Surgical procedures for the treatment of ulnar nerve compression at the elbow are well described. Studies have reported clinical outcomes after decompression of the nerve without transposition and decompression with transposition. Numerous preoperative, intraoperative, and postoperative factors contribute to failure of the surgical procedures. Although the techniques available for revision decompression of the ulnar nerve at the elbow are similar to those used in the primary setting, the results after repeat surgical intervention are less predictable.

A rare cause of ulnar nerve compression at the elbow is presented in this report. A 42 year old right-handed mechanic developed subacute, progressive numbness, tingling and weakness in his right hand. Electrophysiologic studies demonstrated a severe conduction block affecting the ulnar nerve in the retrotrochlear groove but without any sign of major axonal loss. His hand functions were carefully studied prior to surgery. While fine motor tasks were not affected, the hand strength was markedly diminished. At surgery, a 1-cm diameter intraneural ganglion at the site of the conduction block was f

'Handball goalie's elbow' has been defined as pain in the elbow region due to repetitive forced hyperextensions of the elbow. Goalkeepers are the players most often suffering from hyperextension trauma to the elbow in European team handball. They may complain of radiating pain or numbness in the ulnar aspect of the forearm in addition to local pain in the elbow region. To detect any injury to the ulnar nerve that could explain the symptoms, we performed a neurological and neurophysiological study in goalkeepers with elbow pain. Nine goalkeepers, with a total of 15 'handball goalie's elbow', we

OBJECTIVE: To investigate the compression factor and clinical manifestation of the compression of deep branch of the ulnar nerve at the wrist.
METHODS: Anatomic study was done on both sides of 10 cadavers, the deep branch of ulnar nerve, the Guyon's canal and the flexor digiti minimi brevis pedis were observed. Then from Jan. 1990 to Jan. 1997, 5 patients with compression of the deep branch of ulnar nerve at the wrist were treated clinically. Among them, there were 4 males and 1 female, aged from 37 to 48 years and the course of disease ranged from 1 to 5 months.
RESULTS: The motor branch of

Knowledge of general ulnar anatomy aids in the understanding of the possible site of ulnar nerve entrapment and the rationale for differential diagnosis possibilities. Understanding of internal nerve topography sheds light on reasons for ulnar nerve susceptibility at the elbow region and offers an explanation for the sparing of forearm muscles in ulnar neuropathy at the elbow. Dynamic anatomy or biomechanics of the elbow help elucidate the pathophysiology of ulnar nerve compression at the elbow. The anatomy interface with electrodiagnosis illustrates rationale for particular methods employed a

Controversy continues to surround the treatment of ulnar nerve compression at the elbow in the cubital tunnel. By performing extensive segmental electrodiagnostic studies of the ulnar nerve through the cubital tunnel, accuracy of diagnosis is improved. In those patients with documented compression where the ulnar nerve enters the flexor carpi radialis, decompression of this area alone yields excellent results with rapid recovery.

Carpal tunnel syndrome is often treated nonoperatively with temporary wrist immobilization and local steroid injections. A direct injection into a peripheral nerve can result in permanent damage. Two cases of median nerve injection injury and one involving the ulnar nerve are presented; all were treated with neurolysis and debridement of the injected material. At follow-up ranging from 1 to 11 years, all patients showed significant improvement, but with some functional loss. The literature is confusing because of the variety of injection techniques used for the treatment of carpal tunnel syndr

Spasmodic torticollis is thought to be a central nervous system disorder with no peripheral nervous system component. Several consecutive patients with spastic torticollis were found to have ulnar nerve compromise at the elbow on the side ipsilateral to the direction of chin turning. This article describes three such patients in detail. It is postulated that the cause is the stereotypical posture assumed by these patients in using their hand to maintain a neutral head position. The chronic leaning and elbow flexion place the ulnar nerve at risk for the development of pressure neuropathy.

