radical debridements, intravenous administration of amphotericin B, and eventually split thickness skin graft coverage resulted in successful functional salvage of the forearm, wrist, and hand. Of the nine previously reported cases of mucormycosis affecting the forearm or hand in immunocompromised patients, all either required amputation of the hand or below elbow amputation, or resulted in t

PURPOSE: Children with radial longitudinal deficiency have very short forearms. The Ilizarov method of distraction osteogenesis has been used to increase extremity length, but results differ with the underlying condition and the limb being lengthened. The purpose of this study is to examine retrospectively the outcomes of Ilizarov lengthening of the ulna in children with radial longitudinal deficiency.
METHODS: Nine children with radial longitudinal deficiency had 13 ulnar lengthenings using the Ilizarov method; 8 with unilateral deficiency had 9 lengthenings to improve appearance, and 1 chil

A 45-year-old woman completely lost the ability of active supination of the forearm after a Darrach resection for malunited fracture of the distal radius. A three-component reconstruction was performed to stabilise the distal stump of the ulna and prevent convergence between the two forearm bones. The procedure combined advancement lengthening osteotomy of the ulna, longitudinal intramedullary tenodesis of the extensor carpi ulnaris tendon, and dorsal transfer of the pronator quadratus through the interosseous space. Four months after the salvage procedure she again had full active supination

Three patients presented with upper limb problems caused by the loss of the proximal ulnar shaft due to osteomyelitis which occurred in early childhood. The three patients had several features in common: instability of the elbow, dislocation of the radial head and shortness and bowing of the forearm, but hand and wrist function were largely unaffected. One patient had evidence of impaired function of the posterior interosseous nerve, presumably a traction injury. The treatment of these problems is discussed. In one patient, a child who had a functioning ulno-humeral joint, successful reconstru

Fortunately, most bone forearm fractures in children heal with minimal functional disability. My approach to management of the most common injuries is presented. By following the guidelines outlined in this article, the generalist as well as the most sophisticated upper extremity surgeon should be able to avoid pitfalls and complications when managing forearm fractures in children.

A 70-year-old woman incurred a stress fracture of the radius following a fracture of the ulna. The patient had osteomalacia secondary to a Billroth II procedure. The primary reasons for the development of osteomalacia following a Billroth II procedure are: decreased transit time through the gut, which causes less Vitamin D and calcium absorption, and bypassing of the duodenum, where calcium absorption is highest. After treatment of the osteomalacia with calcium and Vitamin D, the fracture healed uneventfully. The present case report emphasizes the need to identify and treat diseases associated

The author has reported a case of volar dislocation of the ulnar head. This exceptional lesion occurred in a young woman with Crouzon's disease. No past history of trauma was found; the ulnar head was not avulsed and the triangular articular disk was not torn. The instability of the ulnar head was due to a flattened sigmoid cavity of the radius and aplasia of the extensor carpi ulnaris muscle. Recovery was made after a low resection of the diaphysis combined with lower radioulnar arthrodesis. This operation restored both power and a full range of movements to the wrist as well as preserved the
