Blood flow of the urinary bladder: effects of outlet obstruction and correlation with bioenergetic metabolism.

This study investigated the effects of outlet obstruction on blood flow and high energy phosphates content in the rabbit urinary bladder. Mild bladder outlet obstruction was induced by placing a silicon ring (diameter 7 mm) around the bladder neck of each male New Zealand White rabbit (n = 7). Before and immediately after inducing obstruction, and 2 weeks later, the bladders were emptied and regional blood flow measured using laser Doppler flowmetry (LASERFLO BPM2, Vasamedics Inc., St. Paul, Minnesota). Six different areas of each bladder were measured, and the average blood flow calculated fo

[Identification of lower urinary tract voiding dysfunction in females by using video-urodynamic study].

OBJECTIVE: To assess the value of video-urodynamic study (VUD) in the identification of lower urinary tract voiding dysfunction in female. METHODS: A total of 126 female patients with sign and symptoms of lower urinary tract voiding dysfunction underwent VUD from December 2008 to January 2011 in Beijing Chaoyang Hospital. The causes of voiding dysfunction were analyzed based on VUD findings. RESULTS: Neurogenic voiding dysfunction was found in 30 patients (23.8%), non-neurogenic voiding dysfunction was found in 96 patients (76.2%). The 72 patients suffered from recurrent urinary tract infect

Prostatism. I. The correlation between symptoms, cystometric and urodynamic findings.

One hundred and seven consecutive patients referred with symptoms of bladder outlet obstruction were studied using cystometry and combined pressure-flow-electromyographic investigation. The symptoms of infravesical obstruction were not statistically significantly correlated to the hydrodynamic documentation of increased bladder outlet resistance as judged by maximum flow rate and the calculated urethral resistance. Irritative symptoms such as frequency, nocturia, urgency and urgeincontinence were statistically significantly correlated to the presence of detrusor hyperreflexia. The functional d

Transurethral incision of the bladder neck and prostate.

Between March 1983 and December 1988, 66 men 50 years old and older with symptomatic bladder outlet obstruction underwent transurethral incision of the bladder neck and prostate. Patients selected for incision had a small, clinically benign, prostate and peak urinary flow rate of less than 15 ml. per second. Preoperative and postoperative evaluation included symptom questionnaires and uroflowmetry. A single midline incision was made extending from the bladder neck to the verumontanum. Results were available in 64 of the 66 men who underwent the procedure with a mean followup of 2.24 years. Mea

Metastatic bladder tumor from prostate cancer causing a ball valve-like obstruction in urination.

We report a rare case of prostate cancer that apparently metastasized to the bladder and formed a pedunculated mass that caused a ball valve-like obstruction in urination. A 57-year-old man with metastatic prostate cancer was referred to the emergency department for acute urinary retention. Cystoscopy and magnetic resonance imaging revealed a pedunculated mass measuring approximately 2 cm in size in the bladder neck that appeared to cause urinary obstruction in a ball valve-like manner. Transurethral resection of the bladder tumor was performed that resolved his symptom, and histopathological

Prediction of Bladder Outlet Obstruction in Male Patients with Lower Urinary Tract Symptoms Based on Symptom Scores and Noninvasive Office-Based Diagnostic Tools.

Purpose: The purpose of this study was to investigate the predictive values of combining symptom scores, prostatic parameters, uroflowmetry parameters, intravesical prostatic protrusion, and prostatic urethral angle for the establishment of a bladder outlet obstruction (BOO) risk score for diagnosing BOO in men with lower urinary tract symptoms. Materials and Methods: A total of 355 men with lower urinary tract symptoms were enrolled and divided into a training set (N = 285) and validation set (N = 70). Videourodynamic studies were used to diagnose lower urinary tract dysfunction subtypes, whi

The Long-term Effect of Superficial Bladder Neck Incision on Ejaculation and Incontinence in Boys with Primary and Secondary Bladder Neck Obstruction.

OBJECTIVE: Superficial bladder neck incision (SBNI) is controversial at young age, with retrograde ejaculation after puberty as main concern. The aim of the study is to investigate the long-term effect of SBNI on ejaculation and incontinence in boys with primary and secondary bladder neck obstruction (BNO). MATERIALS AND METHODS: In boys with infravesical obstruction, SBNI was performed in case of a persistent BNO after earlier desobstruction or in case of primary severely obstructive bladder neck. SBNI was performed with a diathermy hook, always superficially (2-3 mm) and unilaterally at 7 O

Botulinum Toxin A Injection in the Bladder Neck: A Promising Treatment for Urinary Retention.

Secondary to failure of optimal medical therapy and the high morbidity that accompanies surgical techniques in high risk patients, the use of de novo treatments including botulinum toxin A is emerging in the treatment of benign prostatic hyperplasia (BPH). However, the treatment of urinary retention secondary to BPH via injecting botulinum toxin into the bladder neck is not well established in the literature. This case report describes the case of a 75-year-old male patient with a chronic history of obstructive lower urinary tract symptoms (LUTS) and multiple comorbidities who was admitted to

[Physiopathology of BPH obstruction].

The underlying BPH related mechanisms of outlet obstruction are outlined (modification of the shape of the bladder neck, failure of funnel formation, geometric variations of the urethra, alpha 1 adrenoceptors mediated sympathetic hyperactivity at the level of the lower genito-urinary tract). Bladder voiding dysfunction in response to prostate obstruction is also discussed and correlated to ultrastructural patterns and clinical symptoms. Finally, a pathogenetic mechanism for detrusor overactivity in obstructed bladder is suggested.

[Female urethral obstruction and bladder neck stenosis - fact or myth - how to proceed].

The female urethra is probably the most neglected organ in women. Female urethral stricture and primary bladder neck obstruction are rare clinical entities. Traditional and new surgical techniques have been described for the treatment of female urethral stricture. However, they are based on limited data. There is no consensus on best management. The techniques of urethroplasty all have a higher mean success rate (80-94%) than urethral dilatation (< 50%), albeit with shorter mean follow-up. Urethroplasty performed by experienced surgeons appears to be a feasible option in women who have failed

Stem cell therapy in bladder dysfunction: where are we? And where do we have to go?

To date, stem cell therapy for the bladder has been conducted mainly on an experimental basis in the areas of bladder dysfunction. The therapeutic efficacy of stem cells was originally thought to be derived from their ability to differentiate into various cell types. Studies about stem cell therapy for bladder dysfunction have been limited to an experimental basis and have been less focused than bladder regeneration. Bladder dysfunction was listed in MESH as "urinary bladder neck obstruction", "urinary bladder, overactive", and "urinary bladder, neurogenic". Using those keywords, several artic

Bladder neck resection and transurethral resection of the prostate: a randomized prospective trial.

Transurethral resection of the prostate and bladder neck incision are accepted methods in the treatment of obstructive prostatic hyperplasia. Bladder neck incision is particularly useful in cases of small prostates. We have modified the method of bladder neck incision to bladder neck resection. A randomized prospective trial was done to compare the results of conventional transurethral resection of the prostate (30 patients) and the new method of bladder neck resection. Bladder neck resection was comparable to transurethral resection of the prostate with respect to postoperative hospital stay,