A patient with immediate palsy of the ulnar nerve secondary to a distal fracture of the radius and possibly a dislocation of the distal radio-ulnar joint (Galeazzi's fracture) is described. Surgery demonstrated a serious contusion of the nerve, scarring around the nerve, and thrombosis of the ulnar artery. Symptoms rapidly disappeared after neurolysis.

Tardy ulnar nerve palsy (TUNP) is a progressive neuropathy commonly associated with cubitus valgus deformity following lateral humeral condyle nonunion. However, concurrent ulnar nerve schwannoma as a contributing factor remains undocumented. We present a rare case in which both chronic traction neuropathy and intraneural compression coexisted, with the schwannoma remaining undetected on high-resolution ultrasound (HRUS) preoperatively. A 44-year-old male professional driver presented with a six-month history of progressive ulnar neuropathy, characterized by paresthesia, intrinsic muscle weakn

INTRODUCTION: In the assessment of hand and upper limb function, grip strength is of the major importance. The measurement by dynamometers has been established.
PURPOSE OF THE STUDY: In this study, the effect of a simulated ulnar nerve lesion on different grip force measurements was evaluated.
METHODS: In 25 healthy volunteers, grip force measurement was done by the JAMAR dynamometer (Fabrication Enterprises Inc, Irvington, NY) for power grip and by a pinch strength dynamometer for tip pinch strength, tripod grip, and key pinch strength.
STUDY DESIGN: A within-subject research design was us

Interfascicular nerve suture with autografts is the operation of choice for repairing peripheral nerve injuries because it ensures more precise alignment of the fasciculi and so better chances of reinnervation of the sectioned nerve. The procedure as described by Millesi et al has been used at the Istituto Neurologico di Milano in 30 patients with traumatic lesions of the median, ulnar and radial nerves. All have been followed up for 2 to 7 years since operation. The results obtained are compared with those of other series obtained with interfascicular suture and with epineural suture. Microsu

PURPOSE: The ulnar nerve might undergo compression due to traumatic or non-traumatic pathological conditions or altered anatomy within the extent of the distal arm to the proximal hand, leading to ulnar neuropathy. There are certain anatomical sites where the increased ulnar nerve cross-sectional area (CSA) values indicate probable compression and can be diagnosed by the high-resolution Ultrasonography (USG) or Magnetic Resonance Imaging (MRI) in symptomatic patients. The ulnar nerve CSA varies across the population, and there is limited data available for the Indian population. Hence, we aime

Cubital tunnel syndrome is a common compressive neuropathy in the upper limb. When nonoperative management has failed to sufficiently improve the condition, various surgical techniques including in situ decompression and ulnar nerve anterior transposition have been undertaken to decompress the nerve, and these can be performed via open or arthroscopic assisted approaches. With open surgery or arthroscopic surgery, good clinical results have been reported. We describe a technique of all-arthroscopic medial bi-portal ulnar nerve release with an anterolateral portal for transient nerve retraction

BACKGROUND AND OBJECTIVES: Cubital tunnel syndrome is the second most common nerve entrapment, and understanding the anatomy is crucial for the success of the nerve release. During ulnar nerve release for cubital tunnel syndrome, a motor branch is frequently encountered crossing anteriorly over the ulnar nerve from its medial/ulnar side proximally to the lateral/radial side distally. Little has been noted about this crossing branch in the literature. In this anatomic study, we sought to characterize this branch further and discuss its potential significance in cubital tunnel release.

Background: Several anatomical variations of the median nerve recurrent motor branch have been described. No previous reports have described the anatomical variation of the ulnar nerve with respect to transverse carpal ligament. In this article, we present a patient with symptomatic compression of the ulnar nerve found to occur outside the Guyon canal due to a transligamentous course through the distal transverse carpal ligament. Methods: A 59-year-old, right-hand-dominant male patient presented with right hand pain, subjective weakness, and numbness in both the ulnar and the median nerve dist
