Consequences of an upper motor neuron syndrome (UMNS) include voluntary weakness or paresis, superimposed involuntary phenomena such as spastic co-contraction and associated reactions, and superimposed rheologic changes in affected muscles. This article describes the use of dynamic poly-electromyography to assess UMNS muscle overactivity and inform muscle selection for chemodenervation. Cases are presented that involve spastic co-contraction, spastic dystonia, associated reactions, hyperextended wrist and finger flexor tenodesis, differentiating neural versus non-neural (rheologic) hypertonia,

During the last years, the prosthetic replacement in upper limb amputees has undergone different developments. The use of new nerve surgical concepts improved the control strategies tremendously, especially for high-level amputees. Technological innovation in the field of pattern recognition enables the control of multifunctional myoelectric hand prostheses in a natural and intuitive manner. However, the different levels of amputation pose different challenges for the therapeutic team which concern not only the prosthetic attachment; also the expected functional outcome of prosthetic limb repl

Cutaneous malignancies are the most common primary malignancies of the hand. The hand surgeon may be the first physician to see these patients or may have the patients referred to them because of expertise in this anatomical region. This article reviews diagnosis and treatment, including margin of resection and need for sentinel lymph node biopsy, for the 3 most common cutaneous malignancies: basal cell carcinoma, squamous cell carcinoma, and malignant melanoma.

The overhead throwing motion is a complex sequence of maneuvers that requires coordinated muscle activity in the upper and lower extremities. The shoulder and elbow are subject to multidirectional forces and are particularly vulnerable to injury during specific phases of the overhead throwing motion. Ligamentous, tendinous, neural, and osseous pathology that may occur in the shoulder or elbow of an overhead-throwing athlete will be discussed, with an emphasis on the role of MR imaging and MR arthrography.

Injuries are becoming increasingly prevalent in pediatric athletes. Many of these injuries are diagnosed clinically and do not require imaging for diagnosis, but the sheer increase in numbers of injuries means that radiologists are evaluating more of them. Some injuries that young athletes sustain are simply due to trauma, such as falls, and may be experienced outside sports just as easily; however, others are peculiar to athletic activities. Many of the latter are chronic overuse injuries as opposed to acute injuries and have characteristic appearances. The types of injuries young athletes in

OBJECTIVES: To review the occurrence of mycotic aneurysm affecting upper extremity arteries.
DESIGN: Literature review.
MATERIALS AND METHODS: A MEDLINE search from 1950 until 2007 and an extensive manual search were carried out using bibliographies from relevant published papers including cases involving arteries distal to the subclavian.
RESULTS: A total of 149 cases (68 papers) were identified. The brachial artery was the most frequently reported site, mostly associated with drug abuse, catheterization procedures or endocarditis. Since 1950 arterial trauma (drug abuse or catheterization)

OBJECTIVE: To report a case of bilateral upper extremity edema associated with amlodipine use in a child.
CASE SUMMARY: A previously well and normotensive 6-year-old girl presented with a generalized vasculitis of unknown origin and severe hypertension. Large vessels predominantly affecting the neck, chest, and abdomen were found to be involved, resulting in abnormal arterial circulation and significant blood pressure differences between the upper and lower extremities. Multiple antihypertensive agents were initially required to control blood pressure. She was stabilized and discharged on aml

Nerve compression syndromes of the upper extremity occur at predicable locations. The diagnosis of nerve compression or nerve entrapment is based on the neurologic and electrodiagnostic examinations. The anatomy, neurophysiology, and electrodiagnosis of nerve compression are discussed. Common and uncommon compression and entrapment syndromes of the upper extremity are described. Errors in diagnosis occur when the neurologic or electrodiagnostic examinations are incomplete or inaccurate.

Golf has demonstrated increasing popularity and with this heightened enthusiasm has come an increased awareness of the significant number of injuries associated with playing golf. While back injuries represent the most commonly injured specific body part, upper extremity injuries are most frequent overall and the most likely to result in loss of play. Patterns of injury differ based on level of play and time spent playing or practicing golf. Among golf professionals, the hand/wrist is the most commonly injured upper extremity structure. Among amateurs, the elbow is most commonly injured. The v

Upper extremity arterial injury resulting in hand and digit ischemia can occur in athletes who perform repetitive, high-stress, overhead arm motions. The initial presentation of these injuries often mimics the more common musculoskeletal injuries found in these athletes, and therefore, a high index of suspicion is essential to establish the diagnosis in a timely fashion. There are several described mechanisms of injury, most of which involve extrinsic compression and injury to the axillary artery or its branches from the humeral head. The clinical diagnosis is confirmed with noninvasive vascul

Vascular injuries of the upper extremity represent approximately 30% to 50% of all peripheral vascular injuries. The majority of injuries are to the brachial artery, and 90% of injuries are due to penetrating trauma. Return of function is often related to concomitant injury to peripheral nerves. However, timely restoration of blood flow is essential to optimize outcome. The diagnosis is made by physical examination and limited Doppler ultrasonography. Arteriography may be helpful if there are multiple sites of injury. Anticoagulation with heparin should be given if not otherwise contraindicate

Water polo is a contact sport combining the skills of swimming, swim conditioning, throwing and, occasionally, the elements of wrestling and boxing. As such, the athletes frequently sustain upper extremity injuries involving the shoulder, the elbow, or the hand and fingers; moreover, injuries may result from either overuse or acute trauma. Successful treatment of these injuries requires an understanding of the peculiarities of the game and the likely mechanisms of injury, and the experience to properly recognize when they occur.

This article provides a comprehensive review of the diagnosis and management of mononeuropathies of the long thoracic, suprascapular, axillary, and musculocutaneous nerves. Although these nerves are frequently injured in conjunction with other portions of the brachial plexus, this discussion focuses on isolated injury to each of these nerves. The anatomy, clinical presentation, differential diagnosis, findings on electrodiagnostic evaluation, origin, management, and prognosis of each mononeuropathy is discussed.

Symptoms and disease processes can overlap, and neurologic symptoms of the hand or arm are not necessarily due to local pathology. To combat these difficulties, the authors present a basic review of neuroanatomy, the neurologic examination, the principles of electromyography, and the process of formulation.

Crush syndrome is the severe systemic manifestation of prolonged muscle compression and compartment syndrome. Careful patient assessment, early diagnosis, and aggressive treatment are vital to prevent multiorgan failure and death. Medical management of systemic complications, along with operative procedures of fasciotomy and debridement, are indicated with accompanying compartment syndrome. Debridement of necrotic and nonviable tissue is necessary; significant risks of infection and hemorrhage remain until the wounds can be subsequently closed or covered with skin graft. Crush syndrome and mus

Twelve cases of necrotizing fasciitis were identified retrospectively over a 5-year period. All were associated with a history of substance abuse by injection or with diabetes. Eleven of the 12 infections were associated with beta-hemolytic Streptococcus, a mixed anaerobic aerobic infection, or both. Three of five patients tested for human immunodeficiency virus had positive test results. A wide extensile approach was used to debride necrotic fascia. An average of 3 debridements were necessary, with a range of 1-6 debridements. Two patients under-went shoulder disarticulation because of uncont

With the knowledge base of normal anatomy, development, biomechanics, and differential diagnosis, the sports medicine professional can treat injured young athletes with greater efficiency. In addition, microtraumatic injuries may be prevented by emphasizing safe parameters of participation, proper throwing techniques, and careful monitoring of the amount of practice time and intensity. Gymnasts using apparatus should always have spotters. The height of towers and basket tosses by cheerleaders should be limited by age and ability. Proper pitching techniques, not the fastest pitch or youngest cu
